1.Effect of Naoxintong Capsule on the Myocardial Perfusion and Platelet Activation Function in Patients with Acute Myocardial Infarction after PCI

OBJECTIVE:To explore the effect of Buchang naoxintong capsule on the myocardial perfusion and platelet activation function in patients with acute myocardial infarction (AMI) after PCI. METHODS:100 AMI patients were randomly divided into control group and observation group by parallel random sampling,50 cases in each group. Control group was given conventional PCI, preoperatively chewing 300 mg aspirin+300 mg clopidogrel,postoperatively taking 100 mg/d aspirin+75 mg/d clopidogrel. Observation group was additionally treated 2-4 with Buchang naoxintong capsules(after bracket release),3 times a day,the treatment course for both groups was 7 d,observation period was 1 month. Infarct-related artery myocardial perfusion grade(TMPG)in 2 groups,platelet activation function indexes [CD62P,CD63 and monocyte platelet aggregation (MPA) positive expression rates] before and after treatment,the incidences of adverse cardiac events (MACE) and complications were observed and compared 15 min after PCI. Meanwhile,the incidence of adverse reactions was recorded. RESULTS:After treatment,the cases of TMPG perfusion grade 3 in observation group were significantly higher than control group,the incidence of MACE was significantly lower than control group, with statistical significance(P<0.05);there was no significant difference in the incidences of complications and adverse reactions in 2 groups(P>0.05). Before treatment,there was no obvious difference in CD62P,CD63,MPA positive expression rates in 2 groups;after treatment,CD62P,CD63,MPA positive expression rates in 2 group were significantly lower than before,and observation group was lower than control group,with significant differences(P<0.05). CONCLUSIONS:In PCI treatment of AMI patients,Buchang naoxintong capsule can rapidly improve platelet activation functions,TMPG perfusion,reduce the incidence of MACE,with good safety and no increase of complications.
2.Epidemic characteristics of anthrax in Chengde City, Hebei Province from 2005 to 2021

ObjectiveTo determine the epidemic characteristics of anthrax in Chengde City, Hebei Province from 2005 to 2021, and to provide evidence for formulating prevention and control measures of anthrax and reducing incidence rate. MethodsThis study collected the data of anthrax epidemic in Chengde City and conducted descriptive analysis. ResultsFrom 2005 to 2021, a total of 11 anthrax cases were reported in Chengde City with no death. The average incidence rate was 0.08/105, which remained low. Furthermore, 10 cases were cutaneous anthrax and 1 case was pulmonary anthrax. The cases were mainly reported in Weichang County, accounting for 90.91% of the total reported cases, followed by Pingquan City. In addition, the cases were mainly reported from July to August and mainly between 30 and 59 years old with a gender ratio of 2.67∶1. ConclusionThe anthrax epidemic in Chengde City is likely to increase. It mainly occurs in summer, rural areas, and male young and middle-aged farmers. It is necessary to improve epidemic monitoring, health education, disease prevention capacity, early identification of the epidemic, and active response.
3.Effect of Modified Bazhentang on Nutritional Status and Immune Function of Gastric Cancer Patients with Neoadjuvant Chemotherapy

Objective:To observe the effect of modified Bazhentang on the nutritional status and immune function of patients with Qi and blood deficiency syndrome in neoadjuvant chemotherapy (NAC) for gastric cancer. Method:One hundred and ten patients were randomly divided into observation group and control group with 55 cases each. Both groups accepted FOLFOX6 protocol. Patients in control group took Jianpi Shengxue tablets orally, 3 tablets/time, 3 times/day. Patients in observation group received modified Bazhentang, 1 dose/day. The course of treatment was six weeks in both groups. Before and after treatment, scores were graded according to patient generated-subjective global assessment (PG-SGA), Qi and blood deficiency syndrome, and the Revised Piper Fatigue Scale (PFS-R). Levels of serum total protein (TB), albumin (ALB), prealbumin (PAB), CD4+, CD8+, helper T lymphocyte 17 (Th17), regulatory T cell (Treg), immunoglobulin G (IgG), IgM, and IgA were detected before and after therapy. With the continuous advancement of health informatization and the wide application of medical big data, electronic health records came into being and spread rapidly. However, because electronic health records contain a large amount of private information, privacy protection is the primary consideration for the sustainable development of electronic health records. By analyzing the shortcomings of privacy protection of electronic health records in law, technology, management and protection consciousness, this paper put forward some countermeasures, such as perfecting the relevant laws and regulations of privacy protection of electronic health records, improving the technical level, improving the management defects of electronic health records, and cultivating the privacy protection consciousness of professionals and the public, so as to improve the overall privacy protection level of China’s health records information management system and provide effective protection for the privacy information of Chinese residents’ electronic health records.
5. Clinical research of transcatheter arterial chemoembolization combined with three dimensional conformal radiotherapy and hyperthermia for primary hepatic carcinoma

Objective:
To investigate the effect of transcatheter arterial chemoembolization(TACE) combined with three dimensional conformal radiotherapy(3-DCRT) and hyperthermia on inoperable primary hepatic carcinoma.
Methods:
This study retrospectively analyzed 96 patients with hepatic carcinoma in the Third Hospital of Chengde between 2014 and 2017 who received TACE or combined therapy (TACE combined with 3-DCRT and hyperthermia). Kaplan-Meier curve was used to compare the overall survival(OS) of the two groups.
Results:
There were 50 cases in the combined therapy group, 46 cases in the TACE group.The effective rate of the combined therapy group was 76.1%, and that of TACE group was 45.6%, the difference was statistically significant(

Objective To explore the predictive value of the ratio of C-reactive protein to high-density lipoprotein cholesterol (CHR) for carotid plaque vulnerability in patients with ischemic stroke.Methods A retrospectively enrolled 349 patients with acute ischemic stroke in the Department of Neurology of Baoding First Central Hospital from January 2019 to January 2020.All subjects underwent ultrasound examination to evaluate the condition of carotid artery plaques,and based on the ultrasound results,they were divided into no plaque group,stable plaque group,unstable plaque group.The differences in risk factors,blood lipids,and inflammatory indicators in each group were compared.The receiver operating characteristic (ROC) curve was used to estimate the predictive value of each index on the vulnerability of carotid plaque.Results C-reactive protein and CHR of the unstable plaque group were higher than those of the stable plaque group,and the difference was statistically significant (P<0.05).ROC curve shows that CHR can predict carotid plaque instability more reliably than a single C-reactive protein.Conclusion CHR has high predictive value for carotid artery vulnerable plaque and can be used as a new circulating biomarker to identify carotid artery plaque vulnerability.
7.Correlation between the trigiuceride-glucose index and early neurological deterioration in patients with mild ischemic stroke

To investigate the correlation between the trigiuceride-glucose index and early neurological deterioration(END) in patients with mild ischemic stroke(MIS). Methods This study was retrospective. Patients with acute MIS who were admitted to the Department of Neurology of the First Central Hospital of Baoding from January 2019 to December 2020 for the first time and within 72 h of onset were consecutively included. The National Institute of Health Stroke Scale(NIHSS) scores were recorded at the time of admission,and fasting venous blood was drawn in the early morning of the following day to determine routine blood,liver function,renal function,lipids and other relevant blood indices,and the trigiuceride-glucose index was calculated ln[TG(mg/dl)×FBG(mg/dl)]/2.The patients were divided into END and non-END groups according to their neurological deterioration. Multi-factor logistic regression was used to analyze the factors associated with the occurrence of END in acute MIS patients,and the subject operating characteristic curve(ROC) was used to evaluate the predictive effect of the trigiuceride-glucose index on END in acute MIS patients. Results A final total of 149 patients with acute MIS were included,39 in the END group and 110 in the non-END group. Multi-factor logistic regression analysis showed that neutrophil ratio(OR 1.054,95%CI 1.009~1.101,P=0.017)、TBIL(OR 0.718,95%CI 0.611~0.845,P<0.001)and trigiuceride-glucose index(OR 5.072,95%CI 1.645~15.640,P=0.005) were independent risk factors for the development of END in patients with acute MIS. The ROC curve analysis showed that the diagnostic cut-off value of trigiuceride-glucose index for END was 1.74 with a sensitivity of 53.8% and specificity of 81.8%. Conclusion Elevated trigiuceride-glucose index is one of the risk factors for the development of END in patients with acute MIS and has a predictive role for the development of END in patients with acute MIS.

Chorioretinal disease has become a significant problem affecting human vision. Abnormal expression of vascular endothelial growth factor(VEGF)leads to increased fundus permeability and neovascularization. Vitreous injection of anti-VEGF agents can rapidly inhibit the level of VEGF in the eye and effectively control the development of the disease. At present, anti-VEGF agents are widely administered in ophthalmology. However, studies have shown that intravitreal anti-VEGF agents can reduce plasma VEGF concentration after entering the circulatory system, and the pointless off-target effects of drugs may lead to systemic adverse reactions. For elderly patients, patients with serious comorbidities, lactating women, premature infants and other special populations, attention should be paid to systemic VEGF inhibition after multiple injections. In this paper, in order to provide reference for clinical anti-VEGF therapy, the pharmacokinetics therapy, systemic adverse reactions, contralateral eye effects, and effects of anti-VEGF on breast milk and preterm infants were discussed, and the systemic effects of vitreous injection of anti-VEGF agents were reviewed.
9.Study on in vitro dissolution and in vivo pharmacokinetics of Luteolin solid dispersion

OBJECTIVE To explore in vitro dissolution and in vivo pharmacokinetics of Luteolin solid dispersion in Beagle dogs. METHODS The dissolution of Luteolin solid dispersion was investigated according to the second method (paddle method) of the “dissolution determination method” in the 2020 edition of Chinese Pharmacopoeia (Part Ⅳ). UPLC-MS/MS method was established to determine the concentration of luteolin in the plasma of Beagle dogs. Twelve Beagle dogs were randomly divided into luteolin group and Luteolin solid dispersion group, with 6 dogs in each group. They were given relevant medicine orally at the dose of 10 mg/kg luteolin. Blood was collected before medication (0 h), at 5, 10, 15, 30, 45 min and 1, 2, 4, 6, 8, 10, 12, 24, 48 h after administration. After protein precipitation with acetonitrile, the blood concentration of luteolin in Beagle dogs was determined by UPLC-MS/MS and the major pharmacokinetic parameters were calculated with non-compartmental models by using DAS 3.2.8 pharmacokinetic software. RESULTS The dissolutions of Luteolin solid dispersion in purified water and 0.1% sodium dodecyl sulfate solution was significantly higher than those of luteolin; the dissolution rate reached 95% in 0.1% sodium dodecyl sulfate solution for 120 minutes. The peak concentration (cmax) of luteolin in the Luteolin solid dispersion group of Beagle dogs was 5.62 times higher than the luteolin group, and the relative bioavailability was 348%. Compared with luteolin group, cmax and the area under the drug time curve of luteolin in the Luteolin solid dispersion group of Beagle dogs were significantly increased, while the apparent distribution volume was significantly reduced (P<0.05). CONCLUSIONS Luteolin solid dispersion can improve in vitro dissolution and bioavailability of luteolin in Beagle dogs.
10.Effect of moxibustion at "oppositely-located points" on neurogenic bladder after spinal cord injury and endoplasmic reticulum stress pathway in rats.

OBJECTIVE:
To observe the effect of moxibustion at oppositely-located points "Mingmen" (GV 4) and "Shenque" (CV 8) on the motor function of the hind limbs and bladder function in rats with neurogenic bladder after suprasacral spinal cord injury (SCI), so as to explore the effect of this therapy on bladder tissue apoptosis mediated by endoplasmic reticulum stress pathway.
METHODS:
Twenty-eight female Wistar rats were randomly divided into a sham-operation group (8 rats) and a model establishment group (20 rats). Using the modified Allen's method, the spinal cord of T10 segment was injured to establish a neurogenic bladder model in the model establishment group. Sixteen rats were modeled successfully and then divided into a model group (8 rats) and a moxibustion group (8 rats). In the moxibustion group, 2 h after consciousness regaining from modeling anesthesia, moxibustion was exerted at "Shenque" (CV 8) and "Mingmen" (GV 4), 2 cones at each acupoint in one intervention. The intervention was administered once every two days and 5-time intervention was required totally. After intervention, Basso, Beattie and Bresnahan locomotor rating scale (BBB) score for the motor function of the hind limbs, and the urodynamics indexes (maximum bladder capacity, urine leakage pressure and bladder compliance) were compared among groups. HE staining method was adopted to observe the morphological changes of bladder tissue. With Western blot method and real-time PCR assay, the protein and mRNA expressions of the endoplasmic reticulum stress-related genes (glucose- regulated protein 78 [GRP78], activating transcription factor 4 [ATF4] and cysteinyl aspartate specific proteinase-12 [Caspase-12]) were determined.
RESULTS:
The transitional epithelial cells were arranged irregularly, the bladder wall was getting thinner, and the cellular vacuolar degeneration and neutrophil infiltration were found in the model group. Whereas, compared with the model group, in the moxibustion group, the arrangement of transitional epithelial cells was clear and continuous in layers, the cellular vacuolar degeneration was mild and the infiltration presented in a small amount of neutrophil granulocytes. Compared with the sham-operation group, in the model group, the BBB score was reduced (P<0.01), the maximum bladder capacity and bladder compliance were increased (P<0.01), and the protein expression levels of GRP78, ATF4 and Caspase-12, as well as mRNA expressions were all increased (P<0.01). In comparison with the model group, in the moxibustion group, BBB score was increased (P<0.01), the maximum bladder capacity and bladder compliance were decreased (P<0.01), and the protein and mRNA expression levels of GRP78, ATF4 and Caspase-12 were all decreased (P<0.01).
CONCLUSION
Moxibustion at the "oppositely-located points" improves the urination function, alleviate urine retention in neurogenic bladder rats after spinal cord injury. The underlying mechanism may be related to the down-regulation of the expressions of GRP78, ATF4 and Caspase-12 in the endoplasmic reticulum stress pathway of the bladder tissues, and thus to alleviate the apoptosis of bladder tissue.
