Objective To investigate the clinical effect of sitagliptin combined with metformin in the treatment of sulfonylurea-deficient elderly patients with diabetes mellitus.Methods Sixty patients with diabetes mellitus were enrolled in our hospital from January 2013 to December 2016.The patients were divided into observation group(30 cases) and control group(30 cases) according to the random number method.The patients in the observation group were treated with sitagliptin and metformin.The patients in the control group were treated with gemcitabine, and the clinical treatment effect, fasting blood glucose, postprandial blood glucose, glycosylated hemoglobin, fasting C peptide and postprandial 2hC peptide Level of statistical comparison.Results The clinical effect of the observation group was significantly higher than that of the control group.Before and after treatment, the fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin, fasting C peptide and postprandial 2hC peptide level were not significantly different After fasting blood glucose, 2h postprandial blood glucose and glycosylated hemoglobin levels were significantly lower than before treatment, after treatment of fasting C peptide and postprandial 2hC peptide levels were significantly higher than before treatment.Conclusion The clinical efficacy of serotretine combined with metformin in the treatment of elderly patients with diabetes mellitus is similar to that of Yuxing, and the combination is more convenient and more suitable for clinical treatment.It is worthy to be popularized and applied in clinical practice.

MircoRNAs (miRNAs) are widely existing inorganisms, non-coding small RNA. MircoR-NAs play important role in cell proliferation, apoptosis, stress resistance and fat metabolism. In vitro studies indicate that miRNAs are important factors in platinum resistance of ovarian cancer cell lines. However, the pathway of platinum resistance of ovarian cancers is complex. Studies find miRNA expression play important roles in the sensitivity of platinum resistance of ovarian cancer cell lines , and can be used as an early alert for platinum resistance. Changing the expression of miRNA even can reverse the platinum resistance . But the regular role of miRNAs in target genes network is still unconfirmed. Thus, more studies about miRNA in vivo and animal tumor models are needed to identify the function of miRNAs in platinum resistance of ovarian cancer.

Neovascularization plays an essential role in the process of renovation in tissue damage,tumor growth, and hormones action. Parathyroid hormone (PTH) is one of the basic hormones that regulate the serum calcium, phosphorus, and bone metabolism. In recent years, studies involving the neovascularization-mediated effects of PTH on metabolism are becoming more and more popular and wider. Based on recent researches, the purpose of this paper is to summarize the changes of angiogenesis while PTH interacts with its target organs, especially interacts with osseous tissue.

In mice, adriamycin (l7.5mg/kg, ip ) produced heart peroxida-tion abnormality including increased TBA value & decreased gluta-thione peroxidase activity, respectively. But the change of former was not parallel to the latter. The drug also produced early pathological change of the heart i.e. more D-PAS positiveness. The results suggested that the early heart pathological change produced by adriamycin may be related to increased accumulation of pero-xi dative products.

Objective To evaluate 18F-FDG PET/CT imaging in primary adrenal lymphoma (PAL). Methods Three male patients with clinically diagnosed PAL, aged 59 to 77 years old, were examined by18F-FDG PET/CT before histopathologic examination. The SUVmax ratio of tumor to liver (T/L)was calculated on 18F-FDG PET images. All patients underwent R-CHOP ( rituximab-cyclophosphamide,doxorubicin, vincristine, and prednisone) chemotherapy regimens. One patient had serial follow-up 18F-FDG PET/CT post therapy. Results The final diagnosis of all 3 cases was bilateral diffuse large B-cell lymphoma. The pathologic results of bone marrow biopsy were negative. Case 1 was in disease remission based on four times of follow-up 18F-FDG PET/CT. The patient's clinical follow-up was also negative and he was currently alive for more than 19 months. Case 2 was of late stage and suffered from adrenal insufficiency. He died 6 months after the diagnosis. Case 3 (the eldest patient) was treated with partial excision followed by the chemotherapy. He died of lung infection 12 months after treatment. Conclusion 18 F-FDG PET/CT may be of important value for maximizing patient management and survival for patients with PAL, a rare and highly malignant disease.

This paper found some unsuitability of cognition,attiude and behavior when people deal with the various profit relationships in the gene treatment because of effect theory,benethamism intention and breaking the game rule.In my opinion, we must persist in the suitable ethic principle in the gene treatment.

· AIM: To analyze the pathogeny of corneal endotheliitis and to investigate the causes of its misdiagnosis and the key to treatment.endotheliitis presented with predisposing factors, clinical manifestations and treatment were analyzed retrospectively.junctiviis, and the other 7 were misdiagnosed as acute close-angle glaucoma, glaucomatocyclitic crisis, inflammatory reaction after cataract extracapsular extraction and IOL implantation, disciform stromal keratitis and iridocyclitis, respectively. All the 7 cases were cured through treatment, and recurrence appeared in 1 case.unclear yet. Properly grasping its features and key points for differential diagnosis and seriously and detailed examining patients will benefit to timely diagnosis. After clinical treatment, maintaining treatment for more than 4wk will lower the recurrence of corneal endotheliitis.

AIM: To observe the clinical features of glaucomatocyclitic crisis and to investigate its pathogenesis, diagnosis,treatment and prognosis.METHODS: The clinical data of 15 cases (15 eyes) subjected to glaucomatocyclitic crisis were retrospectively analyzed including clinical manifestation and treatment.RESULTS: Visual field was normal in 13 eyes, and 2eyes had glaucomatous visual field defects. C/D ratio was ≤0.3 in 10 eyes and ≥0.6 in 2 eyes. Through pharmacotherapy the symptoms in 14 eyes disappeared after 2wk. One eye was diagnosed as being complicated by primary open-angle glaucoma and well treated with surgery.CONCLUSION: Glaucomatocyclitic crisis is a kind of ocular syndrome with unknown pathogenesis. It should be differentiated from primary glaucoma and uveitis clinically. Its prognosis is not absolutely benign, and the patients should be followed up regularly.

Natural biocompatible nanomaterials such as self-assembled triterpene natural small molecule products with favorable anticancer activity show great potential for biomedical application. However, the mechanisms of their molecular self-assembled structures have not been investigated systematically, while there are still few reports of the natural active carrier for drug delivery. Herein, in this work, we further explored the molecular assembly mechanism and common regularity of tetracyclic triterpenes ergosterol, stigmasterol as well as pentacyclic triterpenes glycyrrhetinic acid and ursolic acid, which suggested that the coplanarity and orderliness of molecular arrangements which are speculated to be responsible for their self-assembly into the spherical, rod-like or lamellar nanostructure. Besides, ergosterol (ET) with better anticancer activity was chosen as a representative substance for construction of the synergistic antitumor nanodrug. By intermolecular hydrogen bonding and π-π stacking, chemotherapeutic drug paclitaxel (PTX) was encapsulated into ET-PTX NPs successfully. Then, the anti-cancer efficacy of the tumor-bearing mice was evaluated according to the protocol approved by the Experimental Animal Research Center of Harbin Medical University. The resulting nanodrug exhibited excellent biosafety and enhanced in vivo anticancer activity efficiency of 52.3%, higher than free PTX (29.4%) or ET NPs (32.5%) alone, further verifying the potential medical application value of triterpene natural products. This work provides not only a theoretical basis for exploring the self-assembly behavior of small molecule natural products, but also a promising perspective for the fabrication of active natural biocompatible nanodrug delivery systems for synergistic antitumor therapy and other biomedical applications.

Objective To observe the expression of induced nitricoxide synthase(iNOS)in ligustrazine against cisplatin-induced ototoxicity.Methods Fifty white guinea pigs with normal hearing were randomly divided into the control group,ligustrazine group and traditional Chinese drug(TCD)group.The alternations of hearing function were evaluated by the examination of auditory brainstem response(ABR).Some specimens were perfused by AgNO3 and observed the damage of bristle cell.And some were sent for immunohistochemistry to detect the expression of iNOS.Results Compared with the control group,the threshold of ABR in ligustrazine group and TCD group were significantly different.There was a significant difference between the ligustrazine group and TCD group.There was no expression of iNOS in Cortis',spiral ganglion cell and stria vascularis in normal cochlear(control group).In the ligustrazine group,the expression became stronger in spiral ganglion cell,stria vascularis,spiral ligament and Cortis'.INOS also expressed in TCD group.Conclusion Ligustrazine can antagonize cisplatin-induced ototoxicity by inhibiting the expression of iNOS.
