Bilateral sequential cataract surgery is very common in clinical,but the ophthalmologists often ignore how to choose the timing of operation and whether the first eye can be used to improve the intraocular lens (IOL) power calculation for the second one.Despite the refinements in IOL selection with the use of third-generation IOL formulas,5％ of eyes still result in a postoperative refractive surprise of ± 1.00 D or more.This paper introduced the methods of using refraction error of the first eye to improve the IOL power calculation for the second eye,and the refractive error of the second eye can be improved by modifying the IOL power to correct up to 50％ of the error from the first eye in details.Because of the variation of the postoperative refractive surprise within a month,the interval of three to four weeks between two eyes cataract surgery is highly recommended.These conclusions may provide the advantageous guidance for us in the selection of IOL in bilateral cataract surgery.

OBJECTIVE: To characterize the pharmacokinetics of a single dose of 2.5 mg fondaparinux sodium in Chinese patients after thoracic surgery. METHODS: Thirteen patients were enrolled in the study and were given a single dose of 2.5 mg fondaparinux sodium via a subcutaneous injection. Plasma concentrations of fondaparinux sodium were measured throughout a 24 h period. The pharmacokinetic analysis was performed using WinNonlin software. RESULTS: The following pharmacokinetic parameters were derived; t1/2(7.92 ± 3.13) (4.21-15.89) h, tmax(2.52 ± 0.93) (1.38-4) h, p-1 (0.73 ± 0.18) (0.45-1.02) mg · L-1, AUClast(6. 56 ± 2.19) (3.07-9. 75)mg · h · L-1, Vz/F(3.54 ± 1.00) (2.24-5.73) L, CL/F(0.34 ± 0.14) (0.16-0.63) L · h-1, and MRTlast (6.98 ± 2.1) h. The pearson correlation coefficient, r was 0.7283(P = 0.005) between the serum creatinine(Scr) and mean residence time(MRTlast) of fondaparinux. CONCLUSION: The disposition of fondaparinux is highly variable in patients. A statistical significant correlation between Scr and MRTlast of fondaparinux was observed. Fondaparinux sodium administered by intravenous injection is well tolerated in patients.

The microarray technology used in biological and medical research provides a new idea for the diagnosis and treatment of cancer. To find different types of cancer and to classify the cancer samples accurately, we propose a new cluster ensemble framework Dual Neural Gas Cluster Ensemble (DNGCE), which is based on neural gas algorithm, to discover the underlying structure of noisy cancer gene expression profiles. This framework DNGCE applies the neural gas algorithm to perform clustering not only on the sample dimension, but also on the attribute dimension. It also adopts the normalized cut algorithm to partition off the consensus matrix constructed from multiple clustering solutions. We obtained the final accurate results. Experiments on cancer gene expression profiles illustrated that the proposed approach could achieve good performance, as it outperforms the single clustering algorithms and most of the existing approaches in the process of clustering gene expression profiles.

Objective To investigate the clinical effect of uterine arterial embolization combined with hysteroscopes in the treatment of cesarean scar pregnancy(CSP).Methods Seventy CSP patients were divided by willingness of patients and their familiality into treatment group and control group with 35 cases each.The treatment group was treated with uterine arterial embolization combined with hysteroscopes,while the control group was treated with uterine curettage after crug therapy.The intraoperative and postoperative situations of two groups were observed.Results The intraoperative blood loss,erythrocyte suspension transfusion,plasma transfusion,preparation time before curettage,hospital stay and pestoperative serum β- human chorionic gonadotropin in treatment group were all obviously lower than those in control group [(420.7 ± 18.9) ml vs.(1893.9 +39.8) ml,(2.1 + 1.3) U vs.(11.1 ±1.4) U,(400.2 ±23.6) ml vs.(1700.3 ± 31.4) ml,(3.9 ± 2.3) d vs.( 18.8 ± 6.7) d,( 14.32 ± 3.89) d vs.(37.83 ± 8.98) d,(312 ± 54) U/L vs.(532 ± 85) U/L,P＜ 0.01].There was 1 case with massive hemorhage in treatment group,while 3 cases in control group.Conclusions Uterine artery embolization combined with hysteroscopes is a safe and effective treatment with less trauma for CSP.It is worthy of clinical promotion and application.

ObjectiveTo investigate the expression of Twist and N-cadherin in pancreatic cancer,and to study its relationship with clinicopathological parameters and patients' prognosis.Methods The expression of the Twist and N-cadherin in 62 tissue samples from patients with pancreatic ductal adencocarcinoma and 10 normal pancreatic tissue samples was determined by using immunohistochemistry MaxVision two step method,and the relationship with clinicopathological parameters and patients' prognosis was analyzed.ResultsThe positive expression rate of Twist in pancreatic cancer was higher than that of normal pancreatic tissues (96.8％ vs.30％,P ＜0.01 ),and the positive expression rate of N-cadherin was higher than that of normal pancreatic tissues (75.8％ vs.0,P ＜0.01),but there was no correlation between them (r =0.100,P =0.441 ).The expressions of Twist and N-cadherin was significantly correlated with TNM stage,lymph node metastasis,infiltration of portal vein or nerves and tumor location ( P ＜0.05 ),but not with age,gender and degree of differentiation ( P ＞ 0.05 ).The post-operative survival of patients decreased with the increasing Twist expression,but the survival was not associated with the expression of N-cadherin.TNM stage,the expression of Twist was independent predictive factors of prognosis for pancreatic cancer patients.ConclusionsTwist and N-cadherin are highly expressed in pancreatic cancer,and the expression was associated malignant behavior of pancreatic cancer.The abnormal expression of Twist may be a potential marker for prognosis evaluation of pancreatic cancer patients.

Objective To explore the influence of vacuum sealing drainage(VSD) technology in snake bite treatment.Methods 108 patients of snake bites need to incision decompression were randomly divided into two groups,VSD group (VSD) 58 cases,conventional group (wound change dressings ordinarily) 50 cases.The secondary suture of time,the number of change dressings,material consumption fees,antibiotics use cost,wound infection rate,in hospital,average length of hospital stay were compared between two groups.Results Compared the VSD group with the conventional group,the secondary suture of time (t =11.743,P ＜ 0.05),the number of change dressings (t =50.661,P ＜ 0.05),material consumption fees (t =9.054,P ＜ 0.05),antibiotics use cost (t =21.515,P ＜ 0.05),wound infection rate (x2 =9.155,P ＜ 0.05),in hospital,average length of hospital stay (t =10.350,P ＜ 0.05) had significant differences.Conclusion VSD technique can be a ideal way to treat snake bites patients and worth popularizing at this stage.

Questionnaire survey of case teaching was conducted among medical marketing majors,the results showed that case method was widely used in the teaching for medical marketing majors and was approved by most students,however,deficiencies still existed including lack of required teaching skills,outdated teaching concept,low-quality cases,inappropriate teaching methods,few participation,etc.To solve these problems,during the preparation and implementation of case teaching,teachers must update their teaching concept,participate in more training programs,improve case qualities,reengineer case activities and evaluate the effects at each phase.

Objective To explore the clinical value of cervical lesion screening,which was detected by Pap smears,thinprep cytologic test (TCT),full automatic cellular DNA quantitative analysis system,human papilloma virus detection,colposcopy staining biopsy and blind biopsy.Methods 8 780 peoples were as the study object,divided into four groups,positive rate and accuracy of different screening methods;In addition,which were the positive rate of colposcopy staining biopsy and blind biopsy,the positive rate of HPV in all positive biopsy people and negative biopsy people,and the HPV infection rate of various lesions in positive patients were compared,respectivey.Results (1) The positive rates between the four groups showed an increasing trend (P ＜ 0.05).The positive rate of Pap smears and TCT had no statistical difference between every two of them (P ＜ 0.05),the rest were statistically significant (P ＜0.05).(2) The accuracy rates between the four groups showed an increasing trend (P ＜ 0.05).The accuracy rate of Pap smears and TCT had no statistical difference between two of them(P ＞ 0.05),the rest were statistically significant (P ＜ 0.05).(3)The positive rate of colposcopy biopsy under vision inspection with acetic acid (VIA) was higher than that of blind biopsy (P ＞ 0.05).(4) The positive rate of HPV detection of positive biopsy people was higher than that of negative biopsy people(P ＜0.05).(5)In colposcopy biopsy under vision inspection with acetic acid(VIA)group,the HPV positive rates of inflammation showed an increasing trend(P ＜ 0.05) ;in blind biopsy group,the HPV positive rates also showed an increasing trend (P ＜ 0.05).Conclusion (1) HPV test has the highest sensitivity and strongest specificity,cellular DNA quantitative analysis is second,Pap smears is worst,so HPV and DNA detection have more valuable in clinical.(2)There is no significant difference between colposcopy biopsy under vision inspection with acetic acid(VIA)and blind biopsy in clinical,but HPV infection is the key factor,which affects the occurrence,development of cervical intraepithelial neoplasia and gradually progresses to cervical cancer.

OBJECTIVE　To assess the efficacy and safety of Lipobay (cerivastatin) and Jingbishuxin (simvastatin) on lipid profiles.METHODS　40 patients with hypercholesterolemia were randomizedly divided into Lipobay group (0.3 mg,QN) and Jingbishuxin group (domestic simvastatin tablet) (20 mg,QN)for 4 weeks.Total cholesterol (TC),low density Lipoprotein cholesterol (LDL-C),triglyceride (TG) and high density lipoprotein cholesterol (HDL-C),blood sugar,hepatic and renal function,blood and urine regular test were measured before and after the treatment.RESULTS　After 4 weeks of treatment,TC,LDL-C and TG were reduced by 29.02%,29.95%,17.21% in Lipobay the group and 22.48%,28.68%,14.74% in Jingbishuxin group,respectively.Compared the measurements after treatment with that before treatment,there were significant differences in both groups,but there was no significant difference between them.No serious adverse effect was found.CONCLUSION　Lipobay had a same efficacy and safety on lipoprotein profiles as that of Jingbishuxin in elderly.It has a good tolerance with smaller dose.
