1.CT and MRI Manifestations of Hepatic Tuberculosis:A Report of 2 Cases and Literature Review

Objective To discuss CT and MRI manifestations and the differential diagnosis of hepatic tuberculosis.Methods The imaging manifestations of hepatic tuberculosis proved by pathology in 2 cases were analyzed retrospectively with literatures review.The plain and contrast-enhanced CT scans were performed in 2 cases and 1 case underwent plain MRI.Results On plain CT scan,the lesion was non-homogeneous low density with even more low density in the center in 1 case,and nearly isodensity in another case.On contrast-enhanced CT scan,the lesions were no enhancement in arterial phases,and slight enhancement in venous phase and balance phase.In 1 case,slight enhancement was also shown surrounding the necrotic area.On MR plain scan,the lesion demonstrated low signal intensity on T_1WI,iso-and high mixed signal intensity on T_2WI.Conclusion Hepatic tuberculosis is rare seen,so it is easy to misdiagnosis.CT and MRI scanning can demonstrate some features of hepatic tuberculosis,especially dynamic enhanced scanning is very helpful in diagnosis and differential diagnosis.
2.CT and MRI Manifestations of Prostate Sarcoma:A Report of 4 Cases and Review of the Literature

Objective To explore the CT and MR imaging features and differential diagnosis of the prostate sarcoma.Methods CT and MRI findings of prostate sarcoma in 4 patients confirmed by pathology were retrospectively analyzed with literature review.Results All cases underwent plain CT scan and one case underwent contrast-enhanced CT scan,MR imaging was performed in 2 cases.The prostates were increased obviously in volume,the average anterior-posterior diameter was 60.5 mm in all cases,lobulated heterogeneous low-density with lowest-density in the center and the tumors were undefinitive with surrouding structures in 3 cases,1 case showed oval homogeneous low-density with sharp margin on plain CT scan.On contrast-enhanced CT,the lesions appeared as heterogeneous enhancement in periphery of prostates and no enhanced at the centre.MRI showed heterogeneous hypo-signal intensity on T1WI and miscellaneous slight high-signal intensity at center and hyper-signal intensity at periphery on T2WI.Conclusion CT and MRI are the effective methods in diagnosing the prostatic sarcoma.
3.Urachal carcinoma:clinicopathologic features and surgical outcomes

Objective To discuss the clinical and pathological features of urachal carcinoma.Methods The clinical and pathological data of 7 patients diagnosed as urachal carcinoma were retrospectively analyzed,and the cIinicopathologic features,diagnosis and treatment,surgical characteristics and surgical outcomes were reviewed.There were 6 males and 1 female.Patient's age ranged from 26-75 years,with average of 52 years.Examinations before surgery included ultrasound,cystoscopy,urine cytology,CT and IVU.Six patients underwent extensive partial cystectomy and 1 patient underwent conventional partial cystectomy. Results Pathological diagnosis were 5 cases of mucinous adenocarcinoma,1 case of not classified adenocarcinoma,1 case of small cell neuroendocrine carcinoma.Clinical stages according to Sheldon staging system were 6 cases of stage ⅢA and 1 case of ⅢC.One patient died of bone metastasis 3 months after operation,1 patient experienced recurrence in bladder neck and urethra in 15 months and 24 months after operation and received TUR-Bt,the other 5 patients were alive without recurrence and metastasis with follow-up of 2-30 months. Conclusion Urachal carcinoma is a rare malignancy,and patients with this disease haye a poor prognosis.
4.Acellular Organismic Scaffolds for Small-diameter Vascular Tissue Engineering

Objective To prepare the acellular carotid artery of adult pig using biotic enzymes for small-diameter vascular tissue-engineering scaffold.Methods Carotid artery of adult pig was removed cells by using Trypsin/EDTA,ribonuclease and desoxyribonuclease.Residual cellular composition was studied with quantitative DNA analysis and the Hematoxylin and Eosin(H&E)staining.Extracellular matrix composition was evaluated with elastin van Gieson's(VG)staining and scanning electron microscopy(SEM).Results Quantitative DNA analysis and H&E staining confirmed that carotid arteries were completely decellularized.Elastin van Gieson's staining demonstrated that elastin morphology appeared unchanged.Scanning electron microscopy examination of the acellular scaffolds revealed a well-oriented porous decellularized structure that maintained natural architecture of the aorta.Conclusion Carotid artery of adult pig rendered acellular with Trypsin/EDTA,ribonuclease and desoxyribonuclease has well-preserved extracellular matrix for vascular scaffold,which can be used as a scaffold for further small-diameter vascular tissue engineering.
5.Multiple analysis of the relationship between correlation of gene function and expression by GeneHub software.

GeneHub software developed by us can be applied to evaluate the significance of expression similarity of genes in each functional unit of different gene function classification systems, and thus to select experiment condition related gene functional units. With different expression similarity measurement and different experiment data, we have applied GeneHub to analyze the expression correlation of functional related genes classified respectively with gene chromosome location, celluar location and interaction, metabolic pathway and signal transduction pathway relationship of protein products of genes. Genes classified according to these multiple ways all showed significant experimental condition related co-expression in our analysis. Our results provide further evidence for the assumption that functional related genes express similarly, which is widely adopted in gene expression profile analysis.

6.Effects of problem-based learning applied in medical imaging education in China:a Meta-analysis

Objective To evaluate the effects of problem-based learning (PBL) teaching model in medical imaging education in China. Methods Such databases as PubMed, Medline, CNKI, WanFang, VIP Data were electronically searched for literature on PBL versus lecture-based learning (LBL) applied in medical imaging education in China up to April, 2015. According to the strict quality evaluation of the in-cluded studies, meta-analysis was performed using RevMan 5.2 software. Results Fourteen studies were included totally. Studies included 1 233 students, of whom the PBL group had 608 cases, while LBL group had 625 cases. Compared with LBL, PBL was superior in medical imaging theoretical scores [WMD=5.22, 95%CI(3.06, 7.37), P=0.000], and the case analysis scores [WMD=6.45, 95%CI(4.77, 8.12), P=0.000]. PBL was also superior in the autonomous learning ability [RR=1.78, 95%CI (1.47, 2.16), P=0.000], the unity cooperation ability [RR=1.42,95%CI (1.25, 1.61), P=0.000] and analysis ability [RR=1.73,95%CI (1.42, 2.11), P=0.000]. There were significant differences between PBL group and LBL group. Conclusion PBL can improve teaching results in medical imaging education.
7.Efficacy of continuous infusion of lidocaine through urinary catheter for postoperative analgesia in patients undergoing urological surgery

Objective:To evaluate the efficacy of continuous infusion of lidocaine via urinary catheter for postoperative analgesia in patients undergoing urological surgery.Methods:Forty male American Society of Anesthesiologists physical status Ⅱ or Ⅲ patients, aged 65-75 yr, with body mass index of 18-25 kg/m 2, scheduled for elective percutaneous nephrolithotomy, were divided into 2 groups ( n=20 each) using a random number table method: continuous infusion of lidocaine through urinary catheter group (group L) and patient controlled intravenous analgesia (PCIA) group (group PCIA). All the patients underwent total intravenous anesthesia, and a matched type of sterile urethral irrigation catheter was inserted after the operation.In group L, 0.5% lidocaine was continuously infused at a rate of 5 ml/h via the urinary catheter, while the equal volume of 0.9% normal saline was continuously infused via the urinary catheter, and PCIA was connected in group PCIA.PCIA solution contained sufentanil 125 μg (diluted to 250 ml in normal saline), and the PCA pump was set up with a 5 ml bolus dose, a 15 min lockout interval and background infusion at a rate of 5 ml/h.When visual analogue score was>4, sufentanil 0.05 μg/kg was injected intravenously as rescue analgesic.The development and severity of catheter-related bladder discomfort (CRBD) were recorded immediately at the end of the operation (T 1), and at 6 h (T 2), 24 h (T 3) and 48 h (T 4) after the operation, respectively.Riker sedation-agitation scale (SAS) score was recorded at T 1, 2, and QoR-9 scale was recorded at T 3, 4.The concentrations of serum cortisol (Cor), norepinephrine (NE), epinephrine (E) and blood glucose (Glu) were measured by enzyme linked immunosorbent assay.First off-bed time, exhaust time, length of hospital stay after surgery, and the requirement for rescue analgesia and adverse reactions (nausea and vomiting, respiratory depression, hypotension, skin itching) within 48 h after the operation were recorded. Results:Compared with group PCIA, the incidence of CRBD and the severity were significantly decreased at T 1-4, SAS score was decreased at T 1, 2, QoR-9 score was increased at T 3, 4, Cor, NE, E and Glu concentrations were decreased at T 1-4, the incidence of postoperative rescue analgesia was decreased, first off-bed time, exhaust time and length of hospital stay after surgery was shortened, and the incidence of postoperative nausea and vomiting, respiratory depression, hypotension, skin itching was decreased in group L ( P<0.05). Conclusion:Continuous infusion of lidocaine through the urinary catheter can provide good postoperative analgesia, reduce postoperative stress response and adverse reactions, and facilitate early postoperative recovery in patients undergoing urological surgery.
8.The correlation analysis of donor renal histopathology scores with Lifeport parameters and donor grades

Objective To analyze the relationship of donor kidney histopathology scores with Lifeport parameters and donor grades before kidney transplantation.Methods Retrospective analysis was including 25 cases of organ donation (DCD) donor kidney transplantation from January 2017 to May 2018.The correlation analysis was performed on histopathological data of donor kidney,Lifeport parameters,and donor grades.Histopathological examination was carried out by rapid paraffin method,and the scores were graded according to the Remuzzi standard and acute tubular injury (ATI) degree.The donor grades were based on the standard score of our center.Data were analyzed by SPSS(R) 17.0 software.Results Remuzzi scores were positively correlated with protopathy scores (r =0.546,P =0.005) and hypertension scores of donor scores (r =0.500,P =0.011).Remuzzi scores and Lifeport parameters flow (Flow) and resistance index (RI) were found to be correlated.Remuzzi scores of glomeruli sclerosis (GS),tubule atrophy (TA),interstitial fibrosis (IF),arterial narrow (AN) and Lifeport parameters were also correlated,among which the correlation of AN scores were strongest correlated with Lifeport parameters (Flow:r =0.539,P<0.001;RI:r =0.497,P =0.001).ATI had a correlation with Lifeport parameters (Flow:r =0.368,P =0.016;RI:r =0.456,P =0.002),which is most relevant to the RI.However,there was no correlation of the donor grades with Lifeport parameters,and ATI (Flow:r=0.216,P =0.169;RI:r=0.06,P =0.707;ATI:r =0.005,P =0.977).Of 50 cases of kidney transplantation,recover time of DGF group was significantly higer than that of nomal recovery group F =14.729,P =0.001);Lifeport flow parameters of normal recovery group were statistically higher than those of other groups (F =3.612,P =0.020).Conclusion Histopathology scores of donors kidney are correlated with protopathy scores,hypertension scores,and Lifeport parameters,and could be used to evaluate the renal quality effectively.In the clinical work,the donor grades,histopathology scores and Lifeport parameters should be combined to evaluate the kidney quality.
9.Clinical efficacy of artificial vascular replacement for external iliac artery dissection during renal transplantation

Objective:To explore the clinical efficacy of treating external iliac artery dissection in renal transplantation by artificial vascular replacement.Methods:Four sudden intraoperative cases of external iliac artery dissection were selected.After removing vascular sutures, intimal arterial peeling blocked external iliac artery( n=3)and transplanted renal artery( n=1). Artificial vascular replacement of external iliac artery was performed using artificial vessels made from puffed polytetrafluoride ethylene(ePTFE). Secondary perfusion was performed in four transplanted kidneys for anastomosing with internal iliac artery. Results:One patient regained normal renal function within 1 week post-operation.Two cases had delayed graft function.Another case had delayed graft function plus acute rejection.After hemodialysis, renal function normalized at 2-3 weeks post-operation.During a follow-up period of(0.5-5.0)years, transplanted kidney function remained stable, blood supply, skin temperature and movement of operated lower extremities normalized.Conclusions:The incidence of vascular dissection of external iliac artery is not high during renal transplantation.However, the disease has a rapid and dangerous progression.The consequences of delayed intervention are quite serious.Treating external iliac artery dissection with renal transplantation may achieve satisfactory clinical outcomes.
10.The relationship and mechanism between accessory renal artery and essential hypertension by CTA

Objective To explore the relationship between accessory renal artery(ARA)and essential hypertension,and the possible mechanism using CT angiography(CTA).Methods The patients who underwent CTA examination on renal artery were reviewed retrospectively in our hospital.A variety of CTA reconstruction techniques were used to observe the type and number of ARA,as well as the diameter of ARA and the main renal artery.Results A total of 126 ARA(66 left ARA and 60 right ARA)were found in 253 patients. 10% of the patients had more than two ARAs.In 164 patients with essential hypertension,ARA detection rate was 40.2%(66/164).In the non-hypertension patients,the proportion of ARA was 31.5%(28/89).There was higher proportion of young(P=0.002)and male (P=0.022)patients in ARA hypertension group compared with ARA non-hypertension group.There were no significant differences on the prevalence of type(P=0.826)and number of ARA(P=0.501)between these two groups.In all of the patients with single ARA,no significant differences were detected on the ratios of diameter of ARA and main renal artery between the two groups(P=0.32). However,in ARA hypertension group,the diameter of main renal artery on the ARA side was significantly smaller than that on the opposite side(P=0.01).In non-hypertension ARA group,no statistical difference was found between the diameter of bilateral main renal arteries(P=0.06).Conclusion ARA is more prevalent in essential hypertensive patients,especially in young male.The decrease of the diameter of main renal artery in the ARA side may be a possible mechanism for essential hypertension.
