Objective To explore the patterns of motor and linguistic activation in cortical and its correlations with abnormal gray matter in patients with malformations of cortical development(MCD)and epilepsy. Methods Seven MCD patients with epilepsy(2 patients with focal cortical dysplasia,2 heterotopia,2 schizencephaly, and 1 polymicrogyria)underwent blood-oxygen-level-dependent(BOLD)functional MRI(fMRI)in a 3 T MR scanner when practicing bilateral fingers tapping, toes twisting, verb generation, and picture naming.Functional images were post-processed by using SPM 5 software based on a general linear model(GLM)to generate activations above a uniform threshold with the cluster size (≥30 voxels,P <0.001 corrected). The activations were recognized and classified by two experienced neuroradiologists, and then compared with that in abnormal gray matter. Results The clusters and intensities of motor activations were mainly located in the sensormotor cortex(SMC)and premotor area (PMA).In linguistic tasks, activations produced by verb generation were found in language-associated cortical regions and PMA with higher activation in Wemicke area, picture naming significantly in the visual cortex, and language in Broca area.Combination of the two linguistic tasks produced significant clusters and intensities in language cortex. For MCD patients with abnormal cortical abnormalities, motor and language task could produce neuronal activities within normal as well as abnormal cortex regions. In 6 patients who underwent resective surgery, epileptic seizures decreased significantly, and the follow-up images demonstrated no new neurological dysfunctions and cognitive impairments. Conclusions fMRI can visualize neuronal activities in patients with MCD and epilepsy and demonstrate the motor and linguistic activations occurring in normal and abnormal gray matter. It should be cautious for surgery in patient with MCD and epilepsy.

Objective To explore the diagnostic value of CT cerebral perfusion imaging on the brain injury of the high +Gx in Rhesus. Methods Seven healthy male adult Rhesus were randomly divided into control group and +15 Gx group. The +15 Gx group underwent parabolic G curve in animal centrifuge. The animals were all examined by CT cerebral perfusion before +Gx exposure, 2 h, 24 h, and 1 week after ~+Gx exposure. The results were compared with pathologic examination. Results 2 h and 24 h after +15 Gx exposure, brain ischemia was showed on CT cerebral perfusion imaging. After 1 week, the brain ischemia was almost recovered to normal. Mild ischemic atrophy was observed in pyramidal neurons in cerebral cortex by light microscopy. Electron microscopic observation showed chromatin marginating and mitochondria cristae blurring in pyramidal cells after +Gx overload. Conclusion High G from simulating spaceship emergency return can cause ischemic injuries of the brain in Rhesus, and CT brain perfusion imaging can provide valuable diagnostic information.

Objective To study the role of ubiquitin ligase FBW7 in the sensitivity of glioma to temozolomide and its mechanism in glioma cells. Methods FBW7 overexpression lentivirus was constructed. Glioma cell line U251 was divided into 4 groups: the control group, temozolomide group, FBW7 overexpression group, and FBW7 overexpression + temozolomide group. Compared with the intervention on U251 cell lines, the differences of cell inhibitory rates in 4 different groups were analyzed by using contrast microscopy and methyl thiazolyl tetrazolium (MTT) colorimetric assay after 36 h and 72 h respectively. Flow cytometry (FC) was used to determine the cell cycle and apoptosis rate. Results The survival number of U251 cells in the three treatment groups was increased compared with the control group at both 36 h and 72 h. The inhibitory rates of temozolomide group, FBW7 overexpression group, and FBW7 overexpression +temozolomide group at 36 h were (17.6±0.8) %, (10.4±0.6) %, (18.6±0.6) % respectively compared with the control group (F=67.02, P<0.01); while at 72 h, the inhibitory rates of the three treatment groups were (25.1 ±0.4) %, (16.7 ±0.7) %, (29.0 ±0.9) % respectively compared with the control group (F= 74.61, P<0.001). Moreover, FBW7 overexpression + temozolomide group presented much greater inhibitory rate than temozolomide group (P<0.01). The G2/M arrest ratio and the cell apoptotic rate at 72 h in the three treatment groups were higher than those in the control group (F=41.63, P<0.001;F=42.30, P<0.01). The increased degree of G2/M arrest ratio and the cell apoptotic rate in FBW7 overexpression + temozolomide group were more obvious compared with temozolomide group (P<0.05, P<0.01). Conclusion FBW7 could enhance the sensitivity of glioma cells to temozolomide treatment, which is associated with G 2/M arrest and the increased apoptosis rate induced by FBW7.

Objective:To compare the clinical effect of transpedicular impaction and grafting of allogeneic bone containing enriched bone marrow combined with posterior internal fixation and posterior subtotal vertebrectomy combined with posterior internal fixation in the treatment of stage III Kümmell′s disease. Methods:A retrospective cohort study was made on clinical data of 40 patients with stage III Kümmell′s disease admitted to Zhengzhou Orthopedic Hospital from June 2015 to December 2018. There were 10 males and 30 females, at age range of 57-79 years[(67.7±6.1)years]. A total of 19 patients were treated by transpedicular impaction and grafting of allogeneic bone containing enriched bone marrow combined with posterior internal fixation (impaction bone graft group), and 21 patients by posterior subtotal vertebrectomy combined with posterior internal fixation (subtotal vertebrectomy group). Operation time and intraoperative blood loss were compared between the two groups. Degree of pain, lumbar dysfunction and degree of kyphosis were evaluated by visual analogue scale (VAS), Japanese Orthopedic Association (JOA) score and kyphotic Cobb angle before operation, at 1 week after operation and at the last follow-up. Bone healing time was compared between the two groups. The complications of the two groups were observed. Results:All patients were followed up for 25-64 months[(40.6±10.4)months]. Operation time and intraoperative blood loss were (130.0±10.1)minutes and (284.5±43.5)ml in impaction bone graft group, lower than those in subtotal vertebrectomy group[(253.8±33.2)minutes, (889.1±95.7)ml](both P<0.01). There were no significant differences in VAS, JOA score or kyphotic Cobb angle between the two groups before operation, at 1 week after operation and at the last follow-up (all P>0.05). Both VAS and JOA score showed significant differences within each group at any time point (all P<0.01). In both groups, the kyphotic Cobb angle reduced significantly at 1 week after operation when compared with that before operation (all P<0.01), and the angle showed a slight increase at the last follow-up, but remained significantly lower than that before operation (all P<0.01). There were no relapse of pain or aggravation of kyphosis. Bone healing time in impaction bone graft group[4.4(4.0, 5.0)months]was significantly shorter than that in subtotal vertebrectomy group[6.4(5.2, 8.1)months]( P<0.01). There were 2 patients with delayed healing of surgical incision in impaction bone graft group, with the complication rate of 11%. There were 2 patients with dural tear and 3 patients with delayed healing of surgical incision in subtotal vertebrectomy group, with the complication rate of 24%. The complication rate was not statistically significant between the two groups ( P>0.05). No loosening or breakage of internal fixation was observed during the follow-up. Conclusions:Transpedicular impaction and grafting of allogeneic bone containing enriched bone marrow combined with posterior internal fixation and posterior subtotal vertebrectomy combined with posterior internal fixation are effective in the treatment of stage III Kümmell disease. However, the former can shorten the operation time, reduce the intraoperative blood loss and accelerate the healing of injured vertebral bone, suggesting a relatively minimally invasive surgical method for reconstruction and maintenance of spinal biomechanical stability.

Objectives To investigate the diagnostic performance of the combination of ultrasound elastography and automated breast volume scanner (ABVS) in differentiation of benign and malignant breast imaging reporting and data system (BI-RADS) 4 breast lesions. Methods Data from 137 breast cancer patients (147 tumors) confirmed pathologically were analyzed. Each tumor was examined by ABVS and ultrasound elastography. All tumors were diagnosed as BI-RADS 4 by ABVS. With final pathology results as the gold standard, the predictive value in differentiating BI-RADS 4 breast lesions between ultrasound elastography and the combination of ultrasound elastography and ABVS were compared. Results There were 54 benign nodules and 93 malignant nodules in this study. The diagnostic sensitivity of ultrasound elastography and the combination of ultrasound elastography and ABVS were 94.6% and 98.9%,the specificity were 57.4% and 57.4%, the accuracy were 81.0% and 83.7%, the area under the curve were 0.858 and 0.965, respectively. The diagnostic performance of ultrasound elastography combined with ABVS was better than that of ultrasound elastography. Conclusions Ultrasound elastography have certain value in differential diagnosis of BI-RADS 4 breast lesions, especially when combining with ABVS, which will improve its diagnostic accuracy. Ultrasound elastography combined with ABVS can improve the detection rate of malignant lesions in BI-RADS 4 breast lesions and reduce the rate of preoperative biopsy, and it has a good application prospect.
