Objective To investigate the risk factors of delirium in elderly patients with intertrochanteric fracture after internal fixation. Methods The data of 160 patients with intertrochanteric fractures who received internal fixation in our hospital from January 2012 to July 2014 were analyzed retrospectively. The risk factors such as age, sex, preoperative complications, preoperative cognitive function, fracture location, operation mode, operation time, anesthesia method,hospital-to-operation time and intraoperative blood loss were summarized. Results The incidence of postoperative delirium was 28.15% in postoperative elderly patients with intertrochanteric fractures.Univariate analysis showed that delirium had correlated with preoperative cognitive impairment, preoperative preparation time, serum sodium, fentanyl, atrial fibrillation, anesthesia method, operation time and perioperative blood loss (P < 0.05).The multivariate Logistic regression analysis showed that the independent risk factors of postoperative delirium were preoperative cognitive dysfunction, operative time more than 2 hours and preoperative preparation time more than 4 days. Conclusion The occurrence of postoperative delirium was associated with anesthesia method, cognitive deficits, preoperative preparation time and perioperative blood loss. The anesthesia method which had less effect on the whole body condition and less time of operation preparation can decrease the occurrence of postoperative delirium in a certain extent, which is conducive to improving the prognosis.

OBJECTIVETo seek a new method for reconstructing bilateral intemrnal jugular vein invaded by metastasis lymph node in advanced oral cancer patients.
METHODSA combination of microvascular anastomosis and longitudinal constriction suture venoplasty was performed to reconstruct internal jugular vein. We resected the part of the bilateral internal jugular vein of advanced oral cancer patients invaded by metastasis lymph node and used the external carotid vein to reconstruct the internal jugular vein. A part of the vessel wall of the internal jugular vein could also be resected to reconstruct the vein. Longitudinal constriction suture venoplasty could slowly narrow the lumen diameter of the internal jugular vein. Thus, difference in anastomosis diameter should be avoided because it generates eddy currents and subsequently causes blood clots. A total of five advanced cases of oral squamous cell carcinoma were involved in this study. We performed bilateral radical neck dissection on all patients to reconstruct the internal jugular vein and observed their postoperative conditions.
RESULTSPostopera-tive follow-up of 5 months to 19 months was performed on all patients. Doppler or CT angiography and related tests showed no internal jugular vein thrombosis. No patient with facial edema, throat swelling, cerebral edema, and high intracranial pressure or other serious complications caused by blocked venous blood was observed. The one-year survival rate of five patients was 60% (3/5).
CONCLUSIONMicrovascular anastomosis combined with longitudinal constriction suture venoplasty is a new method for reconstructing internal jugular vein. This method was proved successful and clinically feasible.

Objective To sum up a new surgical method to treat knee ankylosis with skeletal traction following arthrolysis. Methods Skeletal traction through supracondyle of femur or through patella was applied for 48 patients with knee ankylosis after dissection of both intra and extra articular adhesion and quadricepsplasty. Results The follow up period ranged from 3 to 36 months. The flexion angles of the knee joint increased 70?to 105?in 34 patients. Conclusion Skeletal traction following arthrolysis to treat knee ankylosis can prolong the length of quadriceps, increase the flexion angle of the knee joint step by step and facilitate early functional recovery after operation.
