Translational medicine is a new term recently which aims to eliminate the barrier between preclinical and clinical medicine,to efficiendy and effectively translate basic scientific findings into drugs and medical devices that benefits patients.This article discussed the meaning of translational medicine in cultivating postgraduates of neurology.According to the preliminary attempt,it is beneficial for postgraduates to form translational medicine philosophy and be high-quality medical talents.

ObjectiveTo counsel the life style from the first trimester based on the fasting hyperglysemia to decrease the rate of GDM and adverse effect of it. MethodsThe pregnancy women with fasting hyperglysemia were recruited. All subjects were divided into two groups, one was counselled the life style from the first trimester but the other not. The pregnant outcome were compared. ResultsIn the study group 1 100 with complete information and 29 were abnormal glucose intolerance, 1 120 of 47 in control. The rate of GDM or GIGT,hypertensive disease,spontaneous abortion,premature dilivery, polyhydramnios, fetal macrosomia, fetal growth restriction, neonatal respiratory distress syndrome and the mean GHbA1c was lower than that in control ( x2 = 8.110,6.124,5.815,4.518,6.355,3.957,5.453,4.619, t = 3.385, all P ＜ 0. 05 ). But the body mass index at labor( t = 0.698 ), infection disease ( x2 =0.082),mean neonatal weight (t = 2.09 )and neonatal hypoglysemia (x2 = 0.035 )of term labor had no significant differences. ( t = 0.698, χ2 = 0.082,t = 2.090, χ2 = 0.035, all P ＞ 0.05 ). ConclusionCounseling the life style from the first trimester based on the fasting hyperglysemia could decrease the rate of GDM or GIGT and improve the pregnancy result.

Objective To explore the auxiliary efficacy of microbial ecological agents for elderly patients with acute pancreatitis.Methods Totally 144 elderly patients with acute pancreatitis were selected and randomly divided into the observe group and the control group (n=72,each).All patients received the regular treatment and enteral nutrition.Microbial ecological agents were added in enteral nutrition in the observe group.The levels of endotoxin (ET),tumor necrosis factor (TNF-α),interleukin (IL)-6 and C-reactive protein (CRP)in peripheral blood were determined and compared between the two groups.The clinical index and the occurrence of adverse reactions were observed.Results Compared with pretreatment,the levels of ET,TNF-α,IL-6,CRP were significantly reduced in both two groups after treatment.The levels of ET,IL-6,TNF-α,CRP were lower in the observe group than in the control group [(0.14±0.07)EU/ml vs.(0.31±0.12)EU/ml,(44.77±17.82)pg/ml vs.(63.25 ± 14.26)pg/ml,(0.15 ± 0.05)μg/L vs.(0.29 ± 0.14)μg/L,(108.33 ±11.84)mg/Lvs.(135.41±21.45)mg/L,t=14.722,43.824,7.071,5.811,respectively,allP＜0.05].The recovery time of symptoms and signs,the anal exsufflation time and hospitalization time were shorter in the observe group than in the control group [(25.8±4.8)h vs.(36.4±6.2)h,(30.9±6.7)hvs.(44.4±7.3)h,(24.9±3.2)hvs.(37.6±3.4)h,(6.9±0.4)dvs.(13.1±1.2)d,t=4.747,4.401,10.125,33.443,respectively,all P＜0.05].There were no statistically differences in death rate and the occurrence of adverse reactions between the two groups (x2 =0.15,0.64,both P＞0.05).Conclusions Microbial ecological agents are safe and effective as an adjuvant therapy for elderly patients with acute pancreatitis.It is worth promoting in clinical medicine.

Precision medicine significantly promotes the diagnosis and treatment of various diseases,which also induces the revolution of evidence-based clinical practice guidelines,as well as multimodality therapy for hepatocellular carcinoma (HCC).In the application of the concept of precision medicine and evidence-based medicine,surgeons will concretize the precise surgery,establish an improved multimodality therapy for HCC,and ultimately achieve the goal of overall benefit.This is also a new task of surgeons in the precision medicine era.

Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) is used for patients with advanced hepatocellular carcinoma who cannot tolerate major hepatectomy due to an insufficient future liver remnant,but the morbidity and mortality rate in the perioperative period are still high.Available studies indicate that damage control surgery variations such as laparoscopic procedure and associating radiofrequency/microwave ablation/liver tourniquet and portal vein ligation could improve the morbidity and mortality associated with ALPPS,as could portal vein embolization.However,randomized controlled trials are needed to determine benefits in technical variations.

Objective To explore the effect of dexmedetomidine preconditioning on antioxidant ability in Rats Brain with focal cerebral ischemia/reperfusion.Methods Forty-two healthy male SD rats,weighted 250-280 g,were randomly divided into three groups (n = 14):sham-operation group (group Sham):in which carotid artery was exposed but MCAO was not performed;ischemia/reper-fusion group(group IR):NS were injected intraperitoneally at 30 minutes before the MCAO;dexme-detomidine group (group Dex):dexmedetomidine 100 μg/kg were injected intraperitioneally injected at 30 minutes before the MCAO.Focal cerebral ischemia-reperfusion (IR)model in rats was made by transient occlusion of the middle cerebral artery occlusion (MCAO)using a nylon thread with rounded tip inserted into internal carotid artery and advanced cranially until resistance was met.MCAO was maintained for 2 hours followed by 24 hours reperfusion.Neurologic deficit scores(NDS),the infarc-tion volume as well as the activities of endogenous antioxidants (such as superoxide dismutase(SOD), glutathione peroxidase(GSH-PX),glutathione reductase (GR),catalase (CAT))in ischemic brain were evaluated 24 h after reperfusion.Results Compared to group Sham,the neurologic deficit scores and the infarction volume in group Dex and IR were significantly higher,but the concentration of SOD,GSH-PX,GR,CAT were significantly lower(P <0.05).Compared to group IR,however,the neurologic deficit scores and the infarction volume in group Dex were significantly lower,but the concentra-tion of SOD,GSH-PX,GR,CAT were significantly higher (P <0.05).Conclusion Dexmedetomidine can protect the brain from focal cerebral ischemia-reperfusion injury,and its mechanism maybe relate to preserving the activities of endogenous antioxidants.

Based on the review of the existing evidence of the research on the hospital management institutional ethics , this paper found researches in this area in the last two decades focused on the concept and level definition of hospital management institutional ethics;the construction of hospital management institutional ethics;the value and function of hospital management institutional ethics, and the reason and result of lack of hospital management insti-tutional ethics. On this basis, it pointed out the deficiency of the current researches and outlook the prospects.

The increasing need for ideal body shape makes more and more patients choose to receive different types of breast reconstruction surgery after mastectomy. Right now, it is a key problem for the multidisciplinary treatment to find an optimal sequence of comprehensive treatment from which those patients can receive the maximum benefits and a good balance between ideal body shape and satisfactory antitumor outcomes. This paper reviews the latest research advances in the effect of postoperative radiotherapy on breast shape after breast reconstruction, the sequential relationship between the replacement of the temporary tissue expander with the permanent breast prosthesis and postoperative radiotherapy, breast construction surgery, and radiotherapy techniques. To provide a basis for clinical practice, this paper proposes a flow chart for decision?making in reconstruction surgery and adjuvant treatment based on the current literature and clinical data.
