3.Treatment of fracture of elbow joint with internal fixation

Objective To describe the methods of treatment of fresh and old fractures of the elbow joint with internal fixation.Methods Ten cases of fracture of the elbow with complete records were reviewed; 9 of them were male and 1 female; 6 of them were Han people and 4 Tibetans. The average age was 31 years. Four cases had open fractures and 6 had closed fractures. Among the cases of open fractures, 3 were treated with routine debridement and internal fixation of the fragments or internal fixation plus advanced skin flaps. One was treated as closed fractures after wound healing. The 6 cases of closed fractures were treated with a single tension band wiring, tension band and screw fixation, or screw fixation. Results The mean duration of the follow up was 8 months. Fracture healing was seen at 3- 4 months. The range of motion of fresh fractures of the elbow and forearm was 4.2° - 144.2° . The old fracture cases had the range of motion of 31.3° - 115° without restriction in pronation and supination. Conclusion The intra articular fracture of elbow joint must be treated with early surgery to attain anatomical reduction with firm internal fixation. Early functional exercise is also important for the recovery.
4.Comparison of different implants for internal fixation of intra-articular fractures of the proximal tibia

BACKGROUND: Intra-articular fractures of the proximal tibia are complex periarticular fractures, for which, the best implant is the focus of clinical attention.
OBJECTIVE: To explore the application methods and therapeutic effects of different implants in the internal fixation of intra-articular fractures of the proximal tibia in order to choose the appropriate implants.
METHODS:From April 2006 to May 2012, 41 patients with intra-articular fractures of the proximal tibia combined with ligament tissue injury were selected at the Second Department of Orthopedics, the 401st PLA, who had received different implant internal fixations. According to the Schatzker classification, there were 18 cases of type IV, 12 of type V, and 11 of type VI. Bone defect also be bone;simultaneously or two rows ligament tissue repair observed fracture healing and functional recovery. Bone grafting was given in patients with bone defects, and ligament tissue repair was done simultaneously or at the second stage. Fracture healing and functional recovery were observed. RESULTS AND CONCLUSION:Al the 41 patients were fol owed up for 6-36 months. Bristol scores in al the patients were 32-50, with a mean score of 46. Excellent was in 20 cases, good in 13 cases, and fair in eight cases. The excellent-good rate was 80%. Implants were removed in 29 of 41 cases within 12-28 months after treatment. Treatment methods are selected based on the type of fractures. Good reduction and stable internal fixation are necessary. Less Invasive Stabilization System can provide the maximal stability and lead to lowest probability of axial and lateral fracture deviation and the lowest incidence of complications, which is the best choice.
5.Proteomics technology application in screening of pediatric surgery diseases

6.Progress of the intestinal microflora in human immune and allergic diseases in children

Microorganisms can affect the development of body immune system.Intestinal flora regulate immune function by promoting the intestinal immune system development,inducing T cell differentiation to avoid or reduce the incidence of immune-related diseases.Allergic diseases are related with the body's immune system hypoplasia and imperfect immune regulation mechanism.The immune system can be affected by intestinal flora,and the distribution of flora in the body between children with allergies and healthy children are significantly different,indicating that the occurrence of allergic diseases are associated with intestinal microflora.Studies have reported probiotic are helpful to the prevention and treatment of allergic disease,providing a new way for the prevention and treatment of allergic diseases.
7.Discussion on TCM strategies and skills of the treatment of refractory jaundice

[Conclusion]Recommended that through played curriculum, scientific curriculum, to strengthen the course construc-tion of improvements in areas such as mental health course.
9.Diagnosis and managements of intracranial aneurysms with three-dimensional CT angiography

Objective:To explore of clinical value of three-dimensional computed tomography angiography(3D-CTA) in the etiological diagnosis and management of intracranial aneurysm.Methods:One hundred and fifteen patients with spontaneous SAH admitted from September 2007 to August 2008 were examined with 3D-CTA,and were examined with digital subtracted angiography(DSA),and eighty nine patients were found suffering from intracranial aneurysm which underwent DSA.Repeated 3D-CTA was performed 1 week after clipping in eighty one patients or embolization in eignt cases.The findings of 3D-CTA were compared with the results of DSA.Results:A total of ninty seven aneurysms in eighty nine cases were detected by 3D-CTA,but DSA revealed ninty three anurysms.DSA confirmed the findings of CTA except for five cases of small anterior communicating aneurysms and three cases posterior communicating aneurysms which were demonstrated by 3D-CTA and verified by surgery but missed by DSA and four cases which were found by DSA and verified by surgery but missed by 3D-CTA.The sensitivity of 3D-CTA to diagnose the intracranial aneurysms is 95.6% and the specificity is 100% and the accuracy is 96%.The diagnose accordance rate among 3D-CTA and DSA and/or surgery is 88.1%.Conclusion:3D-CTA and 2D-DSA and/or surgery have high diagnose accordance rate,and we should combine the two diagnostic techniques to examine intracranial aneurysms accurately.
10.Study of clinical value of noninvasive intracranial pressure monitoring with TCD in patients with traumatic brain injury

Objective: This study was designed to analyze the results from ICP and TCD monitoring in patients with traumatic brain injury (TBI) in order to further evaluate the clinical significance of noninvasive ICP monitoring with TCD. Methods:The monitoring and measurements with TCD were performed beside beds on the following day of surgery to record the spectral atlas and parameter of the middle cerebral artery (MCA) of suffering side in 32 patients with TBI. The values of ICP were recorded at the same time. The patients were divided into four groups according to ICP. The comparative analyses of results of TCD and ICP monitoring were carried out.Results:(1)Spectral atlas: The spectral atlas of Mild ICP Increase Group showed no visible differences from those of Normal ICP Group.A phenomenon of deepened notch before reaching D peak was found in some cases of Moderate ICP Increase Group while occurrence of single cusp peak and disappearance of S2 peak with decreased diastolic velocity were observed in Severe ICP Increase Group. (2) Parameter of TCD: With the increase of ICP,Vs,Vd and Vm were decreased accordingly with the most significant dropping in Vd while PI and RI were elevated gradually. Conclusion:This study suggests that TCD can be applied to monitor noninvasively ICP and indicate the significant increase of ICP in patients with TBI,but fail to detect mild increase of ICP. Spectral atlas is superior to parameters of TCD in judgment of increased ICP
