1.Progresses of hepatic segments imaging

Precise location of liver segments has great significance in diagnosis and treatment of hepatic disease. In recent years, medical imaging and three-dimensional imaging techniques provide more diagnostic information about the location of liver segments and research methods for finding more precise liver segment classification. The progresses of hepatic segment imaging were reviewed in this article.
2.Mechanism of Aspirin Resistance (review)

Aspirin is widely used for the prophylaxis of ischemic events in patients with atherothrombotic complications or in populations with high risk of thrombotic events.However,despite the treatment of aspirin some patients experience thrombotic events,such as stroke.So the concept of "aspirin resistance" has been put forward.The mechanism of aspirin resistance is discussed in this article.
3.Cheng Jing's Experience in Treating Decline in Ovarian Reserve

Objective]To discuss and analyze Professor Cheng Jing's clinical experience for treating decline in ovarian reserve(DOR). [Method]Through learning from the tutor in the outpatient service and case studies ,we analyzed and summarized Pro. Cheng's recognition on the etiology and pathogenesis of DOR,together with his dialectical way of thinking, combined therapy of Chinese and western medicine ,and empirical prescriptions. [Result] Pro. Cheng believes that this disease lies in the kidneys as wel as in the thoroughfare and conception meridian ,the pathogenesis is kidney deficiency and the imbalance of the thoroughfare and conception meridian,or combined with the stagnation of liver qi,spleen deficiency,and blood stasis and phlegm fire (heat); the therapy is mainly nourishing and warming kidneys as wel as conditioning the thoroughfare and conception meridian ,combined with Chinese medicine's periodical therapy,such as adjusting the circulation of qi and blood in livers,spleens,hearts and other organs. [Conclusions] Pro. Cheng's combined therapy of Chinese and western medicine for curing DOR ,of which his empirical prescriptions the Bushenyichongkangshuaitang is the main part,is of remarkable curative effect and significant clinical value.
4.Expression and clinical significance of VCAM-1 and E-selectin in thromboangiitis obliterans

Objective To investigate the expression of VCAM-1 and E-selectin in vessel walls of the patients with thromboangiitis obliterans(TAO).Methods The vessel tissues were taken from 18 cases of amputation operation due to TAO and 16 cases of amputation operation due to non-vascular diseases.The expression of VCAM-1 and E-selectin were assessed by the immunohistochemical method.Results The positive expression rate of VCAM-1 in vessel wall of the patients with TAO was 77.8%(14/18),which of E-selectin was 72.2%(13/18);both of them were obviously higher than 6.3%(1/16) and 6.3%(1/16) in the control group.The positive rate of VCAM-1 and E-selectin had no correlation with the age,smoking amount,smoking time and drug use (P>0.05).Conclusion The expression of VCAM-1 and E-selectin have close correlation with the occurrence and development of TAO.Detecting the expression of VCAM-1 and E-selectin can serve as a diagnostic indicator of TAO.
5.Bioanalytical Methodology Validation

OBJECTIVE: To introduce the methodology validation and quality control in bioanalytical procedures and its expected trend of development.METHODS: According to the bioanalytical validation reflected in FDA guidelines and other publications,based on the guidance of SFDA and the standard operating procedure of our phase I clinical trial lab,we summarized the procedure and questions of bioanalytical methodology validation.RESULTS: The methodology validation and quality control in bioanalytical procedures and the possible risks involved in the methodology validation were summarized.CONCLUSIONS: The established methodology validation in bioanalytical procedure and quality control contents were basically consummate,but the standards for the methodology validation should be formulated based on reality.
8.Selected three-field lymphadenectomy in thoracic middle-lower section esophageal carcinoma

Objective To explore the technique and effect of selected three-field lymphadenectomy by left thoracotomy in treatment of thoracic middle or lower section esophageal squamous carcinoma. Methods From Jun. 2005 to Mar. 2009, 213 patients with thoracic middle or lower section of esophageal carcinoma received esophagectomy, bilateral mediastinal lymphadenectomy and pleural membrane resection.Group 1 -5, 7 - 12a, 16al, and 19 were performed to dissect abdominal lymph node and extended thoracic and abdominal lymphadenectomy and only lymph node extraction of mesoesophagus in neck field. Results 14197 lymphatic nodes(LN) were detected in 213 case. The average number of resected LN was 66. 65 ±24. 73. The metastatic lymph node was detected in 105 cases. The metastatic rate was 49.05% (105/213).There were 423 metastatic lymph nodes. The lymph nodes metastasis was 2. 97% (423/14197) of all dissected lymphatic nodes. No remnant carcinoma in the upper and lower cutting edge was found in pathological examination. The operation time ranged from 2. 92 ~ 4. 67 ( 3. 37 ± 0. 42) hours. Blood transfusion during perioperative period was 0 ~ 6u ( 1.08 ± 0. 93 ) u. Perioperative plasma transfusion was 0 ~ 1400( 103.77 ± 184. 89) ml. The hospital-time was 14 ~ 39 ( 17.64 ±4. 12) days. There were no anastomotic leakage and recurrent laryngeal nerve injury. One case died from respiratory failure, the mortality was 0. 04% ( 1/213). Conclusion Surgical approach in the management of left thoracotomy in the sixth intercostals could extend resection of chest-field lymph node dissection, decrease neck field lymph node dissection. Abdomen-field lymph node dissection reached selected D3. The selected lymphadenectomy procedure had the advantages of small traumas and few complications.
9.Effect of artificial circadian rhythm of melatonin on postoperative cellular immune function in patients undergoing gynecological operation

Objective To investigate the effect of artifictal circadian rhythm of melatonin on the postoperative cellular immune function in patients undergoing gynecological operation Methods Eighteen ASA Ⅰ or Ⅱ patients,aged 25-50 yr,weighing 45-80 kg,scheduled for elective gynecological operation,were randomly divided into 3 groups (n =6 each):control group (group C),placebo control group (group P) and melatonin group (group M).In group M.melatonin 6 mg was given orally at 10 min before lights-out (21:00) on 1 day before operation,on the day of operation and on 1 day after operation,while placebo was given orally instead of melatonin in group P.The operation was performed under epidural anesthesia.Patient-controlled epidural analgesia with ropivacaine was used for postoperative analgesia.VAS score was maintained < 5.Blood samples were collected from the peripheral vein at 1 day before operation (baseline),the end of operation and 1 day after operation to measure CD4+,CD8+ and CD3+ cell count by flow cytometry.The ratio of the number of CD4+ cells to the number of CD8+ cells was calculated.Results There were no significant differences in the number of CD4+,CD8+ and CD3 + cells and ratio of the number of CD4 + cells to the number of CD8 + cells between groups C,P and M (P >0.05).Conclusion Artificial circadian rhythm of melatonin exerts no influence on the postoperative cellular immune function in patients undergoing gynecological operation.
10.Preparation and pharmaceutical characterization of glycyrrhetinic acid liposomes

Objective To prepare and characterize glycyrrhetinic acid cationic liposomes.Methods Liposomes were prepared by ethanol injection technique.An orthogonal test was utilized to optimize the formulation and preparation of glycyrrhetinic acid liposomes.The unencapsulated glycyrrhetinic acid and liposomes were separated by Sephadex gel G-50,the encapsulation efficiency was detected by HPLC.The morphological examination of glycyrrhetinic acid liposomes was performed using transmission electron microscopy.The particle size and Zeta potential of the liposomes were measured.The release rate of glycyrrhetinic acid from liposomes was tested.Results The liposomes with spherical or ellipsoidal shape and better stability featured the encapsulation efficiency of(91.61?1.16)%,the mean partical size of(141?10)nm,and Zeta potential of(35.9?5)mV.The in vitro release kinetics were consistent with Higuchi equation.The stability of glycyrrhetinic acid liposomes was better.Conclusion The selected formulation and preparation technic of glycyrrhetinic acid liposomes could be rational,stable,and with a sustained feature in vitro release.
