1.Effect of Cefotiam Sodium Combined with Furosemide on Renal Function in Rats

Objective:To evaluate the safety of the combination of cefotiam sodium and furosemide and to provide the reference for clinical medication. Methods:Three groups were established, including the blank control group, cefotiam sodium group at the dosage of 500 mg·kg-1 ·d-1 , cefotiam sodium combined with furosemide group at the respective dosage of 500 mg·kg-1 ·d-1 and 15 mg ·kg-1 ·d-1 . After the continuous administration for 12 days, the renal structure, serum uric acid, creatinine and urea nitrogen, u-rine of α1 microglobulin and β2 microglobulin in the rats were detected. Results:Cefotiam sodium at the dosage of 500 mg·kg-1 · d-1 showed no significant effects on the renal structure, serum uric acid, creatinine and urea nitrogen,urineα1 microglobulin andβ2 microglobulin in the rats. The combination group showed significantly increased urine β2 microglobulin (P<0. 05) and significantly decreased serum uric acid (P<0. 05). Conclusion:Short time use of cefotiam sodium exhibits no significant effect on the renal struc-ture and function in rats, while the combination of cefotiam sodium and furosemide has significant effects on urineβ2 microglobulin and serum uric acid in rats.

Objective To evaluate the effect of anterior cervical fusion or non-fusion surgery for the treatment of Hirayama disease.Methods The patients with Hirayama disease who underwent anterior cervical surgery in our hospital from July 2008 to December 2015 were retrospectively enrolled in this study.These cases were divided into two groups:fusion group(11 cases)and non-fusion group(12 cases).In fusion group,there were 9 males and 2 females,with an average age of 17.92±0.61 years(range,15-29).In the non-fusion group,there were 10 males and 1 female,with an average age of 19.18±1.14 years(range,15-23).Muscle strength,muscular atrophy,and the Michigan score for hand function were compared between the two groups preoperatively and postoperatively.The indexes such as arc cervical physiological curvature index,the maximum flexion angle,the maximal range of motion(ROM),flexion ROM of adjacent segments of lower(cervical vertebra were measured from the X-ray of the two groups,and the cross-sectional area of spinal cord was measured and compared on neutral position MRI.Results There were no significant differences between the two groups in grip strength,preoperative hand function Michigan score,cervical physiological(curvature index,the maximum flexion angle,the maximal ROM,flexion ROM of adjacent segments of lower cervical vertebra,and cross-sectional area of spinal cord on neutral position MRI(P>0.05).Patients in fusion group were followed up for 38-103 months with an average of 59.73±5.57 months,while patients in non-fusion group were followed up for 40-95 months with an aver-age of 56.25±4.57 months.Clinical outcomes:At the last follow-up,the finger tremor and cold paralysis were alleviated in all patients,but the symptoms in fusion group and non-fusion group were not completely disappeared in 2 and 3 cases,respectively.The grip strength of fusion group(preoperative 17.14±6.09,postoperative 17.47±5.64)and the non-fusion group(preoperative 17.75±5.49,postoperative 17.40±4.58)were not significantly improved compared with preoperative(P>0.05).The Michigan score for hand function of fusion group(preoperative 10.27±0.41,postoperative 6.64±0.24)and the non-fusion group(preoperative 9.25±0.35,postoperative 6.83±0.24)were significantly improved compared with preoperative(P<0.05).There was no significant difference in postoperative Michigan score between the two groups(P>0.05).Radiological outcomes:The cervical physiological curvature index and cross-sectional area of the spinal cord of the two groups increased after operation,while the maximum flexion angle,the maximal ROM decreased.There was no significant difference in these indexes between the two groups postoperatively(P>0.05).The spinal cord had no compression on flexion MRI.The cross-sectional area of C4-C7 spinal cord in Neutral MRI were significantly increased at C6 and C7 levels in both groups.There was no significant difference in the cross-sectional area of C4-C7 spinal cord between the two groups postoperatively(P>0.05).Two cases of internal fixation loosening occurred in non-fusion group 3 years after the operation,while there were no cases of loosening in the fusion group.Conclusion Anterior cervical fusion or nonfusion surgery can limit excessive cervical kyphosis in patients with Hirayama disease,reduce atrophy of compressed cervical spinal cord,and alleviate the disease progress of muscular atrophy of the distal upper limb.It can be selected as a treatment method for Hirayama disease.However,there is a risk of internal fixation loosening in non-fusion cases,and also a risk of adjacent segment degeneration in fusion cases.The appropriate cases should be selected carefully for surgery.
4.Simultaneous Determination of the Contents of 8 Components in Pingzang Tiaoshen Granule by HPLC

OBJECTIVE:To establish a method for simultaneous determination of berberine hydrochloride,icariin,acteoside,isoflavone glucoside,paeoniflorin,mangiferin,salvianolic acid B and puerarin in Pingzang tiaoshen granule.METHODS:HPLC method was adopted.The determination was performed on InertSustain C18 column with mobile phase consisted of acetonitrile-0.05mol/L potassium dihydrogen phosphate solution (pH to 3,gradient elution) with the flow rate of 1.0 mL/min.The detection wavelength was set at 250 nm (0-23 min,puerarin,mangiferin),230 nm (>23-30 min,paeoniflorin),220 nm (>30-50 min,isoflavone glucoside,acteoside),286 nm (>50-60 min,salvianolic acid B),265 nm (>60-75 min,berberine hydrochloride),220 nm (>75-90 min,icariin).The column temperature was set at 30 ℃,and sample size was 10 μL.RESLUTS:The linear range of berberine hydrochloride,icariin,acteoside,isoflavone glucoside,paeoniflorin,mangiferin,salvianolic acid B and puerarin were 4.000-400.0,4.843-484.3,0.498-49.8,2.366-236.6,23.26-2 326.0,3.067-306.7,3.629-362.9 μg/mL,48.23-4 823.2 μg/mL(r≥0.999 4),respectively.The limits of detection were 0.02,0.02,0.02,0.02,0.01,0.02,0.01,0.01 μg/mL;the limits of quantitation were 0.07,0.05,0.06,0.05,0.03,0.07,0.02,0.03 μg/mL,respectively.RSDs of precision,stability (24 h)and repetition tests were a11<2.0% (n=6).The average recoveries were 95.77%-103.50% (RSD=0.77%-2.22%,n=6).CONCLUSIONS:Established method can be used for simultaneous determination of 8 components such as berberine hydrochloride in Pingzang tiaoshen granule.
5.Effects of Bifidobacterium Combined with L-carnitine on Intestinal Flora of Dysbacteriosis Diarrhea Model Rats

OBJECTIVE:To study the effect s of Bifidobacterium combined with L-carnitine on intestinal flora of dysbacteriosis diarrhea model rats. METHODS :Totally 30 SD rats were randomly divided into blank control group ,model group ,probiotics group(Bifidobacterium triple viable enteric coated capsules 70 mg/mL),L-carnitine group (L-carnitine injection 50 mg/mL)and L-carnitine+probiotics group (L-carnitine injection 50 mg/mL+Bifidobacterium triple viable enteric coated capsules 70 mg/mL). Except for blank control group ,the rats in other groups were given 50 mg/mL clindamycin phosphate intragastrically (2 mL/rat, once a day ,for 4 consecutive days )to establish the model of dysbacteriosis diarrhea. On the 5th day of the experiment ,the rats in administration groups were given corresponding drugs intragastrically ,blank control group and model group were given equal volume of normal saline intragastrically ;with the dosage volume of 1 mL/rat,once a day ,for consecutive 7 days. The general situation of rats in each group was observed during the experiment. The feces of normal control group and model group at the end of the modeling and the feces of the rats in administration group after the last administration were collected for genomic DNA extraction,polymerase chain reaction amplification ,library construction and high-throughput sequencing. After processing ,the effective data were analyzed by operational taxonomic unitsclustering and species annotation ,as well as Alpha and Beta diversity of compared with blank control group ,grade 1 feces and grade 2feces were found in model group. The diversity and richness of intestinal flora ,the ratio of Firmicutes/Bacteroidetes and zhongjuanwang7@163.com the abundance of probiotics such as Lactobacillus, Bifidobacterium and Ackermann were significantly decreased (P<0.05),while the abundance of pathogenic bacteria such as Enterococcus was significantly increased (P<0.05). At the end of the recovery period ,compared with model group ,the activity,fecal morphology and color of rats in probiotics group ,L-carnitine group and L-carnitine+probiotics group returned to normal,and the diversity and richness of intestinal flora had no significant difference (P>0.05). However ,the abundance of Lactobacillus in intestinal tract was increased to a certain extent ,and the abundance of Ackermann in intestinal tract of rats in L-carnitine+probiotics group was significantly increased (P<0.05). CONCLUSIONS :Although Bifidobacterium combined with L-carnitine have no significant effect on improving the diversity and richness of intestinal flora in dysbacteriosis diarrhea model rats,it could increase the abundance of probiotics to a certain extent.
