1.Effects of astragalus injection auxiliary on the levels of Serum troponin I and related inflammatory factors inpatients with infantile viral myocarditis

Objective To investigate the effects of astragalus injection auxiliary on the levels of serum troponin I ( cTn I ) , creatine kinase isoenzyme(CK-MB), creatine kinase(CK), interleukin 4(IL-4), interleukin 6(IL-6) and interleukin 23(IL-23) in patients with infantile viral myocarditis.Methods 70 patients with infantile viral myocarditis who were treated in Zhejiang Quzhou Hospital were selected and randomly divided into control group andexperiment group,with35 cases in each group.Patients in control group were treated with 1, 6 -two-diphosphate fructose, a lot of vitamin C and polarized liquid conventional treatments, patients in experiment group were treated withastragalus injectionon the base of control group.After treatment with two weeks, the serum cTn I, CK-MB, CK,IL-4,IL-6 andIL-23 levels of two groups were compared, and the clinical total effective rate were observed.Results Compared with pre-treatment, the serum cTn I, CK-MB, CK, IL-6 and IL-23 levels of the two groups were decreased, the IL-4 level increased(P<0.05).ompared with the control group, the serum cTn I, CK-MB, CK, IL-6and IL-23 levels of experiment group were lower, theIL-4 levelwashigher ( P <0.05 ) . The clinical total effective rate of experiment group was higher than control group ( P <0.05 ) . Conclusion Astragalus injectioncombined with conventional therapy aremore effective in treatment with infantile viral myocarditis .
3.Clinical evaluation of peripheral vascular disease withwall thickness, stiffness and hemodynamics of femoral artery in type 2 diabetes mellitus

Objective To evaluate the relationship between femoral artery intima-media thickness (FA-IMT), stiffness, hemodynamics factors of femoral artery and peripheral vascular disease (PVD) in type 2 diabetes mellitus. Methods According to the presence of PVD, 151 patients with type 2 diabetes were divided into PVD group and non-PVD group. FA-IMT was tested with 2D ultrasound. Ds, Dd, PSV, EDV were observed with color flow rate and volume quantitative technology. FA-IMT, FA-stiffness β, peak systolic velocity (PSV) and end diastolic velocity (EDV) were compared between the two groups. Results Compared with non-PVD group, FA-IMT and FA-stiffness β increased in PVD group (P<0.01), whereas PSV and EDV decreased in PVD group (P<0.01). There was no relationship between FA-stiffness β and FA-IMT in PVD group (P=0.01). Multiple Logistic regression indicated the presence of PVD symptoms associated closely with FA-stiffness β, PSV, EDV and duration of diabetes. Conclusion Stiffness β plays an important role in the clinical evaluation of type 2 diabetes mellitus.
4.Optimization of Activated Carbon Decolorization Technology for Bletilla Striata Polysaccharide by Orthogonal Test

Objective: To optimize the decolorization technology of activated carbon for the plant pigment in Bletilla striata polysaccharide.Methods: Using L 9 (3 4) orthogonal test with activated carbon as the decolorizer, the amount of activated carbon, decolorization temperature, decolorization liquid pH and decolorization time were investigated.The decolorization rate and polysaccharide retention rate were investigated.The decolorization rate and polysaccharide retention rate were taken as the indices.Results: The optimum decolorization technology was as follows: the amount of activated carbon was 1.0%, the decolorization temperature was 40 ℃, the pH value was 5 and the decolorization time was 30 min.Under those conditions, the decolorization rate of Bletilla striata polysaccharide was 91.3% and the retention rate of polysaccharide was 80.6%.Conclusion: The selected decolorization technology of activated carbon can make Bletilla striata polysaccharide get the best decolorizing effect.
5.Depression,anxiety,and health-related quality of life in adult epilepsy

Objective To assess health-related quality of life and the mood disorder in adults with epilepsy,and to evaluate factors contributing to the quality of life.Methods Quality of life was measured by the Quality of Life in Epilepsy Inventory(QOLIE-31)and the World Health Organization Quality of Life Assessment-Bref(WHOQOL-BREF).Psychotic conditions were evaluated by Self-rating Depressive Scale (SDS)and Self-rating Anxious Seale(SAS).The multivariate analysis was used to assess the determinant factors.Results The study included 141 epilepsy patients in the teat group and 59 sex,age,and education matched normal controls. WHOQOL-BREF scores in the physical and psychological aspects were significantly lower in epilepsy patients(12.7 ±1.8 and 12.4±1.9,respectively)than those in the normal controls(15.1 ±2.3 and 13.9 ±1.9,respectively,t value were 11.75 and 8.625.both P<0.05).The survey reported that 57.4 % of the epilepsy patients suffered with depression.and 39.7 % anxiety.The patients with both anxiety and depression scored lower in all aspects in QOLIE survey except medical effect.Multivariate analysis showed that factors that effect the overall quality of life in order were anxiety.depression and disease duration.Conclusion Our results support that complications of anxiety and depression and long disease duration are key factors affecting the quality of life in epilepsy patients.
6.Correlations of neurophysiological features with clinical manifestation in patients with diabetic peripheral neuropathy

Objective To explore the correlation of electrophysiological features with clinical manifestation in patients with diabetic peripheral neuropathy (DPN).Methods The clinical data of 306 patients with DPN undergoing nerve conduction study (NCS) during May 2013 to June 2015 were retrospectively analyzed.Among 306 patients,304 were type 2,and 2 were type 1 diabetes with a mean age of 64.1 years and disease duration of 1 week-31 years.The relationship between the electrophysiological features and clinical manifestations was evaluated.Nerve conduction studies and F-wave and in sympathetic skin response were analyzed.Results ① NCS in lower limbs presented more abnormalities than that in upper limbs.There was the highest prevalence of not elicited sensory potential in Tibial nerve [56.9%(174/306),x2 =175.9] and the highest prevalence of not elicited motor potential in peroneal nerve [7.8%(24/306),x2 =1 103.1].Amplitude abnormality was more severe in sensory nerve than that in motor nerve[41.4% (507/1 224) vs.10.8% (132/1 224),x2 =559.2].The ratio of abnormal motor conduction velocity or distal latency was higher than that of amplitude decreased [22.9% (280/1 224) vs.10.8%(132/1 224),x2 =1 077.4].② The abnormal ratios of the shortest F-wave latency was lower than that of the prolonged distal latency in median and tibial nerve [5.4% (33/612) vs.20.6% (126/612),x2 =607.6].③ The abnormal ratio of sympathetic skin response (SSR) in lower limbs were higher than that of upper limbs [40.2% (246/612) vs.27.3% (167/612),x2 =129.4,P < 0.001],abnormal ratio of amplitude was higher than that of the prolonged latency [30.1% (368/1 224) vs.4.7% (58/1224),x2 =1040.5,P <0.001].Among the 99 cases presenting abnormal SSR in upper limbs,32 cases were normal in conventional NCS.Among the 127 cases presenting abnormal SSR in lower limbs,7 cases were normal in conventional NCS.④Disease course and glycosylated hemoglobin values were positively correlated with the distal motor latency,and negatively correlated with the nerve conduction velocity and amplitude.Conclusion Electrophysiological features of DPN is mainly shown in axon of the distal sensory nerve.Long duration and hyperglycemia may lead to more abnormalities in electrophysiological tests.

Objective :
To learn the characteristics of second-line drug resistance and related gene mutations of multidrug-resistant Mycobacterium tuberculosis ( MDR-TB ) Beijing genotype strains.
Methods:
The MDR-TB isolates in Hwa Mei Hospital from 2017 to 2019 were enrolled and detected using RD105 deletion-targeted multiplex polymerase chain reaction (PCR). The proportion method for drug susceptibility test was used to detect the drug-resistant profiles against kanamycin, amikacin, capreomycin, ofloxacin and levofloxacin. The gene sequencing of rrs, tlyA, eis, gidB, gyrA and gyrB was conducted by PCR compared with H37RV strain. The differences in the rates of drug resistance and mutation between Beijing and non-Beijing genotype strains were examined to understand the characteristics of Beijing genotype strains.
Results:
There were 106 Beijing genotype and 27 non-Beijing genotype strains in 133 MDR-TB isolates. The drug resistance rates of kanamycin, amikacin, capreomycin, ofloxacin and levofloxacin in Beijing genotype strains were 9.43%, 7.55%, 3.77%, 32.08% and 32.08%, respectively. The rates of quasi-extensive and extensive drug resistance in Beijing genotype strains were 30.19% and 7.55%. The gene mutation rates of rrs, tlyA, eis, gidB, gyrA and gyrB in Beijing genotype strains were 7.55%, 7.55%, 1.89%, 2.83%, 36.79% and 2.83%, respectively. There were no significantly differences between Beijing and Non-Beijing genotype strains in the factors above ( P>0.05 ). The gene rrs, tlyA, eis, gidB, gyrA and gyrB had 2, 1, 2, 2, 5 and 3 mutation types, respectively, with single base substitution as the main type.
Conclusion
Beijing genotype strains are dominant in MDR-TB, with high resistance to fluoroquinolones and mainly gyrA gene mutation.

Objective To fabricate an immunofluorescence probe system of carbon dots conjugated antibody based on antigen-antibody reaction principles.Methods A green one-step microwave assisted pyrolysis method was applied to preparing fluorescent carbon dots (CDs) using aminoglucose as carbon source and the obtained CDs were conjugated with antibody via EDC/NHS reactions to build CDs based fluorescent probe.Furthermore,the properties of CDs and CDs based probe system were evaluated by Fourier transform infrared (FTIR) spectra,transmission electron microscopy (TEM),UV-vis absorption and so on.Results The as-prepared CDs showed excellent fluorescence and hydrophilicity and CDs based immunofluorescence probe exhibited the capability of quick detection of E.coli O157:H7.Cinclusion Fluorescent CDs as one new emerging environment-friendly nanomaterial has great potential in biosensors.
10.Comparison of different urinary diversion regimens after minimally invasive resection for bladder cancer patients

Objective To investigate the clinical effects of different urinary diversion regimens after minimally invasive resection of bladder cancer. Methods 127 patients with muscular infiltrating bladder cancer from January 2010 to June 2015 were enrolled in this study. According to the patients' condition, they were divided into orthotopic ileal cystectomy group (58 cases), Bricker bladder surgery group (33 cases), ureteral skin ostomy group (36 cases), then compare the clinical data, postoperative complications and quality of life of the three groups. Results There was no significant difference in the ratio of sex, the distribution of age and tumor staging among the three groups. The operation time and postoperative hospital stay in orthotopic ileal cystectomy group were longer than those in the other two groups. The operation time and postoperative hospital stay of the Bricker bladder surgery group were longer than that in ureteral skin ostomy group, the difference was statistically significant; the bleeding in orthotopic ileal cystectomy group was more than the other two groups, and in Bricker bladder surgery group was more than ureteral skin ostomy group, the difference was statistically significant. There was no significant difference in the recovery time of intestinal function among the three groups. The recovery time of intestinal function in ureteral skin ostomy group was shorter than that in the other two groups, the difference was statistically significant. There was no significant difference in the incidence of short-term complications of the three groups of patients. The incidence of long-term complications of orthotopic ileal cystectomy group was significantly higher than the other two groups, the difference was statistically significant. The social function scores, and overall health score of orthotopic ileal cystectomy group were higher than the other two groups, the difference was statistically significance. Conclusion Without external device, closer to the characteristics of physiological urination, orthotopic ileal cystectomy holds higher postoperative quality of life, and higher incidence of long-term complications. With advantages of quick recovery and less complications, ureteral skin ostomy is best for patients who can not stand for long time surgery. It should choose the appropriate surgical approach for patients according to individuals' conditions.
