2.Clinical observation on combined treatment of chronic nephritis proteinuria,a report of 22 cases

Objective To observe the curative effect of Zhengqingfengtongning release tablets and lumbroki-nase enteric -coated capsules and losartan potassium tablets joint for the treatment of chronic nephritis proteinuria, and explore its mechanism.Methods 42 patients with chronic nephritis were randomly divided into two groups. 20 cases in the control group were given losartan potassium tablets,50mg,1 ~2 times a day.22 cases in the treatment group received Zhengqingfengtongning release tablets 120mg,2 times a day,lumbrokinase enteric -coated capsules 600 000u,3 times a day on the basis of the control group.The 24h urine protein,liver and kidney function before and after the treatment were observed.Results 20 days after treatment,24h urinary protein in the treatment group decreased 57.08%,which in the control group decreased 41.59%,the difference was statistically significant (t =2.39,P <0.05).30 days after treatment,24h urinary protein in the treatment group decreased 81.28%,which in the control group decreased 52.34%,the difference was statistically significant (t =3.65,P <0.01 ).Renal function change had no significant difference between the two groups.Conclusion Zhengqingfengtongning release tablets and lumbrokinase enteric -coated capsules combined with losartan potassium tablets in the treatment of chronic nephritis proteinuria work fast,and has good curative effect,less side effects,and is worth to recommend.
3.Analytical performance evaluation of HBsAg ELISA kit using CLSI EP1 2-A2 document

Objective To evaluate the performance of enzyme linked immunosorbent assay(ELISA)reagent kit of hepatitis B surface antigen(HBsAg)with Clinical and Laboratory Standards Institute(CLSI)document EP12-A2,and to compare two different manufacturer kits′performance.Methods According to CLSI document EP12-A2,to evaluate the performance of two different manufacturers′HBsAg ELISA kits.Results The positive rates both were ≥95% by using the kits of two manufacturers to detect the same sample with 120%of the critical concentration.The negative rates were both ≥95%by using the kits of two manufactur-ers to detect the same sample with 80% of the critical concentration.The intra assay coefficients of variation were ≤15%,and the interassay coefficients of variation were ≤20%.The sensitivities of two manufacturers′kits were respectively 94.2%(95%CI:84.3%-98.0%)and 92.3%(95%CI:81.8%-97.0%),and the specificities were 98.0%(95%CI:89.5%-99.6%)and 100.0%(95%CI:92.8%-100.0%).The 95%interval of sensitivity of two manufacturers′s kits was 5.46%-10.19%,and which of specificity of two manufacturers′s kits was -2.00%-5.32%.Conclusion The kits of two manufacturers can provide stable re-sults of HBsAg detection with concentrations ±20% outside the critical concentrations at the cutoff point with high sensitivity and specificity.Both of the kits have no significant differences in sensitivity and specificity.
4.Study on the correlation between red cell distribution width and microalbuminuria in patients with chronic heart failure

Objective To observe changes of red blood cell distribution width (RDW) and trace albumin urine (MAU) in patients with chronic heart failure (CHF) and to explore the relationship between them and cardiac function classification. Methods A total of 190 patients with CHF were divided into mild group (NYHA grade II), moderate group (NYHA gradeⅢ) and severe group (NYHA gradeⅣ), according to the grade of cardiac function. A total of 100 subjects with normal physi?cal examination during the same period were selected as the control group. The changes of MAU, urinary creatinine (Scr) ra?tio (UACR), RDW, Scr, hemoglobin (Hb), type B urine sodium titanium (BNP) and left ventricular ejection fraction (LVEF)were compared between four groups. The correlation between UACR, RDW, Scr, Hb, BNP and LVEF were analyzed. Re?sults The levels of RDW, Scr, BNP, hypersensitive C-reactive protein (hsCRP), LVEDd, LVM, LVMI and UACR were sig?nificantly higher in chronic heart failure group than those of control, showing a trend of increase gradually with the rising of heart function classification (P<0.05). The values of Hb and LVEF showed a gradually decreasing trend with the increased heart function classification (P < 0.05). The values of UACR and RDW showed a gradually increasing trend with the in?creased heart function classification (P<0.05). There was a positive correlation between UACR and RDW, and both had a positive correlation with BNP, and a negative correlation with LVEF and Hb(P<0.05). Conclusion RDW and MAU are re?lated to the cardiac function classification, and both can be used for the evaluation of the prognosis in patients with CHF.
5.Effects of Rho associated kinase inhibitors on glaucoma and current researches

Glaucoma is one of the main causes of blindness in the world,resulting from a slow and progressive loss of retinal ganglion cells (RGCs) and their axons.So far,intraocular pressure reduction is the main treatment modality to control disease progression.However,not all the patients benefit from this therapy,and the pathophysiology of glaucoma is not always associated with an elevated intraocular pressure.These limitations,together with the multifactorial etiology of glaucoma,urge the pressing medical need for novel and alternative treatment strategies.Such new therapies should focus on not only preventing or retarding RGCs death,but also repairing the injured axons.Glaucoma optic nerve protection therapy is becoming a hot spot of current research,especially Rho associated kinase (ROCK) inhibitors as a new drug for the glaucomatic neuroprotection,which can reduce intraocular pressure,inhibit scar formation after anti-glaucoma surgery,improve blood circulation,prevent RGCs death and axonal degeneration and induce axon regeneration.In this review,we focused on the research status and progress of the classical signal pathway of ROCK inhibitor in glaucoma optic nerve protection.
6.Diagnosis and Treatment of Patients with Refractory Functional Gastrointestinal Disorders Accompanying with Psychological Disorders

Patients who have functional gastrointestinal disorders(FGIDs)often together with psychological disorders and damaged quality of live. Management of FGIDs patients failed to conventional drug therapy,so-called the refractory FGIDs,is a challenge for gastroenterologists. Till now,FGIDs are still not fully recognized and clarified and having only limited therapeutic approaches. In this article,we reviewed the clinical characteristics of refractory FGIDs accompanying with psychological disorders and the progress in research on their diagnosis and treatment for improving the understanding and formularizing the management of this disease.
7.Research progress on regulatory mechanism of liver cancer stem cells

The theory of cancer stem cells proposed that recurrence and metastasis of liver cancer are closely related to liver cancer stem cells.The traditional surgical treatment of liver cancer simply kills those rapidly proliferating tunor cells instead of eliminating hepatic cancer stem cells which play decisive role in recurrence and metastasis.As far as people have concerned,there are many signaling pathways and regulatory factors modulating liver cancer stem cells,such as the Wnt/β-catenin pathway,TGF-β pathway,Notch pathway,Hedgehog pathway,hepatitis B virus,as well as epigenetics.Further studies on regulatory mechanisms of liver cancer stem cells are of great significance,in hope of providing new evidence for treatment and prevention of recurrence and metastasis of liver cancer.
9.Reasearch progress of cervical cancer screening

As to clarify the cause of cervical cancer,early normative and effective screening can decrease the incidence and mortality of precancerous cervical lesions and cervical cancer.At present the main methods for cervical cancer screening are pap smear method,acetic acid shading to naked eye observation,iodine staining to the naked eye observation,liquid based cytology and human papillomavirus DNA testing methods.Screening techniques and strategies are different in different areas and countries because of unbalanced economic development.
10.Forensic analysis of 109 cases of traumatic visual loss

Objective To analyze types, mechanism and forensic identification for traumatic visual loss. Methods 109 cases of the evaluation for impairment and disability on traumatic visual loss were retrospectively analyzed, including interocular differences, anisometropia, left or right eye and the distributions of visual acuity, etc. Results The male-to-female ratio was 5.8:1, mainly 30~59 years of age. The ratio of monocular versus binocular injuries was 17.17:1. The ratio of left versus right injured eye was 1.39:1 in intentional injury cases. 115 eyes were injured, 83 of which were in low vision or blindness. 95 patients(87.16%) had large interocular differences( ≥ 3 lines). There were 9 patients with anisometropia from 2.25D to 6.00D and 23 patients with anisometropia larger than 6.00D. Conclusion Interocular differences and anisometropia are common in practice, which are not included in the expertise standard yet. Functional vision should be considered when developing the expertise standard.
