1.Correlation study between hope and depression with the elderly in a welfare institution in Henan Xingyang city

Objective To investigate the status of the level of hope and depression with the elderly in a welfare institution in Henan Xingyang city,and to explore the relationship between them. Methods A total number of 200 elderly people were recruited conveniently from a welfare institution in Henan Xingyang city. They were investigated by General Material Questionnaire, Herth Hope Scale and Geriatric Depression Scale. Results The total score of hope and depression with the elderly in a welfare institution were (21.28±3.07) , (13.15±2.77) points respectively, the depression detection rate was 82.38%(159/193);the level of hope was negatively related with depression (r=-0.447,P<0.01). Conclusions The hope of the elderly in welfare institution is not optimistic,and the depression detection rate is high,the level of hope is positively related with their depression,so we should take effective measures to improve their mental health according to the emotional state to give emotional care and improve the level of their hope.
2.Walking ability and cognitive function changes in normal pressure hydrocephalus patients after cerebrospinal fluid tap test

Objective To explore the walking ability and cognitive function changes in normal pressure hydrocephalus patients after cerebrospinal fluid ( CSF ) tap test for helping clinicians choose evaluation time and methods.Methods Twenty-seven patients with probable normal pressure hydrocephalus in Peking Union Medical College Hospital from 2013 to 2014 were included.All patients were evaluated using Minimum Mental State Examination, the Montreal Cognitive Assessment, Ability of Daily Life, and Idiopathic Normal Pressure Grade Scale, underwent 1.5 T head MRI scan and had ventriculo-peritoneal shunt after informerd consent.A lumbar tap with removal of 30 ml of CSF was performed in all patients.Evaluations included the 10 m walking time and steps, Trail Making Test A, number code and Stroop test.Those tests were performed 1 day before and 4, 8, 24, 72 hours after CSF tap test.The walking test and neuropsychological test results were compared between those before and after the CSF tap test.Correlation analysis was conducted between the normal pressure hydrocephalus featured MRI characters and CSF tap test responses including Evan′s index, callosum corpus angle, mismatch between narrowed high-convexity and medial subarachnoid spaces and enlarged Sylvian fissure associated with ventriculomegaly . Results Compared with 0 h walking time (23.56(14.00) s), the 10 m walking time on the 8 hours and 24 hours after CSF tap test, which were 19.41 ( 9.00 ) s and 19.67 ( 11.00 ) s respectively, were significantly improved ( Z values in Wilcoxon signed ranks test were -3.416 and -3.443 respectively,both P<0.01).There were no statistically significant differences on every evaluation time point.The neuropsychological tests changings were significant on 24 hours and 72 hours.Compared with 0 h neuropsychological test z scale (-10.28(21.60)), the z scale on the 24 hours and 72 hours after CSF tap test, which were -6.29 (26.72), -3.37(36.15)respectively, were significantly improved (Z values in Wilcoxon signed ranks test were -3.506,-2.701 respectively, both P<0.01).The Evan′s index, callosum corpus and the feature of mismatch between narrowed high-convexity and medial subarachnoid spaces and enlarged Sylvian fissure were not statistically correlated with the response of CSF tap test.Conclusions Walking ability in normal pressure hydrocephalus patients was improved after the CSF tap test.The Evan′s index, callosum corpus and the feature of mismatch between narrowed high-convexity and medial subarachnoid spaces and enlarged Sylvian fissure might not be correlated with the response of CSF tap test.
3.Continuous low-dose cyclophosphamide and prednisone treatment of 84 cases relapsed and refractory multiple myeloma

Objective To evaluate the efficacy andtolerability of continuous low-dose cyclophosphamide and prednisone treatment of relapsed and refractory multiple myeloma. Methods84 relapsed and refractory multiple myeloma patients were enrolled, including 46 males and 38 females, the assess patients of 81 cases with average age of 69.7 (45-91)years. They were treated continuous with oral cyclophosphamide (50 mg/d) and prednisone (15 mg/d) and monthly follow-up.ResultsAverage follow-up time were 23.5(1-71)months.The assessed patients were 81 cases,with 52 cases (64.2 %) responded.There were 2 cases(2.5 %)CR,21 cases(25.9 %) of PR,29 cases(35.8 %)MR,19 cases(23.5 %)NC and 10 cases (12.3 %)PD.The median time to response was 2 months.In the patients who responded to the treatment,the median progression-free survival(PFS)and overall survival (OS) were 18(95 %CI 12.8-23.2),29(95 %CI 24.1-33.9)months.In the non-responding patients,the PFS and OS were 4(95 % CI 2.2-5.8) and 6(95 % CI 4.9-7.1)months.Two groups were statistically significant(P<0.05).The most common toxicities included fatigue,nausea, neutropenia, hyperglycemia and lung infection. No patient withdrew from the study because of toxicity. Conclusions Continuous low-dose cyclophosphamide combined prednisone is a treatment options for relapsed and refractory MM patients.
4.Preimplantation genetic diagnosis for carriers of thalassemia and chromosomal abnormality.

OBJECTIVETo provide preimplantation genetic diagnosis(PGD) for two couples carrying thalassemia mutations and chromosomal abnormalities.
METHODSCouple 1 were both carriers of β 41/42 thalassemia mutations, while the husband has carried a reciprocal translocation with a karyotype of 46,XY,inv(9)(p11;q13),t(11;22)(q25;q13). Couple 2 were both carriers of α (-SEA) thalassemia mutation. Their chromosome karyotypes were both normal, but had two spontaneous abortions. The couples had received 1 and 3 blastocysts respectively through in vitro fertilization(IVF) cycles. Following the biopsy, the cells underwent whole genome amplification, and the amplified DNA from each embryo was subjected to genetic testing and a 23-chromosome single nucleotide polymorphism(SNP) microarray assay.
RESULTSThe embryo of couple 1 was diagnosed as carrier of β 41/42 thalassemia with euploid chromosomes. The embryo was transferred and resulted in intrauterine pregnancy. Similarly, an embryo of couple 2 was verified as carrier of α (-SEA) thalassemia with euploid chromosomes.
CONCLUSIONPGD for aneuploidy coupled with testing for single gene disorders via trophectoderm biopsy and whole genome amplification is feasible. The approach can attain diagnosis with minimal damage with sound clinical outcome.

5.Study on Improvement Effects and Mechanism of Astragalus complanatus Total Flavonoids on Reproductive Dysfunction Model Male Rats

OBJECTIVE: To stu dy the improvement effects of Astragalus complanatus total flavonoids (TFA) on adenine-induced reproductive dysfunction model male rats and its mechanism. METHODS :Male SD rats were divided into blank group,model group ,positive group (Wuzhi yanzong pill ,2 g/kg),TFA high-dose,medium-dose and low-dose groups (0.05, 0.1,0.2 g/kg),with 12 rats in each group. Male rats in each group were given the corresponding drugs intragastrically at 9:00 a.m. every day ,while the model of reproductive dysfunction was reproduced by intragastric administration of adenine suspension (except for blank group )at 3:00 p.m. on the same day ,for consecutive 35 days. After medication ,male rats in each group were mated with female rats by ratio of 1∶1 in cages ;7 days later ,inducing-pregnancy rate of male rats were measured. ELISA assay was used to detect the serum levels of T ,E2,FSH,LH,GSH-Px,SOD,MDA,Scr and BUN in male rats ;the testis , epididymis,seminal vesicle ,thymus and kidney of male rats were extracted and the organ index was calculated ;HE staining was used to observe the pathological changes of testis ,epididymis and kidney ;the protein expression of Bcl- 2,Bax,Caspase-3 and Caspase-9 in the renal tissue of male rats were detected by immunohistochemistry. RESULTS :Compared with blank group ,the inducing-pregnancy rate of male rats ,the testis ,epididymis,seminal vesicle gland ,thymus indexes ,the serum levels of T ,E2, SOD and GSH-Px ,the protein expression levels of Bcl- 2 in renal tissue were reduced significantly in model group (P<0.01);the renal index ,the serum levels of FSH ,LH,MDA,Scr and BUN ,the protein expression levels of Bax ,Caspase-3 and Caspase- 9 in renal tissue were increased significantly (P<0.01);the diameter of seminiferous tubules in testis became thinner ,the spacing was wider ,and the arrangement of seminiferous cells was disordered ;the wall of epididymis was thickened ,the arrangement was loose,the stroma was enlarged ,the lumen was proliferated and atrophied ,and the sperm in the lumen wass aggregated ;in renal tissue,interstitial fibers was proliferated ,and a large number of yellow crystals were seen in renal tubules. Compared with model group,the inducing-pregnancy rate,the testis ,epididymis,seminal vesicle gland ,thymus indexes ,the serum levels of T ,E2, SOD and GSH-Px ,the protein expression levels of Bcl- 2 in renal tissue of male rats were increased significantly in the positive group,TFA medium-dose and high-dose groups (P<0.05 or P<0.01),while the renal index ,the serum levels of FSH ,LH, MDA,Scr and BUN ,the protein expression levels of Bax ,Caspase-3 and Caspase- 9 in renal tissue were decreased significantly (P<0.05 or P<0.01);there was the same chage trend and most with significant difference in above indexes of TFA low-dose group (P<0.05 or P<0.01);the pathalogic changes of testis ,epididymis and renal tissue were improved significantly. CONCLUSIONS:TFA can significantly improve adenine-induced reproductive dysfunction model male rats. The mechanism may be associated with reducing hormone FSH and LH level ,the expression of apoptosis-related proteins in renal tissue ,while increasing the level of hormone T and E 2 and improving oxidant stress level.
6.Assessment of cheese sign and its association with vascular risk factors: Data from PUMCH dementia cohort

Background::In the clinic, practitioners encounter many patients with an abnormal pattern of dense punctate magnetic resonance imaging (MRI) signal in the basal ganglia, a phenomenon known as "cheese sign". This sign is reported as common in cerebrovascular diseases, dementia, and old age. Recently, cheese sign has been speculated to consist of dense perivascular space (PVS). This study aimed to assess the lesion types of cheese sign and analyze the correlation between this sign and vascular disease risk factors.Methods::A total of 812 patients from Peking Union Medical College Hospital (PUMCH) dementia cohort were enrolled. We analyzed the relationship between cheese sign and vascular risk. For assessing cheese sign and defining its degree, the abnormal punctate signals were classified into basal ganglia hyperintensity (BGH), PVS, lacunae/infarctions and microbleeds, and counted separately. Each type of lesion was rated on a four-level scale, and then the sum was calculated; this total was defined as the cheese sign score. Fazekas and Age-Related White Matter Changes (ARWMC) scores were used to evaluate the paraventricular, deep, and subcortical gray/white matter hyperintensities.Results::A total of 118 patients (14.5%) in this dementia cohort were found to have cheese sign. Age (odds ratio [OR]: 1.090, 95% confidence interval [CI]: 1.064-1.120, P <0.001), hypertension (OR: 1.828, 95% CI: 1.123-2.983, P = 0.014), and stroke (OR: 1.901, 95% CI: 1.092-3.259, P = 0.025) were risk factors for cheese sign. There was no significant relationship between diabetes, hyperlipidemia, and cheese sign. The main components of cheese sign were BGH, PVS, and lacunae/infarction. The proportion of PVS increased with cheese sign severity. Conclusions::The risk factors for cheese sign were hypertension, age, and stroke. Cheese sign consists of BGH, PVS, and lacunae/infarction.
7.Study on Suitability Zoning of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province Based on MaxEnt and ArcGIS

Objective To grasp the main environmental factors affecting the growth of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao;To predict the distribution of suitable areas of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province;To provide references for the rational distribution of the resources in Shanxi Province.Methods This study utilized the sample point longitude and latitude information collected in the"Fourth Survey of Chinese Materia Medica Resources"database in Shanxi Province.The data were supplemented by searching the China Digital Herbarium and retrieving related literature records.347 sample points distribution data and environmental factors were added to the MaxEnt model.The main environmental factors and contribution rates affecting the geographical distribution of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao were screened out.The ArcGIS software was used to divide the ecological suitable area of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province.Results The area under the ROC curve of the established MaxEnt model was 0.909,indicating that the model prediction results were accurate.The model screened 19 environmental factors.Among them,climate factor was the most important environmental factor,followed by biological factor and topographic factor,and soil factor had the least influence.The potential suitable areas of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province were mainly distributed in the northern mountainous areas,presenting a trend of gradually decreasing suitability levels from north to south.Under the current climate conditions,the most suitable area for Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province was 15 424 km2,the suitable area was 19 856 km2,the sub suitable area was 59 436 km2,and the unsuitable area was 61 894 km2.Conclusion Based on MaxEnt model and ArcGIS software,this study predicts the distribution of suitable areas of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao in Shanxi Province,which has certain reference value for the protection and rational distribution of Astragalus membranaceus(Fisch.)Bge.var.mongholicus(Bge.)Hsiao resources in Shanxi Province.
8.Efficacy of continuous low-dose cyclophosphamide and prednisone regimen in refractory multiple myeloma patients with severe heart failure.

OBJECTIVETo observe treatment response, survival, safety and the improvement of ECOG in patients with refractory multiple myeloma (MM) with serious heart failure after the administration of continuous low-dose of cyclophosphamide combined with prednisone (CP).
METHODSFrom January 2005 to September 2013, a total of 75 patients were treated by metronomic chemotherapy with continuous low-dose cyclophosphamide (50 mg/d) and prednisone (15 mg/d).
RESULTSAmong the 75 patients, 2 were lost for follow-up. In the 73 available patients, the overall response was 64.4%, including 2 patients (2.7%) with complete remission (CR), 4 cases (5.5%) very good partial remission (VGPR), and 24 patients (32.9%) partial remission (PR). The median survival was 12 months (1-70 months) with a median onset time of 90 days (16-120) and a median progressive freedom survival of 12 months (1-60). The level of B-type natriuretic peptide in responders declined significantly, as compared to no responders [(336.6 ± 30.3) ng/L vs (906.4 ± 104.8) ng/L, P<0.01]. Common adverse events were as follows: 32 (43.8%) cases of bone marrow suppression, 26 (35.6%) cases of infection, 8 cases of dizzy as well as sleepiness (11.0%), 7(9.6%) cases of Cushing syndrome, 4 (5.5%) cases of secondary diabetes mellitus, and 3 (4.1%) cases of edema respectively.
CONCLUSIONThe metronomic chemotherapy of cyclophosphamide combined with prednisone had satisfactory impact on the treatment outcome, including the improvement of cardiac functions and physical capacities, better survival and safety in refractory MM with serious heart failure. It provides a novel regimen for such patients.

9.Prokaryotic expression, purification and identification of recombinant human atrial natriuretic peptide.

In order to improve the expression of recombinant human atrial natriuretic peptide (ANP), a new plasmid (pET28a(+)/ANP₃) containing 3 tandem ANP genes with lysine codon as the interval linker, was constructed. Target gene was transformed into Escherichia coli BL21 (DE3) and induced by IPTG, about 60% of the total-cell-protein was the target protein, His₆-ANP₃. After denaturation and refolding, it was digested by Endoproteinase Lys-C and Carboxypeptidase B (CPB) and then purified by a series of purification processes, about 16 mg purified ANP monomer could be obtained from one liter bacteria broth of shaking culture. Ultimately, the purity of protein was above 90% determined by UPLC and Tricine SDS-PAGE, its molecular weight was 3 080 Da according to LC-MS identification and it was proved to be equivalent to the reference product by ELISA. The use of tandem gene expression can provide a new possible model for the expression of other peptide drugs.
