1.Effect of Stroke Unit Therapy on Cognitive Impairment in the Cerebral Infarction Patients

Objective To evaluate the effect of stroke unit therapy on cognitive impairment in the cerebral infarction patients.Methods 96 patients with cognitive impairment were divided into two groups: the stroke unit treatment(treatment groups, 52 cases) and routine treatment (control group, 44 cases). Cognitive function examinations were performed in two groups before and 2 months after treatment.Results Comparing with control group, the score of Mini-Mental State Examination(MMSE), span of digit and 100 to 1 counting were significantly different in treatment group. Conclusion Stroke unit is effective to improve cognitive function of cerebral infarction patients.
2.Tensile mechanical properties of lateral collateral ligaments in animal models of osteoarthritis

BACKGROUND:It is very nscessary for the prevention and treatment of ostaoarthritis(OA)to understand the tensile mechanical properties of lateral collateral ligaments(LCLs)of knee joints with osteoarthritis,while reports on it are still few.OBJECTIVE:To compare the LCL tensile mechanical property indexes of knee joints between normal animals and OA medels established for this purpose,and to provide qualitative and quantitative referential data about effects of OA on the LCL tensile mechanical properties of animal knee joints.DESlGN,TIME AND SETTING:A randomized controlled trial was finished at the Mechanical Expedmental Center of Jilin University from December 20th to 30th in 2008.MATERIALS:A total of 20 male SD rats of 6 months old were divided into a normal control group and a model group,with 10ones in each group.METHOOS:Rats in the model group were copied into OA models.after which test on tensile was performed to each of the ten rats in the two groups at the speed of 5mm/min on Universal Testing Machine,Shimadzu.Data obtained from the test were processed in polynomial though least square method.MAIN OUTCOME MEASURES:Tensile maximum load.maximum displacement,maximum stress,maximum strain and stress-strain curves.RESULTS:The tensile maximum load,maximum stress,maximum displacement and maximum strain in the normal control group and the model group were(12.754±2.795)N,(27.681±5.832)MPa,(2.754±0.707)mm,(11.679±2.373)%and (7.183±1.817)N,(16.162±3.403)MPa,(1.827±0.768)mm,(8.019±2.811)% respectively,from which we could see that each index in the normal control group was greater than that in the model group obviously (P<0.05).CONCLUSION:The tensile maximum load,maximum stress,maximum displacement and maximum strain all decreased after OA,which indicates that osteoarthritis has certain effects on tensile mechanical properties of LCLs of knee joints as well as stress relaxation.
3.The Influence of Different Blood Pressure Circadian Rhythm on Cardiac Function in Patients With Hypertension

Objective: To investigate the inlfuence of different blood pressure (BP) circadian rhythm on cardiac function in hypertensive patients.
Methods: A total of 180 hypertensive patients with well controlled day-time blood pressure were divided into 3 groups. Dipper group, n=30, Non-dipper group, n=99 and Reverse dipper group, n=51. The relationship between cardiac function, relevant clinical index and blood pressure circadian rhythm were analyzed.
Results: The mean systolic and diastolic BP, BP classiifcation and the antihypertensive medication were similar among 3 groups.①The ratio of peak mitral E wave to peak mitral annulus E' wave (E/E') increased accordingly from Dipper group, Non-dipper group and Reverse-dipper group as (8.1 ± 2.4), (8.6 ± 2.5) and (9.6 ± 3.2), P<0.05. ②Left atrial volume index (LAVi) in Reverse-dipper and Non-dipper groups were higher than that in Dipper group as (43.8 ± 11.8) ml/m2, (40.7 ± 9.8) ml/m2 and (36.6 ± 8.5) ml/m2, P<0.05 or P<0.01. ③E/E' was positively related to nocturnal systolic BP and age, negatively related to nocturnal BP decline rate (nocturnal BPR); E/E' was independently related to age and BPR. LAVi was positively related to nocturnal systolic BP, pulse pressure, age and hypertension course, negatively related to the mean diastolic BP, day-time diastolic BP and BPR; LAVi was independently related to BPR, pulse pressure and hypertension course. Left ventricular ejection fraction had no statistic differences among 3 groups.
Conclusion: Abnormal blood pressure circadian rhythm was associated with the cardiac injury in hypertensive patients. The cardiac injury increased accordingly from Non-dipper group to Reverse-dipper group and the main damage was in diastolic function.
4.The relevance between single nucleotide polymorphism in the downstream of Toll-like receptor signal transduction pathway and rheumatoid arthritis susceptible genes

Objective To study the correlation between single nucleotide polymorphism (SNP) of downstream mononucleotide in signal transduction of Toll-like receptors and predisposing genes of rheumatoid arthritis (RA).Methods One hundred and sixty-two RA patients were selected from northern part of Han people in China,and 188 healthy subjects were enrolled as normal controls.Rs7744 of Myd88,rs5030445 of TRAF6,rs11265618 and rs4845626 of IL-6R were analyzed.The data of genotypic frequency and allele genotypic frequency were statistical analyzed by x2 test between the two groups.Results The difference of allele frequency of TRAF6 rs5030445 between the two groups were remarkable (x2=5.716,P<0.05),and so did the IL-6R rs11265618 (x2=5.704,P<0.05).But the difference of genotypic frequency was not statistically significant for locus of Myd88 rs7744,TRAF6 rs5030445 and IL-6R rs11265618 (P>0.05).And the differenceof allele frequency and genotypic frequency between the two groups had no statistical significance in locus of Myd88 rs7744 and IL-6R rs4845626 (P>0.05).Conclusion There is significant correlation between single nucleotide polymorphism and predisposing genes of RA.The G allele of TRAF6 rs5030445 gene locus is a possible predisposing gene for RA.The T allele of IL-6R rs11265618 is a possible predisposing gene of RA.
5.Correlation of radius and respiration variation of inferior vena cava with central venous pressure in elderly patiens with septic shock

Objective To explore the correlation of the radius and respiratory variation of inferior vena cava(IVC)with central venous pressure(CVP)for rapid evaluation of blood volume with ultrasound in elderly patients with septic shock.Methods The radius of IVC was measured using bedside ultrasound,respiration variation index(RVI)was calculated as following:RVI =(maximum radius-minimum radius)/maximum radius × 100% and central venous pressure(CVP)was also recorded in 28 elderly patients with septic shock before and during 2 h and 6 h fluid recovery.Radius and RVI of IVC were compared between 28 shock patients and 22 healthy volunteers as control.Correlation of radius and RVI of IVC with CVP were analyzed.The thresholds of radius and RVI of IVC to estimate CVP 8 mmHg were determined by Receiver Operator Characteristic Curve (ROC)curves.Results The maximum and minimum radius[(1.23±0.28)cm and(0.48±0.18)cm]in the elderly patients with septic shock were smaller than in control group[(1.95±0.14)cm and (1.73±0.13)cm].RVI in the elderly patients with septic shock were larger than in control group [(55.88±11.18)% vs.(11.23± 1.82)%].The maximum and minimum radius were positively(r=0.668 and 0.863,both P<0.01)and RVI negatively(r=-0.848,P<0.01)with CVP.The thresholds of maximum radius,minimum radius and RVI of IVC to estimate CVP 8 mmHg were 1.56cm(sensitivity 85.2%,specificity 86.3%),1.13 cm(sensitivity 96.3%,specificity 94.1%)and 30%(sensitivity 88.2%,specificity 96.3%),respectively.Conclusions Using ultrasound to measure radius of IVC and calculate RVI might estimate CVP to certain degree.It might be an option for physicians to rapidly estimate blood volume in the elderly patients with septic shock.
6.Correlation between radius and respiration variation of inferior vena cava and hemodynamicmonitoring values of pulse-indicated continuous cardiac output(PiCCO)in septic shock pigs

Objective To explore the correlation between radius and respiratory variation of inferior vena cava(IVC)and hemodynamic monitoring values of pulse-indicated continuous cardiac output(PiCCO)in septic shock pigs.Methods A total of 8 pigs were used to establish animal model of septic shock by intravenous infusing LPS(100 μg/kg),and fluid resuscitation was followed with normal saline.Ultrasound was used to measure the maximum radius(IVCmax)and minimum radius(IVCmin)of IVC,and respiration variation index(RVI)was calculated at basic status,septic shock,1 hour and 6 hours after fluid resuscitation,respectively.Respiratory variation index of IVC were calculated as:RVI =(IVCmax-IVCmin)/ IVCmax × 100%.Hemodynamic monitoring values,including ITBV,GEDV,SVV and CI of PiCCO,were recorded at the same time.Radius and RVI of IVC and PiCCO values between before and after fluid resuscitation were compared by LSD-t test.Correlation between radius and RVI of IVC andhemodynamic monitoring values were calculated by Pearson correlation coefficient.Results Compared with the moment of septic shock,IVC IVCmin,GEDV,ITBV and CI at 1 after hour fluid resuscitation were larger(P < 0.01)and SVV and IVCrvi were smaller(P < 0.01).Compared with the moment of septic shock and 1 after hour fluid resuscitation,IVC[VCmin,GEDV,[TBV and CI at 6 hours after fluid resuscitation were larger(P < 0.01)and SVV and IVCrvi were smaller(P < 0.01).IVCmax correlated with SVV(P=0.024)and it failed to correlate with GEDV,ITBV and CI.IVCmin correlated with GEDV(P=0.003),ITBV(P =0.001),SVV(P =0.009)and CI(P =0.015),respectively.RVI was correlated withGEDV(P<0.01),ITBV(P<0.01),SVV(P=0.007)and CI(P<0.001),respectively.Conclusions Radius and RVI of IVC was correlated with hemodynamic monitoring values of PiCCO.It can serve as a parameter to rapidly estimate the blood volume.
7.Review of prevention and treatment on influenza A (H1N1) with traditional Chinese medicine.

The influenza A (H1N1) broke out in Mexico and the USA in March 2009, which soon spread over the world in coming months. This paper comprehensively analyzed the work of prevention and treatment to Influenza A (H1N1) by using traditional Chinese medicine in recent years, and briefly introduced the research of Influenza A (H1N1), specially summarized the possibility of using theory of TCM for predicting epidemic disease and the relationship between traditional Chinese medicine and influenza A (H1N1). Meanwhile some suggestions are made on the future work of prevention and treatment to Influenza A (H1N1) by using traditional Chinese medicine.

8.Experimental research on degradation and biocompatibility of super-high-molecular-weight poly-DL-lactic acid.

OBJECTIVEThe super-high-molecular-weight poly-DL-lactic acid (PDLLA), with the molecular weight of 900 kD, is a newly emerging biomaterial and potentially used in the therapy of bone fracture because of its excellent mechanical property. However the biocompatibility of this material has not been reported so far, therefore this experiment was designed to examine whether the super-high-molecular-weight PDLLA was harmful to creatures, when it was implanted in the body of animals for a long period.
METHODSThe material was prepared in small cuboids, with the size of 1.0 mm x 1.5 mm x 2.0 mm, and these blocks were implanted into the masseteric space of SD rats and, the activity of the SD-rats was monitored continuously. The animals were sacrificed in the 3rd, 6th, 9th, 12th months after the operation and, the specimens were taken out from the animals. The examination included anatomical, pathological and haematological methods. The data were analyzed with SPSS 8.0.
RESULTSThe wound healed well after the operation. Super-high-molecular-weight PDLLA degraded 6 months after the implantation. In the 3rd month after the operation, a thin fiber membrane around the materials was formed. In the 6th month, the membrane was much thinner than that in the 3rd month and completely disappeared in the 9th month. The pathological examination showed that slightly inflammatory reaction appeared in the tissue around these blocks in the 3rd month, but the inflammatory reactions were gradually remitted in the following 6th, 9th and 12th months. Further, the haematological examination did not show any abnormity during the 12-month observation period.
CONCLUSIONThe super-high-molecular-weight PDLLA can be degrade when it is implanted into the body of creatures, which proves its good biocompatibility.

9.Five kinds of vitrified cryoprotectants:toxicity of their alone or combination to nucleus pulposus cells

BACKGROUND:Transplantation of alogeneic intervertebral disc can be facilitated by the cryopreservation of the intervertebral disc. But the traditional cryopreservation methods always lead to the appearing of ice crystals inside and outside the cels which can cause celular injury. The vitrification method that can avoid the formation of ice crystals have been widely applied in the cryopreservation field. However, only a few reports have assessed the vitrified cryopreservation of the intervertebral disc, and the toxicity of cryoprotectants to the nucleus pulposus cels have not been fuly explored.
OBJECTIVE:To determine the order of toxicity of five commonly used cryoprotectants that are used alone or in combination to rabbit nucleus pulposus cels, and to select the optimal cryoprotectant for the vitrification of the intervertebral disc.
METHODS: We chose five most commonly used cryoprotectants including dimethyl sulphoxide, formamide, ethylene glycol, propylene glycol and glycerol. Then, 5 single commonly used cryoprotectants, 10 mixed agents containing any 2 commonly used cryoprotectants, and 10 mixed agents containing any 3 commonly used cryoprotectants were formulated. Cel viability of nucleus pulposus cels was determined using cel counting kit-8 and fluorescein diacetate/propidium iodide method. Al data obtained were analyzed statisticaly to choose the appropriate combining scheme with less toxicity.
RESULTS AND CONCLUSION: The order of the toxicity of these five commonly used cryoprotectants from low to high was ethylene glycol, glycerol, formamide, dimethyl sulphoxide, and propylene glycol. The toxicity of the combined agents containing two or three commonly used cryoprotectants was lower than that of any commonly used cryoprotectants that were used to formulate them. The toxicity of the mixed agents that contained ethylene glycol or glycerol was lower than that of any other mixed agents. So we can choose the mixed cryoprotectants that contain ethylene glycol and (or) glycerol for the vitrification of the intervertebral disc.
10.The effect of sevoflurane combined with remifentanil on anesthetic effect and stress indexes in patients undergoing laparoscopic hysterectomy

Objective:To investigate the effect of conducting general anesthesia with sevoflurane combined with remifentanil on the anesthetic effect and stress indexes in patients undergoing laparoscopic hysterectomy.Methods:A total of 80 patients undergoing laparoscopic hysterectomy in the Affiliated Hospital of Jining Medical College from April 2020 to August 2020 were selected and divided into the observation group and the control group according of the number table method, with 40 cases in each group. In the control group, sufentanil, propofol and rocuronium were used to induce anesthesia, and propofol and remifentanil were intravenously administered during anesthesia maintenance.The observation group received the same induction anesthesia, followed by 2% sevoflurane inhalation combined with remifentanil to maintain anesthesia. The anesthetic effect, stress indexes at different postoperative time points and adverse reaction between the two groups were compared.Results:The scores of mini mental state evaluation (MMSE) at 0.5, 1 and 2 h after the surgery and the scores of observer′sassessment of alertness/sedation scale (OAA/S) atimmediately after extubation, 1 and 2 h after surgery in the observation group were higher than those in the control group, there were statistical differences ( P<0.05). The wake up time and extubation time in the observation group were shorter than those in the control group: (9.22 ± 1.67) min vs. (15.94 ± 1.44) min, (10.34 ± 1.46) min vs. (17.11 ± 1.33) min, there were statistical differences ( P<0.01). The systolic blood pressure, diastolic blood pressure, epinephrine and cortisol at 10 min after intubation and at the end of surgery in the observation group were lower than those in the control group, there were statistical differences ( P<0.05). Conclusions:Sevoflurane combined with remifentanil conducting anesthesia can better improve the cognitive function of patients, shorten the recovery time of postoperative consciousness, and reduce the intraoperative stress response.
