1.Comparison between Changchun'an sustained-release capsules and Nimodipine for the treatment of vascular cognitive impairment

OBJECTIVE: To compare the therapeutic effect of Changchun'an sustained-release capsules and Nimodipine tablets on the treatment of vascular cognitive impairment.METHODS: A total of 120 patients with vascular cognitive impairment were randomly divided into treatment group and control group, with 60 patients for each group. Patients In the treatment group were treated with Changchun'an sustained-release capsules, 30 mg for each administration, twice a day; the other patients in the control group were treated with Nimodipine tablets,30 mg for each administration, 3 times a day. The treatment course was 6 months for both groups. The scores of clinical efficiency,Mini-Mental State Examination (MMSE), and Activity of Daily Living Scale (ADL) were analyzed.RESULTS: The total effective rates of Changchun'an sustained-release capsules and Nimodipine tablets were 60.0% and 53.3%,respectively, and there was significant difference in the total effective rates between treatment and control groups (P < 0.05). The scores of MMSE and ADL in both groups were improved obviously after treatment compared with before treatment (P < 0.01),and there was a significant difference between the two groups after treatment (P < 0.05). The side effects of Changchun'an sustained-release capsules were obviously posterior to those of Nimodipine tablets.CONCLUSION: Changchun'an sustained-release capsules are effective in treating vascular cognitive impairment with few side effects.
2.Angioplasty and stent implantation for treating lower limb atherosclerosis obliterans:report of 34 lesions

30%. For inferior genicular artery lesions,only balloon angioplasty were carried out. All patients were followed up for 6 months and 12 months to assess of restenosis and clinical efficacy. Results All the patients' therapy was successful,their clinical symptoms were improved or disappeared. The 6-month follow-up showed that restenosis rates were 5.5%,16.7% and 25% respectively in the iliac artery,femoral popliteal artery,and inferior genicular artery. The rates in 12-month follow-up were 11.1%,33.3%,and 75% correspondingly. Conclusion Angioplasty and stent implantation are safe and effective in treatment of lower limb atherosclerosis obliterans. Standardized postoperative anti-platelet,anticoagulant,and anti-lipid therapy is essential to maintain blood vessel recanalization.
3.Congenital Cystic Disease of the Lung.

Congenital cyst of the lung is a type of rare pulmonary disease. This lesion was initially introduced by Morgagni in 1769. Thereafter many other cases were reported. Etiology or developemental pathogenesis of this lesion is not certain, but abnormal developement of embryonic lung bud during the embryonal period is highly suspected. This lesion is two times more frequently located at the left lung, but both sides of the lung may be involved. This lesion is largely asymptomatic and found incidentally but can be evoked respiratory symptoms and also can be died due to only this lesion. So accurate diagnosis and treatment in the early period are very important. Therefore, 32 cases of congenital cyst of the lung reported at the department of pathology, seoul national university hospital from 1. 1986 to 12. 1984 were reviewed and classified with clinical, gross and microscopic findings based on the Buntain's classification. Among 32 cases of congenital cyst of the lung, 16 cases were bronchogenic cysts and were largest in number, 8 cases were pulmonary sequestration, 6 cases were congenital cystic adenomatoid malformation (CCAM) and 2 cases were pulmonary emphysema. In cases of bronchogenic cyst, involved age was variable from 7 months to 51 years and large cases were asymptomatic. Grossly, 15 cases were solitary cyst and only one case was multiple. Size was also variable from 3 cm to 11.5 cm. Microscopically, cyst wall was lined by pseudostratified ciliated columnar epithelium with gobet cells, mucous secreting glands, smooth muscle fibers and fibrous interstitial tissue. Three cases were also exhibit cartilage at the wall. In cases of CCAM, there were involved below the 7 months old boys and girls. Four cases were confirmed at the autopsy, other 2 cases were confirmed at the surgical specimens which were found incidentally or due to generalized cyanosis. Microscopic findings were similar in all cases showing relatively well demarcated multiple small cysts with adenomatous proliferation. Lining epithelial cells were tall columnar, pseudostratified ciliated columnar epithelium with occasional mucous secretion. Stroma also exhibit adenomatous proliferation lined by simple columnar or cuboidal epithelium. In cases of pulmonary sequestration, 7 cases were intrapulmonary, other one case was extrapulmonary sequestration. In five cases, systemic large vessel were confirmed. Microscopically it was composed of irregularly dilated bronchiolar structures lined by pseudostratified ciliated columnar epithelium.

6.Spinal Pleomorphic Xanthoastrocytoma.

Pleomorphic xanthoastrocytoma is a rare tumor, presenting superficially over the cerebral hemispheres of young subjects. We report a case of Pleomorphic xanthoastrocytoma of the cervical cord in view of its unusual location. The patient is a 20-year-old female, presenting with left neck pain radiating to the left shoulder one month before the admission. Magnetic resonance imaging revealed a high signal lesion in the upper cervical cord lower brainstem in T2 weighted image. On operation, intramedullary tumor was present and it was relatively well delineated. Near-total removal of the spinal cord mass was done. Microscopically, the tumor was composed of pleomorphic astrocytes, which were spindle-shaped with cytoplasmic processes and hyperchromatic nuclei. Other cells were round with vesicular nuclei and abundant eosinophilic cytoplasm. A few giant cells were seen. Despite the marked pleomorphism, mitosis or necrosis was not encountered. Scattered foamy cells were also seen, which showed negative staining in GFAP immunostaining in contrast to other GFAP-positive tumors cells.

7.Rhein improves insulin sensitivity of diabetic rats by increasing the protein expression levels of PPAR-? and GluT-4 in adipose tissue

Objective To investigate the mechanism for rhein to improve the insulin sensitivity in diabetes mellitus. Methods Male Wistar rats were randomly divided into normal control group (NC,n=15) and diabetic group (DM,n=40) induced by STZ and high-fat diet.Diabetic rats were randomly divided into diabetic control group (DM-C,n=15) and diabetic group treated with rhein (DM-T,n=15).Then DM-T group was given intragastric administration with rhein at the dose of 100 mg/ (kg-1?d-1) for 11 weeks.FBG,fasting serum insulin (FIns),triglyceride (TG),total cholesterol (TC),insulin sensitivity index (ISI) and other biochemical parameters in each group were measured.The protein expressions of PPAR-? and GluT-4 were determined by immunohistochemistry and Western blot. Objective To investigate the expression of MMP-2 in rabbit carotid artery injured by airing drying. Methods Fifty-four rabbits were assigned to normal diet (n=18), fat diet (n=36), 18 of fat diet rabbits underwent airing drying of carotid intima after one-week fat diet. 9.Effect of Air Formaldehyde Exposure of Medical Students on DNA-Protein Crosslinks in Their Buccal Mucosa Cells

Objective To study the effect of formaldehyde exposure on DNA-protein crosslinks (DPC) in buccal mucosa cells in students who were taking anatomy course.Methods The modified SDS-KCl precipitation assay published by Zhitkovich and Costa in 1992 was applied to detect DNA-protein crosslinks in human cells.And this method has been used to explore DPC induced by different pollutants in numerous studies.37 medical students (20 males and 17 females) aged 19.24?1.09 (mean?standard deviation) and 40 students (20 males and 20 females) in natural science college aged 19.55?0.99 (mean?standard deviation) were studied.The frequency of exposure to formaldehyde was 6 h per week in exposed group.Results Concentrations of formaldehyde in anatomy laboratory ranged from 0.42 to 1.57 mg/m3.Exposure to formaldehyde resulted in an increase of DNA-protein crosslinks.The percentage (mean?standard deviation) of DNA-protein crosslinks in exposed and nonexposed students were 25.72%?6.48% and 22.88%?5.34% respectively(P0.05).Conclusion The result of the present study suggests that formaldehyde exposure in medical students increases the frequency of DNA-protein crosslinks in buccal mucosa cells and females may be more sensitive to formaldehyde exposure.
10.The effectiveness of wechat-based transitional care in the acoustic neuroma patients with postoperative facial palsy

Objective To evaluate the effectiveness of wechat-based transitional care in the acoustic neuroma patients with postoperative facial palsy.Methods 90 acoustic neuroma patients with postoperative facial palsy were randomly enrolled into 2 groups by tossing coin,45 cases in each group.Control group received routine discharge education,while the experimental group received wechat-based transitional care for three months.The rehabilitation adherence,the level of facial palsy,the ocular infection and the quality of life at patients discharged and three months after operation were compared between the two groups.Results Three months after operation,the cases of high,middle,low level of rehabilitation in the experimental group were 28,12,5,which were more than the control group whose cases were 15,21,9 (x2=7.528,P< 0.05).Facial palsy and the quality of life of the experimental group were 66.7% (30/45) and (71.62±6.36) points,which was significantly higher than 42.4% (19/45) and (63.75±11.28) points in the control group (x2=5.421 and 4.073,P < 0.01).The incidence of ocular infection in the experimental group was 8.9% (4/45) which was significantly lower than 31.1% (14/45) in the control group (x2=6.671,P <0.05).Conclusions Wechat-based transitional care achieves good effectiveness in patients with postoperative facial palsy,which could improve the level of rehabilitation,facial palsy and the quality of life,and reduce the incidence of ocular infection,is worthy of promotion.
