1.A retrospective analysis of clinical features and death factors of infants and young children with severe pneumonia

Objective Through retrospective data analysis,we tried to further understand the epide-miological characteristics,clinical feature and death factors of infant and young children with severe pneumo-nia. Methods The study objects were inpatients( age between≥28 days and≤3 years) who were diagnosed severe pneumonia from 1 January,2011 to 31 December,2013 of the Chengdu Women′s and Children′s Cen-tral Hospital. We used retrospective case study to understand the epidemiology,clinical feature,death factors of infant and young children with severe pneumonia. And we used chi-square test and Logistic multivariate regression analysis to analyze the death factors of infants and young children with severe pneumonia. Results (1) Among 1 411 cases of severe pneumonia,the ratio of male and female was 1. 8∶1,and the ratio of urban and rural areas was 1∶3. 62. The proportion of less than 3 months old infant was 46. 00%. And 62. 93% infant and young children with severe pneumonia occurred in the spring and winter. (2) Average hospitalization time was (9. 99 ± 6. 27 ) days, longer than the hospitalization time of mild pneumonia patients. ( 3 ) A total of 64. 21% of infant and young children with severe pneumonia had basic diseases. (4)A total of 91. 99% of the infant and young children with severe pneumonia had complications. (5) The most common etiology of infant and young children with severe pneumonia was bacteria,the second was virus. (6) In all cases,there were 44 cases died. The mortality of infant and young children with severe pneumonia was 3. 12%. And 72. 73% of the death cases were infants less than 3 months old. (7) The results of Logistic multiple regression analysis showed that there were significant differences in age, congenital heart diseases, repeating infection history, multiple drug-resistant strains infection, surgical history, multiple organ dysfunction, internal environment disorder. Conclusion Infant and young children with severe pneumonia have the following characteristics:most of them occurred in the winter and spring, and come from rural more than from the city. The smaller the age, the incidence of a disease is higher,and the mortality is higher. Most of infant and young children with severe pneu-monia have basic diseases. Most of the infant and young children with severe pneumonia have complications. If having one of the following high-risk factors:less than 3 months old,congenital heart diseases,repeating infec-tion history,multiple drug-resistant strains infection,surgical history,multiple organ dysfunction,internal envi-ronment disorder,the infant with severe pneumonia should be intensively monitored and actively treated.
2.A case report of childhood Pseudomonas aeruginosa sepsis

Objective To summarize the clinical features of Pseudomonas aeruginosa sepsis in children. Methods We retrospectively review the clinical data of a baby boy with Pseudomonas aeruginosa sepsis and summarize its clinical character-istics. Results A ten-month male infants with onset symptom of fever, irritability, drowsiness, cough and sputum was diagnosed as Pseudomonas aeruginosa sepsis through blood and sputum culture. The baby recovered well after anti-infection treatment. Conclusions Timely and appropriate use of sensitive antibiotics can decrease severe complications and mortality rate of Pseu-domonas aeruginosa sepsis in children.

Gut microbiota means a highly diverse and dense microbiota which inhabits in the gastrointestinal tract of humans.It plays a vital role in many important physiological processes including exerting nutritional and metabolic activities,regulating the immune system,and protecting against pathogens.Recent studies suggested that the gut microbiota was associated with the development of many lung diseases,such as pneumonia,asthma and tuberculosis.There is a gut-lung axis in human body.Now,the relationship between the gut microbiota and the gut-lung axis is reviewed.
4.VEGF antisense oligodeoxynucleotides in enhancing the apoptosis-inducing effect of Cinobufotalin on K562 cells

Objective To study the vascular endothelium growth factor(VEGF) antisense oligonucleotide(ASON) Cinobufotalin in enhancing apoptosis induced by human chronic myeloid leukemia K562 cell line.Methods The synthesis of VEGF ASON was transfected into the K562 cells;Cinobufotalin of four concentrations(1,3,5 and 7mg/L) acted on the K562 cell line for 24h,48h and 72h.Western blot was used to detect VEGF protein expression,while in situ apoptosis(TUNEL) and flow cytometry method(FCM method) were employed to detect the apoptosis.Results The different doses of Cinobufotalin all inhibited K562 cell line in time-and dose-dependent manners.K562 cell line transfected by VEGF ASON had a more pronounced induction of apoptosis.Conclusion The in vitro Cinobufotalin at a certain range of concentration can induce apoptosis in K562 cell line,and VEGF ASON enhances Cinobufotalin's effect in inducing apoptosis of K562 cells.
5.Clinical analysis of 113 cases of tuberculous pleurisy in children

Objective To investigate the clinical characteristics of tuberculous pleurisy(TP) in children in order to provide a good solution to the diagnosis and treatment of this disease.Methods Retrospective analysis was performed on the clinical data of 113 cases confirmed with TP who were admitted to Children's Hospital of Chongqing Medical University from January 2009 to December 2014.Results Among the 113 pediatric inpatients with TP,85 cases (75.2%) were sick for less than 1 month;92.9% (105/113 cases) age ≥≥5 years old by onset;and 21.2% (24/113 cases) patients demonstrated the contact with tuberculosis ,among whom 70.8% (17/24 cases) attributed to linear relatives.The primary clinical manifestations included fever[96.5% (109/113 cases)], cough [76.1% (86/113 cases)] and chest pain[38.9% (44/113 cases)] ,with as high as 89.0% (87/109 cases) of the patients experiencing moderate or ardent fever.The positive rates of acid-fast bacilli smear and mycobacterium tuberculosis culture were 12.2% (17/139 cases) and 22.7% (15/66 cases), respectively;among the 41 patients who received pleural biopsy,40 cases (97.6%) were identified with the typical pathological changes in tuberculosis;and all 11 patients who received acid fast stain test on pleural tissues were positive to the test.Pulmonary parenchymatous lesions were revealed on CT examination in 89.1% patients(98/113 cases),82.4% (90/109 cases) of the patients had normal body temperature within a short period,and reduced effusion was revealed in 86 out of 98 patients who received repeated chest imaging examinations.Conclusions TP in children has acute onset typically, and the diagnosis of TP in children is more difficult than in adults,requiring considering multiple factors.Contact history with tubercular patients, etiological examination, pleural biopsy and chest CT are essential for the diagnosis of this disease.
6.Application experience of postoperative gastrointestinal decompression tube in 15 gastric cancer cases of basic-level hospitals

Objective To sum up the experiences of the application of gastric cancer postoperative gastroin-testinal decompression tube.Methods Retrospectively analyzed the implementation of postoperative gastrointestinal decompression tube undergoing elective surgery.The clinical data of 15 cases of gastric cancer,preoperative education communication,preoperative preparation,intraoperative and postoperative treatment were analyzed.Results 14 cases recovered,1 case was not completely correct anastomotic fistula by malnutrition.The average for the first time the anus exhaust time was (48.7 ±16.4)h,for the first time defecation time was (65.1 ±5.7)h,postoperative length of hospi-tal stay was (12.4 ±3.1)d,postoperative nausea and vomiting occurred in 2 cases,diarrhea in 2 cases,sore throat discomfort in 3 cases.Conclusion Postoperative gastric cancer patients with gastrointestinal decompression tube can reduce stress damage,improve postoperative experience,accelerate the postoperative rehabilitation,does not increase the incidence of postoperative complications,it is safe and feasible.
7.TAD regimen with melphalan to treat 18 cases of multiple myeloma

Objective To observe the clinical results of combination therapy for multiple myeloma by arsenic trioxide,melphalan,vitamin C and dexamethasone.Methods 18 cases with multiple myeloma,12 males and 6 females were given the treatment including arsenic trioxide,melphalan,vitamin C and dexamethasone combination therapy;the dosage of arsenic trioxide 10 mg,d1~5,d8~12;vitamin C 1.0 g,d1~5,d8~12;dexamethasone 20 mg,d1~4,d8~11;melphalan 2 mg,tid,d1~12.Complete response(CR),partial response(PR),minimal response(MR),no ehange(NC)were observed.Results The CR werel6.7%(3/18),PR were 77.7%(14/t8),MR were 5.6%(1/18).Conclusion Combinational arsenic trioxide,melphalan,vitamin C and dexamethasone is an effective therapy with less side effects,which can be considered for treating the initial or relapsed or refractory MM.
8.Effects of Hip Flection and Extension in Supine Position on Lumbar Traction for Prolapse of Lumbar Intervertebral Disc

Objective To compare the curative effects of lumbar traction in supine position with hip flection and extension for prolapse of lumbar intervertebral disc (PLID).Methods 84 PLID patients were all treated by lumbar traction in supine position. But, group A (42 cases) with hip flection and group B (42 cases) with hip extension. Before and after three weeks therapy, all patients were assessed by Fairbank JC index and Visual Analogue Scales (VAS) to observe the symptom and pain condition.Results The scores of Fairbank JC index and VAS scale of the patients in group A were significantly lower than those in the group B (P<0.001).Conclusion Lumbar traction in supine position with hip flection for the PLID can obviously promote the curative effects.
9.The value of rapid species identification of Mycobacterium tuberculosis by two kinds of multi-locus ;polymerase chain reaction in children

Objective To evaluate the clinical value of multi-locus polymerase chain reaction (PCR) for identifying Mycobacterium tuberculosis complex isolated in children. Methods The isolates were collected and were first determined by PNB/TCH medium. 7-point PCR sites including 16SrRNA, Rv0577, IS1561, Rv1510, Rv1970, Rv3877/8 and Rv3120, and 4-point PCR sites including ropB, RD1, RD8 (present), RD8 (deleted) were used to amplify them by PCR. Results Total of 204 isolates were collected, in which 199 were Mycobacterium tuberculosis, 3 were Mycobacterium bovis, and 2 were non-tuberculous mycobacteria by the PNB/TCH method. 4-point PCR analysis showed that 196 were Mycobacterium tuberculosis, 2 were Mycobacterium bovis, 3 were BCG species and 3 were non-tuberculous mycobacteria. 7-point PCR analysis showed that 191 were Mycobacterium tuberculosis, 2 were Mycobacterium bovis, 3 were BCG species, 4 were African Mycobacterium type I, 1 was Mycobacterium caprae, 1 was Mycobacterium microti and 2 were non-tuberculous mycobacteria. Conclusion Compared with the conventional method, the PCR identification in 4-point PCR method and 7-point PCR method could rapidly identify the BCG among the complex group in children tuberculosis. 7-point PCR method was able to identify all the subspecies of Mycobacterium, except Africa Mycobacterium. 4-point PCR method would be more rapid and easier in the identification of BCG strains.
10.In vitro study of atrial natriuretic peptide circadian secreted by encapsulated atrial natriuretic peptide cDNA transfection cells

AIM:To investigate atrial natriuretic peptide(ANP) circadian in the encapsulated human ANP(hANP) cDNA transfected cells,to alter the ANP circadian by artificial control to achieve the objective of effectively treat hypertension or congestive heart failure(CHF). METHODS:ANP cDNA was transfected into Chinese hamster ovary(CHO) cells,which were encapsulated in polycarprolactone(PCL) tubes.The longterm survival of transfected CHO cells and the levels of ANP secreted were detected.Circadian rhythm of ANP secreted by encapsulated transfected cells was also studied,which was regulated by melatonin. RESULTS:During culturing,the ANP level secreted by transfected CHO cells in 2 mL of culture medium within 24 hours could reach 210.3 ng/L in a 20 mm-long and 2 mm-diameter PCL tube.The section of ANP displayed a circadian variation:higher in daytime,but lower at night.The acrophase of circadian rhythm was 4:15 but could be shifted to 7:55 after melatonin management. CONCLUSION:ANP cDNA transfected CHO cells that encapsulated into PCL tube can secret ANP,which might be suitable for the future implantation into human body.Our research provides a new approach in the treatment of hypertension and CHF by ANP.
