1.Clinical diagnosis rather than aquaporin-4 immunoglobulin status predicts the cognitive performance in central demyelinating disease

Background:Reports on the aquaporin-4 immunoglobulin G (AQP4-IgG) status for cognitive performance
and neuroimaging correlations are limited in neuromyelitis optica (NMO) and multiple sclerosis (MS)
literature. Methods: Cognitive results of 19 MS and 15 NMO patients were compared with 47 agematched
controls. Apparent diffusion coeffi cient (ADC) values were used to delineate gray matter
and white matter damages and correlate with neuropsychological results. Results: Verbal memory test
showed signifi cant differences between MS and NMO in the late registration, early and delay recall
(p<0.05), while their retention rates were even. In MS, ADC values were signifi cantly elevated in the
dorsolateral prefrontal and occipital gray matter which was in contrast with NMO group that showed
elevation in the dorsolateral prefrontal gray matter and parieto-occcipital white matter. AQP4-IgG
status exerted a limited effect on ADC values and neuropsychological results.
Conclusions: Verbal memory test might be helpful in differentiating NMO and MS. ADC values
can be used as a surrogate marker for tissue injury in NMO and MS since they were in line with the
cognition scores. Anatomical regions with elevated ADC values were different in NMO and MS.
2.Sociocultural Factors Associated with Caregiver-Psychiatrist Relationship in Taiwan.

OBJECTIVE: Research on sociocultural factors associated caregiver-provider relationship is needed to enhance family involvement in psychiatric care. This study examines from the caregiver's perspective the associations of schizophrenia attributions, stigmatization, and caregiving experiences with caregiver-psychiatrist working relationship in Taiwan. METHODS: This cross-sectional study used a convenience sample of 152 Taiwanese family caregivers of persons diagnosed with schizophrenia, recruited from a grassroots organization, 4 community mental health rehabilitation centers and 2 psychiatric hospitals between July 2012 and March 2013. Multiple linear regression models were used for analysis. RESULTS: Biological attribution was positively associated with perceived family collaboration, and so was environmental attribution with perceived informational support. Internalized stigma was negatively associated with perceived family collaboration. Caregiving rewards were positively related to both perceived family collaboration and informational support, and so was experience of problems with services to perceived family collaboration. CONCLUSION: The examination of family perceptions informs Western psychiatric care providers of the importance of culturally sensitive practices in developing an effective working relationship with family caregivers, particularly in regards to caregivers' casual attributions, impact of stigma, and caregiving experiences.

3.TRODAT-1 and Tc-99m ECD observations in hyperglycemia hemichorea

We describe two cases of right hyperglycemia hemichorea (HGHC) with identical Tc-99m TRODAT-1/
Tc-99m ECD scan findings. While the brain MRI showed signal alterations within the left putamen,
there was evidence of hyperperfusion on Tc-99m TRODAT-1 but hypoperfusion on Tc-99m ECD
within the left putamen, in association with hyperperfusion within left thalamus on Tc-99m ECD.
The discrepancy between the Tc-99m TRODAT-1 and Tc-99m ECD scan provides insight into the
imbalance between direct and indirect circuits along the nigrostriatal pathway, as the fundamental
genesis of HGHC. Furthermore, the hyperperfusion at the left thalamus represents thalamic disinhibition
secondary to loss of pallidal negative control, which ultimately leads to HGHC through re-entrant
outflow to the motor cortex.

5.Patients' Perceptions and Expectations of Family Participation in the Informed Consent Process of Elective Surgery in Taiwan.

PURPOSE: This study investigated patients' perceptions and expectations of their families' participation in the informed consent process of elective surgery. METHODS: This is a survey study. Anonymous questionnaires that were mailed to potential participants included a demographic data sheet and a scale, measuring patients' perceptions of themselves and their families' participation level in the informed consent process. A convenience sample of patients who had undergone surgery and had been discharged within 4 months from a medical center in southern Taiwan (n = 1,737) were recruited. RESULTS: One hundred and forty-five recipients replied, gaining a response rate of 10.0%, and 120 provided complete data. The mean age of the participants was 56.0 years (range 20e85, SD = 14.98), and more than half were female (54.2%). Twenty-one participants (17.5%) perceived having less family participation than their own participation in the surgery informed consent process, and 40% expected more family participation in the process. The mean scores of the self-rated participation level was significantly higher than that of the families (p < .001). Patients' expectations of family participation were significantly higher than their perceptions of family participation (p < .001). Eleven patients (9.2%) reported having more family participation than they expected. Participants received the most information about the disease during the informed consent process and the least information about alternatives to surgery. Age, gender, number of previous surgeries and admissions influenced the study participants' perceived level of participation in the informed consent process. CONCLUSION: This preliminary study demonstrates that patients' perceptions and expectations for family participation in the surgical informed consent process vary. Healthcare providers should be aware of patients' expectation to appropriately invite their family into the informed consent process.

6.Taking a Closer Look at Bus Driver Emotional Exhaustion and Well-Being: Evidence from Taiwanese Urban Bus Drivers

Background:
Urban bus drivers work under conditions that are among the most demanding, stressful, and unhealthy with higher rates of mortality and morbidity as well as absenteeism and turnover.
Methods:
Drawing on the job demand-resource model, this study investigates the impacts of job characteristics on emotional exhaustion and the effects of emotional exhaustion on job outcomes (including job satisfaction, life satisfaction, organizational commitment, and turnover intention) in the context of bus drivers.
Results:
Using self-reported survey data collected from a sample of 320 Taiwanese urban bus drivers, results reveal that role overload and work-family conflict (as job demand factors) positively relate to emotional exhaustion, and organizational support (as a job resource factor) is negatively associated with emotional exhaustion. Emotional exhaustion has negative effects on both job satisfaction and organizational commitment. Job satisfaction positively leads to life satisfaction, whereas organizational commitment negatively relates to turnover intention.
Conclusion
This study concludes that role overload and work-family conflict as two stressors related to job demands and organizational support as the job resource factor to affect emotional exhaustion which further influence well-being in bus driver context. The moderating effects of both extraversion and neuroticism on the relationship between job demands and emotional exhaustion are evident.
7.Exploring orthostatic hypotension in patients with multiple system atrophy by a non-invasive cardiac output system

Objective: To detect early subclinical signs of autonomic dysfunction in the cardiovascular system
and explore the mechanism of orthostatic hypotension (OH) in patients with multiple system atrophy
(MSA). Methods: Eighteen male patients with possible MSA and 10 healthy men were recruited.
The hemodynamic responses to head-up tilt and tilt-reversal were studied by an electrically-powered
tilt table and a non-invasive cardiac output measurement (NICOM) system. Results: At supine, there
was no signifi cant difference in blood pressure, heart rate (HR), stroke volume, cardiac output and
total peripheral resistance between MSA patients and healthy controls. During tilting upright, OH
developed in 5 MSA patients, with a 23.7±4.8 mmHg drop in systolic blood pressure. Patients with
OH were older and exhibited higher scores in unifi ed Multiple System Atrophy Rating Scale part I
than patients without OH. The stroke volume, cardiac output and total peripheral resistance did not
differ between groups. The controls had the most signifi cant HR elevation (6.5±2.5 bpm) during tiltup,
followed by patients without OH (2.8±1.6 bpm) and those with OH (-0.2±2.2 bpm). A similar
trend of HR decrease was observed during return to supine posture. The process of tilt-reversal altered
HR more signifi cantly than head-up tilt in controls (8.0±2.9 vs 6.5±2.5 bpm; P=0.031) and patients
without OH (4.2±2.1 vs 2.8±1.6 bpm; P=0.032), but not in patients with OH (1.2±1.5 vs -0.2±2.2
bpm; P=0.380).
Conclusions: The HR change during postural challenge showed signifi cant difference between MSA
patients and healthy controls. Impaired HR responsiveness contributed to OH in MSA. Monitoring HR
during the tilt table test may be a practical and useful method to detect early autonomic dysfunction
in patients with MSA.
8.The Effects of Personality and Attitude on Risky Driving Behavior Among Public van Drivers: Hierarchical Modeling

Background:
Traffic injuries have become a significant public health problem in low- and middle-income countries. Several studies have examined the role of personality and attitude toward traffic safety in predicting driving behaviors in diverse types of drivers. Few studies have investigated risky behavior among public passenger van drivers. This study aims to identify the predictors of self-reported risky driving behavior among public van drivers.MethodA total of 410 public van drivers were interviewed at terminal stations in Bangkok. Hierarchical regression models were applied to determine the effects of demographics, personality traits, and attitude on self-reported risky driving behaviors.
Results:
The results indicated that drivers with a high education level, more working days, and high scores for normlessness and anger were more likely to report risky driving behaviors (p < 0.05).
Conclusion
The personality traits and attitude toward speeding account for aberrant self-reported risky driving behavior in passenger van drivers. This could be another empirical basis for evidence-based road safety interventions in the context of public transport.
9.Investigating the Effects of Job Stress on the Distraction and Risky Driving Behaviors of Food Delivery Motorcycle Riders

Background:
Occupational safety issues related to food delivery riders emerge with evidence of an increase in associated traffic accidents and injuries along with the rapid growth of the online food delivery business. This paper focuses on food delivery riders' job stress and investigates its relationships with both antecedents and risky riding outcomes.MethodSurvey data were collected from 279 Taiwanese food delivery motorcycle riders and analyzed using hierarchical regression analysis.
Results:
The results show that job overload and time pressure positively impact riders' job stress, while self-efficacy slightly reduces job stress. Job stress positively leads to risky driving behavior and distraction. In addition, time pressure can strengthen the impact of job overload on job stress. Riders' risky riding attitude can also strengthen the impacts of job stress on risky riding behaviors and distraction.
Conclusion
This paper advances the literature on online food delivery as well as the occupational safety improvement of food delivery riders. Specifically, this study provides insights into the job stress of food delivery motorcycle riders and the effects of job characteristics and risky behavioral consequences.
