In-utero intestinal volvulus is a rare but potential life
threatening foetal complications. It is a surgical emergency
and delay in diagnosis or treatment can increase the
morbidity and mortality to the foetus. We report a case of
mild foetal bowel dilatation diagnosed at 21 weeks of
gestation. She was closely follow up and at 31 weeks of
gestation, in-utero intestinal volvulus was diagnosed with
the characteristic ‘whirlpool’ sign on ultrasound
examination. This case emphasises the importance of early
recognition and quick decision to delivery when intestinal
volvulus is diagnosed. This enabled early surgical
intervention to prevent further foetal morbidity.

Aromatase inhibitors(AIs) are standard therpy for postmenopausal women with estrogen responsive breast cancers.By inhibiting the aromatase enzyme,causing decreases in endogenous estrogens,the treatment of Als is responsible for lower bone mineral density(BMD) and increased fractures.Therefore,early recognition, prevention,and/or treatment of AI-induced bone loss is needed.Zoledronic acid is specific inhibitors of osteoclasts and extensively used in bone metastasis patients.Recently, there are several trials evaluating the use of in- travenous zoledronic acid as prevention and treatment of AI-induced bone loss in postmenopausal women with breast cancer.In this article, we aim to review the use of zoledronic acid in this population including the response and safety.
3.Design on High-Reliability Key Hospital Database Server System of "No.1 Millitary Medical Project"

Objective To improve the reliability of the core database system by ameliorating the scheme of the server system architecture. Methods Combined with practice of the hospital informatization, the security and potential hole of the database server system were analyzed and evaluated. Results Aimed to deal with the security hidden-trouble, a mini-server was adopted instead of PC server, while SAN(Storage Area Network) was utilized instead of RAID-5, and mirror copy technique was used to implement the real-time hot copy of the data. Conclusion Consequently the reliability of the system has been greatly improved.
4.The fluctuation of cytokine mRNA expression level of a novel T-cell-mediated immune hepatic fibrosis model in Balb/c mice

Objective To explore the fluctuation of cytokine mRNA expression level in a novel T-cell-mediated immune hepatic fibrosis model induced by repeatedly injections of Concanavalin A in BALB/c mice. Methods BALB/c mice were divided into different groups. Model group mice were injected weekly up to 20 weeks with Concanavalin A (15mg/kg), via retro-orbital venous plexus under ether anesthesia. Normal control group mice were treated in the same manner weekly with normal saline. Twenty-four hours after Concanavalin A challenge at 1, 5, 12 and 20 week, 8 mice from each time were killed by cervical dislocation, repectively. The livers of different group were excised and fixed in 10% formalin for HE staining and Gomori Ag staining or frozen in optimal cutting temperature (O.C.T.) media in liquid nitrogen for immunohistochemical staining for CD4 +T or CD8 +T cell. After extracting total RNA from liver tissues, IL-2, IL-4, IL-10 and transforming factor ?1 messenger RNA were amplified by reverse transcription polymerase chain reaction. PCR products were electrophoresed on agrose containing ethidium bromide and visualized under ultraviolet light. Densitometric RT-PCR data were standardized with ?-actin signals. Results The histological change of HE staining and Gomori Ag staining indicated the fibrogenesis in model group mice. Immunohistochemical staining for CD4 + or CD8 + T cell indicated that the infiltrating lymphocytes in liver parenchyma were mainly CD4 +T lymphocytes. IL-2 mRNA expression level only increased after the first injection of Concanavalin A. The expression levels of IL-4, IL-10 and transforming growth factor ?1 mRNA significantly increased over the whole experiment period as compared with control group. Conclusions Repeated administration of Concanavalin A can induce T-cell-mediated immune hepatic fibrosis model in BALB/c mice. The expression levels of IL-4, 10 and TGF-?1 increase over the whole experiment period and may play an important role in creating mouse fibrotic model.

6.Development of the mouse model for acute hepatitis B virus infection

A mouse model for acute hepatitis B virus (HBV) infection was established by using the hydrodynamical injection of mouse tail vein, in which the immunocompetent BALB/c mice were hydrodynamically injected with a competent replication plasmid pAAV-HBV1.2 having 1.2 fold over-length of HBV DNA. On day 1, 2, 4, 6 and 8 after injection, the levels of HBsAg, HBeAg and HBV DNA in blood serum were detected by using ELISA and fluorogenic quantitative PCR assay (FQ-PCR). And on day 8. HBsAg and HBeAg in liver tissue were assayed by immunohistochemical staining. It was found that HBsAg in blood serum could be detected on day 1 after infection in 14 of 16 mice (85.7%) injected with pAVV-HBV1.2 by using ELISA assay and the peak levels of HBsAg and HBeAg were attained during the first day after injection and then it dropped down gradually up to day 8 following injection. The titer of HBV DNA in blood serum attained its peak on day 2 and maintained a high level later on. On day 8 after injection, its titer was 1.9×10~4 copies/mL. The percentage of HBcAg-positive hepatocytes and HBsAg-positive hepatocytes in liver tissues were 5% and 2% respectively. Thus, by using the hydrodynamic injection with the competent replication plasmid, a mouse model for acute HBV infection is successively developed.
7.Influence of open access journals of biomedicine in China on academic exchanges

Objective To compare the influence of open access journals ( OAJ ) of biomedicine on academic exchanges by empirically analyzing the journals enrolled in the Chinese science citation database-the core (CSCD-C).Methods Multivariate statistical analysis was performed on OAJ and non OAJ in CSCD by using bibliometric and statistical methods,SPSS software and rank-sum test.Results The OAJ were accounted for only 31.63% of the total enrolled journals,the average ratio of funded papers in OAJ was 72.29%.Statistical journals had high influence in the discipline.The selfcitation rates of clinical medicine OAJ and special medical OAJ were high.The average impact factor and h index in biological OAJ were high.The average impact factor of preventive medicine OAJ was high.The average h index of comprehensive non OAJ was high.Conclusions The distribution of OAJ is uneven among different disciplines.The ratio of funded papers is higher in OAJ than in non OAJ with insignificant differences in discipline influence.

9.Study on TCM Clinical Research Primary Quality Control and Management

ObjectiveTo investigate and analyze the current primary quality management situation in TCM clinical studies.Methods One sub-center unit from 39 projects of “Prevention and Treatment of Difficult and Complicated Disease of TCM” plan of the“11th Five-year National Key Technology R&D Program” was chosen. Executive condition of the primary quality control of each unit was under field investigation, and quality control quantitative evaluation indexes were used to conduct quantitative evaluation.Results Results were shown as the forms of statistical description and radar map. The average and median scores of primary quality management were 5.05 and 5.2, respectively. The best and preferable proportions of 39 units were 25.64% and 28.21%. Other 46.15% of these units performed poor. The problem mainly manifested on insufficient frequency and bad normalization of primary quality management, such as comprehensiveness and integrity of inspection contents, cases, problem recordings and problems timely feedback.ConclusionAlthough the primary quality management is widely conducted in TCM clinical studies, there are still some problems to be settled and improved.
10.Diagnosis and treatment of primary small bowel tumors

Objective To study the clinical manifestation of small bowel tumors and the prognostic factors. Methods Retrospective analysis of the clinical data were made on 42 cases of primary small bowel tumors (except periampullary tumors) admitted to our hospital from 1974 to 1999. Results In this series, there were 6 cases of benign small bowel tumors, 5 of them were leiomyoma and 1 adenoma. Of the 36 cases of malignant tumors, there were malignant lyphoma in 17 cases (47.2%), adenocarcinoma in 11 (30.6%), leiomyosarcoma in 6(16.7%), carcinoid tumor in 1 (2.8%) and neurofibrosarcoma in 1 (2.8%). There was no characteristic clinical sign in small bowel tumors; endoscopy and barium series were the main diagnostic methods. About 50 per cent of patients with malignancy had metastasis, and only 18 cases (50%) received radical resection. Follow-up were available on 36 cases. The overall 5-year survival rate in the patients with malignant small bowel tumors was 47.3%. Multivariate analysis showed that metastasis and diameter of tumors were the main prognostic factors of the 5-year survival. Conclusions Early diagnosis and early treatment play the key roles in improving the prognosis of malignant small bowel tumors. Radical resection should be taken if no metastasis is found.
