BACKGROUND:Delayed gastrointestinal function recovery often appears in patients after liver transplantation. It may influence patient’s prognosis. OBJECTIVE:To retrospectively analyze the recovery time of gastrointestinal function in patients after liver transplantation, and to explore the influence of the gastrointestinal function recovery after liver transplantation on the prognosis. METHODS:The data of 254 liver transplantation patients selected from two hospitals of Fujian province from January 2006 to October 2011 were col ected. The recovery time of gastrointestinal function in liver transplantation patients was descriptively analyzed. This study was divided into three groups (<7 days group, 7-14 days group and>14 days group) according to the recovery time in order to comparatively observe the incidence of postoperative complications, hospital stay and costs in hospital. RESULTS AND CONCLUSION:Among the 254 cases, 142 cases had the ful recovery of gastrointestinal function for<7 days, 90 cases 7-14 days, and 22 cases for>14 days. The patients in the<7 days group had the lowest incidence of complications (lung, abdominal cavity and incision), 2 weeks postoperative serum albumin level, hospital stay and cost when compared with other two groups, fol owed by the 7-14 days group, and highest in the>14 days group (P<0.05);the 2 weeks postoperative serum albumin level was highest in the<7 days group, fol owed by 7-14 days group, and lowest in the>14 days group (P<0.05);there were no significant differences in the renal dysfunction after liver transplantation and the incidence of acute rejection (P>0.05). Higher incidence for patients after liver transplantation with delayed gastrointestinal function recovery may result in higher incidence of postoperative infection complications, longer hospital stay and higher hospital costs.

Objective To study the correlation between the hope levels and medical coping modes of maintenance bemedialysis(MHD) patients.Methods 118 hemodialysis patients were sampled from a hospital in Fuzhou city in October,2007.A survey was conducted among these patients by questionnaires.And the col-lected data were analyzed with SPSSll.5.Results 98.3% of surveyed patients got a mediate or high-level in hope level.Compared to common patients,MHD patients got a higher score both in wriggly and subdued coping modes,and a lower score in facing mode.The choice of coping modes was affected by the hope levels of MHD patients.Conclusions Pafients with higher hope level tend to adopt wriggly and facing coping modes,while those with lower hope level generally take subdued coping mode.The nurses should help MHD patients seek supports from others in order to increase their hope level and promote them to adopt effective coping modes,in doing 80,it Can promote their mental healthy and improve their qu~ty of life.

Objective To evaluate the clinical efficacy and safety for the CAG regimen combined with thalidomide in elderly patients with acute myelogenous leukemia (AML).Methods 51 untreated elderly cases with AML were randomly divided into treatment group (treated with CAG regimen combined with thalidomide,26 cases) and control group (treated with CAG regimen alone,25 cases),and the clinical efficacy and safety were observed.Results After 2 cycles of treatment,the complete remission (CR) rates of treatment group and control group were 61.5 ％ (16/26) and 48.0 ％ (12/25) respectively,the difference was not significant (P =0.404).With regard to prostecdtive efficacy,the difference was markedly significant,1-year CR rates of treatment group and control group were 53.8 ％(14/26) and 24.0 ％ (6/25) respectively (P =0.045),and 2-year CR rates were 42.3 ％ (11/26) and 12.0 ％ (3/25) respectively (P =0.027).During the follow-up of 2 years,overall 1-and 2-year overall survival rates were 73.1％ and 46.1％ in treatment group versus 52.0 ％ and 24.0 ％ in control group.During the follow-up of 2 years,the estimated mean overall survival time in treatment group was 22 months and it was 13 months in control group (P =0.018).The induction mortality rates and myelosuppression showed no difference between the two groups.Compared with control group,the side effect rate of nausea and vomiting was significantly decreased in treatment group (P =0.025).Conclusion The CAG regimen combined with thalidomide is effective with little side effects for elderly AML,and is worthy of further study and clinic application.

Objective To evaluate clinical treatment method and efficacy of newly diagnosed acute promyelocytic leukemia (APL),and analyze the relevant factors about the long-term survival.Methods The clinical data of 48 patients with newly diagnosed APL were analyzed retrospectively.All of them used alltrans retinoic acid (ATRA) combined with anthracycline as induction remission therapy.After induction remission,ATRA combined with chemotherapy was used as consolidation therapy,and ATRA,arsenic trioxide and conventional chemotherapy alternated as maintenance therapy.Short-term efficacy was analyzed.Patients were followed up,and the rates of overall survival (OS) and disease-free survival (DFS) were analyzed.Long-term efficacy was analyzed by COX proportional hazards regression models univariate analysis.Results The complete remission (CR) rate was 87.5％(42/48) in all 48 patients with APL.The time from treatment beginning to CR was (30.7 ± 4.6) d.Age was the only factor affecting the rate of CR.The rates of 1-year,3-year and 5-year OS were (87.5 ± 4.8)％,(85.4 ± 5.1)％ and (78.3 ± 6.7)％ in 48 patients with APL.The rates of 1-year,3-year and 5-year DFS were (97.6 ±2.4)％,(93.9 ±4.2)％ and (89.5 ± 5.9)％ in 42 patients with CR.COX proportional hazards regression model univariate analysis result showed that the patient' s age,gender,lactate dehydrogenase,diffuse intravascular clotting,risk stratification and bone marrow abnormalities promyelocyte ratio had no correlation with the rate of DFS (P ＞0.05).Conclusions ATRA combined with anthracycline as induction remission therapy,after induction remission ATRA combined with chemotherapy as consolidation therapy,and ATRA,arsenic trioxide and conventional chemotherapy alternated as maintenance therapy can get a higher rate of CR and long-term survival in patients with newly diagnosed APL.It is worthy of clinical application.

BACKGROUND:The stress changes of bone are the main simulation sources for bone remodeling. At present, the research on the relationship between stress and bone remodeling focuses on the effect of axial stress on bone remodeling, while the research on bone remodeling under bending force is rare.
OBJECTIVE:To observe the bone remodeling of rabbit tibia diaphysis cortical bone in growth phase under bending force.
METHODS:A bone window with the diameter of 2.5 mm was made 0.5 cm from the medial distal epiphyseal line of rabbit proximal left tibia, and then ice saline was used to cool the tibia and memory al oy film. After cooling, the memory al oy stripe was straightened and implanted into the tibial bone marrow cavity through the bone window. The direction of memory al oy stripe implantation:two ends of the memory al oy stripe arrived to the lateral cortex after rewarming, and the curve apex arrived to the medial cortical bone, fol owed by the sutured wound. The continuous internal bending force was loaded on the rabbit tibia diaphysis through the shape memory al oy stripe implanted into the tibial bone marrow cavity in order to observe the bone remodeling mode of tibia diaphysis cortical bone under long-term bending force.
RESULTS AND CONCLUSION:After 6 months continuous bending force, the thickness of cortical bone on the tension stress side of tibia diaphysis was decreased, while the thickness on the compressive stress side was increased, and no obvious bending deformation was observed. The thickness of the cortical bone on the compressive stress side was associated with the stress distribution on tibia. The tibia with integrity structure exhibited an inherent“resistance”to the bending stress, this indicating that the bone shaft of the long bone without fracture had no potential of force line correction basical y. The results suggested that the bone remodeling model of tibia diaphysis cortical bone had the advantages of less trauma, control ed force, less influence on the growth and metabolism, no limitation on the animal activities and meet the physiological state, which was an ideal model of bone remodeling.

Objective To investigate the inhibitory effects of cationic polymeric liposomes (CPLs) vector-based RNA interference (RNAi) technology on the expression of rat MHCⅡ transactivator ( CⅡTA) and MHCⅡgenes .Methods According to the genetic information of CⅡTA downloaded from GenBank, three short hairpin RNA (shRNA) sequences targeting CⅡTA sequences were designed .CPLs vectors were constructed and coupled to shRNA plasmid vectors to form pCⅡTA-CPLs vectors .Six groups including control group , CPLs control group , pHK-CⅡTA control group and three pCⅡTA-CPLs groups were set up.Rat dendritic cells (DCs) were transfected in vitro.Real time PCR and flow cytometry analysis were used to detect the expression of CⅡTA and MHCⅡat mRNA and protein levels in DCs after transfec-tion.Results The pCⅡTA-CPLs vectors were successfully constructed .Compared with control groups ,the transcription level of CⅡTA and MHCⅡand the expression of MHCⅡat protein level were significantly in-hibited in all pCⅡ TA-CPLs groups ( P<0 .01 ) .The strongest inhibitory effects of pCⅡTA-CPLs on the ex-pression of CⅡTA and MHCⅡgenes were observed in the second pCⅡTA-CPLs group.There was a positive correlation between the expression of CⅡTA and MHCⅡ.Conclusion CPLs vectors were effective gene carriers.The constructed pCⅡTA-CPLs vectors significantly inhibited the in vitro expression of rat CⅡTA and MHCⅡ, which provided evidences for further investigation on pCPLs-CⅡTA vectors in vivo.

Obiective To evaluate the efficacy of health package for the nurse in train evacuation, Methods The self-manufactured new mobile health package for the nurses provides shortcut and completed equipment in various basic nursing care, therapic procedure, condition observation, emergency aid and treatment for nursing staff who performed transportation in train evacuation.Results The potable health package can play an important role in several medical support training and have special mission. Conclusion The health package has mobile applicability and operability, which is an important equipment for successful medical support training.

Objective To investigate a retrieval strategy for dentistry literature based on medical subject headings (MeSH). Methods We analysed the MeSH Tree Structures of PubMed in 2009 and combined it with the core glossary of dentistry to find out highly frequent dental words in MeSH. Then 6 words were singled out from the whole MeSh Tree to apply for a retrieval strategy for dentistry literatures. Results 102,500 pieces of dentistry literature were obtained from the year 2006 to 2008. Conclusions This research strategy can improve the efficiency of literature search in dentistry field for clinical or laboratory researches.

Objective To establish a kind of medical box for wound debriding and change dressing in sanitary train,which could be used to satisfy the requirements of natural and manmade disasters for medical service.Methods With tri-grade modularization theory,the medical box were divided into four functional sections,including disinfection cleaning solution section,debriding and change dressings common section,contaminated wound change dressings section and infected wound change dressing section.Through the four sections,the medical box could be provided 5 wound debirding and 50 change dressings.Results The medical box can satisfy all requirements for wound treatment with useful and convenient function.Conclusions The invented medical urgent box has so many advantages of modularization,complete functions and expansionary,which could satisfy all requirements for sanitary train.

Objective To compare the cost-effectiveness and cost-utility of venlafaxine and mirtazapine in patients with treatment-resistant major depression (TRD).Methods One hundred and five patients with TRD were enrolled in this study and grouped into venlafaxine treatment (n=50) and mirtazapine treatment (n=55) based on the double-blind randomization scheme generated by computer.The treatment costs of antidepressants during 8 weeks were calculated,the rates of clinical response and remission were taken as treatment effectiveness,and the quality-adjusted life years (QALYs) as treatment utility.The descriptive analysis and nonparametric test were used to compare the cost-effectiveness and cost-utility of different groups.Results During 8 weeks,the treatment cost of antidepressant was ￥ 1 396.44 for venlafaxine and ￥ 1 206.90 mirtazapine,and the difference between two groups was ￥ 189.54.The cost-effectiveness ratios between venlafaxine and mirtazapine were very close (differed ￥ 0.06 for remission rate and ￥ 1.08 for response rate respectively).There was no significant difference for cost-utility ratios between two groups (physical functioning Z=-0.15,P＞0.05 ; mental health Z=-0.54,P＞0.05).Conclusion Both cost-effectiveness and cost-utility of venlafaxine in patients with TRD are close between venlafaxine and mirtazapine.
