2.Influence of clinical results of early low frequency training for acute Achilles tendon rupture after anchors repair

Objective To investigate the influence of clinical results of early low frequency training for acute Achilles tendon rupture after anchors repair.Methods A total of 35 patients with acute Achilles tendon rupture were treated with early low frequency training after anchors repair,including 28 males and 7 females.Aged 18 to 37 years, mean 26 years old.Duration 2h to 5d from injury to therapy.All patients palpated the discontinuity of Achilles tendon, limitative activity of ankle plantar flexion,Thompson test was positive,and MRI examination was taken.Results 35 patients were followed up for 12 to 18 months,with an average follow -up of 14.5 months.According to the Amer-Lindholm score,excellent in 30 cases,good in 5 cases,the good rate was 100%.All the wounds were healed, without infection,dehiscence,sural nerve injury and re -rupture occurred.Conclusion Early low frequency training after anchors repair for acute Achilles tendon rupture is safe and reliable,patients can receive a better functional outcome.
3.Analysis of chemical components in Aconitum kusnezoffii leaves and their extracts by infrared spectroscopy.

Aconitum kusnezoffi leaves and their extracts were analyzed and evaluated by fourier transform infrared spectroscopy, second derivative spectra, and two dimensional correlation infrared spectroscopy in this research. There was characteristic IR absorption peaks of polyose (767 cm(-1)) in the Aconitum kusnezoffii leaves. The IR spectra of the aether extract were similar with the ethanol extract of Aconitum kusnezoffi leaves. The strong absorption peaks of carbonyl (1 715 cm(-1)) and saturated alkyl groups (2 921, 2 851 cm(-1)) could be detected. Some fatty acids were also detected in the water extract. Infrared spectroscopy, combined with two dimensional correlation infrared spectroscopy, can provide much structural information for herbs, which will benefit the quality control, components tracing and identification of extraction process of herbs roundly and qualitatively.

4.Chinese drugs for supplementing Qi and activating blood circulation in preventing deep venous thrombosis after big operations in orthopaedics and traumatology.

OBJECTIVETo evaluate the efficacy and feasibility of the Chinese drugs for supplementing Qi and activating blood circulation in preventing DVT after big operations in orthopaedics and traumatology and observe the change of D-dimer before and after treatment.
METHODSeventy patients ranging in age from 39 to 94 years who were treated by big operations in orthopaedics and traumatology were divided into two groups randomly. Traditional Chinese medinine (TCM) group (group A) and western medicine group (group B). On the 2rd day after operation d-dimer was tested from both groups and the patients in group A were given one dose of TCM every day and the patients in group B were subcutaneously injected low-molecular-weight heparins calcium (LMWH). On the 8th after operation d-dimer was tested again from both groups. The parameters, such as the incidence of DVT and pulmonary thromboembolism, bleeding condition, were evaluated respectively.
RESULTThe D-dimer decreased in group A and had significant difference (P < 0.05), group B had no significant difference, there was also no significant difference between two groups. DVT was found in 1 patient in group B (1/35). Petechiae were found in 10 patients in group A (10/35) and in 26 patients in group A (26/35), which had significant difference (P < 0.05).
CONCLUSIONBoth the Chinese drugs and anti-coagulation drug can prevent the incidence of DVT effectively, and Chinese herbs are feasible in the prevention of DVT.

5.Study on the correlation between nonunion and TCM constitutions and exploration on the application of the concept of "prevention of disease"

Objective:To explore the disease characteristics and TCM constitutions of patients with nonunion; To analyze the risk factors of nonunion and its correlation with TCM constitution; To provide research ideas for the prevention and treatment of nonunion with TCM based on the concept of "prevention of disease".Methods:This study was an observational case-control study. 334 orthopedic patients who visited the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September 2020 to September 2022 were selected. 167 patients were collected according to the diagnostic criteria of nonunion, then 167 patients who met the criteria of fracture healing were included as the control group. General information and the classification of TCM constitutions of the patients were collected. The distribution rules of TCM constitution and patients' risk factors were statistically analyzed, and the correlation between the two was explored.Results:The unbalanced constitution was found in 126 cases (75.45%) in the bone nonunion group and 108 cases (62.87%) in the fracture healing group, with statistical significance ( χ2=4.63, P=0.032). The proportion of TCM constitutional types distributed among the patients with nonunion were in the following order: phlegm-dampness constitution, yang-deficiency constitution, mild constitution, qi-deficiency constitution, yin-deficiency constitution, damp-heat constitution, qi-stagnation constitution, blood-stasis constitution and inherited-special constitution. There was statistical significance between the nonunion group and the fracture healing group in the phlegm-dampness and yang-deficiency in men and yang-deficiency in women ( χ 2 values were 19.12, 4.96, 4.92, respectively, P<0.01 or P<0.05). Diabetes [ OR (95% CI): 2.672(1.067, 6.693), P=0.036], BMI≥24 [ OR (95% CI): 1.903 (1.182,3.063), P=0. 008], phlegm-dampness [ OR (95% CI): 3.099 (1.624,5.913), P=0.001] and yang-deficiency [ OR (95% CI): 2.424 (1.252,4.693), P=0.009] as independent risk factors for the development of nonunion in multifactorial logistic regression analysis. Conclusions:The proportion of unbalanced constitution in patients with nonunion is significantly higher than that in patients with fracture healing. Phlegm-dampness constitution and yang-deficiency constitution are independent risk factors for the development of nonunion. TCM constitutional types can be an important factor in the risk assessment of nonunion and the prevention and treatment of nonunion based on the concept of "prevention of disease" has a sound theoretical basis and wide application prospects.
6.The Short-term Effectiveness of Three Manipulations for Atlantoaxial Joint Disorders and the Effects on Surface Electromyography of Sternocleidomastoid Muscle

ObjectiveTo compare the short-term effectiveness of the three different manipulations for atlantoaxial joint disorders and their effects on surface electromyography of sternocleidomastoid muscle. MethodsNinty patients with atlantoaxial joint disorders were randomly divided into the tendon relaxing manipulation group, the tendon relaxing plus rehabilitation manipulation group, and the conventional manipulation group, with 30 cases in each group, and each group of patients received the corresponding manipulation treatment for 2 weeks. The changes of visual analogue score (VAS) of occipital neck pain, evaluation scale for cervical vertigo (ESCV), and averaged electromyography (AEMG) of surface electromyography of bilateral sternocleidomastoid muscles before and after the treatment were observed, and the clinical effectiveness and safety of the patients were compared among groups. ResultsThe VAS scores of patients in each group decreased, and the ESCV scores increased after treatment (P<0.01), and the tendon relaxing manipulation group and the tendon relaxing plus rehabilitation manipulation group were significantly better than the conventional manipulation group (P<0.01). The AEMG of the bilateral sternocleidomastoid muscles of the three groups increased after treatment (P<0.01); when compared among the three groups, the AEMG of the bilateral sternocleidomastoid muscles of the tendon relaxing plus rehabilitation manipulation group was higher than that of the tendon relaxing manipulation group, and the tendon relaxing manipulation group was higher than that of the conventional manipulation group (P<0.05 or P<0.01). The cure and markedly effective rates of the tendon relaxing manipulation group, the tendon relaxing plus rehabilitation manipulation group, and the conventional manipulation group were 56.67%, 86.67%, and 36.67% respectively, showing statistically difference (K=10.21, P<0.01). ConclusionThe tendon relaxing manipulation and tendon relaxing plus rehabilitation manipulation can effectively improve the symptoms of vertigo, headache, and neck pain for patients with atlantoaxial joint disorders, and can improve the contraction function of sternocleidomastoid muscle, whose effectiveness are better than that of conventional manipulation.
