1.Systemic lupus erythematosus combined with antiphospholipid syndrome and cerebral thrombosis in a child: a case report and literature review

Objective To explore the clinical features of systemic lupus erythematosus (SLE) combined with antiphospholipid syndrome (APS) and cerebral thrombosis in a child. Method The clinical data of SLE combined with APS and cerebral thrombosis in a child was retrospectively analyzed, and the related literature was reviewed. Results This was a 12-year-old boy. The disease onset with recurrent fever, confusion and rash in cheek. He had anemia and thrombocytopenia, and positive antinuclear antibody (ANA) and anticardiolipin antibody (aCL). Magnetic resonance imaging showed multiple cerebral infarction. The diagnosis of SLE combined with APS and cerebral thrombosis was clearly made. Methylprednisolone, cyclophosphamide, warfarin, meropenem and acyclovir were used for the treatment. At the same time, the patient also received intravenous immunoglobulin. Conclusion SLE combined with APS and cerebral thrombosis in children was usually in a severe condition, the prognosis of which can be effectively improve by early diagnosis and reasonable treatment.
2.Clinical Research on Zishen Anshen Decoction Combined with Compound Zaoren Capsule for Treatment of Insomnia with Disharmony Between Heart-yang and Kidney-yin Syndrome

Objective To observe the efficacy of Zishen Anshen Decoction combined with Compound Zaoren (Ziziphi Spinosae Semen) Capsule on insomnia patients with disharmony between heart-yang and kidney-yin syndrome and its effect on sleep quality index. Methods Seventy insomnia patients were randomly divided into treatment group and control group. Both groups conventionally took estazolam as needed, the treatment group was additionally given Zishen Anshen Decoction (in the afternoon and after dinner) and Compound Zaoren Capsule (before sleep at night) for 14 d. The Pittsburgh sleep quality index (PSQI) score and dosage of estazolam were used as evaluation indexes for the comparison between the two groups. Results PSQI score of daytime function disorder and hypnotics dosage of the treatment group was significantly lower than that of the control group, and the dosages of estazolam were reduced (P<0.05). Conclusion The alternative application of Zishen Anshen Decoction and Compound Zaoren Capsule on insomnia treatment can not only improve daytime function, but also reduce the dosage of benzodiazepine, with better effect than estazolam.
3.Effect of recombinant bovine basic fibroblast growth factor ongingival sulcus bleeding index and its efficacy in gingivitis

Objective To investigate the treatment effect of recombinant bovine basic fibroblast growth factor (rb-bFGF) in patients with gingivitis and its influence on gingival sulcus bleeding index. Methods Sixty-eight patients with gingivitis treated in our hospital were randomly divided into control group (34 cases) and observation group (34 cases). The control group were given basic periodontal therapy, while the observation group were treated with rb-bFGF. The clinical effects, gingival sulcus bleeding index (SBI) before and after treatment, and the recurrence were compared between the two groups. Results After treatment, the total effective rate (97.06%) of the observation group was significantly higher than that of the control group (70.59%), the difference was statistically significant (P<0.05). The SBI of the observation group was significantly lower than that of the control group at 1 and 2 weeks after treatment (P<0.05). The recurrence rate of the observation group (5.88%) was significantly lower than that of the control group (23.53%) (P<0.05). The incidence of complications such as pain, periodontal abscess, bromopnea and loosening was significantly lower in the observation group (8.82%) than in the control group (32.35%). The difference was statistically significant (P<0.05). Conclusion After rb-bFGF treatment, the symptoms of toothache and gingival swelling disappear, normal function of teeth is restored, gingival sulcus bleeding improves significantly, with fewer adverse reactions and obvious clinical effect.
4.Flexible Ureteroscopy and Laser Lithotripsy for Renal Calculi:Report of 338 Cases

Objective To evaluate the efficacy of the flexible ureteroscopy and Holmium and FREDDY laser lithotripsy for renal calculi at various locations.Methods From May 2002 to May 2007,338 cases of renal calculi(sized less than 20 mm in diameter)were treated by flexible ureteroscopy and laser lithotripsy in our hospital.The data of the patients were analyzed retrospectively.Among the cases,the calculi were located at the upper or middle calyx or the renal pelvis in 288 patients,at the lower calyx in 37,and in multiple calyxes in 13.All the patients received KUB and IVU before the procedures.With the TerumoTM settled in place,a F8/9.8 Wolf rigid ureteroscope was inserted to observe and dilate the ureteral lumen.Then,an Olympus P3 6.9F flexible ureteroscope was used.When the insertion of the rigid ureteroscope was difficult,a re-ureteroscopy would be tried 2 weeks later with a pig-tail stent in place.The WOM U-100 FREDDY laser and the Lumenis Holmium laser were used to perform the procedure and a pig-tail stent would be set at the end of the procedure.The urethral catheter was removed in the first postoperative day and an intravenous antibiotic prophylaxis was administrated during the first 2 days postoperatively.A follow-up by KUB or non-contrast CT scan was done 4 weeks after the procedure to evaluate the result of the endoscopic nephrolithotripsy.Any residual calculi larger than 4 mm in diameter was taken significant.Results In 28 patients the rigid ureteroscope or the ureteral sheath couldn't be inserted in the first time,and the success rate of the ureteroscopic insertion was 91.7%(310/338).The re-ureteroscopy was successful in the patients after 2 weeks.The calculi were detected in 97.3%(329/338)of the patients,and the procedures succeeded in 306 of them(306/329,93.0%).The success rate of laser nephrolithotomy was 78.0%(32/41)for the lower calyx calculi,which was significantly lower than that for the middle,upper calyxes or renal pelvic calculi 95.1%(274/288),?2=13.601,P=0.000].After 4 weeks,the total evacuation rate of the calculi was 87.6%(296/338).For the calculi in the upper or middle pole or in the renal pelvis,the complete evacuation rate was 90.1%(264/293),which was significantly higher than that for the lower pole 71.1%(32/45),?2=12.929,P=0.000].No patient had ureteric perforation or active bleeding after the surgery.The mean operation time was 35 min(12-55 min).After the operation,11 cases developed nephritic colic.None of them had septicemia or acute renal failure.Gross hematuria disappeared in 1-2 days postperation.Conclusions The flexible ureteroscopy and Holmium and FREDDY laser lithotripsy are safe and effective for renal calculi sized less than 20 mm in diameter,especially for those in the upper,middle poles and renal pelvis.A re-ureteroscopy with the pig-tail stent in place is suggested in two weeks after the failure of the first procedure.
6.Polylatic acid microspheres-induced CTL cytotoxicity against hepatocellular carcinoma cell lines

Objective: To investigate the in vitro efficiency of two kinds of polylatic acid (PLA) immunomicrospheres: M1(hAFP158~166)and M2(hAFP218~226) in specific inducement of CTL and the cytotoxicity of CTL against hepatocellular carcinoma cells. Methods: Pripheral blood monocytes (PBMC) of HLA-A2+ healthy volunteers stimulated in vitro by peptide-loading microspheres were taken as effectors. The experiments were divided into three groups: control group, hAFP+ tumor cells group, and hAFP- tumor cells group. The specific cytolysis of target cells was assessed by 51Cr-release assay. Results: Both M1 and M2 induced proliferation of HLA-A2+ PBMC, forming visible clones. The effector cells induced by M1 and M2 both had an cytolysis rate over 75% for hAFP+ tumor cells, which was significantly higher than that for hAFP- tumor cells (P0.05). Conclusion: The two kinds of microspheres can both induce specific CTL in vitro, which can effectively kill hAFP+ tumor cells.
7.Prediction of miR-199a-3p targets gene and its bioinformatics analysis

Objective To provide theoretical guidance for further research on the role of miR-1 99a-3p in formation and development of bladder cancer.Methods Mature sequence of miR-1 99a-3p was analyzed;target genes and transcription factors of miRNA-1 99a-3p were predicted,and the target genes were analyzed for gene ontology (GO)enrichment and Kyoto Encyclopedia of Genes and Genome (KEGG)pathway.Then TF-miRNA-mRNA network diagram was constructed.Results Sequences of miR-1 99a-3p were highly conserved in various species.In GO analysis,the target genes of miR-1 99a-3p were enriched in many biological processes,such as regulation of cellular process,regulation of macromolecule metabolic process,and regulation of biological process (P <0.01 ).In KEGG pathway,the target genes were mainly located in bacterial invasion pathway of epithelial cells,ECM-receptor interaction pathway,PI3K-Akt signaling pathway,MAPK signaling pathway,small cell lung cancer pathway,and proteoglycans pathway in the cancer (P <0.05).According to the TF-miRNA-mRNA network diagram,the important genes that might be regulated by miR-1 99a-3p were MYC,SP1,mTOR,NFκB,and NFκB1.Conclusion miR-1 99a-3p may directly target mTOR and participate in the formation and development of bladder cancer through regulating PI3K-Akt-mTOR signaling pathway.
8.Expression of miRNA-148a in Bladder Carcinoma Tissues and Its Bioinformatics Analysis

Objective To explore the role of miRNA-148a in bladder tumorous development and progression.Methods Ex-pression of miRNA-148a was assessed in 35 bladder carcinoma tissues and 16 non-carcinoma tissues by fluorescence quanti-tative real time PCR,and correlation with clinical features was evaluated.Target genes and transcription factors of miRNA-148a were predicted using bioinformatic analysis,then TF-miRNA-148a-target genes network diagram was built and the tar-get genes was analyzed of gene ontology enrichment and KEGG pathway.Results Expression of miRNA-148a was lower in bladder carcinoma tissues than in non-carcinoma tissues(0.000 8±0.000 2 vs 0.002 1±0.000 5)(t=2.46,P <0.05),but its expression was no statistical significance in different groups of gender,age,pathological classification,clinical stage, lymph node metastasis (P >0.05).268 target genes of miRNA-148a were predicted by three softwares at the same time,60 transcription factors were predicted and the binding sites with combination scroes above 80 was 657.The target genes of miRNA-148a was enriched in many biological processes,such as neuron differentiation,generation of neurons,neuron projec-tion development,cytoplasmic mRNA processing body,cytoplasm(P <0.001).They also participated in p53 signaling path-way,proteoglycans in cancer-homo sapiens,pathways in cancer,prostate cancer,protein processing in endoplasmic reticulum, focal adhesion and so on(P <0.05).According to TF-miRNA-148a-target genes network diagram,miRNA-148a was regula-ted by SP1,ESR1,AP1,MYC and BRCA1,genes of IGF1,P27kip1 ,NCOA1,PTEN,SERPINE1 might be regulated by miR-NA-148a.Conclusion miRNA-148a which was significantly down-regulation in bladder carcinoma tissues may be participate in bladder tumorous development and progression,bioinformatics analysis provides some ideas for further research.
9.Effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function in patients with hypothyroidism

Objective To investigate the effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function (VEF) in patients with hypothyroidism (PWH). Methods The number of mono-cyte CD141and platelet CD36 from 52 patients wtih PWH and 40 healthy persons (control group) was calculated by flow cytometry. VEF was detected in terms of brachial artery flow-mediated dilatation (FMD) using Doppler ultrasonography. The differences of expressions of monocyte CD141 and platelet CD36 were compared between the two groups, so was FMD. The association of monocyte CD141 and platelet CD36 with FMD (%) was analyzed by Pearson analysis. Results Expression of monocyte CD141 (%) was significantly higher in the PWH group than in the control group (21.79 ± 9.45 vs. 18.84 ± 9.64, P < 0.001), so was expression of platelet CD36 (%) (34.26 ± 10.23 vs. 28.65 ± 9.53, P < 0.001). FMD% was lower in the PWH group than in the control group (8.65 ± 1.97 vs. 11.25 ± 2.72, P < 0.001). CD141 was negatively correlated with FMD% (r = -0.657, P < 0.01), so was CD36 (r = -0.557, P < 0.01). Conclusions Both monocyte CD141 and platelet CD36 are significantly higher in the PWH group than in the control group. CD141 and platelet CD36 are negatively related with VEF.
10.Minimally invasive pie-crusting technique combined with arthrolysis for the treatment of the stiff knee.

OBJECTIVETo study the effectiveness of pie-crusting technique in improving the stiff knee.
METHODSFrom February 2012 to December 2013, 13 patients with stiff knee were reviewed retrospectively. There were 6 males and 7 females, ranging in age from 39 to 70 years old (averaged, 55.6 years old). Of the 13 cases, 8 patients had stiffness following fracture (comminuted tibial plateau fracture in 4, femoral supracondylar fracture in 3 and patellar fracture in 1), 5 patients had TKA-related stiffness.
RESULTSA follow-up lasted 8 to 12 months (mean 10 months)in 13 cases. The mean maximum flexion increased from (37 ± 6)° preoperatively to (52 ± 7)° after arthrolysis, and (108 ± 7)° after pie-crusting. At the final follow-up, mean maximum flexion was (105 ± 6)°. According to Judet evaluation system, 10 patients got an excellent result and 3 good. No major complications, such as extensor lag, skin necrosis, deep infection, dislocation of the patella or recurrent stiffness were found.
CONCLUSIONThe percutaneous technique of pie-crusting is a simple, minimally invasive and effective treatment for knee stiffness.
