Objective To explore different pathogenesis of chronic hepatitis B(CHB)and asymptomatic H BV carriers(ASCs)by identifying differentially expressed genes.Methods Subtracted library was constructed by suppression subtraetive hybridization(SSH),and α-defensin was identified by dot blot hybridization.Peripheral blood was collected from 46 CHB patients and 11 ASCs.and the expressions of α-defensin mRNA in peripheral blood mononuclear cell and protein in plasma were determined by the real time RT-PCR and enzyme-linked immunosorbent assay(ELISA).Results Real time RT-PCR showed that the expression of α-defensin mRNA in blood samples of CHB was 1.4-fold higher than that of ASCs.As shown by ELISA,the plasma level of α-defensin in CHB was higher than that of ASCs [(216.40±81.25)μg/L vs.(156.00±57.26)μg/L,t=2.23,P<0.05].Conclusion α-defensin may involve in the pathogenesis of CHB,for it iS over-expressed in CHB patients.

In order to study the relationship between sex hormone and gynecomastia, the levels of plasma sex hormone in the male patients with gynecomastic were determined by using radioactive immune assay. Seventy-eight gynecomastic patients were regarded as the experimental group which was divided into 3 subgroups: the youth group with the age ≤30 years old, the middle-aged group with the age ranging from 30 to 60 years old and the old-aged group with the age ≥60 years old in the light of age and 24 normal men served as control group. The results showed that the levels of E2 and E2/T in the gynecomastia patients were much higher than in the control group (P<0.05, 0.01)；In the experimental group, the ratio of T reduction in the old-aged group was much higher than in the youth group or the middle-aged group（P<0.05). In conclusion, gynecomastia was related to the dysfunction of the level of sex hormone, to a large extent, with the increasing of estrogen. In addition, senile gynecomasti was closely related with the decreasing of male hormone.

The proteome is the entire complement of proteins expressed by a genome, cell, tissue or organism,which are all types of proteins expressed by one cell in the specific physiological or pathological states. The study of proteomics not only can help us to understand the general rules of activity of proteins,but also illuminate the pathologic mechanisms of numerous diseases. By proteomic analysis of normal individual and pathological individuals, we can find some disease-specific protein molecules, which not only can be a new target for drug design, but also can be a molecular markers of some diseases especially tumors.

This article discusses the relevant factors which influence the assessment and diagnosis of personality disorder, especially the relationships between personality disorder and personality,personality disorder and the axis I conditions in the Diagnostic and Statistical Manual of Mental Disorders (DSM) system and its particularity of assessment. Such discussion helps to improve the clinical worker's comprehension to the psychopathology, personality disorder and their relationships.

Objective To investigate the expression of an inhibitor gene of apoptosis, clusterin, in prostate cancer and its relationship with the genesis and progression of prostate cancer and with the expressions of bcl 2 and p53. Methods The expressions of clusterin, bcl 2, and p53 in 10 cases of normal prostatic tissues, 15 cases of benign prostatic hyperplasia (BPH), and 49 cases of prostate cancer were examined by immunohistochemical staining. Results The rates of strong positive or weak positive of clusterin in normal prostatic tissues, BPH, and prostate cancer were 10% (1/10), 66.6% (10/15), and 91.8% (45/49), respectively. Objective To observe the clinical effects of anti-hepatitis B virus with telbivudine combined with adefovir dipivoxil.Methods 82 cases of patients diagnosed with chronic hepatitis B from January 2014 to February 2016, were randomly divided into two groups, the control group were treated with adefovir ester treatment, the observation group were treated with telbivudine combined with adefovir dipivoxil treatment, patients were followed up and recorded Child-Pugh score, aspartate aminotransferase ALT, total bilirubin TBIL, aspartate aminotransferase AST, albumin ALB, HBV-DNA negative rate, HbeAg seroconversion rate case, the use of statistical methods for data analysis.Results After treatment, the observation group Child-Pugh score, ALT values were(4.01±0.79)points,(37.19±4.82)U/L, were better than the control group(5.46±1.16)points,(61.49±9.78)U/L, and the differences were statistically significant (P<0.05).Observation group after treatment TBIL, AST, ALB, respectively(34.91±5.49)μmol/L,(56.49±5.28)U/L,(45.51±4.24)g/L, were better than the control group(61.62±10.06)μmol/L,(78.64±8.16)U/L,(38.76±3.21)g/L, and the differences were statistically significant (P<0.05).The observation group after treatment for HBV-DNA negative rate and HbeAg seroconversion rate(82.93%, 58.54%)respectively were higher(58.54%, 29.27%), and the difference was significant (P<0.05).Conclusion Telbivudine combined with adefovir dipivoxil anti-HBV effect is good, better than medication alone is worthy of further research and application.

The training of residents constitutes the primary work of Graduate Medical Education.Reviewing the historical development of the training of residents,we find that it has experienced four stages and trended to establish the medical specialist training system.

The quality of doctoral degree conferring depends on the quality of doctoral dissertation.Based on the quality evaluation of doctoral dissertation in medical School of Shanghai Jiaotong University in 2005,the author points out the problems existing in doctoral dissertation nowadays and makes some explorations on how to improve the quality of doctoral degree conferring.
