1.Risk Assessment Tools for Stroke After Transient Ischemic Attack

Transient ischemic attack (TIA) is an independent risk factor of stroke. About two-thirds patients with TIA will reoccur TIA or stroke. To predict the risk of stroke after the onset of TIA may identify the prognosis of patients, so as to have an intervention for the high-risk patients as early as possible. This article reviews the various risk assessment tools for stroke after TIA, in order to provide ideas for the prognostic assessment of TIA.

3.Tissue distribution of Aconitum alkaloids extracted from Radix aconiti preparata after oral administration to rats

Aim To develop an HPLC method for the determination of Aconitum alkaloids extracted from Radix aconiti preparata in rats. Methods Waters 2690@996 PAD system was used. The analytical column was a Halsil 100 C18 column (250 mm×4.6 mm ID, 5 μm). The mobile phase was water, methanol and diethyl amine at the ratio of 75∶ 25∶ 0.1. The flow rate was 0.9 mL·min -1. The wavelength of the detector was 240 nm. Results The linear ranges of aconitine in the heart, spleen, lung and kidney were 0.4-100 μg·mL -1, the correlation coefficients were 0.997 2, 0.998 6, 0.999 3 and 0.999 4, respectively. The linear range of aconitine in liver was 2-200 μg·mL -1 and the correlation coefficient was 0.999 0. The linear ranges of hypaconitine in heart, liver, spleen, lung, kidney, brain and spinal cord were 5-100 μg·mL -1, the correlation coefficients were 0.999 4, 0.999 7, 0.999 8, 0.998 4, 0.999 8, 0.999 8 and 0.999 7, respectively. Detection limits (S/N=3) of aconitine and hypaconitine were 0.4 μg·mL -1. The recoveries of aconitine and hypaconitine ranged from 88.7% to 102.2% and 86.5% to 101.3%, respectively, and the RSD of precision of aconitine and hypaconitine was 10%. Conclusion It appears to be an accurate and effective method that can offer reference basis for in toxication of Radix aconiti preparata clinically.
4.A Survey on the Current Situation of Smoking and Smoking Cessation in 184 Patients with Cardiovascular Disease

Objective To understand the current situation of smoking and quitting smoking in patients with cardiovascular disease. Methods 184 cases of inpatients or outpatients with cardiovascular disease treated in the First People's Hospital of Kunming were surveyed about quitting smoking, and the smoking and smoking cessation-related clinical features of patients with cardiovascular disease were analyzed. Results ( 1) Cardiovascular diseases in smokers was male-dominated (100%), mainly hypertension and/or coronary heart disease (179, 97.3%) . (2) Average age of starting smoking was 21.5, 97.8%of patients had smoked more than 10 years, with an average of 35.1 ±13.4 years, 69.6% of patients smoked ≥10 cigarettes/day, with an average of 16.3 ± 10.1 cigarettes/day, 65.8%of patients were in the low level of nicotine dependence, 81.5%of the patients thought that quitting smoking was important, 76.1% had quitting smoking ideas, 67.4% had confidence in the success of quitting smoking, 50.5%thought that it was difficult to quit smoking, 127 (69%) patients had seriously quit smoking. (3) 65 cases (51.2%) who seriously quit smoking had succeeded, 54 patients (83.1%) received varying degrees of medical staff advised to quit smoking, 62 cases (48.8%) in relapse, only 34 patients (54.8%) accepted medical personnel advised to quit. Two sets of chi-square test, p=0.000, prompted difference was statistically significant. Relapse reasons were pressure, craving and around other smokers. Conclusions Smoking patients with cardiovascular disease are mainly constituted by male patients with hypertension and/or coronary heart disease. Most of the patients recognize that quitting smoking is important, and have idea and seriously quit smoking. However, lack of confidence and smoking which has also become a lifestyle significantly affect the success of smoking cessation. The medical staffs' recommending smoking cessation persuasion can largely improve the success of smoking cessation for patients.

6.Clinical Observation of Electroacupuncture at Huiyang (BL35) and Zhongliao (BL33) plus Scalp Acupuncture for Female Stress Urinary Incontinence

ObjectiveTo observe the clinical efficacy of electroacupuncture at Huiyang (BL35) and Zhongliao (BL33) plus scalp acupuncture in treating female stress urinary incontinence (SUI), for providing clinical evidence in the treatment of SUI.Method Ninety eligible female SUI patients were randomized into an observation group and a control group. The observation group was intervened by electroacupuncture at Huiyang and Zhongliao plus scalp acupuncture, while the control group was asked to practice contracting anus. The urine leakage degree, residual urine volume in bladder and clinical efficacy were compared.ResultAfter treatment, there was a significant difference in comparing urine leakage degree between the two groups (P<0.05); there was a significant between-group difference in comparing the residual urine volume (P<0.05); the cured and markedly-effective rate was 62.2% in the observation group versus 35.6% in the control group, and the difference was statistically significant (P<0.05). ConclusionElectroacupuncture at Huiyang and Zhongliao plus scalp acupuncture can produce a more significant clinical efficacy compared with rehabilitation training in treating female SUI.
7.Stress Resistance of Rhodiola under Freezing Injury Stress

Objective To study the stress resistance of Rhodiola under freezing injury stress. Methods With R.kirilowii, R.crenulata, R.Algida and R.fastigiata as the experimental materials, the effects of freezing injury stress on emerging time, duration of seedling emergence, emergence rate, surviving rate, the activity of SOD and POD were observed by field test and experimental detection. Results Before seeding, the effect of freezing injury stress on R.kirilowii was the largest, and that on R.fastigiata was the least. When injury stress happened during seedling stage, surviving rate, increase of SOD and POD activities of R.kirilowii was the least, indicating the freezing injury was severe. R.fastigiata was the best in those indexes. The stress resistance of Rhodiola was as follows:R.fastigiata > R.crenulata > R.Algida > R.kirilowii. Conclusion R. fastigiata and R.crenulata are good choice in introduction and cultivation of Rhodiola in Gannan area of Gansu province, and R.fastigiata is the best.
8.Nosocomial Infections with Acinetobacter baumannii:Clinical Distribution and Drug Resistance

OBJECTIVE To approach the clinical distribution and drug resistance in nosocomial infections with Acinetobacter baumannii(ABA) to provide the scientific evidence for clinical diagnosis and treatment.METHODS Referring to National Rules of Operation in Clinical Laboratory,215 strains of ABA were isolated.Microscan auto-4 was used to identify strains.And the antibiotic susceptibility test was performed by K-B method.The test results were judged according to CLSI 2007.RESULTS The top four of isolated rates were the intensive care unit(23.3%),department of neurology(17.7%),department respiratory medicine(15.8%) and department of tumor(14.4%).The detection rate of ABA from specimen from respiratory tract was the highest,arriving at 72.6%.The antibiotic susceptibility test showed ABA had been resistant to commonly used antibiotics.The resistance rates to imipenem and meropenem were 22.3% and 23.7%,respectively.The isolated rate of pan-resistant stains was 7.4%.CONCLUSIONS Hospitals should think highly of monitoring and controlling of drug resistance in ABA and prevent nosocomial infections with multidrug resistant strains.
9.Discussion on the Current Situation and the Strategy of Practicing Pharmaceutical Care in a Maternal and Child Health Service Special Hospital

OBJECTIVE:To find suitable pharmaceutical care for our hospital. METHODS:The literature research and expert-consultation were adopted to determine the project of pharmaceutical care. Meanwhile,related information were analyzed by the questionnaire survey,statistical analysis interview and other methods in our hospital,such as the content,methods,level and quali-ty of pharmaceutical care. RESULTS:The determined project of the pharmaceutical care and five-point scale were as follows:pre-scription audit 4.53,medication consultation 4.21,prescription analysis 3.56,information about the medication for doctors and nurses 3.73,to establish medical records of hospitalized patients 3.14,ADR monitoring 4.33,drug therapy monitoring 4.31,to par-ticipate in clinical drug therapy 4.03,education about drug use 4.12,to design individualized dosing regimens 3.98,pharmaceutical research 3.58,to popularize pharmaceutical knowledge and education service 4.01. CONCLUSIONS:The hospital should empha-size clincal pharmacist work and software and hardware construction entirely;make the best of modern approach to carry out phar-maceutical care,mainly involving prescription audit and ADR monitoring. However,the problems are that the content of some pharmaceutical cares are not complete,and become a mere formality;clinical pharmacists do not receive systematic trained,the pharmaceutical researches have not been carried out systematically.
10.Effect of adenoviral-mediated interleukin-10 gene transfection on systemic inflammatory responses in a rat model of ventilator-induced lung injury

Objective To investigate the effect of adenoviral-mediated interleukin-10 (IL-10) gene transfection on systemic inflammatory responses in a rat model of ventilator-induced lung injury. Methods Thirty adult male SD rats weighing 200-220 g were randomly divided into 5 groups (n=6 each): group Ⅰ control (group C); group Ⅱ adenovirus without IL-IO gene + 2 h mechanical ventilation (MV) (group B, ); group II1 adenoviruswithout IL-IO gene + 4 h MV (group B_2); group Ⅳ adenovirus with IL-10 gene + 2 h MV (group A_1) and group Ⅴ adenovirus with IL-10 gene + 4 h MV (group A_2). In group Ⅱ-Ⅴ the animals were mechanically ventilated (airway pressure = 25 cm H_2 O) and adenovirus was injected into right ventricle at 48 h before MV. Blood samples were taken from abdominal aorta at the end of 2 h or 4 h MV for determination of serum IL-8 and IL-10 concentrations. The animals were then killed. The pulmonary specimens were obtained for examination of ultrastructure with light and electron microscope. Results Mechanical ventilation significantly increased serum IL-8 and IL-IO concentrations in a duration-dependent manner in group Ⅱ-Ⅴ as compared with group C. Trans-pulmonary administration of adenoviraI-mediated interleukin-10 gene significantly decreased serum IL-8 concentration and increased serum IL-10 concentration and reduced ventilator-induced lung injury alter MV in group A_1 and A_2 as compared with group B_1 and B_2. Conclusion Adenoviml-madiated IL-10 gene transfection can attenuate the systemic inflammatory response and reduce ventilator-induced lung injury.
