2.Comparison of the effects of different analgesia methods for artificial abortion

Objective To observe the clinical effect of multimodal analgesia for painless artificial abortion . Methods 150 cases of ASA Ⅰ-Ⅱ pregnancy received painless artificial abortion operation ,no contraindication for all patients,were randomly divided into the five groups ,30 cases in each group,the blank control group (group S):do not use any analgesia measure ,intravenous injection of 0.9% sodium chloride 2mL preoperation;the fentanyl group ( F group ) :intravenous injection of fentanyl 1μg/kg preoperation ;the parecoxib group ( group P ) :intravenous injection of parecoxib sodium 40mg preoperation.The patients were given painless artificial abortion after injection the above drugs.Nerve block group ( N group):the implementation of painless induced abortion operation after sleep in patients with the cervical nerve block ,injection of 1%lidocaine 2 mL.Multimodal analgesia group (group M):intra-venous injection of parecoxib sodium 40mg,fentanyl 1μg/kg,then the implementation method the same as N group . The operation time,anesthesia onset time,recovery time,time of accurate orientation and additional dosage of propo-fol;incidence rate of artificial abortion syndrome ,the incidence rate of uterine contraction pain ,10min,30min,1h,6h pain scores after operation; adverse reaction;satisfaction score of analgesic effect postoperative 24h were observed. Results The additional dosage of propofol in the group S ,group F,group P,group N and group M were (78.45 ± 20.36)mg,(15.55 ±12.33)mg,(16.75 ±13.13)mg,(14.55 ±10.25)mg,(9.35 ±8.27)mg.The incidence rates of uterine contraction pain in the group S ,group F,group P,group N and group M were 30%,10%,10%,10%,0. Compared with group M ,the additional dosage of propofol and incidence rate of uterine contraction pain in the other four groups were significantly increased (t=3.05,1.82,1.84,1.78,χ2 =72.1,68.2,68.2,68.2,all P<0.05). Pain scores after operation 10min,30min,1h,6h in the group S were (6.6 ±1.5)points,(5.6 ±1.4)points,(4.6 ± 1.1)points,(3.6 ±1.4)points.Those in group F were (5.1 ±1.5)points,(4.3 ±1.1)points,(3.1 ±0.9)points, (2.8 ±0.8).Pain scores after operation 10min,30min,1h,6h in the group P were (4.6 ±1.5) points,(4.2 ± 0.9)points,(2.9 ±0.8) points,(2.7 ±0.6) points.Those in the group N were (4.5 ±1.5) points,(4.1 ± 0.8)points,(2.9 ±0.8)points,(2.6 ±0.5).Pain scores after operation 10min,30min,1h,6h in the group M were (2.2 ±0.8)points,(1.9 ±0.7)points,(1.5 ±0.5)points,(1.2 ±0.3)points.Compared with the group M,pain scores after operation 10min,30min,1h,6h in the other four groups were significantly elevated ( t=2.03,1.96,1.86, 1.84,1.98,1.82,1.80,1.76,2.05,1.99,1.95,1.86,2.06,1.88,1.82,1.76,all P<0.05).Compared with S group,postoperative 24h satisfaction score of analgesia in the group M was improved significantly ( t =1.98,P<0.05).Compared with group M,the incidence rate of nausea in the other four groups was increased significantly (χ2 =72.2,68.9,68.1,68.1,all P<0.05).Conclusion Multimodal analgesia method can greatly reduce artificial abor-tion postoperative pain ,reduce the adverse reactions ,improve patients'satisfaction score ,ensure the safety of operation .
3.Preparation of monoclonal antibody against CUEDC2 protein and detection of expression profile of CUEDC2 in mouse tissues

Objective To prepare monoclonal antibody(mAb) against CUE domain containing 2(CUEDC2) protein and detect the expression level of CUEDC2 in mouse tissues.Methods Fusion protein GST-human CUEDC2(hCUEDC2) was expressed and purified,and mAb against CUEDC2 was prepared by using hybridoma technique.The specificity of the antibody and CUEDC2 expression in mouse tissues were detected by Western blot analysis.Results We obtained mAb against CUEDC2 protein,which recognized specifically both hCUEDC2 and mouse CUEDC2.The mAb could be used to detect expression level of CUEDC2 protein,which was different in various mouse tissues.Conclusion The prepared CUEDC2 has good mAb specificity and can be used to detect expression of CUEDC2 protein in mouse tissue,thus providing a useful tool for further investigation into the functions of CUEDC2.
4.Optimization of Recombinant E. coli High-density Fermentation for Expressing Human-like CollagenⅡ

To study and optimize the fermentation parameters for expressing human-like collagenⅡduring E. coli high-density fermentation. The effects of pH, temperature, dissolved oxygen and induction instant on the cell growth and human-like collagenⅡproduction were investigated to optimize the fermentation conditions. The results demonstrated that the following conditions were beneficial for cell growth and foreign gene expression, controlling pH in phase induction at 6.8 and initial pH at 6.5, maintaining fermentation temperature and dissolved oxygen concentration was controlled at 34?C and 20% respectively, and implementing induction at the later logarithmic growth phase. Under the optimized condition, the cell density and human-like collagenⅡyield could reach 88.4 g/L and 14.2 g/L, respectively.
5.Anaerobic Biohydrogen Production Bacteria Selection and Its Optimization of Biohydrogen Fermentation Process

This research adopted silt as the sample,and the five highest hydrogen production performing strains contained in the sample were isolated. The strain whose hydrogen production was the highest was identified as Enterobacter cloacae by the analysis of 16S rDNA sequencing and comparison. It is showed by Plackett-Burman Experimental Design that only glucose,citric buffer and reducing agent had significant effects on hydrogen production by Enterobacter cloacae FML-C1. The path of steepest ascent was undertaken to approach the optimal response region of those three factors. Central Composite Design(CCD) and Response Surface Methodology(RSM) were employed to investigate the interaction of the variables and to ascertain the optimal values of the factors,which finally led to the maximum hydrogen production(VH2) . The theoretical optimal medium conditions were:glucose 21.5 g/L,citric buffer 13.6 mL/L,reducing agent10.0 mL/L. The five tentative tests matched this model well. The final VH2 was up to 2347.4 mL/L,which was 127.42% enhanced in comparison to the original. The result shows that PB experiment design and RSM analytical method work well in selecting factors which have significant influences on the hydrogen production and,moreover,achieve the ideal optimal result.
6.Construction of recombinant yeast expressing PreS2120-146 and hepatitis B surface antigen and evaluation of the immune effects of whole yeast cells

Objective To construct the recombinant yeast expressing PreS2 120-146-hepatitis B surface antigen (HBsAg),and to evaluate the immune effects of whole yeast cells.Methods PreS2 120-146 and HBsAg gene sequence were optimized according to the yeast cell codon preference,and were recombined and cloned into pPIC3.5K yeast expression vector to construct pPIC3.5K/PreS2 120-146 plasmid.After digested and linearized by Bgk Ⅱ restriction enzyme,pPIC3.5K/PreS2 120-146-HBsAg recombinant plasmid was electrotransformed into GS115 strain to screen PreS2 120-146-HBsAg-recombinant Pichiapastoris .The expression of PreS2 120-146-HBsAg was identified by sodium doclecyl sulfate polyacrylamide gel electrophogesis (SDS-PAGE),Western blot and enzyme linked immunosorbent assay (ELISA)analysis. BALB/c mice were vaccinated by inactivated whole recombinant yeast cells expressing target protein. Specific antibodies to HBsAg were detected by ELISA.Cytotoxic T lymphocyte (CTL)response induced by interferon (IFN)-γ was detected by reverse transcription-polymerase chain reaction (RT-PCR)when immune spleen cells of mice were stimulated by CTL epitope on HBsAg.Independent sample t test was used. Results Data of PCR detection,restriction enzyme digestion and sequencing analysis showed that recombinant pPIC3.5K/PreS2 120-146-HBsAg plasmid was successfully constructed.SDS-PAGE,Western blot and ELISA verified the expression of PreS2 120-146-HBsAg in the lysate of the recombinant Pichiapastoris induced by methanol.Levels of specific anti-HBsAg IgG antibodies produced by inactivated yeast cells vaccinated mice were comparable to purified HBsAg immunization (t =0.946,P =0.381 ). Analysis of HBsAg-specific CTL responses revealed that the level of IFN-γwas significantly higher when the immune spleen cells of mice were stimulated by CTL epitope peptides on HBsAg (t =2.305 ,P =0.044).Conclusions PreS2 120-146-HBsAg target protein is successfully expressed by construction of recombinant Pichiapastoris . The specific humoral and cellular immune responses are induced by recombinant whole yeast cells vaccinated mice.
7.Mental health status and internet-surfing behavior among rural adolescents

Objective:To investigate the prevalence and influencing factors of mental health status and internet-surfing behavior among rural adolescents in Sichuan province,and explore the mutual effects between mental health status and internet-surfing behavior.Methods:Totally 2745 junior and senior high school students of grade seven and grade ten from two rural schools were selected.Mental health status,self-esteem and social support of students were assessed with Mental Health Inventory of Middle-school students (MMHI),Rosenberg self-esteem scale (SES) and social support rating scale (SSRS) respectively.Demographic characteristics,internet-surfing behavior were obtained using cross-sectional survey.Non-recursive structural equation model was applied to analyze the effects of other variables on mental health status and internet-surfing behavior and the mutual effects between them.Results:The mean score of MMHIwas (2.1 ±0.7),and the dimensions including academic stress (2.4 ±0.9),emotional instability (2.4 ±0.8) and anxiety (2.4 ± 1.0) got the top three.The total prevalence of long-time internet-surfing was 32.8% (899/2745).The structural equation model showed that female and increasing age had positive effects on score of MMHI (β =0.058,0.058,P < 0.001),and male and increasing age positively influenced internet-surfing behavior (β =-0.171,0.149,P < 0.001).The scores of SES and S SRS were directly negatively related to the score of MMHI (β =-0.300,-0.263,P < 0.001),and indirectly negatively affect internetsurfing behavior through the mediating effect of mental health (βi =-0.074,-0.065,P < 0.010).The score of MMHI had positively effects on long-time internet-surfing behavior (β =0.246,P < 0.001),and long-time internetsurfing behavior had positively effects on the score of MMHI in reverse (β =0.008,P < 0.001),but much weaker.Conclusion:There are mild mental health problem among rural adolescents,and internet-surfing behaviors are prevalent among this population.Poor mental health and long-time internet-surfing behavior are risk factors mutually.
8.Clinical features and risk factors of chronic persistent asthma small airway dysfunction

Objective To analyze the clinical characteristics and risk factors of small airway dysfunction(SAD)in patients with asthma.Methods The clinical data of 200 patients with chronic persistent asthma were included,including general data,disease-related condition,pulmonary function test result,compliance assessment and asthma control status.The clinical features of the two groups were compared.Logistic regression was used to analyze risk factors for asthma SAD,and ROC curves were plotted to assess the predictive power of the model.Results Two hundred patients were divided into the SAD group(128 cases)and the non-SAD group(72 cases).The main risk factors of SAD in patients with chronic persistent asthma included smoking history(OR=4.758,95%CI:2.043-11.081),overweight(OR=2.952,95%CI:1.428-6.105),asthma without clinical remission(OR=6.140,95%CI:2.929-12.870),acute asthma attack in recent 1 year(OR= 3.406,95%CI:1.430-8.117)and allergic rhinitis(OR=2.289,95%CI:1.121-4.673).The area under the curve(AUC)of above risk factors were 0.612,0.610,0.716,0.614 and 0.600,respectively.The AUC of the composite prediction model was 0.826(95%CI:0.769-0.883),which had good prediction value.Conclusion Smoking,overweight,acute asthma attack in recent one year,non-remission period of asthma and allergic rhinitis are independent risk factors for SAD in chronic persistent asthma.The risk factors of SAD should be identified as early as possible,and individualized monitoring and treatment should be taken.

Male primary adenocarcinoma of urethra is rare clinically. A patient with primary adenocarcinoma of posterior urethra complicated with intestinal metaplasia was admitted and underwent urethral mass resection. The tumor recurred 3 months after surgery and no further treatment was given for personal reasons. The purpose of this report is to improve clinicians' understanding of urethral cancer, so that patients can undergo more accurate diagnosis and treatment.
10.Establishing and evaluating a robust method based on LC-MS/MS for simultaneous determination of Aβ1-42,Aβ1-40 and A β1-38 in cerebrospinal fluid

Objective:To establish and validate an LC-MS/MS method for simultaneous determination of Aβ 1-42, Aβ 1-40, and Aβ 1-38 in cerebrospinal fluid. Additionally, the consistency between this method and three mainstream detection methods was evaluated.Methods:This study involved method establishment, validation, and consistency evaluation. The N15 labeled β-amyloid protein was used as the internal standard. Extraction was performed using Waters MCX 96-wells solid phase extraction plate, and the eluent was collected to QuanRecovery MaxPeak 700 μl plate. At the positive ion mode, the multi-reaction ion monitoring mode based on electric spray ionization is chosen for the determination of CSF Aβ 1-42, Aβ 1-40, and Aβ 1-38. Referring to the CLSI C62-A and EP-15A3 guidelines, the method is evaluated and verified, including quantitation of limit (LOQ), linearity, recovery, precision, and accuracy. In addition, a total of 57 clinical residual CSF samples were collected and the concentrations of Aβ 1-42 and Aβ 1-40 were determined based on manual INNOTEST ELISA assay and Lumipulse G and Roche Elecsys fully automated biochemical analyzers. The comparison analysis and deviation evaluation were conducted by passing-bablok and Bland Altman methods.Results:The analysis time of this method is 8 min, and the LOQ of Aβ 1-42, Aβ1-40 and Aβ1-38 is 0.1 ng/ml, 0.5 ng/ml, and 0.1 ng/ml, respectively, and the linear range can meet the needs of clinical detection. Respectively, the recovery is 86.2%-93.8%, 100.9%-103.9% and 103.3%-107.1%; the total imprecision is 4.7%-7.4%, 3.5%-4.6% and 5.2%-10.9%. The measured values of Aβ 1-42 certified reference materials are all within the allowable uncertainty requirements. Moreover, the carryover rate of three analytes was all≤0.11%. In addition, the correlations of Aβ 1-42 and Aβ1-40 in CSF between this LC-MS/MS method and the INNOTEST ELISA method, Lumipulse G and Roche Elecsys fully automated biochemical analyzers were all deemed good, with correlation coefficient (r) ranging from 0.920 to 0.970. However, the measured values between the four methods were remarkably different.Conclusion:We established and validated a robust method based on LC-MS/MS technology for simultaneous determination of Aβ 1-42, Aβ 1-40, and Aβ 1-38 in CSF. The method is accurate, simple, and suitable for clinical measurements. However, despite good correlations, there were substantial differences in the measurement results of Aβ 1-42 and Aβ 1-40 among different analytical platforms, indicating the need for further promotion of harmonization and standardization processes for AD classic biomarkers.
