BACKGROUND: Consenting to do-not-resuscitate (DNR) orders is an important and complex medical decision-making process in the treatment of patients at the end-of-life in emergency departments (EDs). The DNR decision in EDs has not been extensively studied, especially in the Chinese mainland.
METHODS: This retrospective chart study of all deceased patients in the ED of a university hospital was conducted from January 2017 to December 2019. The patients with out-of-hospital cardiac arrest were excluded.
RESULTS: There were 214 patients’ deaths in the ED in the three years. Among them, 132 patients were included in this study, whereas 82 with out-of-hospital cardiac arrest were excluded. There were 99 (75.0%) patients’ deaths after a DNR order medical decision, 64 (64.6%) patients signed the orders within 24 hours of the ED admission, 68 (68.7%) patients died within 24 hours after signing it, and 97 (98.0%) patients had DNR signed by the family surrogates. Multivariate analysis showed that four independent factors influenced the family surrogates’ decisions to sign the DNR orders: lack of referral (odds ratio [OR] 0.157, 95% confidence interval [CI] 0.047-0.529, P=0.003), ED length of stay (ED LOS) ≥72 hours (OR 5.889, 95% CI 1.290-26.885, P=0.022), acute myocardial infarction (AMI) (OR 0.017, 95% CI 0.001-0.279, P=0.004), and tracheal intubation (OR 0.028, 95% CI 0.007-0.120, P<0.001).
CONCLUSIONS: In the Chinese mainland, the proportion of patients consenting for DNR order is lower than that of developed countries. The decision to sign DNR orders is mainly affected by referral, ED LOS, AMI, and trachea intubation.
2.Glaucoma patients'contrast sensitivety changes and the effect of tinted filter lens/

A total of 17 cases (30 eyes) of glaucoma were tested for their contrast sensitivity (CS) with the VCTS 6000, without or with yellow or red filter lens. Test results showed that 1) the glaucoma patients′ CS curve was depressed at all the five spatial frequencies (1.5, 3, 6, 12, and 18 cycle/degree) tested; 2) the decrease was most significant at the 12 cycle/degree (e/d), which made the CS curve look like having a notch (v shaped); 3) the peak shifted to the left. Yellow filter lens could signifi-cantly increase the glaucoma patients′CS at 1.5, 3 and 6 c/d, especially at the peak (3c/d), leading to facilitation of mobility and adaptability in daily life. The red filter lens could improve CS at the peak but it decreased CS at 6, 12 and 18c/d, so it seemed that the red filter lens could not improve the visual function.
3.Suppression effect of RNA interference on expression of EGFR of SKOV3 cell line

Objective To investigate the suppression effect of RNA interference(RNAi) on the expression of epidermal growth factor receptor(EGFR) of ovarian cancer SKOV3 cell line. Methods SKOV3 cells were transfected with synthetic EGFR sequence-specific dsRNA by lipofectamin 2000.The expression level of EGFR mRNA and protein were detected by RT-PCR and Western blotting,respectively.The proliferation of transfected SKOV3 cells was detected by WST-1 cell proliferation assay.The abilities of adhesion,migration and invasion were detected by cell adhesion assay,cell migration assay and Matrigel invasion assay,respectively. Results The suppression rates on EGFR mRNA and protein by sequence-specific dsRNA-EGFR were 73.65% and 58.94%,respectively.The cell proliferation was inhibited by 22.54%,35.76% and 31.07%,respectively at 24,48 and 72 h after transfection.The adhesion of sequence-specific dsRNA transfected SKOV3 cells for 30 min and 90 min were reduced by 12.11% and 18.66%,respectively.The migration and invasion of SKOV3 cells were decreased by 25.47% and 22.08%,respectively. Conclusion The down-regulation of EGFR expression of ovarian cancer by sequence-specific dsRNA can lead to the suppression of proliferation,adhesion,migration and invasion of ovarian cancer cells.
4.The bactericidal effect of ozonated water on the putative periodontopathic bacteria

Objective:To evaluate the antimicrobial effect of ozonated water on the putative periodontopathic bacteria.Methods:Pophyromonas gingivalis (P.g)ATCC33277,Haemophilus actinomycetemcomitans (H.a)ATCC29522,Fusobacterium nucleatum (F.n)ATCC1 0957 and clinically seperated strain of P.g(C -P.g)were treated by ozonated water with ozone concentration(mg/L) of 0.03,0.06 and 0.1 2 for 30,60,90 and 1 20 s respectively.The bactericidal effect was tested by bactericidal assay.H2 O2 was used as the positive control and distilled water as the negative control.Results:The antimicrobial rate of ozonated water agaist the bacteria increased with the ozone concentration increase.There was no statistic diffrence of the effect on P.g and C -P.g(P >0.05).Linear regression analysis showed that the βvalues of the concentration factor were over 0.95,that of the time factor under 0.1 1 .Conclu-sion:The ozonated water has dose-dependent bactericidal effect on P.g,H.a and F.n.
5.Relativity between serum vascular endothelial growth factor and its receptors and uterine myoma

Objective To study the relativity between serum vascular endothelial growth factor(sVEGF),its receptors(sVEGFR1 and sVEGFR2) and uterine myoma. Methods Thirty hospitalized patients with uterine myoma were chosen as the study group and the other healthy women of the same age as control group.The levels of sVEGF,sVEGFR1 and sVEGFR2 of the two groups were determined by using ELISA technology. Chlamydia trachomatis and Neisseria gonorrhoeae are important bacterial pathogens
of sexually transmitted infections (STIs) worldwide. This study sought to compare the analytical
sensitivity and specificity of conventional methods against a rapid molecular method in detecting
STIs caused by these bacteria. Methods: Ninety five first-time male attendees of the Genito-urinary
Medicine Clinic in Hospital Kuala Lumpur were included in this cross-sectional study. The detection
of C. trachomatis was achieved through direct fluorescence antibody (DFA) staining of urethral
swabs and real-time polymerase chain reaction testing (Xpert® CT/NG assay) on urine specimens.
N. gonorrhoeae was detected through Gram staining and culture of urethral swabs and Xpert® CT/
NG assay on urine specimens. Results: From the Xpert® CT/NG results, 11 (11.6%) attendees had
chlamydia, 23 (24.2%) had gonorrhoea and 8 (8.4%) had both STIs. The sensitivity and specificity
of DFA in detecting chlamydia compared to Xpert® CT/NG were 5.3% (95% CI: 0-28) and 94.7%
(95% CI: 86-98), respectively. For gonorrhoea, the sensitivity and specificity of Gram staining were
90.3% (95% CI: 73-98) and 95.3% (86-99), respectively, whereas the sensitivity and specificity of
culture compared to Xpert® CT/NG were 32.2% (95% CI: 17-51) and 100% (95% CI: 93-100),
respectively. Conclusion: Although Gram-stained urethral swab smears are sensitive enough to be
retained as a screening tool for gonorrhoea, culture as well as DFA lack sensitivity and are poorly
suited to screen for gonorrhoea and chlamydia, respectively. However, owing to their high specificity,
conventional detection methods are still suitable as confirmatory tests for gonorrhoea and chlamydia.
7.The effect of Smad4 for normal development of mouse lacrimal gland

BackgroundSmad4,a key intracellular mediator in transforming growth factor-β (TGF-β)signaling,plays a critical role in the normal development of many tissues/organs.However,its functional role in the development of lacrimal gland has rarely been reported.ObjectiveThe aim of this study was to investigate the role that Smad4 may play in the development of lacrimal glands using Smad4 conditional knockout (CKO) mice( C57BL/6 mouse line),MethodsSmad4 in lacrimal glands,as well as in the lens,cornea and ectoderm of the eyelids,was conditionally inactivated by the Pax6 promoter-driven Cre transgenic mice and Smad4 conditional gene mice,LacZ reporter was used to visualize the developing lacrimal gland by X-gal staining,and standard histological approaches were used to reveal morphological changes.Six or more mice or embryos in each group were used for comparisons at the same stage.ResultsLacZ staining showed that E15.0,Smad4 CKO mice could still develop primary lacrimal bud,but much shorter than the wild-type ones.At E16.5,the primary lacrimal bud in wild-type mice began branching,but no branching was found in Smad4 CKO mice except that the primary lacimal bud became blunt at the tip.At E18.0,although Smad4 CKO mice develop some acini,the branching and size and number of acini were obviously less than ones in Smad4 wild-type mice.Based on histological findings,lacrimal glands in Smad4 CKO mice developed slowly,and the size was considerably smaller,and the numbers of lobes as well as the numbers of acini were much fewer than those of Smad4 wild-type mice lacrimal glands at various stages.Pigment and adipose tissue were also observed within the lacrimal glands starting from P7 in Smad4 CKO mice and increased with age growing.Lacrimal glands in mutant adult mice were eventually replaced by adipose tissue and accumulation of pigments.Conclusions These results support the notion that Smad4 is essential for the normal development and maintenance of the mouse LG and may be involved in the metabolism of pigment and adipose tissue in LG.
8.Effects of ketamine on the expression of N-methyl-D-aspartate receptor-1 in a rat model of focal cerebral ischemia

Objective To investigate the effects of ketamine on the expression of NMDA receptor-1(NRⅠ)in a rat model of focal cerebral ischemia and the possible mechanism of the neuroprotection.Methods Forty healthy male SD rats weighing 250-290g were randomly divided into 2 group(n=20 each):groupⅠketamine and groupⅡpentobarbital.The aminals were anesthetized with intraperitoneal ketamine 60 mg?kg~(-1) in groupⅠor pentobarbital 40 mg?kg~(-1) in groupⅡ.Focal cerebral ischemia was produced by permanent middle cerebral artery occludion(MCAO).The animals were killed at 24 h and 72 h of MCAO and their brains removed for determination of infarct size,the number of living neurons in the penumbra and the expression of NRⅠprotein(immuno- histochemistry).Results The infarct size was significantly smaller;the number of living neurons in penumbra significantly larger and NRⅠexpression significantly down-regulated in ketamine group than in pentobarbital group.Conclusion Ketamine can protect the brain against ischemia through downregulation of NMDA receptor-1.
9.A multicenter,randomized,double-blinded and placebo-controlled study of acute brain infarction treated by human urinary kallidinogenase

Objective To evaluate the efficacy and safety of a new drug,human urinary kallidinogenase,against acute brain infarction.Method A 15-center,randomized,double-blinded and 3:1 placebo-controlled study was carried out.Acute brain infarction within 48 hours of onset in the territory of the middle cerebral artery were indicated as subjects;kallidinogenase or placebo which was dissolved in 50 ml saline,was slowly injected intraveousely within 30 minutes daily for 3 weeks.The European Stroke Scale and Barthel Index were used to evaluate the neurological deficit and the activities of daily living(ADL),followed by a follow-up at the end of the third month.Results 446 patients were enrolled,who completed ITT analysis,including 330 in kallidinogenase group and 116 in placebo group,meanwhile 421 proceeded with PP analysis(311 and 110 respectively).There were no significant differences of the baseline data between the 2 groups.At the end of treatment,the ESS scores increased by 55.1%?33.0% and 44.7%?32.8% respectively in kallidinogenase group(KG)and placebo group(PG,P=0.0022),the difference being significant.PP analysis had similar results.As for ADL,follow-up 90 days after the treatment showed 374 cases followed,280 in KG and 94 in PG;1 died in PG,while none in KG.In KG,the cases whose BI≥50 were significantly more than those in PG(P=0.0228).Adverse events possibly or definitely attributable to the drug were observed in 27 cases(7.74%),mostly were mild,such as palpitation,flush,dizziness, nausea etc,without special management needed.Only 2 died which was confirmed not correlated to kallidinogenase,and another 2 cases of sudden blood pressure drop were observed.The blood pressure drop, quickly restoring soon after the withdrawal of kallidinogenase and use of hemopiesic drugs,was considered to be caused by the combination use of anti-hypertensive drug ACEI and quick infusion speed.Conclusion Kallidinogenase is efficacious for acute brain infarction in improving the neurological deficits,which is safe in clinical use.

Atherosclerosis, a chronic degenerative disease mainly attacks the middle-aged and the aged population as they grow old. Anti-angiocellular aging has gradually become a new strategy for atherosclerosis. In the process of atherosclerosis developing, endothelial cell renewing is speeding. Various biological function disorders that induce blood vessel aging emerge, which leads to changes of the telomere and telomerase, resulting in aged endothelial cells and dysfunction. Telomere and telomerase may play key roles in the etiological factors such as inflammation and AS plaque. In our previous work we have found that Chinese compounds with Shen invigorating effects could not only obviously ameliorate the symptoms and functions of the senility, but also show significant effects on restraining atherosclerosis. We should actively study the mechanisms of Chinese medicine for treating atherosclerosis from Shen, and the mechanisms of Shen invigorating compounds for regulating angiocellular aging through the telomere pathway, thus providing evidence for establishing vascular cell aging based atherosclerosis prevention and treatment strategies by Chinese medicine.
