Type 2 diabetes is a polygenic and complex disease.Various approaches, including association analysis, positional cloning, positional candidate gene approach and genome-wide association analysis, were used to identify its susceptible genes.This article summarized the recent findings on susceptible gene mapping of type 2 diabetes.
2.Risk factors associated with surgery and postoperative recurrence in patients with Crohn's disease

Objective To explore the risk factors associated with surgery and postoperative recurrence in patients with Crohn's disease (CD).Methods A total of 183 patients diagnosed as CD from January 2008 to December 2013 were enrolled.Forty-six patients after surgery were assigned to surgery group,and 137 non-surgical CD patients were assigned to control group.All the patients were followed up until the endpoint of the study or death,and the follow-up period was at least 12 months.Typing was according to the Montreal classification.The risk factors associated with surgery and postoperative recurrence were analyzed by Logistic regression.Results The surgery rate of CD patients was 25.1 % (46/183),the postoperative clinical recurrence rate of surgery group was 39.1% (18/46),and the recurrence time was 20 months (7~42 months).Before surgery,the rate of definite diagnosis of CD was only 13.0% (6/46).Stricture (odd ratio (OR)=5.836,95% confidence interval (CI) 2.199 to 15.487,P<0.01) and penetration (OR=25.706,95%oo CI 7.091 to 93.190,P<0.01) were independent risk factors for the surgery.Furthermore,perianal disease(OR=23.550,95%CI 1.311 to 422.912,P=0.032) and lack of prophylaxes treatment after surgery (OR=58.701,95%CI 1.803 to 1 991.000,P=0.022) were independent risk factors for postoperative clinical recurrence.Conclusions Stricture and penetration are risk factors for surgery.Perianal disease and lack of prophylaxes treatment after surgery are risk factors for postoperative clinical recurrence.

In order to find out an index for the assessment of chromium deficiency in diabetic patients, the contents of chromium in the hair of twenty-seven diabetic patients and sixty-three healthy subjects were determined. The contents of chromium in hair was determined with a flameless atomic absorption technique. 4.Quality standard for Tongbianling Capsule

AIM:To establish the quality standard for Tongbianling Capsule(Folium Sennae,Radix Angelicae sinensis,Herba Cistanches). METHODS: Folium Sennae,Radix Angelicae sinensis of Tongbianling Capsuls were identified by TLC.Cisttanoside and echinacoside were determined by HPLC together.The HPLC system consisted of shim-pack c_(18) column(4.6 mm?150 mm,5 ?m),acotonitrile-methanol-0.2% pure phosphoric water solution(7∶10∶83) mixture as mobile phase,flow rate of 1.0 mL/min,detection wavelength at 334 nm. RESULTS: The TLC method was suitable for the identification of salidroside and ptyrosol with good reproducibility.The quantitative evaluation of cistanoside had the linear range of 0.114 9-2.298 ?g,the average recovery was 98.2%(RSD=1.70%,n=6).The quantitative evaluation of echinacoside had the linear range of 0.147 9-2.958 ?g,the average recovery was 96.8%(RSD=1.53%,n=6). CONCLUSION: The TLC and HPLC method is found to have good stability and reproducibility for Tongbianling Capsule.
5.Requirement analysis and database design of USIS

Based on the development and application of medical ultrasonic diagnosis information system(USIS) of Nanjing Maternity and Child Health Hospital,this article deeply expounds the requirement analysis and the database design of USIS's development.With the analyses of the distinguishing feature of ultrasonic diagnosis work flow and the design of the relational data structure of USIS,the thought and method to overcome the difficulty of data storage are proposed.
6.Transradial interventional treatment of coronary artery lesions by using sheathless common 7-F guiding catheter: its clinical efficacy

Objective To compare the efficacy and safety of the use of sheathless common 7-F guiding catheter with those of conventional 6-F sheath guiding catheter in performing transradial percutaneous coronary intervention (PCI) for coronary artery lesions.Methods The clinical data of 160 consecutive patients with coronary artery lesions,who were treated with transradial PCI during the period from January 2014 to June 2015,were retrospectively analyzed.The patients were randomly divided into sheathless group (n=80,using sheathless common 7-F guiding catheter) and sheath group (n=80,using conventional 6-F sheath guiding catheter).The accuracy of the guiding catheter placement,the compression time for the punctured radial artery,the success rate of hemostasis,the incidence of radial artery spasm,the postoperative diameter of radial artery,the fading time of swelling at distal arm,the forearm pain or discomfort,the formation of forearm hematoma,the incidence of pseudoaneurysm and radial artery occlusion,the skin ischemia necrosis,etc.were recorded,and the results were compared between the two groups.Results In the sheathless group and the sheath group,the radial artery spasm was observed in 9 patients and 2 patients respectively,the forearm pain or discomfort was seen in 7 patients and one patient respectively,the differences between the two groups were statistically significant (P<0.05).No statistically significant differences in the accuracy of the guiding catheter placement,the compression time for the punctured radial artery,the formation of forearm hematoma,the success rate of hemostasis,the postoperative diameter of radial artery,the incidence of radial artery occlusion and the skin ischemia necrosis existed between the two groups (P>0.05).Conclusion Compared to the use of conventional 6-F sheath guiding catheter,the use of sheathless common 7-F guiding catheter is safe and effective in treating coronary artery lesions with transradial PCI.
7.Clinical Characteristics of Meniscal Cyst of the Knee:Analysis on 104 Cases

Objective To study the clinical features of meniscal cyst of the knee. Methods From January 2001 to December 2007,totally 5986 cases of meniscus operation were performed in our hospital,104 of them were diagnosed as having meniscal cyst in the knee. The clinical data of these patients,including gender and age distributions,causes and mechanism of injury,clinical presentations,characteristics of the cyst,and the type of the injury,were investigated. Results Incidence rate of meniscal cyst was 1.7% (104/5986) and the ratio between medial and lateral meniscal cysts was 1∶12 (8∶96). The mean age of the 104 patients was (30.7?12.2) years (range,13 to 65 years),and the radio of male to female was 1.2 to 1. Of the patients,60 cases (57.7%) had history of trauma,32 or them were hurt in sports,including football (11 cases),basketball (8 cases) and others. The most common primary symptom was pain (102 cases,98.1%),followed by swelling (50 cases,48.1%). Tenderness over the joint line (85 cases,81.7%) and palpable mass (79 cases,76.0%) with or without signs of meniscal tear were common presentations. The anterior and middle portions of the medial meniscus (5/8,62.5%),and posterior extension of the lateral meniscus (88/96,91.7%) were the most commonly involved sites (?2=15.167,P=0.000). The most frequently observed meniscal tear was a horizontal cleavage,alone (25/104,24.0%) or associated with another types of lesions (52/104,50.0%). In 5.8% (6/104) of the cases,the discoid meniscus was involved. Conclusions Meniscus cysts are unusual. The ratio between medial and lateral meniscal cysts favors the lateral meniscus. Meniscal tear and myxoid degeneration are the common causes of the cyst. The most frequently observed meniscal tear is a horizontal cleavage,and the anterior and middle portions of the medial meniscus and posterior extensions of the lateral meniscus are the most commonly involved sites.
8.Preparation of Berberine Hydrochloride Poloxamer 188 Solid Dispersion

OBJECTIVE: To prepare solid dispersion of berberine hydrochloride poloxamer 188. METHODS: Melting method was adopted to prepare solid dispersion. The influences of the ratio of the drug to poloxamer 188, melting temperature and cooling temperature on the dissolution of drugs were investigated. The difference of the dissolution between solid dispersion and physical mixture were explored. RESULTS: When the ratio of the drug to poloxamer 188 rose to 1 ∶ 1, the amount of the carrier was enough to distribute the drug evenly. Melting temperature had little influence on the dissolution of drugs while cooling temperature had strong influence on the dissolution of drugs. The fastest dissolution rate was achieved emerged when the cooling tempreture was 0℃. As compared with physical mixture, the dissolution rate of berberine hydroclride was nearly increased one time. CONCLUSION: Berberine hydrocloride poloxamer 188 solid dispersion improves the in vitro dissolution rate of berberine hydrocloride.
9.Efficacy of Letrozole in the Treatment of Breast Cancer in Postmenopausal Women:A Systematic Review

OBJECTIVE:To evaluate efficacy of letrozole in the treatment of breast cancer in postmenopausal women. METHODS:RCTs about letrozole in the treatment of breast cancer in postmenopausal women were collected using Cochrane systematic evaluation method by retrieving from Cochrane Library,PubMed,CBMdisc and CNKI. The quality evaluation of those trials and data extraction were performed and Meta-analysis was conducted by RevMan 5.0 software. RESULTS:6 RCTs involving 6 316 patients were included. In neoadjuvant endocrine therapy,letrozole was more effective than tamoxifen in PFS but there was no statistical significance in difference of total survival rate;in rescue therapy for recurrence and transform,letrozole was more effective than long-term therapy of tamoxifen in OS,PFS and OR;in new neoadjuvant endocrine therapy before operation,letrozole was more effective than tamoxifen in OR. CONCLUSION:The current evidence shows that letrozole is more effective than tamoxifen for breast cancer in postmenopausal women.More high-quality clinical trials are expected for further study.
10.Management of condylar fractures by using open-mouth jaw plate in children

Objective: To investigate the effect of open-mouth jaw plate in the treatment of condylar fracture in children. Method: Open-mouth plates in the height of 2~3 cm were respectively made in op e n-mouth position for 12 children aged 3~6 years old with condylar fractures an d used by the children with the help of doctors or parents for 3~6 months.Follo w -up was carried out by clinical observation and X-ray radiograph.Re sults: Satisfactory results were obtained in all the patients examine d by clinical observation and X-ray radiograph. Conclusion: The method of open-mouth plate is effective in the treatment of condylar f racture in children.
