2.A comparison of effects between recombinant insulin lispro and insulin lispro in treating type 2 diabetes mellitus

Objective To compare the efficacy and safety of recombinant insulin lispro and insulin lispro in the treatment of type 2 diabetes mellitus(T2DM ) .Methods Forty‐eight T2DM patients with poor blood glucose control were randomly assigned to the recombinant insulin lispro group (observation group ,n=32) and insulin lispro group (control group ,n=16) according to the ratio of 2∶1 .On the basis of injection of the recombinant insulin glargine once daily before sleep ,the two groups were given the re‐combinant insulin lispro injection or insulin lispro injection once before each meal .The period of treatment was 16 weeks .The levels of HbA1c ,2 h postprandial blood glucose(2hPG) and fasting plasma glucose(FPG) before and after treatment were measured and compared between the two groups .The adverse events were evaluated at the end of treatment .Results Forty‐four cases finished the study ,28 cases in the observation group and 16 cases in the control group .The levels of HbA1c ,FPG and 2hPG after 16‐week treatment in the two groups were decreased significantly (P<0 .05) .The decrease amplitudes of HbA1c in the observation group and the control group were (1 .55 ± 1 .50)% and(1 .06 ± 1 .30)% respectively ,which of FPG were (2 .07 ± 5 .01)mmol/L and (1 .09 ± 3 .18)mmol/L respectively ,and which of 2hPG declined (3 .28 ± 5 .71)mmol/L and(3 .60 ± 5 .89)mmol/L respectively . With HbA1c as the main evaluation index and 0 .40 as the non‐inferiority critical value ,non‐inferiority was found in the recombi‐nant insulin lispro injection as compared with the insulin lispro injection (P<0 .05) .The standard‐reaching rate of HbA1c less than 6 .50% in observation group was 14 .29% ,and which of HbA1c less than 7 .00% was 28 .57% ;in control group ,these two stand‐ard‐reaching rates were 18 .75% and 43 .75% respectively ,and there were no statistically significant differences between two groups (P>0 .05) .Conclusion The recombinant insulin lispro injection has non‐inferiority effects in the aspect of effectiveness compared with the lispro insulin injection ,moreover they have the same safety .
3.Percutaneous transluminal angioplasty with stenting for treatment of superior vena cava obstructive syndrome

Objective To evaluate the clinical value of percutaneous transluminal angioplasty with stenting(PTAS) for the treatment of superior vana cava obstructive syndrome (SVCOS). Methods 15 cases of SVCOS including 10 cases of lung cancer with mediastinal lymphatic metastasis, 3 cases as malignant lymphoma and 2 cases of esophageal cancer with mediastinal lymphadenopathy, were undergone right subclavian vein or ellow vein catheterization for pressure measurement and DSA imaging of SVC with displaying the obstructive characteristics. A self expanding stent was then implanted throwgh right fermoral vein catheterization.Results 15 cases were all successfully undertaken angioplasty and stenting, except 1 case with a long stricture over 10 cm requiring 2 stents. After successful stent placement, DSA revealed smooth flow of contrast with almost normal diameter of SVC lumen, together with disappearance or relief of SVCOS. The SVC pressure decreased from 30.5?2.3 cmH 2O down to 8.8?1.5 cmH 2O after recanalization, with a significant difference in statistics( P
5.Conservative surgery for the treatment of breast cancer:Analysis of 148 cases

Objective To investigate the therapeutic procedures,indications and efficacy of breast-conserving therapy (BCT). Methods BCT was carried out in 148 patients with breast carcinoma in this hospital from January 1996 to October 2003.There were 5 patients in stage 0,85 patients in stage Ⅰ,55 patients in stage Ⅱ,and 3 patients in stage Ⅲ.The surgical treatment was quadrantectomy or local wide excision of tumor,combined with axillary lymph node dissection.Routine adjuvant irradiation,chemotherapy and hormone therapy were given postoperatively. Results Pathological examinations of breast specimens found no positive margins.Follow-up observations for a median of 54 months (range,2 ~ 84 months) revealed a local recurrence rate of 2 7% ( 4/148 ;3 patients in breast and 1 patient in axillary fossa).Distant metastasis occurred in 5 patients (3 4%) and 3 of them died.Sites of metastasis included lung,liver,meninges and bone.The duration from surgery to distant metastasis was 6 ~ 43 months.Evaluation of cosmetic results in 108 patients who had received BCT for at least 12 months showed that good outcomes were achieved in 32 4% of the patients (35/108),fair outcomes,in 49 1% of the patients (53/108),and poor,in 18 5% (20/108). Conclusions For patients with early stage breast carcinoma,and part of those with local advanced breast cancer who have received neoadjuvant chemotherapy and gotten downstaging effects,the effects of BCT is satisfactory.Standard excision,postoperative irradiation and systemic combination treatment are crucial to BCT.Most patients receiving BCT have good cosmetic results.
6.Treatment of locally advanced breast cancer with breast-conserving therapy after neoadjuvant chemotherapy: Report of 31 cases

12 months postoperatively,and excellent results were obtained in 16.0% of the patients((4/25)),good in 40.0%(10/25),and poor in 44.0%(11/25). Conclusions For patients with locally advanced breast cancer previously treated with downstaging neoadjuvant chemotherapy,breast-conserving therapy offers satisfactory results.Strict adherence to technique and use of postoperative irradiation and systemic therapy are crucial to breast-conserving therapy.
7.Relationship between apoptosis index,expression of P53 and radiosensitivity of rectal carcinoma

Objective:To investigate the correlation between the levels of the spontaneous apoptosis index(SAI),expressions of P53 and the radiosensitivity of rectal carcinoma.Methods:Thirty-six patients with rectal carcinoma who received preoperative radiotherapy were registered in this study.Specimens were obtained by biopsy before irradiation and after operation.The apoptotic index(AI) of each tissue specimen was evaluated by terminal deoxynucleotidyl transferase-mediated deoxyuridine triphophate nick-labling (TUNEL) assay and expressed as a percentage,Immunohistochemical evaluation for P53 and the radiosensitivity of the tumor was also peformed.Results:Consistent low levels of spontaneous apoptosis were observed in all specimens evaluated,the mean AI of spontaneous apoptosis was 0.98%+/-0.31%;There was an inverse correlation between SAI and the expression of P53 protein(r=0.72,P0.98% was significantly higher than that of SAI
8.Effect of early systematic rehabilitation on the functional recovery in patients after replantation of severed digit

ObjectiveTo observe the effect of early systemic rehabilitation trainings on the function recovery in patients after replantation of severed digit. Methods21 patients were planned systemic rehabilitation trainings more than 2 months after operation. Total Active Measurement (TAM) and Two Points Distance (2-PD) were used for the assessment of motor and sensory function. Employment situation were also followed-up after being discharged from hospital. ResultsThe omni-efficiency of motor and sensory function of the 21 cases was 92.3% and 88.5%. 19 cases of them returned to the original work, 2 cases changed another work. ConclusionThe early systemic rehabilitation trainings are very useful for the hands function and vacation ability of the patients after replantation of severed digit.
9.Intraoperative lumbar cathetering for prevention of postoperative cerebrospinal fluid leakage in lumbosacral vertebral canal

Objective To study the technique of lumbar cathetering in lumbosacral vertebral canal operation and its effect on prevention of postoperative cerebrospinal fluid leakage.Methods Retrospectively analyzed the clinical data of patients who underwent lumbosacral ver-tebral canal surgery and suffered from difficult dural repair during the operation in Department of Neurosurgery from August 2015 to October 2015.These patients were divided into the observation group (11 cases)and the control group (12 cases)according to whether lumbar cathe-ter were placed during the operation or not.Volume of drainage was strictly controlled after surgery.Remove the epidural drainage after primi-tive healing of the dura mater.The lumbar catheter was removed after 7 to 10 days.The indwelling time of each patient was collected and sta-tistical analyzed.Results The observation group got obvious shorter epidural drainage indwelling time than the control group(P <0.05). Short-term postoperative complications did occur in some cases in observation group,however,there was no obvious increase of infection rate in patients with lumbar continuous drainage indwelling.Conclusion Lumbar cathetering during the operation could be an effective method to solve difficult problem of placeing a lumbar drainage after lumbar puncture and to prevent cerebrospinal fluid leakage after operations of lum-bosacral vertebral canal.But it can not replace the delicate operation and tight dural suture.Drainage should be used only as a remedial measure of dural repair failure.
10.Experimental study on relationship between Smad4 and angiogenesis related factors in human gastric cancer cell line

Objective To study the effect of Smad4 gene on angiogenesis related factors in human gastric cancer cell line.Methods Recombinant eukaryotie expressing plasmid pcDNA3.1 (-)-Smad4 containing Smad4 gene and empty vector pcDNA3.1 (-) were introduced into human gastric cancer cell line MKN28 using lipofectam and selected by G418,respectively.Two cell lines were obtained as follows:Smad4+-MKN28 cell line which was MKN28 transfected with a stable hybrid containing Smad4 gene and Smad4--MKN28 cell line with empty plasmid as control.The transcription and expression of VEGF and TSP1 were investigated by RT-PCR and Western blot.Results The mRNA expression of TSP1 in Smad4+-MKN28 cells was higher (P<0.05) than that in control cells,while VEGF was lower(P<0.05).Western blot showed the consistent results as measurement by RT-PCR.Conclusion Smad4 restoration in gastric cancer cells reduced angiogenesis rates through down-regulation of angiogenesis activitor and up-regulation of angiogenesis inhibitor.
