4.Risk factors and pathogenic analysis in the newborn infants with ventilator-associated pneumonia

Objective To investigate the risk factors and microbial causes related to the pathogenesis of ventilator-associated pneumonia(VAP).Methods Totaly 138 intubated newborn infants for longer than 48 hours in Ningbo Women and Children's Hospital were studied,who were divided into VAP group (56 cases)and non VAP group(82 cases).Microbial causes of VAP were analyzed.Results Malnutrition,repeated tracheal intubation,repeated endotracheal suctioning,indwelling tube,posture,hyaline membrane disease,gestational age,birth weight,duration of intubation,duration of hospitalization were independent risk factors for ventilator-associated pneumonia(P<0.05).Klebsiella pneumoniae,pseudomonas aeruginosa,acinetobacter species and enterobacter cloacae were the main pathogenic bacteria of VAP.Conclusion Shortening of mechanical ventilation duration,avoiding repeated tracheal intubation and indwelling tube usage,semi-fowler positioning can lower the incidence of VAP.Pathogenic bacteria control is essential for VAP.

6.Treatment of fracture of elbow joint with internal fixation

Objective To describe the methods of treatment of fresh and old fractures of the elbow joint with internal fixation.Methods Ten cases of fracture of the elbow with complete records were reviewed; 9 of them were male and 1 female; 6 of them were Han people and 4 Tibetans. The average age was 31 years. Four cases had open fractures and 6 had closed fractures. Among the cases of open fractures, 3 were treated with routine debridement and internal fixation of the fragments or internal fixation plus advanced skin flaps. One was treated as closed fractures after wound healing. The 6 cases of closed fractures were treated with a single tension band wiring, tension band and screw fixation, or screw fixation. Results The mean duration of the follow up was 8 months. Fracture healing was seen at 3- 4 months. The range of motion of fresh fractures of the elbow and forearm was 4.2° - 144.2° . The old fracture cases had the range of motion of 31.3° - 115° without restriction in pronation and supination. Conclusion The intra articular fracture of elbow joint must be treated with early surgery to attain anatomical reduction with firm internal fixation. Early functional exercise is also important for the recovery.
7.Progress of the intestinal microflora in human immune and allergic diseases in children

Microorganisms can affect the development of body immune system.Intestinal flora regulate immune function by promoting the intestinal immune system development,inducing T cell differentiation to avoid or reduce the incidence of immune-related diseases.Allergic diseases are related with the body's immune system hypoplasia and imperfect immune regulation mechanism.The immune system can be affected by intestinal flora,and the distribution of flora in the body between children with allergies and healthy children are significantly different,indicating that the occurrence of allergic diseases are associated with intestinal microflora.Studies have reported probiotic are helpful to the prevention and treatment of allergic disease,providing a new way for the prevention and treatment of allergic diseases.

10.Study on the correlation between red cell distribution width and microalbuminuria in patients with chronic heart failure

Objective To observe changes of red blood cell distribution width (RDW) and trace albumin urine (MAU) in patients with chronic heart failure (CHF) and to explore the relationship between them and cardiac function classification. Methods A total of 190 patients with CHF were divided into mild group (NYHA grade II), moderate group (NYHA gradeⅢ) and severe group (NYHA gradeⅣ), according to the grade of cardiac function. A total of 100 subjects with normal physi?cal examination during the same period were selected as the control group. The changes of MAU, urinary creatinine (Scr) ra?tio (UACR), RDW, Scr, hemoglobin (Hb), type B urine sodium titanium (BNP) and left ventricular ejection fraction (LVEF)were compared between four groups. The correlation between UACR, RDW, Scr, Hb, BNP and LVEF were analyzed. Re?sults The levels of RDW, Scr, BNP, hypersensitive C-reactive protein (hsCRP), LVEDd, LVM, LVMI and UACR were sig?nificantly higher in chronic heart failure group than those of control, showing a trend of increase gradually with the rising of heart function classification (P<0.05). The values of Hb and LVEF showed a gradually decreasing trend with the increased heart function classification (P < 0.05). The values of UACR and RDW showed a gradually increasing trend with the in?creased heart function classification (P<0.05). There was a positive correlation between UACR and RDW, and both had a positive correlation with BNP, and a negative correlation with LVEF and Hb(P<0.05). Conclusion RDW and MAU are re?lated to the cardiac function classification, and both can be used for the evaluation of the prognosis in patients with CHF.
