2.Collegiate women’s lacrosse injuries: a 2-year prospective surveillance study

The purpose of this prospective study was to examine the injury characteristics in collegiate women’s lacrosse players. Injury data were collected from the Division I collegiate women’s lacrosse team for a 2-year. The total athlete-hours and athlete-exposures over the 2-year period were 27,621 and 13,437, respectively. The total number of injury was 309 injuries, and the total incidence rate was 11.19 injuries (95% confidence interval, 9.94 — 12.43) per 1000 athlete-hours and 23.00 injuries (20.43 — 25.56) per 1000 athlete-exposures, respectively. The most frequent location of injury was the ankle, followed by the knee and the thigh (20.1%, 14.6%, and 10.7% of all injuries). The proportion of lower extremity injuries accounted for 73.5% of all injuries. The most common types of injury was sprains and muscle cramps/spasms (24.9% and 24.6%, respectively). Overuse, contact (e.g., other players, the cross, and the ball), and non-contact injury was the 3 most common cause of injury (39.1%, 24.9%, and 17.2%, respectively). The primary injury characteristics of collegiate women’s lacrosse players were non-contact ankle sprain. Injury recurrence accounted for 26.1% of all injuries, and the most common period required for the player to return to practice and/or games was between 3 and 7 days. Midfield position player suffered the most number of injuries compared to other position player. An understanding of the injury characteristics of women’s lacrosse players may facilitate the development of the comprehensive strategy for injury prevention which is recommended by the sports medicine community.
3.The Analysis of New Patients in Oriental Medicine Research Center of the Kitasato Institute and its Application to Medical Care

In general hospitals or clinics, medical treatment and instruction in medical therapies and nursing are carried out by the medical staff (doctors, pharmacists and nurses) on hand. It is necessary to understand overall trends in patient illness, in addition to personal information, in order to practice medical care comprehensively. For these reasons we analyzed popular medicines, patient make up, and major disease distributions at our own Kampo clinic institute, for patients admitted since 2001.
34% of our patients were male and 66% were female. The majority of these patients were between 20 to 30 years old or, 50 to 70 years old. There were few patients, either male or female, in their 40s. As for major disease distribution, atopic dermatitis was most common among both males and females. Next in line were cold sensations and endometriosis, for females, while respiratory organ diseases such as bronchial asthma or nasal inflammation, and Alzheimer's disease were most common, for males.
Among major disease types, atopic dermatitis was treated with Oren-gedoku-to (JTDN: Japanese Traditional Drug Name) and Ogi-kenchu-to (JTDN), while diabetes was treated with Hachi-mi-gan (JTDN) and Seishin-renshi-in (JTDN). Cold sensations were treated with Toki-shigyaku-ka-goshuyu-shokyo-to (JTDN) and Toki-shakuyaku-san (JTDN), while hypertension was treated with Cho-to-san and Saiko-ka-ryukotsu-borei-to (JTDN).
The present report contains information useful for diagnosis with Kampo medicines, as well as instruction in the nursing and use of these medications by doctors, pharmacists and nursing staff. This report may be utilized in order to administer appropriate medical care for patients.
4.Reduced Temporal Activation During a Verbal Fluency Task is Associated with Poor Motor Speed in Patients with Major Depressive Disorder

Objective:
Substantial research has revealed cognitive function impairments in patients with major depressive disorder (MDD). However, the relationship between MDD cognitive function impairment and brain activity is yet to be elucidated. This study aimed to reveal this relationship using near-infrared spectroscopy (NIRS) to extensively measure frontotemporal cortex function.
Methods:
We recruited 18 inpatients with MDD and 22 healthy controls. Regional oxygenated hemoglobin changes (oxy-Hb) were measured during a verbal fluency task and its relationship to cognitive function was assessed. Cognitive function was assessed using the Japanese version of the Brief Assessment of Cognition in Schizophrenia.
Results:
Compared to healthy controls, patients with MDD displayed poorer motor speed, attention and speed of information processing, and executive function. In the bilateral prefrontal and temporal surface regions, regional oxy-Hb changes were significantly lower in patients with MDD than in healthy individuals. Moreover, we observed a correlation between reduced activation in the left temporal region and poor motor speed in patients with MDD.
Conclusion
We suggest that reduced activation in the left temporal region in patients with MDD could be a biomarker of poor motor speed. Additionally, NIRS may be useful as a noninvasive, clinical measurement tool for assessing motor speed in these patients.
5.Increased Cooperation with Dentistry by the Palliative Care Team

Recently, the palliative care team (PCT) at our hospital has included dentists. Among a total of 127 cancer patientsand required PCT intervention from 2009 to 2014, 17 patients (13.3%) had oral symptoms. Therefore, the PCT held discussions in order to determine the optimal way to treat each patient. Various symptoms, including oral pain, dry mouth, taste disturbance, furred tongue, excessive amounts of saliva, appetite loss, and trismus were treated by the dentists. As a result, the oral findings improved in all patients, while the oral symptoms improved in 16 of the 17 patients (94%). Thanks to the fact that dentists have joined the PCT, oral symptoms are effectively relieved, and PCT members now have an increased interest in oral cavity complications. Furthermore, conducting thorough examinations of the oral cavity by the PCT not only results in an improved QOL, but it has also increased the interest in the oral cavity on the part of the PCT. Therefore, more effective palliative care is expected to be achieved by promoting increased cooperation with more clinical departments.
6.Subtype of Achalasia and Integrated Relaxation Pressure Measured Using the Starlet High-resolution Manometry System: A Multicenter Study in Japan

Background/Aims:
ManoScan and Sandhill high-resolution manometry (HRM) systems are used worldwide; however, the diagnosis of achalasia on the Starlet HRM system is not fully characterized. Furthermore, the impact of calcium channel blockers and nitrites in treating achalasia has not been investigated using HRM. Management of recurrent cases is a priority issue, although few studies have examined patient characteristics.
Methods:
We conducted a multicenter, large-scale database analysis. First, the diagnosis of treatment-naive achalasia in each HRM system was investigated. Next, patient characteristics were compared between type I-III achalasia, and the impact of patient characteristics, including calcium channel blocker and nitrite use for integrated relaxation pressure (IRP) values, were analyzed. Finally, patient characteristics with recurrent achalasia were elucidated.
Results:
The frequency of type I achalasia with Starlet was significantly higher than that with ManoScan and Sandhill HRM systems. In achalasia, multivariate analysis identified male sex, advanced age, long disease duration, obesity, type I achalasia, and sigmoid type as risk factors related to normal IRP values (< 26 mmHg). Calcium channel blockers and nitrites use had no significant impact on the IRP values, although achalasia symptoms were indicated to be alleviated. In recurrent cases, the IRP value was significantly lower, and advanced age, long disease duration, and sigmoid type were more common than in treatment-naive patients.
Conclusions
We should cautiously interpret the type of achalasia and IRP values in the Starlet HRM system. Symptoms of recurrent cases are related to disease progression rather than IRP values, which should be considered in decision making.
7.Development of Dilated Esophagus, Sigmoid Esophagus, and Esophageal Diverticulum in Patients With Achalasia: Japan Achalasia Multicenter Study

Background/Aims:
Patients with achalasia-related esophageal motility disorders (AEMDs) frequently present with dilated and sigmoid esophagus, anddevelop esophageal diverticulum (ED), although the prevalence and patients characteristics require further elucidation.
Methods:
We conducted a multicenter cohort study of 3707 patients with AEMDs from 14 facilities in Japan. Esophagography on 3682 patients were analyzed.
Results:
Straight (n = 2798), sigmoid (n = 684), and advanced sigmoid esophagus (n = 200) were diagnosed. Multivariate analysis revealed that long disease duration, advanced age, obesity, and type I achalasia correlate positively, whereas severe symptoms and integrated relaxation pressure correlate negatively with development of sigmoid esophagus. In contrast, Grade II dilation (3.5-6.0 cm) was the most common (52.9%), while grade III dilation (≥ 6 cm) was rare (5.0%). We found early onset, male, obesity, and type I achalasia correlated positively, while advanced age correlated negatively with esophageal dilation. Dilated and sigmoid esophagus were found mostly in types I and II achalasia, but typically not found in spastic disorders. The prevalence of ED was low (n = 63, 1.7%), and non-dilated esophagus and advanced age correlated with ED development. Patients with right-sided ED (n = 35) had a long disease duration (P = 0.005) with low integrated relaxation pressure values (P = 0.008) compared with patients with left-sided ED (n = 22). Patients with multiple EDs (n = 6) had lower symptom severity than patients with a single ED (P = 0.022).
Conclusions
The etiologies of dilated esophagus, sigmoid esophagus, and ED are considered multifactorial and different. Early diagnosis and optimal treatment of AEMDs are necessary to prevent these conditions.
