1.Venesection Ventouse Coordinated with Helium-neon Laser Combined with Drugs Treat Early Herpes Zoster

Objective]Observe the clinical effect of early herpes zoster which were treated by venesection ventouse combined with helium-neon laser and drugs. [Methods]The control group 32 cases used Famciclovir Tablet,transfer factor capsules,mecobalamin and acyclovir cream-drug for external use. While the experimental group 32 cases used Famciclovir Tablet,transfer factor capsules,mecobalamin and acyclovir cream-drug for external use. While the experimental group besides taking above-medicine, were treated with venesection ventouse and helium-neon laser at the pathological change. [Results]The control group used to cure for an average of 13 ±2.5 days. The experimental group used to cure for an average of 9.6 ±2.4 days. There were significant differences(P<0.05). [Conclusion] Early herpes zoster treated by venesection ventouse combined with helium-neon laser and drugs, can significantly shorten the duration of symptoms, alleviate suffering, and has good clinical efficacy.
2.Effect of propofoi on LPS-induced TLR4 expression in rat alveolar type Ⅱ epithelial cells

Objective To investigate the effect of propofol on LPS-induced TLR4 expression in rat alveolar type Ⅱ epithelial cells. Methods The primarily cultured alveolar type Ⅱ epithelial cells isolated from male rats were randomly assigned to one of 5 groups: group Ⅰ cells were incubated for 3 h without any additive (control) ; group Ⅱ cells were incubated with LPS 1 μg/ml for 3 h (LPS) ; group Ⅲ , Ⅳ , Ⅴ cells were incubated with LPS 1 μg/ml + propofol 25, 50 and 100 μmol/L respectively for 3 b (P1-3). TLR4 mRNA and TLR4 protein expression was detected by real time PCR and Western blot. TNF-α release amount was measured using ELISA. Results LPS significantly' increased TLR4 mRNA and protein expression in alveolar type Ⅱ epithelial cells as well as TNF-α release amount. Propefol at 50 and 100 μmol/L significantly inhibited LPS-imluced increase in TLR4 mRNA and protein expression and TNF-α release amount. Conclusion Propofol can dose-dependently inhibit LPS-induced inflammation in alveolar type Ⅱ epithelial ceils, through down-regnlation of TLR4 gene and protein expression.
3.Myocardial protective effect of ischemic preconditioning in patients undergoing open heart surgery: a meta-analysis

Objective To systematically review the myocardial protective effect of ischemic preconditioning in patients undergoing open heart surgery.Methods PubMed,EMBASE,Highwire,CENTREN and its affiliated clinical trial registration data center,CBM and CNKI were searched to identify all randomized controlled trials involving the myocardial protective effect of ischemic preconditioning in patients undergoing open heart surgery under general anesthesia.The quality of the studies was evaluated by the method recommended by Cochrane Collaboration.Evaluation indexes included the incidences of postoperative cardiac death,myocardial infarction and ventricular arrhythmias and postoperative inotropic drug requirement.Meta-analysis was conducted using the Cochrane Collaboration's RevMan 5.0 software.Results Twelve randomized controlled trials (8 high-quality trials and 4 medium-quality trials) involving 626 patients were included in our Meta-analysis.Meta-analysis indicated that ischemic proconditioning significantly reduced the incidence of postoperative ventricular arrhythmias and postoperative inotropic drug requirement (P < 0.05) and had no effect on the incidences of postoperative cardiac death and myocardial infarction (P > 0.05).Conclusion Ischemic preconditioning can reduce the postoperative ventricular arrhythmias and have no effect on postoperative cardiac death and myocardial infarction in patients undergoing open heart surgery.
4.Hemagglutinin A of Porphyromonas gingivalis function as adhesion and invasion in human gingival epithelial cells

Objective: To determine if the hemagglutinin A (HagA) of Porphyromonas gingivalis could be involved in the adhesion and invasion in human gingival epithelial cells (HGEC). Methods:P. gingivalis 381 hagA mutant was constructed by conjugation method. The whole length of hagA gene was cloned into pYA292 in Salmonella typhimurium x4072 (S. typhimurium-hagA). The strains were used to test their ability of adhesion and invasion into HGEC using a standard antibiotic protection assay. S. typhimurium x4072 strains containing empty vectors were used as negative control. HagA expression in S. typhimurium-hagA was confirmed by Western blot. Results:Although there were no significant differences between P. gingivalis 381 hagA mutant and wild type in adhesion and invasion into HGEC, the adhesion values of S. typhimurium-hagA to HGEC were increased by 3 times compared to their respective controls, while the invasion ability of S. typhimurium-hagA was 4 times greater than that of the negative controls. Conclusion: These results suggest that HagA may participate in P. gingivalis adhesion and invasion into HGEC.

6.Efficiency evaluation of treatment for tibial plateau fracture

[Objective]To summarize efficiency evaluation of treatment for tibial plateau fracture,to select optimal treatment for tibial plateau fracture in clinical practice.[Method]Retrospective analysis of 138 patients of tibial plateau fracture in our hospital from January 1999 to January 2003.They were all fresh fracture within 2 weeks after trauma,79 cases male 59 cases female,age 15~76,average 43.5 yrs.95 cases complicated with ligament and meniscus injure in knee joint.according to B 1,B2 were treat with manipulative reduction,traction AO classification and sinking degree,they were divided into type B and type C.type and external fixation of splint.Type B3,C1 were treated with prize-up replacement,grafting and internal fixation with screws.Type C2,C3 were treated with firm internal plate fixation.[Result]133 cases were followed up for 6~36months in 138 cases of tibial plateau fracture,average 23 months.Recovery of knee function were divided according to KDLMERI classification.Type B1B2B3 excellent rate 100%.C1 excellent 14cases,fair 2 cases.C2 excellent 10 cases,fair 8 cases.C3 excellent 6cases,fair 3 cases,badly 2 cases.Recovery of knee fruction excellent 118 cases(88.7%),fair and spoor 15cases(11.3%).[Conclusion]sinking degree of articular surface must be emphasized in treatment oftibial plateau fracture.Satisfied therapy effect can be obtained with early function exercise of knee joint.
7.Efficacy and Safety of Sildenafil for Treating the Patients With Congenital Heart Disease Combining Severe Pulmonary Hypertension in Plateau Area

Objective: To explore the efifcacy and safety of sildenaifl for treating the patients of congenital heart disease (CHD) with severe pulmonary hypertension in plateau area.
Methods: A total of 50 CHD patients combining severe pulmonary hypertension treated in our hospital from 2010-01 to 2013-10 were studied. The patients were randomly divided into 2 groups, n=25 in each group. Control group, the patients received conventional treatment and Sildenaifl group, based on conventional treatment, the patients received additional sildenaifl medication. The hemodynamic, blood gas, routine and biochemistry were recorded and compared between 2 groups.
Results: Compared with Control group, Sildenaifl group had more reduction of pulmonary artery pressure, increased arterial pressure of oxygen, left ventricular output, cardiac index and oxygenation index, all P<0.05. The patients’ arterial pressure, blood routine and biochemistry were similar between 2 groups, P>0.05. There was no obvious adverse reaction observed in Sildenaifl group.
Conclusion: Based on conventional treatment, Sildenafil may effectively reduce the pulmonary artery pressure in CHD patients combining pulmonary hypertension in plateau area, it improving the cardiac function without adverse reaction.
8.Intrathecal injection of miR-125b mimic attenuates neuroinflammation after ischemia-reperfusion injury by inactivation of microglial NLRP3 inflammasome

Objective:To observe and investigate the effect and mechanism of intrathecal injecting miR-125b mimic on regulating microglial and its-mediated NOD-like receptor protein 3 (NLRP3) inflammasome activation after spinal cord ischemia reperfusion injury (SCIRI) in rats.Methods:120 Sprague-Dawley (SD) rats were randomly divided into four groups: sham group (sham group, intrathecal injection of normal saline), injury group (IR group, intrathecal injection of normal saline), mimic group (IR+ intrathecal injection of miR-125b mimic) and control group (IR+ intrathecal injection of miR-125b control). In sham group, the aortic arch was exposed without clamping. In other groups, the aortic arch was clamped for 14 minutes, and then the artery clamp was removed to prepare SCIRI model. Rats in each group were given intrathecal injection for 3 consecutive days before operation, once a day. Immunofluorescence was used to observe the morphology of microglia and count the number of activated cells 48 hours after injury; double immunofluorescence was used to observe the distribution of Iba-1 and NLRP3 in cells. The protein expression of Iba-1, NLRP3 and cleaved caspase-1 in spinal cord were determined by Western blot, and the inflammatory factor IL-1 β was determined by enzyme linked immunosorbent assay (ELISA).Results:The activated microglias exhibited the amoeba morphology and increased Iba-1 fluorescence signal in immunofluorescence. The microglias in spinal dorsal horn of Sham group were all branched-like morphology and absent of the increased Iba-1 signal at 48 h post-IR. And the microglias in IR and control groups was all amoeba-like morphology with obviously increased number and intensity of Iba-1 signal, whereas in mimic group, the microglias were mixed morphology with the less increased number and intensity of Iba-1 signal ( P<0.05). And NLRP3 signals were mainly distributed in microglias with increased Iba-1 signal. Meantime, compared with Sham group, the protein expression of Iba-1, NLRP3, cleaved caspase-1 and IL-1β were significantly increased in IR group ( P<0.05). Compared with IR group, the above-mentioned protein expression were decreased in mimic group ( P<0.05). There were no statistical differences between IR and control groups in the above-mentioned protein levels ( P>0.05). Conclusions:Intrathecal injection of miR-125b mimic inhibits the SCIRI-induced microglial activation and then reduces NLRP3 inflammasome expression in spinal cords, thereby exerting an effective anti-inflammatory effects.

目的探讨脊髓损伤患者尿路感染常见细(真)菌及其对不同抗生素的敏感性和耐药情况。方法取脊髓损伤患者中段尿做尿培养及药敏试验,共计586例次。结果共培养出细(真)菌586株,其中革兰氏阴性杆菌475株(81.1%)、革兰氏阳性球菌83株(14.1%)、真菌28株(4.8%);产ESBL大肠埃希氏菌12株(2.05%)、产ESBL肺炎克雷伯氏菌5株(0.85%)。结论大肠埃希氏菌是导致本组患者尿路感染的最主要细菌,且细菌耐药现象较为严重,应引起临床医生高度重视。
10.Relationship between osteocalcin and metabolism of glucose and lipid in type 2 diabetes

A total of 185 patients with type 2 diabetes mellitus (T2DM) were consisted of male group (n =95) and postmenopausal female group (n =90).The parameters of fasting plasma glucose,HbA1c,fasting insulin,fasting C peptide,homeostasis model assessment insulin resistant index (HOMA-IR) and β-cell function (HOMA-β),blood lipid,body mass index,and waist to hip ratio were collected and analyzed by Pearson correlation analysis and stepwise multiple regression analysis.The relationships between osteocalcin and these parameters were investigated.The results revealed that osteocalcin was negatively correlated with HbA1C (P<0.05),and osteocalcin was an independent relevant factor affecting HbA1Clevels.Osteocalcin may be involved in the regulation of glucose metabolism in T2DM.
