2.Comparative study of microsurgical lumbar discectomy and microendoscopic discectomy

[Objective]Microsurgical lumbar discectomy(MSLD)and microendoscopic discoectomy(MED)were compared in methods and curative effect for providing the experience and therapy evidence of lumbar disc herniation(LDH).[Method]It was retrospectively analyzed that mono-segment lumbar disc herniation were treated in minimal invasion in our hospital.MSLD was 45 cases and MED was 32 cases.Operation time,operation information,complication,hospital time and curative effect were compared.[Result]The satisfactory rate of two groups were both beyond 90% and no significant deviation was found.The incision of MED was obviously shorter than MSLD and the operation time of MED was longer than latter.[Conclusion]The curative effects of two minimal invasion methods are satisfactory.But the indication of MED is limited obviously and the method has not obvious predominance by compared with MSLD.MSLD is an more ideal minimal invasion operation at present.
3.Experimental study on repair of cartilage defects with combined BMP/bFGF biomaterial

[Objective]To study the repair of articular cartilage defects with combined bone morphogenetic protein(BMP)/basic fibroblast growth factor(bFGF)biomaterial in order to supply experimental basis for repairing cartilage defects biologically.[Method]Twenty-four 14-week-old rabbits were randomly divided into four groups,BMP/bFGF biomaterial gel(group A),BMP biomaterial gel(group B),bFGF biomaterial gel(group C)and simple fibrin glue treated group(group D).A couple of knees of each rabbit experienced 5 mm-diameter-full cartilage resection and drilling throuth of bone marrow.Gross appearance,histologicol section and electron microscope were axamined at 8,12,24 weeks after operation.[Result]In group A the cartilage defects were smoothly repaired by white translucent hard tissue at 8 and 12 weeks,and defect boundary was hard to be indentified with normal cartilage at 24 weks.No smooth repairing was observered in B,C and D groups.Group A got a better histological score than B,C and group D(P0.05).[Conclusion]Combined BMP/bFGF biomaterial is good for repair of articular cartilage defects and has better result than using BMP or bFGF alone.These results may provide a therapy for articular cartilage defects.
4.Lateral patellar retinacular release combined with decomposition:Repair without damage to the patellar cartilage surface

BACKGROUND:Excessive lateral pressure syndrome is often associated with lateral retinacular tension and radiographic patel ar tilt. CT scan displayed that lateral retinacular release can effectively correct patel ar tilt. OBJECTIVE:To study the effect of arthroscopic lateral retinacular release combined with intraosseous dril ing and decomposition in the treatment of excessive lateral pressure syndrome. METHODS:Thirty-two patients with excessive lateral pressure syndrome were treated by arthroscopic lateral release combined with intraosseous dril ing and decomposition. The Lysholm scoring system was used to evaluate the treatment effect. RESULTS AND CONCLUSION:The mean duration of fol ow-up was 12 months. After 1 month, pain of al patients was released or disappeared;after 1 year, pain of 26 cases disappeared basical y. Lysholm scoring system assessment showed 20 cases were rated as excellent, six cases were as good, four cases were as fair and two cases were as poor. The excellent and good rate was 83.6%. The patients’ subjective satisfaction rate was 92.8%. The results indicate that arthroscopic lateral release combined with intraosseous dril ing and decomposition is a good method to treat excessive lateral pressure syndromewere. It has the advantages of less trauma and rapid recovery. Patel ar decomposition has a good effect in the treatment of patel ofemoral pain associated with patel ar tilt outward and lightens articular cartilage degeneration without damage to patel ar cartilage surface.
5.Overproduction of nitrate and S-nitrosothiols in diabetic patients

Objective The present study was designed to investigate changes in serum or plasma concentrations of nitric oxide and its derivatives in diabetic patients.Methods Serum nitrate concentration of 84 diabetic patients was measured by using an enzyme kinetic method,and the plasma S-nitrosothiols concentration of 10 cases was measured by using HPLC technique.Results Serum nitrate concentration and plasma S-nitrosothiols concentration in the diabetics were significantly higher than in control group (P<0.01 andP<0.05,respectively).The serum nitrate concentration in diabetics also had a significant positive correlation with the serum glucose concentration (R=0.7256,P<0.05),but this correlation was not found in control group.Conclusion These data showed that NO and its derivatives are overproduced in the diabetic patients.(J Geriatr Cardiol 2008;5:25-27)
6.Evaluation on methodological problems in reports concerning quantitative analysis of syndrome differentiation of diabetes mellitus

OBJECTIVE: To evaluate the quality of reports published in recent 10 years in China about quantitative analysis of syndrome differentiation for diabetes mellitus (DM) in order to explore the methodological problems in these reports and find possible solutions. METHODS: The main medical literature databases in China were searched. Thirty-one articles were included and evaluated by the principles of clinical epidemiology. RESULTS: There were many mistakes and deficiencies in these articles, such as clinical trial designs, diagnosis criteria for DM, standards of syndrome differentiation of DM, case inclusive and exclusive criteria, sample size and estimation, data comparability and statistical methods. CONCLUSION: It is necessary and important to improve the quality of reports concerning quantitative analysis of syndrome differentiation of DM in light of the principles of clinical epidemiology.

8.Full arthroscopic dual-beam reconstruction of the posterior cruciate ligament using tibial Inlay technique

Objective To investigate the recent effect of full arthroscopic dual-beam reconstruction of the posterior cruciate ligament(PCL)using tibial Inlay technique.Methods From March 2007 to September 2009,17 PCL injured patients underwent full arthroscopic dual-beam PCL reconstruction using Inlay technique,including 16 males and 1 female,with an average age of 25 years(range,19-54).Of all cases,Lysholm score was(53.4±2.1)points,International Knee Documentation Committee(IKDC)rated C in 7,D in 10,and posterior drawer test(+)in 17.We used self-designed tibia tunnel drill system to produce the deep-limited bone tunnel.Follow-up began at 12 months after operation.Evaluate Lysholm knee score,IKDC rating,and posterior drawer test to compare the knee stability with that of preoperative.Observe the location of the bone block and healing by checking knee X-ray and spiral CT scan.Results Seventeen patients were followed up between 12 to 28 months,with an average of 17.8 months.In the last follow-up study,Lysholm score(93.5±1.7)points compared with that of preoperative was statistically significant different(P=0.016).IKDC rating of A grade in 15 cases,B in 2,compared with that of preoperative was statistically significant different(P=0.021).Posterior drawer test were negative in 15 cases,slight positive in 2.The X-ray and spiral CT scan showed the location of the bone block were perfect and healed well.Conclusion We can accurately produce the deep-limited bone tunnel by the tibia tunnel drill system with minor trauma,and the recent clinical effects of PCL reconstruction were pretty good.
9.Development of up-converting phosphor technology based lateral flow assay for quan-titative detection of foodborne pathogens

Objective To develop an up-converting phosphor technology based lateral flow (UPT-LF) assay for rapid detection of Salmonella paratyphi A, S.paratyphi B, Escherichia coli O157 ∶H7 and Vibrio parahaemolyticus. Methods With up-converting phosphor nano-particles ( UCP-NPs ) as the bio-marker, four double-antibody-sandwich mode based UPT-LF strips for detecting the above mentioned four pathogens were prepared respectively and their sensitivi-ty, accuracy, linearity and specificity were evaluated .Furthermore, the feasibility of detecting bacteria in food samples was evaluated by different food samples artificially contaminated with less than 10 CFU target pathogens .Results The sensitivi-ty of UPT-LF assays for four pathogens was 105 ~106 CFU/ml with excellent specificity .The four strips had a good linear response with the linear fitting coefficient of determination (r) for each target pathogen ranging from 0.985 to 0.996.The positive rate of detecting pathogens from samples was acceptable .Conclusion The four developed UPT-LF strips provide a new choice for rapid , specific and sensitive and quantitative detection of S.paratyphi A , S.paratyphi B, E.coli O157∶H7 and V.parahemolyticus.
10.Efficacy of recombinant tissue-type plasminogen activator thrombolysis and primary coronary stenting after acute myocardial infarction.

OBJECTIVETo compare the efficacy of low dose recombinant tissue-type plasminogen activator (rt-PA) thrombolysis with primary coronary stenting after acute myocardial infarction.
METHODSOf 261 patients with first acute myocardial infarction, 131 were given low dose rt-PA intravenous thrombolysis, and 130 primary coronary stenting.
RESULTSThe age, time from onset of chest pain to hospital presentation and infarct location between these two groups were comparable. The patency rate of the infarct-related artery (IRA) in patients in the thrombolysis group was significantly lower than that of patients in the primary stenting group (P < 0.001). Recurrent myocardial infarction, and selective coronary stenting of patients with thrombolytic therapy were higher than that of patients in the primary stenting group (7.6% vs 1.5%, P < 0.05; 20.6% vs 0, P < 0.001, respectively). Left ventricular ejection fraction (LVEF) in patients in the thrombolysis group was lower than that of the stent group (55.6% +/- 13.4% vs 65.8% +/- 9.2%, P < 0.001). Total hospitalization time of the thrombolysis group was longer than that of the stent group (16 +/- 7 d vs 11 +/- 4 d, P < 0.001). Mortality in the thrombolysis group was higher than that of the stent group, but this difference was not significant (6.1% vs 3.1%, P > 0.05).
CONCLUSIONComparing with low dose rt-PA thrombolytic therapy after acute myocardial infarction, primary coronary stenting has a higher patency rate of the IRA, better cardiac function and shorter hospitalization time.
