1.Partial Nephrectomy of Renal Tumor with Three-dimensional Laparoscopy: Report of 11 Cases

Objective To analyze the effect of partial nephrectomy with three-dimensional laparoscopy for the treatment of renal tumor.Methods From October 2013 to September 2015, 11 patients underwent 3D laparoscopic partial nephrectomy for renal tumor in our hospital.All the cases were given retroperitoneal approach.The renal artery was seperated and blocked, and the renal tumor was exposed and completely resected around the edge.The wound was double layer sutured with Vicryl.Results All the 11 operations were finished smoothly without conversion to open surgery.The operative time was 70-180 min (mean, 100 min), the ischemic time was 14-35 min (mean, 25 min), and the estimated blood loss was 50-300 ml (mean, 90 ml).There were 1 case of intraoperative peritoneal perforation, 2 cases of subcutaneous emphysema, and 1 case of perirenal hematoma.No other operative or postoperative complications occurred.The post-operative hospital stay was 5-10 d (mean, 6 d).Post-operative follow-ups for 3-14 months (mean, 10 months) in 10 cases found no recurrence or metastasis or renal dysfunction.Conclusions Partial nephrectomy of renal tumor with three-dimensional laparoscopy is safe and feasible.The outcome is satisfying and worthy of application.
2.Expression of CD40-CD40L during rat myocardial ischemia-reperfusion injury

AIM: To investigate the expression of CD40 and CD40 ligand(CD40L) during myocardial ischemia/reperfusion injury in rats. METHODS: In the rat modal with myocardial ischemia/reperfusion(MI/R),the changes of CD40 and CD40L expression in blood and myocardial tissues were detected by flow cytometry and immunohistochemistry, respectively. There were seven animal groups in the study, including the normal group ( n= 3), the ischemia 30 min group(I 30 min , n= 6),the ischemia/reperfusion 1 min,5 min,10 min,20 min and 30 min groups(each group, n= 6). 3.Balloon embolization therapy for traumatic carotid cavernous fistula

Objective To analyze effect of balloon embolization therapy for traumatic carotid cavernous fistula (TCCF),so as to provide references for treatment of this disease.Methods A total of 21 patients with TCCF managed with balloon embolization from April 2006 to March 2012 were collected in the study.There were 17 males aged (32.3 ±5.4) years and 4 females aged (40.6 ±5.8)years.Perioperative clinical presentations and imageologic changes were observed as well as operative outcome was evaluated.Results After 3 months of follow-up,20 patients experienced successful detachable balloon embolization,but the other one patient had to have stent angioplasty due to the failure of the procedure.All patients had some improvements on clinical presentations after operation (P < 0.05).Imageologic examination revealed that all patients had complete occlusion of TCCF following operation.In follow-up,one patient had a relapse,with total curative ratio of 95%.Conclusions Detachable balloon embolization is effective in treatment of TCCF.Presently,the technique is an optimal method for such kind of disease and is worthy of wide clinical use.
4.Overexpressed Gene YA61 Cloned from Human Gastric CarcinomaCell SGC-7901 and Its Sequencing

To clone overexpressed gene from human gastric carcinoma cell SGC-7901, DDRT-PCR technique is used with human gastric epithelial cell GES-1 as control. After cloned into pGEM -T vector, YA61, one of the overexpressed genes, was analyzed by dot blot and was sequenced then. The sequence gotten was then compared to GenBank data and analyzed by NCBI ORF Finder. Dot blot results showed that the gene YA61 was overexpressed in human gastric carcinoma cell SGC-7901. NCBI's sequence similarity search indicated that the gene YA61 was a new gene sequence. Open reading frame analysis demonstrated that the gene YA61 had one complete open reading frame. In conclusion, the gene YA61 was a new gene sequence that was overexpressed in human gastric carcinoma cell SGC-7901.
5.Effects of S-allylcysteine on Nitric Oxide Production and Antioxidant Enzyme Activities in Hyperlipidemic Model Rats

Objective To investigate the effects of S-allylcysteine ( SAC ) , on nitric oxide ( NO ) production and antioxidant enzyme activities in hyperlipidemic rats. Methods Male Wistar rats were randomly divided into seven groups. Five groups including normal control group ( normal diet) , model control group ( high-fat diet, HFD) and SAC low,medium,high treated group (high-fat diet +25,50,100 mg·kg-1 SAC) were sacrificed after 4 weeks dosing,while the other two groups including L-arginine group (normal diet+ 20 mg·kg-1 L-arginine) and SAC+L-arginine group (50 mg·kg-1 SAC+20 mg·kg-1 L-arginine) were sacrificed at 4 h after dosing. The serum, livers and kidneys were collected. The levels of NO, the activities of nitric oxide synthase ( NOS ) , antioxidant enzymes in vivo and L-arginine contents in serum were determined. Results Comparing with model control group, the activities of total NOS in serum and liver were significantly reduced in SAC-treated groups (P<0. 05). The level of L-arginine in SAC-treated groups was (8. 25 ± 1. 15), (7. 76 ± 1. 24) and (7. 22 ± 1. 64)μg·mL-1 , respectively. Compared with model control group, the level of L-arginine were significantly reduced in SAC-treated groups (P<0. 05). Comparing with L-arginine group, the activities of total NOS (T-NOS) and iNOS were reduced in SAC+L-arginine group. SAC treatment (100 mg·kg-1) significantly increased the activities of superoxide dismutase (SOD) (P<0. 01) and the level of glutathione (GSH) (P<0.01), and decreased the level of malondialdehyde (MDA) in serum and liver of hyperlipidemic rats. Conclusion These data suggest that SAC inhibits the NO production by reducing iNOS activity, arginine concentration and exhibited antioxidant activity, which may play a pharmacologically important role in protection from oxidative injury and pathogenesis of atherosclerosis.
6.Clinical research on the application of VIABAHN stent in the treatment of branching area lesions in lower extremity arterial diseases

Objective To explore the security and the short term efficacy of VIABAHN stents in the treatment of branching area lesions in lower extremity arterial diseases. Methods The data of 16 patients (11 male and 5 female, aged 59.0 to 81.0 with median of 71.6) with lower extremity arterial occlusive disease from November 2014 to June 2015 were analyzed retrospectively. All lesions were located around the branching area of the lower extremity artery (3 cases in the internal iliac artery orifice, 6 cases in the deep femoral artery orifice and 7 cases in the vascular net around the knee). The short?term effects on these patients in the follow-up period, such as the success rate of surgical technique, the improvement of symptoms and the incidence of complications were summarized. Results Revascularization was technically successful in all 16 patients, and ischemic symptoms relieved significantly after the operation. The ABI were 0.36±0.12 before and 0.89±0.10 after the operation. The patients were followed up for 3 to 9 months (median 5.5 months). All the patients were achieved limb salvage and no complication occurred. Conclusions VIABAHN stent is safe and effective for the treatment of lower extremity arterial occlusive disease. The branches around the lesion can be covered with VIABAHN stents.
7.Expression and characterization of fusion pro-tein hTNFα -hPgnK5 containing thrombin cleav-age site in E. coli

Aim To express the fusion protein hTNFα -hPgnK5 in E.Coli and to identify its activity. Methods The hTNFα -hPgnK5 gene express vector was constructed and the fusion protein was expressed in E.coli. The activity of fusion protein to inhibit the proliferation of tumor cells or angioendothelial cells in vitro was determined by MTT. Results Fusion protein hTNFα -hPgnK5 possessed immunoactivity in Western blot analysis and could inhibit proliferation of tumor cells or angioendothelial cells in vitro test. Conclusion The hTNFα gene and hPgnk5 gene can be coexpressed in prokaryotic system and expression products can inhibit the proliferation of tumor cells or angioendothelial cells in vitro.
8.Clinical study of mechanical thrombectomy in treating in-stent restenosis of lower extremity arteriosclerosis obliterans

Objective To evaluate the safety and efficacy of mechanical thrombectomy in treating in-stent restenosis of lower extremity arteriosclerosis obliterans. Methods From May 2015 to Sep 2016, the clinical data of 9 cases of lower extremity arteriosclerosis obliterans who were with in-stent restenosis(3 were stent graft)were retrospectively collected, and of which 7 were males and 2 were females with a mean age of (75.4 ± 6.3)years old. All the cases were treated by mechanical thrombectomy of Rotarex catheter. All the patients were diagnosed via low-extremity artery CTA, and treated by means of the Rotarex catheter, combined with angioplasty and stent if necessary. All patients received antiplatelet therapy. Doppler ultrasonography was taken during the followed-up. Results All the 9 cases were successed in technology, without complications in hospital. All patients received Rotarex mechanical thrombectomy. Six of which used balloon and 1 stent implanted. The ABI increased from 0.29 ± 0.07 to 0.88 ± 0.07 after treatment, the difference was statistically significant(t=28.875,P<0.05). All the patients were followed up for 3—18 (median time, 11.5)months. No death and symptoms recurrence appeared during the follow-up. Conclusions Mechanical thrombectomy using Rotarex catheter is a safe and effective treatment for in-stent restenosis in lower extremity arterial diseases.
9.Research advances in acute-on-chronic liver failure

Acute-on-chronic liver failure (ACLF) is a severe liver disease with high mortality in China. Early diagnosis could reduce complications and improve the survival rate. In the present review, the definition, etiology, and pathogenesis related to inflammatory response, reactive oxygen species, and metabolism are reviewed. Also, the new approaches to the treatment of ACLF and prognostic factors are summarized. Generally, the development of ACLF is considered to be life-threatening for patients. Therefore, a universal definition of ACLF should be proposed, which would provide good guidance and standard for the clinical diagnosis and treatment of ACLF in China. In addition, the risk factors for ACLF in patients with chronic liver diseases need to be determined by prospective studies.
10.Thinking of current situation of animal model of tradition Chinese medicine combining disease and syndrome.

Combination disease and syndrome to construct animal model must be accord with the diagnosis and treatment mode of tradition Chinese medicine (TCM). The construct ion method should abide by the mode of combination prescription, syndrome, disease and efficacy. Prescription refers to those classic and effective formula. Syndrome refers to the symptoms and signs of prescription. Disease refers to model medical disease. Efficacy refers to the results of treatment. Constructing animal model should choose the classical prescription and formulate the clinical signs and symptoms. According to the main pathological process of modem diseases related to the syndrome to mine the methods of constructing animal models to express the main signs and symptoms, at last to testify the efficacy through the corresponding prescription. This method of constructing animal model is not only accord to the mode of diagnosis and treatment of TCM, but also affords a certain example of constructing animal model of combination model disease and syndrome.
