OBJECTIVE:To provide reference for investigating the disease burden of diabetic patients and promoting rational drug use. METHODS:Open-ended questionnaires were adopted to survey the willingness to pay(WTP)of some diabetic patients in Sichuan Provincial People’s Hospital and collect the data about personal information, health status, the affordability of disease and WTP,the data was sorted with Excel (version 2013) software by two operators and analyzed with SPSS 17.0 software by step-wise multiple linear regression to explore the influential factors of WTP values. RESULTS:Totally 120 questionnaires were sent out and 106 were effectively received with effective rate of 88.3%. The average age of respondents was (59.9 ± 13.9) years old,the average body mass index (BMI) of respondents was (24.5 ± 0.5) kg/m2,the highest proportion of monthly diabetes fees was no more than 300 yuan, followed by more than 300-500 yuan and more than 500-1 000 yuan;the awareness rates of fasting plasma glucose (FPG),2 h postprandial blood glucose (2 h PG) and glycated hemoglobin (HbA1c) were 95.3%,86.8% and 68.9%,respectively;and patients with more than 2 complications accounted for 29.5%. The WTP values of respondents to the complete recovery of HbA1c,FPG and 2 h PG were 1 604.1,1 617.9 and 1 683.7 yuan/month,the median values were 1 000 yuan/month;the WTP values of respondents to the 10% recovery were 867.4,908.5 and 877.9 yuan/month,the median values were 500,575 and 510 yuan/month,respectively. The influential factors with positive effects and significant differences on WTP values in step-wise multiple linear regression were total annual household income, complications, hospital experience, family history of di-abetes,diabetes mental status and cost of diabetes in the past months;those with negative effects and significant differences on WTP values were age,FPG level and education level. CONCLUSIONS:Diabetic patients, particularly, with peripheral neuropa-thy,high blood lipids or diabetic foot have higher WTP value, which shows they hole heavier mental burden and disease burden than others. Patients recovered 10% percent were closer to actual treatment state, which better show the influential factors of WTP values actually, and patients with a family history of diabetes, depressed mood, higher fees in the average monthly cost, and hospi-tal patients have higher WTP values.

Chinese Traditional and Herbal Drugs 1994;0(02):-
Objective To study the absorption and transportation of flavonoids in Herb Epimedii by using Caco-2 monolayer model. Methods Caco-2 cell monolayer model was used to study the bi-direction transport of icariin, epimedin A, epimedin B, epimedin C, and baohuoside Ⅰ. The concentration of the five flavonoids in cell culture medium was measured by UPLC and the apparent permeability coefficients (Papp) were calculated. Results The absorptive permeability coefficients (PAB) of icariin, epimedin A, epimedin B, epimedin C, and baohuoside Ⅰ were 5.91?10-7, 3.22?10-7, 2.76?10-7, 4.23?10-7, and 1.46?10-6 cm/s, respectively. Except baohuoside Ⅰ, the other four flavonodes had lower permeabilities, and the secretive permeabilities (PBA) of all the flavonoids were larger than their absorptive permeabilities. Among them, the PBA of baohuoside Ⅰ was 9.8 times as much as the PAB. Conclusion The results suggest that the intestinal absorption of the five flavonoids is lower, which might have efflux mechanism by transporters, and the absorption of monloglycoside (e.g. baohuoside Ⅰ) is better than that of diglycoside (e.g. icariin) and triglycoside (e.g. epimedin A, epimedin B, and epimedin C).

Objective To observe changes of the partial oxygen pressure in the deep tissues along the Large Intestine Meridian (LIM) during acupuncture stimulation,so as to reveal the characteristics of energy metabolism in the tissues along the LIM. Methods Thirty-one healthy volunteer subjects were enlisted in the present study. Partial oxygen pressure (POP) in the tissues (at a depth of about 1.5 cm) of acupoints Binao (LI 14),Shouwuli (LI 13),Shousanli (LI 10),2 non-acupoints [the midpoints between Quchi (LI 11) and LI 14,and between Yangxi (LI 5) and LI 11) of the LIM,and 10 non-meridian points,1.5-2.0 cm lateral and medial to each of the tested points of the LIM was detected before, during and after electroacupuncture (EA) stimulation of Hegu (LI 4) by using a tissue oxygen tension needle-like sensor. Results In normal condition, the POP values in the deep tissues along the LIM were significantly higher than those of the non-meridian control points on its bilateral sides. During and after EA of Hegu (LI 4),the POP levels decreased significantly in the deep tissues along the LIM in comparison with pre-EA (P0.05). Conclusion POP is significantly higher in the deep tissues along the LIM of healthy subjects under normal conditions, which can be downregulated by EA of Hegu (LI 4),suggesting an increase of both the utilization rate of oxygen and energy metabolism after EA.

Objective To investigate the role of OX40 in the mechanisms of memory T cells in islet transplant tolerance. Methods The expression of OX40 on native, like memory and memory CD8+T cells was detected by RT-PCR. Splenic T cells from B6 mice were injected into Rag-/- mice via the tail vein, and the Rag-/- mice were divided into three groups (n=8 each): control group, given IgG; treatment group, given anti-OX40L; and OX40 knock-out group, given T cells from OX40 knock-out B6 mice spleen. All recipients were induced into diabetes mellitus model after adoptive transfer. Islet transplantation was performed on all Rag-/- mice as recipients. The mean survival time of islet was observed. Results The expression of OX40 in native T cells, like memory T cells and memory T cells was 2.87, 111.24 and 146.15 respectively. The expression of OX40 in like memory and memory T cells was higher than in native T cells (P<0.05). Comparison with control group , The mean survival time of the DBA/2 islet allografts in treatment group (130 days) and OX40 knock-out group (125 days) was significantly longer than in control group (21 days, P<0.05).Conclusion The OX40 expression is high in memory T cells. The mean survival time of the islet allografts can be prolonged by blocking OX40/OX40L pathway. OX40/OX40L pathway may be the key point of transplant tolerance.
