1.Environmental object surfaces contamination with carbapenem-resistant gram-negative bacteria in intensive care units of tertiary hospitals in Shanghai

OBJECTIVE To investigate the current status of contamination with carbapenem-resistant gram-negative bacteria in environment of intensive care units(ICU)of tertiary hospitals in Shanghai and find out the potential contamination sources so as to provide bases for prevention and control of multidrug-resistant organisms infec-tions in the ICUs.METHODS The surroundings of the ICU patients detected with CRGNB and environmental ob-jects surfaces in public area were sampled by mSuperCARBA chromogenic media from Dec.2024 to Jan.2025,the strains were isolated,and the targeted strains were identified by matrix-assisted laser desorption/ionization time-of-flight(MALDI-TOF)mass spectrometer.RESULTS A total of 653 samples were collected in the survey,76 of which were positive for bacterial culture,60 were detected with CRGNB,and the isolation rate of CRGNB was 9.19%.The isolation rate of CRGNB was 53.40%in the water-source group,0.91%in the non-water-source group,and there was significant difference(x2=286.450,P<0.001).The result of whole genome sequencing for 17 strains of CRKP showed that ST11 and ST15 were the two major types of multilocus typing(MT),respective-ly carrying 2-12 types of drug resistance genes.CONCLUSIONS The CRGNB strains are detected in some environ-mental sites of the ICUs of 15 tertiary hospitals in Shanghai,and the isolation rate of CRKP is highest among them.The colonization rate of CRGNB is relatively low on the highly frequent-contact object surfaces of the ICUs,however,sink drain holes poses a risk of hospital-acquired CRGNB infections transmissions.Additionally,the ba-sins and towels of the CRGNB patients are hard to be thoroughly cleaned,disinfected and dried,resulting in a high contamination rate.
2.Application of flipped classroom in teaching of rehabilitation assessment

Objective To analyze the application of flipped classroom in the practical teaching of rehabilitation assessment courses for rehabilitation therapy majors,and to explore a new teaching method for rehabilitation therapy.Methods Eighty students majoring in rehabilitation therapy from the Department of Rehabilitation Medicine of Guangzhou Xinhua University in grade 2020 were selected as the research objects.They were assigned into observation group or control group according to the random number table method,with 40 cases in each group.The observation group adopted the flip classroom teaching mode,and the rehabilitation assessment course teaching based on flipped classroom was carried out,while the control group adopted the traditional teaching mode.The students'theoretical and operational assessment results,teaching effect,and teaching model satisfaction were analyzed.Results No statistically significant differences were found in theoretical test scores between the two student groups(by gender/course).The students in the observation group had higher theoretical scores[(80.25±11.65 vs 72.50±14.28)score].The observation group showed significantly higher satisfaction scores than the control group across all three dimensions:classroom learning enthusiasm,teacher-student interaction,and overall teaching satisfaction.(P<0.05).Conclusion The application of flipped classroom in the rehabilitation therapy teaching is helpful to improve the professional theoretical level of students,and to a certain extent,it is beneficial to improve the professional level of teachers.
3.Integrated single-cell and transcriptome sequencing to construct a prognostic model of M2 macrophage-related genes in prostate cancer

Objective·To explore the prognostic value of M2 macrophage-related genes in prostate cancer(PCa),aiming to predict tumor prognosis more accurately and enable personalized treatment.Methods·RNA sequencing(RNA-seq)data of PCa were downloaded from The Cancer Genome Atlas(TCGA)database,and single-cell RNA sequencing(scRNA-seq)data were obtained from the Gene Expression Omnibus(GEO)database.The immune infiltration of TCGA samples was assessed using the CIBERSORTx algorithm.Differential genes in scRNA-seq data were identified using the FindMarkers function,and immune cell subtypes were characterized.M2 macrophage-related pathways and interactions with surrounding cells were explored through Gene Set Enrichment Analysis(GSEA)and the CellChat algorithm.M2 macrophage signature genes were selected to construct a prognostic model for PCa using univariate Cox and LASSO analyses.Based on the risk model,clinical characteristics,immune suppression,drug resistance,and drug sensitivity analyses were conducted.Results·In TCGA samples,patients with high M2 macrophage infiltration exhibited significantly lower progression-free survival(PFS).scRNA-seq analysis identified multiple subpopulations of tumor microenvironment(TME)cells.M2 macrophages interacted with various immune cells in TME,contributing to an immunosuppressive microenvironment and playing a key role in tumor promotion.Based on these findings,a PCa risk model was developed,incorporating TREM2,OTOA,SIGLEC1,and PLXDC1,which showed robust predictive performance in both training and validation cohorts.Patients with higher risk scores demonstrated a more immunosuppressive TME,decreased androgen receptor(AR)signaling activity,and worse clinical characteristics,leading to poorer outcomes.Drug prediction and sensitivity analyses identified six potential therapeutic agents that may offer improved efficacy for patients with higher risk scores.Conclusion·A prognostic model based on M2 macrophage-related genes in the TME has been constructed,providing a theoretical foundation for precision treatment in PCa.
4.Plateau hypoxia induces pyroptosis in mouse kidney cells through NOD-like receptor signaling pathway

Objective:
To investigate the molecular mechanism of NOD-like receptor(NLR) signaling pathway induced cellular focal death in mouse kidney tissues under plateau hypoxia based on transcriptomic sequencing technology.
Methods:
Animal models were constructed in high-altitude kidney test group(HKT group) and plain kidney control group(PKC group),and C57/BL6 mice were bred at an altitude of 4 200 m and 400 m,respectively.Kidney tissues were aseptically taken out after 30 d and used for observation of renal histopathological changes and transcriptomics sequencing,respectively,and then differentially expressed genes(DEGs) were enriched by KEGG and GO analysis.Key gene and protein expression levels were verified by reverse transcription quantitative polymerase chain reaction(RT-qPCR) and Western blot analysis.
Results:
Hematoxylin-eosin(HE) staining showed that the renal tubules of HKT mice were disorganized,and the tubular epithelial cells were edematous with inflammatory cell aggregation.Transcriptomics sequencing analysis revealed that a total of 3 007 DEGs were identified in the HKT group,and KEGG analysis showed that DEGs were significantly enriched in the NLR signaling pathway.The mRNA expression of the key differentially expressed genes,CASP1,CASP11,GSDMD,IFI16,NAIP5,IRF9,and the downstream inflammatory factors,IL-1β and IL-18,was upregulated,and protein expression of CASP1,CASP11,and GSDMD was up-regulated.
Conclusion
The plateau hypoxic environment induces cellular pyroptosis by regulating the expression of NLR signaling pathway and releases downstream inflammatory factors to cause inflammatory responses.
5.Current status and prospects of hyperbaric oxygen therapy for children with autism

Autism spectrum disorder(ASD)is a severe neurodevelopmental disorder with onset in child-hood,characterized by persistent social communication deficits and restricted,repetitive patterns of behavior.In recent years,its global prevalence has shown a continuous upward trend.However,there remains a lack of effective treatments capable of improving brain function in ASD.This article systematically reviews the mechanisms,clinical efficacy,and safety of HBOT in ASD,while exploring its potential therapeutic value.Research indicates that HBOT exerts its effects through the following mechanisms:enhancing plasma oxygen saturation,thereby improving oxygen supply to ischemic brain regions;inhibiting microglial activation,reducing pro-inflammatory cytokine levels;modulating gut microbiota homeostasis,restoring brain-gut axis dysfunction.Clinical studies demonstrate that when HBOT is combined with Applied Behavior Analysis(ABA),it significantly reduces behavioral scale scores in children with ASD while improving their social interaction,self-care abilities,and verbal communication.Additionally,HBOT positively regulates oxidative stress markers and enhances perfusion in specific brain regions.Regarding safety,strict adherence to operational protocols minimizes the risk of adverse effects(e.g.,baro-trauma).This review provides evidence-based support for the standardized application of HBOT in ASD.Future re-search should focus on optimizing treatment protocols(e.g.,pressure and oxygen concentration parameters)and evaluating long-term efficacy.
6.Online risk calculator and nomogram prediction model for urinary incontinence after robot-assisted laparoscopic radical prostatectomy

Objective·To develop a nomogram prediction model and an online risk calculator,and to predict the continence of patients after robot-assisted laparoscopic radical prostatectomy(RARP).Methods·A total of 604 prostate cancer patients who underwent RARP and had preoperative prostate magnetic resonance imaging at the Shanghai General Hospital,Shanghai Jiao Tong University School of Medicine from September 2022 to December 2024 were analyzed and included.All patients were randomly resampled and divided into a training set(n=420)and a validation set(n=184)at a ratio of 7∶3.The patients'continence was followed up every month from the first month after the operation.The least absolute shrinkage and selection operator(LASSO)model was applied to screen the features.A Logistic multivariate regression analysis was used to establish a prediction model integrating the features selected from the LASSO analysis.The receiver operator characteristic(ROC)curve was drawn to predict the recovery of continence in patients after RARP,and the areas under the curve were compared by the DeLong test to evaluate the discrimination of the model.Calibration curves and decision curve analysis(DCA)were used to evaluate the calibration and clinical utility the model.Results·According to the postoperative continence follow-up data of the patients,the continence rate of the patients at 3 months after the operation was 58.28%(352/604).The length of the membranous urethra,the thickness of the right levator ani muscle,and blood loss were identified as independent predictors of early postoperative(3-month)incontinence by Logistic multivariate regression analysis of the training set.The area under the ROC curve was calculated as 0.976(0.954,0.998)for the training set and 0.977(0.945,1.000)for the validation set,demonstrating good discrimination of this model.No significant difference between the ROC curves of the training set and the validation set was confirmed by the DeLong test(P=0.949).A good goodness of fit of this model was demonstrated by the Hosmer-Lemeshow test(P=0.179).The clinical utility of the nomogram prediction model was indicated by the DCA plot.This nomogram prediction model was incorporated into an online calculator(https://yitingd.shinyapps.io/DynNomapp).Conclusion·This study developed and validated a nomogram prediction model that can effectively predict the early continence of patients after RARP.The length of the membranous urethra,the thickness of the right levator ani muscle,and the intraoperative blood loss are significant independent predictors of early postoperative incontinence.

9.Case report and literature analysis of Mycobacterium iranicum infection

OBJECTIVE To offer a reference for the treatment of Mycobacterium iranicum infection by analyzing the diagnosis and management of a single case alongside literature-reported cases.METHODS Through case report and literature reviews,this study synthesized the clinical features,therapeutic regimens,and patient outcomes of those infected with M.iranicum.RESULTS In the single case documented in this report,subsequent to clinical pharmacists'involvement in the consultation,the patient was prescribed a therapeutic regimen comprising levofloxacin(0.5 g,qd,ivgtt)+Clarithromycin sustained-release tablets(1 000 mg,qd,po)+Ethambutol tablets(0.75 g,qd,po).The patient exhibited clinical improvement and was discharged after treatment.This article integrated 12 published studies,encompassing 13 patients(7 male and 6 female),of whom 69.23%were aged≥50 years.Patients infected with M.iranicum exhibited non-specific clinical manifestations and imaging features,with pulmonary infection as the primary presentation.Antimicrobial susceptibility test revealed that M.iranicum was susceptible to multiple agents,including amikacin,clarithromycin,linezolid,and ethambutol.The three-drug combination therapy was the most frequently employed regimen.In terms of clinical outcomes,there were 9 cases(69.23%)of clinical cure,3 cases(23.08%)of bacteriological negativity conversion,and 1 case(7.69%)of treatment failure.CONCLUSIONS For M.iranicum infection,a triple-drug therapeutic regimen consisting of three agents with distinct mechanisms of action selected from amikacin,clarithromycin,moxifloxacin,levofloxacin,minocycline,ethambutol,and other relevant drugs may represent a relatively optimal strategy.
