2.Balloon dilatation of intrahepatic biliary strictures in liver transplantation

Objective To evaluate the efficacy of balloon dilatation in the treatment of intrahepatic biliary strictures in patients with liver transplantation. Methods Of the 100 patients with liver transplantation, 16 patients had intrahepatic biliary strictures and received balloon dilatation treatment. Results Initial technical balloon dilatation was successful in 14 cases but failed in 2 cases. There were no procedure-related complications. 4 restenosis occurred and they were treated with repeated balloon dilatation treatment. Conclusion Balloon dilatation represented an effective and relatively safe treatment for biliary stricture in liver transplant recipients. For restenosis, balloon dilatation was also an effective treatment.
3.Design and Quality Evaluation of Dihydromyricetin in Self-emulsifying Drug Delivery System

Objective To design and optimize the self-emulsifying drug delivery system ( SEDDS) of dihydromyricetin, and evaluate its quality in vitro. Methods The solubility of dihydromyricetin was measured in different oil phases,emulsifier and co-emulsifier solution. The best prescription was selected by comprehensively evaluating emulsification speed,particle size and drug loading via using the ternary phase diagram and orthogonal design test. Results The optimized formula of the self-emulsifying drug delivery system was oleic acidTween-80span-80N-butanol =1514714. Conclusion The optimized dihydromyricetin self-emulsifying drug delivery system significantly improves the solubility of dihydromyricetin, increases the physical and chemical properties of the formulation,and raises the dissolution of dihydromyricetin in artificial gastric fluid.
4.Progress in clinical research of asteroid hyalosis

Asteroid Hyalosis (AH) is a common clinical disease,which has been considered a benign disorder as it rarely impairs visual acuity.It was often discovered when the patient was treated for other eye diseases.The mechanism was unclear.Its characteristic B-ultrasound property makes the B-ultrasound a very helpful diagnostic technique.In the case of the patients with other fundus diseases associated with AH,optical coherence tomography (OCT) and fluorescein angiography (FA) may be used to reduce the interference from asteroid bodies,therefore improve the fundus visibility.Recent studies have shown that AH can incorporate with many other eye diseases.For example,in patients with cataracts,asteroid hyalosis can cause surface calcification of silicone plate intraocular lenses,which in most cases may lead to the need for explantation of the calcified intraocular lenses.The efficacy of pars plana vitrectomy (PPV),the removal of some,or all,of the eye`s vitreous humor for AH remains controversial.In this paper,we provide a review of the recent literature on AH disease: the etiology,diagnosis and treatment.We hope to thus improve the awareness and outcomes of AH disease.
5.Pathology of diabetic myocardiac microangiopathy

Objective To investigate the cause of high cardiovascular lethality in patients with diabetics mellitus. Methods Sections from autopsied coronary arteries and myocardium of dead patients with non-insulin-dependent diabetics mellitus and 12 dead control subjects were used for histomorphometric studies. Results The coronary atherosclerotic lesion in diabetics patients was not different in severity from those in controls. Nor was there difference in number of myofibers or diameters of myocardic fibers and capillaries.But the capillary density and the ratio of capillary number to myocardic fiber number in diabetics group were significeantly reduced compared with control group(P<0.0 l),and the capillary basement membrane in the former was significantly thicker than in the latter(P<0.01).Conclision The decrease in number of capillaries and the thickening of basement membrane enhance myocardiac vulnerability to further ischemia and hypoxia,which may undelie high lethality of myocardiopathy in diabetic patients.

7.Research progress on hyalinizing trabecular tumor

Hyalinizing trabecular tumor (HTT) is a primary thyroid tumor discovered 20 years ago. This tumor is usually misdi-agnosed as papillary thyroid carcinoma because of their many similar nuclear features. Although controversy has focused on the malig-nant potential of HTT, results show that this tumor has a favorable clinical prognosis. Improving the preoperative diagnosis of HTT can reduce the scope of operation because misdiagnosis before or during surgery may lead to unnecessary total thyroidectomy. This study re-viewed the pathology, diagnosis, therapy, and prognosis of HTT.
8.One case report and analysis of patient with renal immunoglobulin light and heavy chain deposited disease

Objective To report 1 case with renal immunoglobulin light and heavy chain deposited disease (IgG-κ light chain and γ1 heavy chain).Methods The clinical manifestations,serum immunofixation electrophoresis,light and heavy chain abnormalities of blood and urine,bone marrow biopsy and renal biopsy data laboratory data were recorded and analyzed.Results A 63-year-old woman presented with massive proteinuria,microscopic hematuria,hypertension,anemia and serum IgG-κ light chain.Bone marrow aspirate revealed 4% plasma cells.Renal biopsy revealed nodular glomerulopathy with congo red staining-negative.Immunofluorescence showed that κ light chain and IgG1 (γ1 heavy chain) were deposited along the glomerular basement membranes (GBM) and tubular basement membranes (TBM).Electron-microscopy revealed electrondense deposits that delineate the outer aspect of TBM and endothelial aspect of GBM.Conclusion The diagnosis of renal immunoglobulin light and heavy chain deposited disease (IgG-κ light chain and γ1 heavy chain) should be addressed combine with clinical and is pathology,especially with immune pathological examination.
9.Ischemic-type biliary lesions without hepatic artery occlusion after liver transplantation: early diagnosis of cholangiography

Objective To evaluate the value of cholangiography for early diagnosis of ischemic-type biliary lasions(ITBL) after liver transplantation. Methods Two hundred and fifty-three patients with liver transplantaion between Jan 2004 and Oct 2006 were recruited. Initial cholangiography was compared with terminal cholangiography to evaluate the value of initial cholangiography of ITBL. The t test, Chi-square test, sum rank test were used for statistics. Results Based on initial cholangiography, 189 patients were diagnosed with normal appearance, while 64 patients were diagnosed with abnormal appearance. The abnormal initial cholangiography appearances included poor filling in 33 patients and irregularity in 31 patients. Based on terminal cholangiogruphy, 199 patients were diagnosed with normal appearance and 54 patients with ITBL In patients with abnormal initial cholangiography, ITBL was occurred in 39 of 64 patients including 10 of 33 poor frlling patients and 29 of 31 irregularity patients. In patients with normal initial appearance, ITBL was only occurred in 15 of 189 patients. The abnormal initial cholangiography was associated with ITBL significantly (X2 = 79.999, P = 0.000, r = 0.490). Initial cholangiography had an overall sensitivity of 72.22%, and specificity of 87.44%, with positive and negative predictive values of 60.94% and 92.06%, respectively. The abnormal initial cholangiogruphy was a risk factor of ITBL by logistic regression analyses(OR=15.193, P=0.000). Conclusion The abnormal initial cholangiography is associated with ITBL Initial cholangiography, especially minimal irregular of intrabepatic biliary tract, is a sensitive and specific method for the detection of ITBL after liver transplantation.
10.Observation of Histomorphology and Biomechanics on Tendons Preserved by Vitrification

Objective To investigate the effect of vitrification on the biomechanics of tendon tissue in rabbit.Methods Two frozen methods were adopted.The first group was treated with Cryoprotective Agent,which was composed of 18.64%DMSO(V/V),13.37% Acetamide(V/V),9.17% 1,2 Propylene glyco(V/V),0.10mmol/LTrehalose and 10% Calf serum.The tendon tissue with three steps of preliminary treatment,preserved in Cryoprotective Agen was conserved in liquid nitrogen(-196℃)for 14 days;The second group treated with 15% DMSO and 10% Calf serum served as control group.Transmission electron microscope was used to observe the Histiooytic shape of tendon.Each group was performed with tendon tensile test,which could detect maximum load,the maximum shifting quantity and Young's Elastic Modulus.Results There was no significant damage in the tissue's micromechanism of vitrified tendon.But in cryopreservation group,the tissue's micromechanism was apparently damaged.There was no significant difference between test group and control group in maximum load(P=0.256).The same was the maximum shifting quantity(P=0.065).There was significant difference between test group and control group in Young's Elastic Modulus(P=0.006).Conclusion The damage of vitrification to tendon is less than that of profound hypothermia preservion,especially to tendon's corpuscular shape,but there is no significant difference between test group and control group in tendon's biomechanics.
