Objective To explore the variation about the application of fast-track surgery and laparoscopy in treatment for colorectal cancer in recent years.To investigate the probability of combining protocols of the two for treatment for colorectal cancer.Methods The clinical and basic literatures of related researches about colorectal treatment of laparoscopy and fast-track surgery were collected and reviewed.Results Compared with the traditional treatment modalities,both of fast-track surgery and laparoscopy used for the treatment of colorectal cancer have better clinical effects.Conclusions Fast-track surgery and laparoscopic techniques used for the treatment of colorectal cancer are feasible,but the combination of the two should be confirmed by further randomized controlled trials.

Objective To investigate the effects of Benazepril on inflammatory cytokines in patients with primary hypertension and its efficacy analysis.Methods 72 patients with mild to moderate primary hypertension were randomly divided into benazepril group(37 cases)and control group(35 cases).The control group patients were given felodipine sustained release tablets Stag,1 times daily.Benazepril group were given benazepril tablets 10mg,1 times daily.Both groups were treated for 8 weeks.Changes in blood pressure and heart rate,plasma inflammatory cytokine levels and the adverse reactions were observed.Results After 8 weeks of treatment,the patients' blood pressures were significantly decreased (t =2.87,2.43,2.31,2.23,P ＜ 0.05 or P ＜ 0.01) than before in both groups.And the decrease in the benazepril group was more obvious than that in the control group (t =2.21,2.13,all P ＜ 0.05),while the patients' heart rates in the two groups showed no significant change (P ＞ 0.05) ; Patients' plasma IL-4 and IL-8 levels decreased significantly,plasma IL-10 levels increased significantly (t =2.99,2.87,3.21,2.13,2.19,2.23,all P＜0.05 or P＜0.01),and the magnitude of decrease or increase in the benazepril group were more obvious compared with the control group (t =2.21,2.13,2.34,all P ＜ 0.05).Conclusion Benazepril in the treatment of primary hypertension has good antihypertensive effect and security which can significantly reduce the plasma levels of proinflammatory cytokines IL-4 and IL-8 levels,improve the anti-inflammatory cytokine IL-10 levels,and has the role of suppressing the inflammatory response of the vessel wall.

Objective]To discuss and analyze Professor Cheng Jing's clinical experience for treating decline in ovarian reserve(DOR). [Method]Through learning from the tutor in the outpatient service and case studies ,we analyzed and summarized Pro. Cheng's recognition on the etiology and pathogenesis of DOR,together with his dialectical way of thinking, combined therapy of Chinese and western medicine ,and empirical prescriptions. [Result] Pro. Cheng believes that this disease lies in the kidneys as wel as in the thoroughfare and conception meridian ,the pathogenesis is kidney deficiency and the imbalance of the thoroughfare and conception meridian,or combined with the stagnation of liver qi,spleen deficiency,and blood stasis and phlegm fire (heat); the therapy is mainly nourishing and warming kidneys as wel as conditioning the thoroughfare and conception meridian ,combined with Chinese medicine's periodical therapy,such as adjusting the circulation of qi and blood in livers,spleens,hearts and other organs. [Conclusions] Pro. Cheng's combined therapy of Chinese and western medicine for curing DOR ,of which his empirical prescriptions the Bushenyichongkangshuaitang is the main part,is of remarkable curative effect and significant clinical value.

Fetal intrauterine growth restriction is one of the common diseases in the perinatal period,which not only increases the child morbidity and mortality in the perinatal period,affecting the long-term development of the nervous system,and can make the risk of adult obesity,hypertension,type Ⅱ diabetes increased significantly.A number of studies suggest that the down-regulation of A amino acid transporter expression is closely associated with intrauterine growth restriction.The down-regulation of A amino acid transporter expression may be an important cause of intrauterine growth restriction.

Bone marrow microenvironment is a complex network consisting of hematopoietic stem/pro-genitor cells (HSPCs),non-hematopoietic cells,extracellular matrix and various cytokines.Its components interact to support normal hematopoiesis.Emerging evidence indicates that the dysfunction of mesenchymal stem cells,myeloid-derived suppressor cells,cytokines and the epigenetic alterations of HSPCs in the bone marrow microenvironment could influence normal hematopoiesis.Abnormal hematopoiesis contributes to the occurrence of hematological malignancies,such as myelodysplastic syndromes (MDS).Animal models have confirmed that bone marrow microenvironment plays an important role in the original generation and maintenance of malignant diseases of hematopoietic system.

AIM: To observe the effect of ischemic preconditioning (IPC) on the subsequent permanent ischemic rat retina. METHODS: A model of two-vessel occlusion (2VO) was used to give an ischemic insult to rat retina. Bilateral carotid arteries were occluded directly in the simple ischemic groups. A procedure of double 2 min ischemia- 3 min reperfusion was applied to IPC groups before their carotid arteries were occluded. Experimental control groups received the same operation except that their exposed vessels were not occluded. Other 6 normal rats served as immunohistochemical controls. Eyeballs were taken out after being subjected to 1, 3 and 7 days of ischemia. The morphometry of retina was measured by stereologic methods. The apoptosis during the retinal ischemia was detected by TUNEL. Immunohistochemistry staining was used for the localization of bcl-2 in retina. RESULTS: In IPC groups, the thickness of retina was thinner than that in simple ischemic groups. The apoptosis was less when compared with the simple ischemic groups at the same ischemic time. The apoptotic cells show only in INL and the apoptosis of RGCs was not appeared until 7 days postischemia. There was no significant difference of the numerical density of RGCs at the different time after the operation. The bcl-2 immunoreactivity was weaker than that of the simple ischemic groups too. CONCLUSION: IPC reduced the injury caused by ischemia in retina, showing a protective effect.

Objective To evaluate the effectiveness of Leprosy Elimination Compaigns (LEC) integrated with routine case- finding of leprosy in the high leprosy- epidemic areas in Yunnan, Guizhou and Sichuan Provinces. Methods Extensive health education of leprosy in the community, training of paramedical workers on leprosy, clue survey and contact survey were carried out to make diagnosis and provide treatment. Results A total of 122 new leprosy cases were detected in 6 high leprosy- epidemic counties through LEC in 1999 with a detection rate of 3.7/100 000, but in contrast, the annual number of newly detected leprosy cases was 67 cases with an average detection rate of 2/100 000 during the period of 3 years before LEC. Among the newly detected leprosy cases, about 47％ were leprosy household contacts. Conclusion LEC combined with routine activities of case finding can improve the detection of the back- log cases in the communities. In the high epidemic areas, especially in the remote and difficult- to- access areas, majority of leprosy cases can be detected through combining active and passive case- finding methods, providing extensive health education, improving leprosy services in skin clinic, leprosy contact and clue surveys.

Gene therapy has emerged as an efficient modality to treat human diseases.This method is based on the transfer of genetic material to tissues to induce a curative effect.Gene therapy vectors are molecular constructs used to facilitate the penetration of genomic sequences inside the cells.Viral vectots have however several limitations when administered directly to the patient.They may cause significant toxicity by activating innate immunity or by eliciting an adaptive immune response against viral proteins.In addition,targeting the vector to the desired site is an issue when given systemically.The use of cells as vehicles for gene therapy vectors has many advantages.The combination of cell-viro-gene therapy has been thought as a new and promising strategy for therapy of cancer.The targeting vector to cancer stem cells will become a new direction in the field of gene therapy.In this article,we will introduce progressions,limitations and future directions of gene therapy of liver cancer.

In view of the current conditions and reasons about the reform of medical and health system in China,the paper expounds the feasibility of authorized operation with state-owned assets of public hospital and the concrete operational pattern from the angle of hospital operational mechanism, which proposes a new idea about how to effectively solve the contradiction between the limited investment on state-owned health resources and the growing demands on medical health.
