1.Gastrointestinal function recovery influences the prognosis of patients after liver transplantation

BACKGROUND:Delayed gastrointestinal function recovery often appears in patients after liver transplantation. It may influence patient’s prognosis. OBJECTIVE:To retrospectively analyze the recovery time of gastrointestinal function in patients after liver transplantation, and to explore the influence of the gastrointestinal function recovery after liver transplantation on the prognosis. METHODS:The data of 254 liver transplantation patients selected from two hospitals of Fujian province from January 2006 to October 2011 were col ected. The recovery time of gastrointestinal function in liver transplantation patients was descriptively analyzed. This study was divided into three groups (<7 days group, 7-14 days group and>14 days group) according to the recovery time in order to comparatively observe the incidence of postoperative complications, hospital stay and costs in hospital. RESULTS AND CONCLUSION:Among the 254 cases, 142 cases had the ful recovery of gastrointestinal function for<7 days, 90 cases 7-14 days, and 22 cases for>14 days. The patients in the<7 days group had the lowest incidence of complications (lung, abdominal cavity and incision), 2 weeks postoperative serum albumin level, hospital stay and cost when compared with other two groups, fol owed by the 7-14 days group, and highest in the>14 days group (P<0.05);the 2 weeks postoperative serum albumin level was highest in the<7 days group, fol owed by 7-14 days group, and lowest in the>14 days group (P<0.05);there were no significant differences in the renal dysfunction after liver transplantation and the incidence of acute rejection (P>0.05). Higher incidence for patients after liver transplantation with delayed gastrointestinal function recovery may result in higher incidence of postoperative infection complications, longer hospital stay and higher hospital costs.
2.Study on correlation between hope levels and medical coping modes of maintenance hemodialysis patients

Objective To study the correlation between the hope levels and medical coping modes of maintenance bemedialysis(MHD) patients.Methods 118 hemodialysis patients were sampled from a hospital in Fuzhou city in October,2007.A survey was conducted among these patients by questionnaires.And the col-lected data were analyzed with SPSSll.5.Results 98.3% of surveyed patients got a mediate or high-level in hope level.Compared to common patients,MHD patients got a higher score both in wriggly and subdued coping modes,and a lower score in facing mode.The choice of coping modes was affected by the hope levels of MHD patients.Conclusions Pafients with higher hope level tend to adopt wriggly and facing coping modes,while those with lower hope level generally take subdued coping mode.The nurses should help MHD patients seek supports from others in order to increase their hope level and promote them to adopt effective coping modes,in doing 80,it Can promote their mental healthy and improve their qu~ty of life.
3.Clinical observation of elderly patients with acute myelogenous leukemia treated with CAG regimen combined with thalidomide

Objective To evaluate the clinical efficacy and safety for the CAG regimen combined with thalidomide in elderly patients with acute myelogenous leukemia (AML).Methods 51 untreated elderly cases with AML were randomly divided into treatment group (treated with CAG regimen combined with thalidomide,26 cases) and control group (treated with CAG regimen alone,25 cases),and the clinical efficacy and safety were observed.Results After 2 cycles of treatment,the complete remission (CR) rates of treatment group and control group were 61.5 % (16/26) and 48.0 % (12/25) respectively,the difference was not significant (P =0.404).With regard to prostecdtive efficacy,the difference was markedly significant,1-year CR rates of treatment group and control group were 53.8 %(14/26) and 24.0 % (6/25) respectively (P =0.045),and 2-year CR rates were 42.3 % (11/26) and 12.0 % (3/25) respectively (P =0.027).During the follow-up of 2 years,overall 1-and 2-year overall survival rates were 73.1% and 46.1% in treatment group versus 52.0 % and 24.0 % in control group.During the follow-up of 2 years,the estimated mean overall survival time in treatment group was 22 months and it was 13 months in control group (P =0.018).The induction mortality rates and myelosuppression showed no difference between the two groups.Compared with control group,the side effect rate of nausea and vomiting was significantly decreased in treatment group (P =0.025).Conclusion The CAG regimen combined with thalidomide is effective with little side effects for elderly AML,and is worthy of further study and clinic application.
4.Development of Medical Box for Debriding and Change Dressings in Sanitary Train with Application Module Theory

Objective To establish a kind of medical box for wound debriding and change dressing in sanitary train,which could be used to satisfy the requirements of natural and manmade disasters for medical service.Methods With tri-grade modularization theory,the medical box were divided into four functional sections,including disinfection cleaning solution section,debriding and change dressings common section,contaminated wound change dressings section and infected wound change dressing section.Through the four sections,the medical box could be provided 5 wound debirding and 50 change dressings.Results The medical box can satisfy all requirements for wound treatment with useful and convenient function.Conclusions The invented medical urgent box has so many advantages of modularization,complete functions and expansionary,which could satisfy all requirements for sanitary train.
5.Application of Portable Health Package for the Nurses in Train Evacuation

Obiective To evaluate the efficacy of health package for the nurse in train evacuation, Methods The self-manufactured new mobile health package for the nurses provides shortcut and completed equipment in various basic nursing care, therapic procedure, condition observation, emergency aid and treatment for nursing staff who performed transportation in train evacuation.Results The potable health package can play an important role in several medical support training and have special mission. Conclusion The health package has mobile applicability and operability, which is an important equipment for successful medical support training.
6.Comparative study of assessment of pain among medical and nursing staff and cancer patients

Objective This study aimed to discuss the different assessment of pain among medical and nursing staff and cancer patients and supply reference for proper analgesic precept for clinical application and nursing measures. Methods We collected the assessment of psychological pain and physiological pain by 55 hospitalization cancer patients, 40 physicians in-charge and 55 nurses in-charge in one week by Johnson inventory. The assessment results were compared and at the same time the relevant problems of the attitude to cancer pain by patients was also investigated. Results Improper recognition existed in cancer pain treatment by most cancer patients. The physiological pain was higher than the psychological pain assessed by both patients and nurses (P<0.05). But the pain assessment by patients was higher than that by the nurses (P<0.05). The assessment of psychological pain was higher than the physiological pain by doctors and both aspects were lower than those by patients, but no statistical difference was seen (P>0.05). The assessment by doctors was more accurate than that by nurses. Conclusions Routine establishment of pain assessment inventory for patients could instruct patients how to record the degree of their pain. We should strengthen the standard training about pain management knowledge and give timely communication with patients' cancer pain.
7.Effect of Acupuncture Combined with Moxibustion on Heart Rate Variability in Patients with Post-stroke Depression

Objective To observe the effect of acupuncture combined with moxibustion on heart rate variability (HRV) in patients with post-stroke depression. Methods 120 patients were divided into control group (n=60) and treatment group (n=60). Both groups took fluoxetine and accepted acupuncture therapy, and the treatment group accepted moxibustion additionally. Hamilton Depression Rating Scale-17 (HAMD-17) and the change of HRV were compared before and after treatment. Results The score of HAMD was lower, and the reductive ratio was higher in the treatment group than in the control group (P<0.05). The standard deviation of the mean of qualified normal sinus interval (SDNN) and standard deviation of sequential five-minute R-R interval means (SDANN) increased in both groups after treatment (P< 0.05), and were higher in the treatment group than in the control group (P<0.05). The root mean square differences of successive R-R intervals (RMSSD) and percentage of differences between adjacent normal sinus intervals greater than 50 ms (PNN50) decreased in both groups after treatment (P<0.05), and were lower in the treatment group than in the control group (P<0.05). The low frequency (LF), high frequency (HF) and LF/HF were higher in both groups after treatment (P<0.05), and were higher in the treatment group than in the control group (P< 0.05). Conclusion Acupuncture combined with moxibustion therapy can better adjust the function of autonomic nerve and reduce depression in patients with post-stroke depression.
8.Effect of Needle Warming Moxibustion on Cognitive Dysfunction after Stroke

Objective To observe the effect of needle warming moxibustion on cognitive dysfunction after stroke. Methods 76 stroke patients with cognitive dysfunction were randomly divided into treatment group (n=38) and control group (n=38). The treatment group received needle warming moxibustion and routine rehabilitation for 4 weeks, while the control group only received routine rehabilitation. They were assessed with Montreal Cognitive Assessment (MoCA) and activities of daily living (ADL) scale before and after treatment. Results The score of MoCA and ADL improved in both groups (P<0.05), and improved more in the treatment group than in the control group (P<0.05) after treatment. Conclusion Needle warming moxibustion could improve cognitive function in patients after stroke and activities of daily living.

The potential medicinal value of Ma bamboo (Dendrocalamus latiflorus), one of the most popular and economically important bamboo species in China, has been underestimated. In the present study, we found that D. latiflorus leaf extract (DLE) reduced fasting blood glucose levels, body weight, and low-density lipoprotein cholesterol with low liver toxicity in db/db mice. In addition, gene expression profiling was performed and pathway enrichment analysis showed that DLE affected metabolic pathways. Importantly, DLE activated the AKT signaling pathway and reduced glucose production by downregulating glucose-6-phosphatase (G6PC) and phosphoenolpyruvate carboxykinase 1 (PCK1) expression. Moreover, network pharmacology analysis identified rutin as an active component in DLE through targeting insulin growth factor 1 receptor (IGF1R), an upstream signaling transducer of AKT. Due to its hypoglycemic effects and low toxicity, DLE may be considered an adjuvant treatment option for type 2 diabetes patients.
