1.The key techniques and complications related with autologous liver transplantation

Autologous liver transplantation,a type of ex situ liver surgery along with hypothermic organ preservation,a venovenous bypass and complicated vascular reconstructions,allows better access to unresectable tumor and is considered as an effective strategy to overcome the shortage of donors.However,the high complication incidence and mortality related with the technique limit its wide application.To popularize this technique,based on our current experience,we review the key parts of the surgery and the treatment strategy for the complications.
2.Interpretation of the international guidelines for management of severe sepsis and septic shock ,2012

The 2012 version of international guidelines for management of severe sepsis and septic shock got re-revised on the basis of the 2008 version.For pediatric severe sepsis,the 2012 version enforces initial resuscitation;develops detailed ant-infection measures;stresses isotonic crystalloid fluid resuscitation and the use of albumin,hetastarch is no longer recommended ;reduces the value of early use of dopamine,and clears the use of corticosteroids indications and the use of blood products;mechanical ventilation is recommended to use lung protective ventilation strategy; standardizes sedation and analgesia applications; clears the level of blood glucose control;others include blood purification,nutrition,stress ulcer prevention recommendations.
3.Research on expression of granzyme B in myocardium with experimental viral myocarditis

Objective To explore the role of granzyme B( GzmB) in infiltrated myocardial cells of mice with viral myocarditis( VMC) induced by coxsackievirus B3(CVB3) during the injury of myocardium and further to elucidate the pathogenesis of VMC. Methods Thirty-six BALB/c male mice, 5 to 6 weeks old, were randomly divided into two groups: the normal group, 10, and the model group, 26. On the eleventh day all mice were killed and the hearts were taken out for inspection whether there were pathological changes in the myocardium by macroscopy and microscopy. The expression of GzmB was detected in myocardium with immunohistochemistry and hemi-quantitive reverse transcript-polymerase chain reaction ( RT-PCR). Results GzmB positive cells in the mice myocardial cells infiltrated were detected by immunohistochemistry in the model group,and the number of GzmB positive cells is remarkable positive correlation to myocardial pathological score(r =0. 859, P =0.000) , however, no such cells were discovered in the normal murine hearts. Test of RT-PCR showed markedly high expression of GzmB mRNA in mice myocardium of the model group, however, no expression of GzmB mRNA in the normal group. Conclusion The expression of GzmB in the infiltrating myocardiocytes probably plays some role during the injury of myocardium, which mediated cytotoxicity effect is one of important mechanisms of myocardial injury in viral myocarditis. The expression quantity of GzmB in the infiltrating myocardiocytes is significant positive correlation to the severity of myocardial injury.
5.Effect of cryosurgery on bronchial stump and adjacent organs

Objective To analyze the biochemical effect of cryosugery on residual bronchial tissue and the adjacent organs in dogs. Methods Bronchial stumps of 18 dogs after lobectomy and the adjacent tissues and organs(including 4 samples of esophagi, 3 diaphragms, 3 stomachs, 2 livers, 7 tracheae and 3 superior saphenous vein sinuses) of 22 dogs after thoracectomy were frozen. The observation from 1~212 d was conducted after surgery. Results Cryonecrosis and fibrosis were found in all 18 samples of the frozen bronchial stumps, but no fistula of bronchial stump was found. Three cryodamaged esophagi in 4 dogs developed fine sheet of fibrous membrane like scars in the situs of cryosurgery. Perforation was found in 2 dogs, one in the esophagus, another in stomach. No perforation was found in dogs with diaphragm cryosurgery. Cartilaginous ring disappeared in 7 dogs by cryosurgery of the trachea and a membrane like scar developed in the inner wall of trachea. There were yellow scars on the liver after cryosurgery. Conclusion Exfoliation of the necrotic tissues is slow due to the slowly developed cryonecrosis and good tolerance to cryosurgery of the fibrotic tissues. The original construction disappears but fibrosis develops. At the edge of cryo necrotic area, proliferation of capillary can be found.

8.Pharmcokinetic study of cyclosporin A in rabbit eyes by HPLC

Objective To study the pharmacokinetic characters of cyclosporin A(CsA) in aqueous humor in rabbit after implanting different dosages of CsA in eyes and to provide a theoretical basis for the treatment of after cataract. Methods ECCE was performed in all rabbit eyes. CsA-MS was injected into the anterior chamber and the capsular bag in left eyes as expression group and MS was given in the same way in right eyes as control group. The concentration of CsA in the aqueous humor was monitored with high-performance liquid chromatogram. The follow-up period was 4 weeks. The samples were separated on a C18 column at 60℃ and detected at 210nm. The mobile phase was acetonitrile-water (67∶33). Results The correlation analysis showed a positive correlation within the range of 0.13-1.25mg/L (r=0.9951) and the detection limit was 0.13mg/L. The accuracy was 95.91% and the inter-day and intro-day precision was less than 5%. CsA in aqueous humor sustained a high concentration within 2 weeks. There were no significant differences in t1/2Ka and CL between the two dosage groups. AUC and Cmax increased in a dose-dependent manner. Conclusion The sustain-released CsA ophthalmic gels provided significant ocular bioavailability in rabbit eyes and they can reach the therapeutic dose in order to inhibit after cataract.
9.Comparison of growth and apoptosis of fibroblasts derived from kidneys with and without interstitial fibnwis

Objective To study the role of fibroblasts in the pathomechanisms of renal interstitial fibrosis(RIF). Methods Growth behavior and apoptos, of renal interstitial fibroblasts in cultures, established using renal biopsies of casts with and without RIF, were studied. Results There were significant alterations in the growth behavior, the differentiation pattern of potentially mitotic fibroblast populations and programmed cell death in cultures derived from kidneys with RIF, as compared with cultures of normal origin. Conclusion The abnormal growth and apoptosis of fibroblasts may play an im-portant role in RIF. Inhibiting the proliferation and promoting the programmed death of fibroblasts may be benificial to patients with RIF.
10.Bilateral multiple renal angiomyolipomas associated with tuberous sclerosis (report of 4 cases)

Objective To study the clinical features of renal angiomyolipomas with tuberous sclerosis and to improve the recognization of this disease. Methods The diagnosis and treatment of 4 cases of tuberous sclerosis with bilateral and multiple renal angiomyolipomas were reviewed.The age range was 17 to 44 years,two of them were men.One patient presented with the classic triad:facial angiofibroma,seizures,and mental retardation.Facial angiofibroma occurred in three and seizures were found in two,three patients showed multiple extrarenal organ involvement including liver,spleen and bone.Brain CT revealed classical calcified cortical tuber and(or) subependymal nodule in three cases. Results The diagnosis of renal angiomyolipomas in three cases was affirmed on CT scan.However,the giant renal angiomyolipomas in one case can not be diagnosed from renal CT scan which did not demonstrate characteristic fatty tissue whereas the diagonsis was confirmed by ultrasound-guided right renal tumor biopsy and subsequent right nephrectomy for the large leisions.Regular monitoring of one patient was adopted for his mild symptomatic small renal leisions,the other two with hemorrhage in renal angiomyolipomas were reluctant to aggressive treatment and underwent conservative therapy.Follow-up ranged 6 to 36 months, none had recurrent symptoms and no significant growth of renal lesions was observed. Conclusions CT may not be pathognomonic for renal angiomyolipomas associated with tuberous sclerosis,needle biopsy may be needed to exclude renal carcinoma.The choice of treatment is based on tumor size distribution pattern,progression,and associated symptoms.
