1.Study on the vascular imaging and the pathology of breast cancer after neoadjuvant intraarterial chemotherapy

Objective To observe the vascular imaging the pathological changes in the patients with breast cancer after neoadjuvant intraarterial chemotherapy. Methods Before intraarterial infusion, DSA were performed on 96 patients with breast cancer. Pathological changes were studied after operation. Results The blood supply of breast cancer mainly arises from the lateral thoracic and subscapular arteries. Abnormalities in the vascular architecture of breast cancer include irregular bolus arrangement, contrast staining, pooling of contrast material. Degeneracy and necrosis can be seen in the tumor cells after intraarterial chemotherapy. The stromal edema, infiltration of inflammatory cells and fibroplasia may take place. Conclusions Intraarterial infusion of neoadjuvant chemotherapy is effective in the patients with breast cancer.
2.The role and significance of T-helper cell subtypes in the pathoaenesis of rheumatoid arthritis

Objective To explore the correlation between Th1,Th2,Th17,and Treg cells differentiation and related cytokines in patients with rheumatoid arthritis (RA).Methods Seventy-one patients with active RA were enrolled in this study.They were divided into low,moderate and high disease activity groups according to disease activity score (DAS28).The frequencies of Th1,Th2,Th17,and Treg cells in the peripheral blood of RA patients group (n=71) and healthy conlrol group (n=18) were determined by flow cytometry.T test was used for statistical analysis.Results Significant difference could be detected between the proportions of Th1 [ (6.2±4.5)%],Th17 [ (1.1±0.9)%] and Treg [(1.8±1.2)%] cells in the peripheral blood of RA patients and the control group (P<0.05),and there was correlation between proportions of these three kinds of cell and the DAS28.Conclusion Th1 and Th 17 cells may promote the development of RA disease,but Th2 and Treg cells could prevent further development of RA disease.
3.Expression and significance of C D19+CD5+B cells and interleukin-10 in patients with systemic lupus ;erythematosus

Objective To access the expression and clinical significance of CD19+CD5+B cells and interleukin (IL)-10 in patients with systemic lupus erythematosus (SLE). Methods Forty-six SLE patients and ten healthy controls were recruited in the Second Affiliated Hospital of Shanxi Medical University. CD19+CD5+B cells subsets were detected with flow cytometry. IL-10 level in serum were detected with enzyme linked immunosorbent assay (ELISA). The correlation between the expression of CD19+CD5+B cells and serum level of IL-10 with ESR, systemic lupus erythematosus disease activity index (SLEDAI) score and complement was analyzed. Pair-wise comparison of means of groups was conducted with one-way ANOVA. Comparison between the two groups was conducted by LSD-t test. Correlations between variables were carried out using Spearman's rank correlation test. Results The percentage of CD19+CD5+B cells in peripheral blood of SLE patients [(5.7±2.1)%] was significantly lower than those in healthy control group [(19.1±2.9)%](t=2.431, P=0.005), and it had a negative correlation with SLEDAI score (r=-0.292, P=0.049). The IL-10 serum level of SLE patients [(18.8±13.5) pg/ml] was significantly higher than the healthy control group [(8.3±2.9) pg/ml] (t=3.021, P=0.003), and it had a positive correlation with SLEDAI score (r=0.322, P=0.029). Conclusion CD19+CD5+B cells and IL-10 may participate in the occurrence and development of SLE. The changes of CD19+CD5+B cells and IL-10 in the peripheral blood might contribute to the pathogenesis and correlate with the disease activity of SLE.
4. Repair of the preauricular defects by the superficial temporal artery frontal branch flap and the retrograde retroauricular artery flap

Objective:
To investigate the feasibility and effectiveness of the superficial temporal artery frontal branch flap combine with the retrograde retroauricular artery flap in repairing the preauricular defects.
Methods:
The superficial temporal artery frontal branch flap with hair is designed for sideburns reconstruction, and the hairless retrograde retroauricular artery flap for repair the hairless area which is between the tragus and the temples. The donor sites were closed directly.
Results:
From September 2012 to September 2015, 9 cases were treated. All flaps survived completely. Surgical incisions and wounds at donor sites and recipient sites healed primarily. All cases were followed up for 6-18 months (10 months on average) and cosmetic results were satisfactory without visible scar.
Conclusions
The method of the superficial temporal artery frontal branch flap combined with the retrograde retroauricular artery flap for the repair of preauricular a large skin defect is simple with less and inconspicious auxiliary incision. The sidebums and hairless area can be simultaneously reconstructed with satisfactory appearance.
5.Evaluation of the effect of prone ventilation in severe pneumonia in high-altitude areas

Objective To evaluate the effect of prone ventilation in children with severe pneumonia at high altitude.Methods By convenience sampling method,80 children with severe pneumonia hospitalized in intensive care department of a tertiary A children's hospital in Xining,Qinghai Province from June 2021 to June 2023 were selected as the study subjects,and the regional randomization group method was used to divide into a test group and a control group with 40 cases in each group.On the basis of routine care,the test group received prone ventilation once a day for 6 to 12 h;the control group received supine ventilation.Respiratory mechanical parameters(oxygenation index,oxygen saturation,arterial oxygen partial pressure,arterial CO2 partial pressure),mechanical ventilation duration and safety parameters(incidence of unplanned extubation,stress injury)at 6 h and 12 h of mechanical ventilation were compared.Results There were no shedding cases.The interaction between oxygenation index,blood oxygen saturation and arterial oxygen partial pressure(P<0.05)in the 2 groups was compared(P>0.05).The results of simple effect analysis showed that at 6 h,the oxygen saturation and oxygenation index in the test group were higher than those in the control group,and the difference was statistically significant(P<0.05).At 12 h of mechanical ventilation,the oxygenation index,blood oxygen saturation and arterial oxygen partial pressure in the test group were higher than those in the control group,and the difference was statistically significant(P<0.05).The time of mechanical ventilation in the test group was102.00(60.00,153.00)h and 126.00(108.00,156.00)h in the control group,and the difference was statistically significant(P=0.013).The incidence of unplanned extubation and pressure injury were compared,and the differences in 2 groups were not significant(P>0.05).Conclusion Prone ventilation in children with severe pneumonia at high altitude is safe and feasible,which is helpful to improve the respiratory function and shorten the time of mechanical ventilation.
6.Correlation between lumbar spine and pelvic parameters in Lenke type 5 adolescent idiopathic scoliosis

BACKGROUND:The study of the lumbar spine and pelvis in patients with Lenke type 5 lordosis is limited to the coronal and sagittal planes,and the three-dimensional relationship between the scoliosis and the pelvis has not yet been clarified. OBJECTIVE:To analyze the effect of lumbar scoliosis on the pelvis in patients with Lenke type 5 lordosis and to study the correlation between the lumbar spine and the three-dimensional spatial position of the pelvis. METHODS:Imaging data of 60 patients with Lenke type 5 lordosis scoliosis admitted to the 3D Printing Reception Center of Shanghai Ninth People's Hospital,Shanghai Jiao Tong University School of Medicine from January 2019 to September 2023 were retrospectively analyzed,including Cobb angle,coronal pelvic tilt,lumbar lordosis,left and right pelvic hip width ratio(sacroiliac-anterior superior iliac spine),spinal rotation angle,pelvic tilt,sacral slope,pelvic incidence,coronal deformity angular ratio,sagittal deformity angular ratio,C7 plumb line-center sacral vertical line,apical vertebral translation,and coronal sacral inclination.The information was summarized as a database.SPSS 22.0 software was used to analyze the data related to the lumbar spine and pelvis of the patients with Lenke type 5 primary lumbar curvature adolescent idiopathic scoliosis using Spearman's correlation analysis and linear regression. RESULTS AND CONCLUSION:(1)Cobb angle was highly positively correlated with coronal deformity angular ratio,apical vertebral translation,and spinal rotation angle(r=0.91,r=0.841,r=0.736).(2)Coronal deformity angular ratio was highly positively correlated with apical vertebral translation(r=0.737),moderately positively correlated with C7 plumb line-center sacral vertical line(r=0.514),and moderately negatively correlated with sagittal deformity angular ratio(r=-0.595).(3)There was a high positive correlation between lumbar lordosis and sagittal deformity angular ratio(r=0.942)and a moderate negative correlation with coronal deformity angular ratio(r=-0.554).(4)There was a moderate positive correlation between Cobb angle with coronal pelvic tilt and coronal sacral inclination(r=0.522,r=0.534)and a moderate positive correlation between C7 plumb line-center sacral vertical line and coronal pelvic tilt(r=0.507).Apical vertebral translation with coronal pelvic tilt and coronal sacral inclination showed a moderate positive correlation(r=0.507,r=0.506).Lumbar lordosis with sacral slope and pelvic incidence showed a moderate positive correlation(r=0.512,r=0.538).Sagittal deformity angular ratio was moderately positively correlated with sacral slope and pelvic incidence(r=0.614,r=0.621).(5)Studies have found that the relative position of the lumbar spine and the pelvis is closely related in the horizontal,sagittal and coronal planes.When the lumbar spine affects scoliosis and is rotated,the relative position of the pelvis will also change to compensate,which indicates that while correcting scoliosis,the correction of the pelvis cannot be ignored.
