Objective To establish a new and convenient assay for whole blood lead concentration. Methods Whole blood sample was digested by nitric acid-perchloric. Under the condition of pH 0.8~1.0, lead concentration was examined by Hydridegeneration Atomic Fluorescence Spectrometry. Results The method had good linearity when lead concentration were between 0 to 600 ?g/L, r =0.999. The within-run CV of the method for high, middle and low lead level were 4.1%, 3.6% and 3.5% respectively. The between-run CV were 4.9%, 4.6% and 4.7% respectively. The recovery rates were 105.4%, 98.5% and 94.8% respectively. The single-blind test result of series standard lead samples provided by Chinese Academy of Preventive Medicine were within the acceptable range. The detecting Limit of this method was 0.3 ng. There was no significant interference from main two-valence positiveion. The sample can be storage at lest for 1 months at 4℃. Samples of 869 children from Tianjin were tested by this method. The mean value was 91.9 ?g/L, median was 83.8 ?g/L, all data were within 23.1~386.3 ?g/L. The result showed out positive skewness distribution. Conclusion This method were sensitive, accurate, precise, quick and low-cost. It was suitable for use in clinical laboratories.

Objective To investigate the effects of mild hypothermia ( MH ) on the expression of my- eloperoxidase(MPO) and cyclooxygenase 2 (COX 2) in rats after cerebral ischemia and reperfusion (CIR). Methods Forty-eight Wistar rats were randomly divided into four control groups (n=6 in each) and four MH groups (n=6 in each).CIR models were established by suture occlusion of the left middle cerebral artery. The rats in the MH groups, but not in the control groups, were treated with MH.Rats were killed at 4 h,8 h,12 h and 16 h after CIR.MPO expression was measured, along with the expression of COX 2 as measured by Western blot- ting and immunohistochemical methods. Results Compared with the control groups,MPO activity and the COX 2 expression in the cortex and striatum were significantly lower in all the MH groups at 4 h,8 h,12 h and 16 h after CIR.Conclusion MH treatment can protect neurons by decreasing MPO activity and COX 2 expression,allevia- ting inflammation and reducing secondary injuries after CIR.

Objective To investigate and evaluate the efficacy of elastic stable intramedullary nailing (ESIN) and bone plate set (BPS) in the treatment of femoral shaft fractures in children. Methods The study involved 39 children with femoral shaft fractures, of whom 18 children were treated with ESIN (ESIN group) and the other 21 with BPS (BPS group). In ESIN group, the nails were insert-ed through a micro incision in the medial and lateral epicondyle after closed reduction under C - arm ob-servation. In BPS group, the plates were placed on the lateral side of femur by means of open reduction and internal fixation. Results All the patients were followed up for 4-20 months (mean 10 months), which showed fracture healing in all the patients. The mean operation time, incision length, perioperative blood loss, length of hospital stay, time of fracture healing and time of weight bearing were (40.0±17.0) minutes, (2.0±0.6) cm, (30.0±8.0) ml, (7.0±1.5) days, (8.0±1.2) weeks and (6.0 ±1.0) weeks respectively in ESIN group, and (70.0±25.0) minutes, (12.0±1.1) cm, (150.0±30.0) ml, (14.0±5.2) days, (13.0±1.9) weeks and (4.0±1.3) weeks respectively in BPS group, with statistical difference between two groups (P < 0.05). Conclusions Compared with BPS, ESIN has the advantages of less operation wound, less perioperative blood loss and shorter time for fracture healing. ESIN can protect the microenvironment important for the fracture healing, and is one of ideal op-tions for treatment of femoral shaft fracture in children.

Objective To explore the levels of serum leptin and insulin and their relationships with growth and development in premature infants. Methods The serum leptin and insulin concentrations of 37 premature infants were examined by the radioimmunoassay(day 1 and day 10,respectively),and anthropometric measurements were performed at birth. The blood sugar and triglyeride, cholesterol were tested at the same time.Twenty full-term newborn without severe diseases were controlled in 24 h after delivery and the level of leptin and insulin of them were detected, too. The results were analyzed by SPSS 10.0 software. Results The leptin and insulin levels in preterm infants were significantly lower than those in full-term infants[(6.45?2.24)?g/L vs(8.56?1.33)?g/L,(5.84?1.07)IU/L vs(10.85?5.24)IU/L Pa0.05).Serum leptin was positive correlation with serum insulin(Pa0.05).Conclusions Serum leptin and insulin can be the markers that reflect intrauterine nutrition status of preterm infants and the growth and development of fetus.A dipoinsular axis may be functional before 34 weeks gestation without maturity. The changes of leptin and insulin concentrations are useful for metabolism and growth in preterm infants.

Objective To explore the mechanisms of posterior pilon fractures and evaluate the curative effects of different types of fixation on the treatment of posterior pilon fractures. Methods We included in this retrospective study 20 patients with posterior pilon fracture who had been treated from January 2012 to January 2015 at our department. They were 10 men and 10 women, from 23 to 77 years of age (average,50.6 years).According to the classification by Yu Guangrong,5 cases belonged to type Ⅰ,3 to type Ⅱa,4 to type Ⅱb,and 7 to type Ⅲ.One was not indentified because of lacking CT examination. The mechanisms included ground level fall in 2 cases, motor vehicle accident in 7,fall off stairs in 5,sport injury in 2,fall from a bike in one and fall from a height in 3.More than 25% of the articular surface was involved in 13 patients.Syndesmosis injury was identified in 6 patients by Cotton test during operation.Internal fixation varied accordingly. We recorded the mechanism, classification, proportion of the articular surface involved (more or less than 25%),and syndesmosis injury to figure out the characteristics of posterior pilon fractures. We used the Burwell-Charnley radiographic criteria to assess the postoperative reduction of the articular surface, and the Olerud-Molander scoring scale and visual analogue scale (VAS) to assess the ankle function. The curative effects of different types of fixation on the treatment of posterior pilon fractures were compared. Results Of the 20 patients,17 were available for follow-up for 6 to 36 months (average,17.8 months). Two patients received reoperation because of implant failure after cannulated screw fixation from anterior to posterior. The Burwell-Charnley radiographic evaluation revealed 12 anatoinical reducations and 8 fair reductions. The mean Olerud-Molander score for the 17 patients at the final follow-ups was 81.5 (range, from 35 to 100) and the mean walking VAS was 1 (from 0 to 3).Conclusions Posterior pilon fractures are mostly caused by medium to high energy violence, resulting from a combination of rotational and vertical forces.Since there is a high risk of implant failure, the cannulated screw fixation from anterior to posterior is not recommended.Good clinical outcomes are observed in the cannulated screw fixation from posterior to anterior and the plate/cannulated screw fixation for posterior pilon fractures.
