Chinese Journal of Postgraduates of Medicine 2008;31(36):21-23
Objective To study the pharmacodynamics features of cisatracurium besylate in differ-ent age patients. Methods One hundred and eighty patients were randomized to group Ⅰ (50-60 years old),group Ⅱ (61-70 years old)and group Ⅲ (above 70 years old). Each group was subdivided according to the dose of cisatracurium besylate, 0.10 mg/kg (2ED95)or 0.15 mg/kg (3ED95) into group Ⅰ2, Ⅰ3, Ⅱ2, Ⅱ3,Ⅲ2, Ⅲ3(n = 30).ECG, SpO2, PETCO2, HR and neuromuscular blockade were monitored. Intubation conditions were assessed when T1 was zero after cisatracurium besylate administer ration using a 4-part scale. The onset time, TOF no reaction time, duration time, recovery lime and hemodynamic response time were recorded. Re-sults The onset times of group Ⅰ3, Ⅱ3 and Ⅲ3 were significantly shorter than those of group Ⅰ2, Ⅱ2 and Ⅲ2 (P < 0.05).The percentages of patients with excellent intubation condition in group Ⅰ3, Ⅱ3 and Ⅲ3 were 93%, 90%, and 80%, which were markedly higher than those in group Ⅰ2, Ⅱ2 and Ⅲ2 (67%, 60%, 60%). TOF no reaction time and duration time were longer in group Ⅰ3, Ⅱ3 and Ⅲ3 than those in group Ⅰ2, Ⅱ2 and Ⅲ2. There was no significant difference in muscle chalasis among the six groups. Conclusions Cisatracuri-um besylate 0.15 mg/kg produces quicker onset time and acceptable intubation conditions in the different age patients than 0.10 mg/kg. Recovery time is independent on the dosage and age. Aging has no influence on the phannacodynamics of single injection of cisatracurium besylate.

Objective:To evaluate arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors in the treatment of tibial eminence fracture. Methods:The 23 patients were retrospectively analyzed who had been treated at Department of Orthopedics, Beiing Shunyi District Hospital for tibial eminence fractures by arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors from October 2015 to December 2019. They were 15 males and 8 females, aged from 12 to 55 years (average, 35.5 years). All the injuries were unilateral (14 right and 9 left sides). According to the modified Meyers-McKeever classification, 2 cases belonged to type Ⅱ, 18 cases to type Ⅲ and 3 cases to type Ⅳ. Range of motion of the knee, Lysholm and International Knee Documentation Committee (IKDC) scores were observed before surgery, 1 month and 12 months after surgery. The anterior tibial slope angle (ATSA) on CT was measured preoperatively and 1 month after surgery for evaluation of fracture reduction. Results:All the 23 patients were followed up for an average of 23 months (from 12 to 52 months). Postoperatively, limited knee movement was observed in 2 patients and non-anatomic reduction in one patient. At 1 month and 12 months after operation, the Lysholm scores (61.4 ± 3.5 and 90.4 ± 4.3) and IKDC scores (69.6 ± 4.2 and 88.5 ± 3.0) were significantly improved compared with the preoperative values (45.4 ± 6.8 and 49.6 ± 3.9, respectively) ( P<0.05). ATSA was significantly restored from preoperative 4.2° ± 5.7° to -11.7° ± 2.9° at 1 month after operation ( P<0.05). Conclusion:In the treatment of tibial eminence fracture, arthroscopy-assisted tibial bone tunnel fixation combined with double-row anchors can achieve anatomical reduction and firm fixation, leading to satisfactory surgical outcomes.

The changes of Fas.FasL and Bcl-2 expression in thyrocytes of patients with Graves'disease were investigated before and 2 weeks after 131Ⅰ administration. The results showed that 131Ⅰ couhl induce thyrocytes to express the apoptotic protein Fas, FasL and the anti-apoptotic protein Bet-2 in patients with Graves'disease. A statistically significant correlation was found between the dose of 131Ⅰ and the expression levels of Fas and FasL but not Bet-2 ,suggesting that early onset of hypothyroid after 131Ⅰ administration may be due to the increased expression of Fas and FasL in thyrocytes.

Objective To observe the effects of hyperbaric oxygen (HBO) on learning and memory ability and on the level of myelin basic protein (MBP) in the brain tissue of rats with delayed neurological sequelae (DNS) after acute carbon monoxide poisoning (COP).Methods Forty-eight male SD rats had their cognitive performance assessed with the Morris water maze. After basic training with the Morris water maze and screening, the rats were randomly divided intoanormalcontrol (NC) group (n=11),a COP group (n=17) and an HBO group (n=17).Pure CO gas was injected intraperitoneally to establish acute COP in the latter two groups. The NC group received a similar injection of air.The HBO group was given HBO therapy. The rats in each group were tested for changes in their learning and memory abilities using the Morris water maze.On the 21 st day after the treatment, paraffin tissue sections of the rats' brains were subjected to immunohistochemical (IHC) examination and Western blotting (WB) was used to detect any expression of MBP in the brain tissue. Results After 21 days, morbidity among the COP group was 64.3%,while it was 26.7% in the HBO group,a significant difference. The average maze escape latency in the COP group was significantly longer than in the HBO group.IHC staining and Western blotting showed that MBP in the hippocampal tissue of the COP group was significantly lower than that in the HBO group. In gray scale comparisons of the rats' brain tissue, that from the NC group was significantly better than that from the COP and HBO groups, but that from the HBO group was significantly better than that from the COP group. Conclusion HBO can effectively reduce DNS after acute COP,mitigate the severity of DNS,reduce demyelination of brain tissue and thus play an important role in protecting brain cells.

Objective To summarize the clinical characteristics and outcome of renal cyst infection in patients with autosomal dominant polycystic kidney disease (ADPKD). Methods Clinical data of 40 ADPKD patients with 43 episodes of renal cyst infection admitted in Shanghai Changzheng Hospital from 1st January 1991 to 31st December 2010 were retrospectively analyzed.Differences of microbiological data and treatments between 1st January 1991 to 31st December 2000 and 1st January 2001 to 31st December 2010 were compared. Results Among 473 identified patients with ADPKD and 662 episodes of hospitalization,40 patients had 43 episodes of renal cyst infection, including 8 definite and 35 likely cases.Microbiological documentation was available for 34 episodes (79.0%),Escherichia coli accounting for 82.4% of all retrieved bacterial strains.Resistant Escherichia coli to quinolone and certain β-lactamine increased in recent decade.Clinical efficacy of initial antibiotic treatment was noted in 69.8% of episodes. Antibiotic treatment modification was more frequently required for patients receiving initial monotherapy compared with those receiving combination therapy. In the first ten-year group, initial combination therapy and clinical efficacy were noted in 30.0% and 60.0% of episodes respectively, and hospital stay was (20.2±6.7) d.In the second ten-year group, initial combination therapy and clinical efficacy were noted in 61.9% and 78.2% of episodes respectively, and hospital stay was (16.3±3.2) d.Large infected cysts (diameter >5 cm) frequently required drainage. Conclusions In renal cyst infection, the source of the organisms is often a gram negative enteric organism.Empiric therapy is often initiated with two antibiotics. The drainage of large infected cysts remains the main treatment for cyst infection.
