2.Morphology & Degeneration of Exoerythrocytic Forms of Plasmodium y. yoelii & Reactions of Host Cells in Rats & Mice

The exoerythrocytic forms (EEF) of Plasmodium yoelii yoelii BY265 in the liver of rats and mice are morphologically similar, however, the size of the matured EEF in the former is far much larger than that in the latter. In the sections of the liver of both the animals, which are sliced 48-65 hours after the inoculation of sporozoites, certain degenerated EEF can be found. The degenerated EEF fall into two types:(1) Degenerated EEF without WBC infiltration, These EEF usually remain intact in their outline with many vacuoles of the size 1-6?m in diameter in the cytoplasm. The nuclei of the host hepatic cells parasitized are usually swollen and appear blank after staining. No WBC infiltration can be found.(2) Degenerated EEF with WBC infiltration: The outline of these degenerated EEF is no more intact. They are usually infiltrated and/or surrounded by WBC.Degeneration occurs in 6.6% of the EEF in rats and 5.8% in mice. Once the normally developed parasites become matured and ruptured, large amoants of WBC are attracted and accumlated to form local foci of WBC infiltration of the size 35-90?m in diameter. In some of the foci, remnants of the parasites can be seen, while in some others, no remnants can be found and they most likely are also formed by the degenerated EEF or ruptured matured parasites.
3.Application of dynamic APACHEⅡscore and POSSUM score in patients with severe acute pancreatitis

Objective To discuss the value of combining the physiological and operative severity score for enumeration of mortality and morbidity(POSSUM)and acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)in severe acute pancreatitis(sAP).Methods 60 cases of SAP were divided into surgery group(n=15)and non-surgical group(n=45),Non-surgical group was vahde with APACHE Ⅱ score,the surgery group with POSSUM score,decided to continue conservative treatment or surgery treatment,Observation of two ways with the complications and death.Results The rates of compHcafions and death were(0.37±0.08)and(0.27±0.09)in Single-operation group(n=15)higher than(0.76±0.14)and(0.61±0.15)in surgical group(n=11)(t1=3.125,t2=3.211,P<0.01);APACHEⅡ score of mortality in operation group and non-surgical group were no significant difference(x2=2.28,x2=1.98,P>0.05);APACHE Ⅱ score were(10.12±6.27)in survival group(n=46)were lower than (25.75±7.90)in death group(n=14)(t=2.525,P<0.05.Conclusion The score of dynamic APACHE Ⅱ has better effect to judge the timing of surgery,and the score of POSSUM has high value to predict post-operative complications and deaths occurred in patients with severe acute pancreatitis.
4.Expression of C2orf40 protein and its clinical significance in human nasopharyngeal carcinoma

Objective To evaluate the protein expression of chromosome 2 open reading frame 40 (C2orf40) in nasopharyngeal carcinoma (NPC) tissues and cells,and to explore its association with cell differentiation and clinicopatho]ogical features.Methods A total of 122 patients with NPC between January 2001 and December 2003 were enrolled in Cancer Hospital of Shantou University Medical College.The paraffinembedded tissue sections and medical records were collected.Twenty-five samples with chronic nasopharyngitis were used as controls.Tumor and control tissues from biopsies underwent immunohistochemical staining for C2orf40.C2orf40 expression was analyzed with clinicopathological variables.Besides,the protein expressions of C2orf40 were measured by Western blotting in three NPC cell lines,including CNE1,CNE2 and C666-1.Results Ninety-six percent (24/25) of control tissues showed positive expression,among which 88.0% showed dense staining.Otherwise,only 58.3% (71/122) of NPC samples were positive for C2orf40 protein and 82.0% showed weak staining.There was significantly difference between the two groups (U =255.500,P < 0.001).It was inversely related to lymph nodes status (r =-0.058,P < 0.001) and clinical stage (r =-0.202,P =0.026) by Spearman rank correlation test.In vitro,higher level of C2orf40 protein was found in well differentiated CNE1 cells,while lower levels were found in poorly differentiated cell lines CNE2 and C666-1.Conclusion Down-regulated C2orf40 expression is correlated with tunor cell differentiation,lymph nodes metastasis and clinical stage,and may be a molecular event in the occurrence and development of NPC.C2orf40 is likely to be a potential target of anticancer therapy.
5.Management of chronic acromioclavicular joint dislocation with hook plate and fascia lata graft

Objective To evaluate the therapeutic efficacy of hook plate and fascia lata graft in management of chronic acromioclavicular joint dislocation.Methods From May 2012 to March 2014,a total of 16 cases of chronic acromioclavicular joint dislocation (Rockwood type Ⅲ) were treated with hook plate and fascia lata graft.They were 10 men and 6 women,from 19 to 57 years of age (mean,34.7 years).Seven cases were at the left side and 9 at the right side.All injuries resulted from direct violence.The time from injury to surgery ranged from 8 to 54 weeks (mean,12.4 weeks).At the final follow-ups,the shoulder function was assessed using the Quick Version of the Disabilities of the Arm,Shoulder and Hand Score (DASH) and visual analogue scale (VAS),and the efficacy using Karlsson's evaluation system.Results All surgical wounds primarily healed.The dislocation of acromioclavicular joint was corrected,without any vascular or neural damage.The patients were followed up for an average of 12 months (from 6 to 18 months).No plate breakage,screw loosening or redislocation was observed.The DASH and VAS scores of the shoulder at the final follow-ups (25.42 ± 8.34 and 2.22 ± 1.70,respectively) were significantly improved,compared with the preoperative ones (43.72 ± 12.35 and 6.33 ± 1.37,respectively) (P < 0.05).According to the Karlsson's criteria,10 cases were excellent,4 good and 2 moderate.Conclusion In management of Rockwood type Ⅲ chronic acromioclavicular joint dislocation,hook plate and fascia lata graft can restore dynamic balance and stability of the acromioclavicular joint,resulting in solid fixation,early joint motion and limited complications.

7.Influence of heat and moisture exchanger on sputum of patients with tracheotomy receiving hyper baric oxygen treatment

Objective To observe characters of sputum of patients with tracheotomy by heat and moisture exchanger(HEM)undergoing hyperbaric oxygen therapy(HBOT)in multi-person chamber.Methods In randomly block design,239 patients were selected and received HBOT using HEM combined with L-shape tube(group HEM,n=128)and by L-shape tube(group L,n=111)respectively.The characters and volume of sputum,times of sucking sputum and cases of sealing/extubation of tubes were assessed over first and tenth sessions of HBOT.Results The times of sucking and volume of sputum over first session of HBOT in the group HEM were more than those in group L.The sputum was changing from ropiness over first session to tenuity over tenth sesion(P<0.01).Cases with sealing/extubation of tubes in group HEM were more than those of group L(P<0.01).Conclusions It is helpful to humidify and discharge deposited sputum of patients over routine hyperbaric oxygen by HEM.And the case8 of sealing/extubation of tubes are also reduced.
8.Comparisom of different methods of determination of eosinophil cells

Objective To comparisom five methods of detection of eosinophil cells,in order to analyze the precision of the five methods.Methods The eosinophil cells of 100 patients were tested by using microscope、BC T5difft and flow cytometry are(0.249 ±0.112) × 109/L,(0.253 ±0.105) × 109/L,(0.252 ±0.103) × 109/L,(0.256 ±0.104) × 109/L and(0.259 ±0.101) × 109/L.There are no statistical differences(F = 1.225,P>0.10)among the five methods.Conclusion The results of the five methods are all accurate;the automatic analyzer method and the flow cytometry method are simple and repeatable,but the equipment and reagents are more expensive;the microscope direct counting method is more complicated and less repeatable.
10.Secondary intramedullary nailing following external fixation for open tibial shaft fractures

Objective To explore the clinical efficacy of secondary unreamed intramedullary nailing following primary external fixation for open tibial shaft fractures.Methods From March 2011 to June 2014,we treated 28 open tibial shaft fractures.They were 17 men and 11 women,aged from 21 to 62 years (average,38.2 years).After thorough debridement,the tibia was stabilized temporarily with external fixation.The wounds were handled with primary closure or secondary repair with skin grafts or tissue flaps.After the wounds healed,the external fixation was replaced by unreamed intramedullary nailing.Fracture union and complications were documented.The clinical efficacy was evaluated at the final follow-ups according to the Johner-Wruhs scoring system.Results All the 28 patients were followed up for 12 to 18 months (average,13.5 months).Bony union was achieved in all after 4 to 15 months (average,4.9 months).Delayed union was observed in 4 cases which were healed after dynamic fixation.There was no deep infection,nail breakage,malunion or limb shortening in any one.Mild knee pain was reported by 5 cases after surgery.According to the Johner-Wruhs criteria,18 cases were excellent,7 good,and 3 moderate,giving an excellent to good rate of 89.3%.Conclusion Primary external fixation and secondary unreamed intramedullary nailing can result in good fracture union with satisfactory biomechanical stability and favorable early rehabilitation in the treatmeut of open tibial shaft fractures.
