1.Study on the correlation between red cell distribution width and microalbuminuria in patients with chronic heart failure

Objective To observe changes of red blood cell distribution width (RDW) and trace albumin urine (MAU) in patients with chronic heart failure (CHF) and to explore the relationship between them and cardiac function classification. Methods A total of 190 patients with CHF were divided into mild group (NYHA grade II), moderate group (NYHA gradeⅢ) and severe group (NYHA gradeⅣ), according to the grade of cardiac function. A total of 100 subjects with normal physi?cal examination during the same period were selected as the control group. The changes of MAU, urinary creatinine (Scr) ra?tio (UACR), RDW, Scr, hemoglobin (Hb), type B urine sodium titanium (BNP) and left ventricular ejection fraction (LVEF)were compared between four groups. The correlation between UACR, RDW, Scr, Hb, BNP and LVEF were analyzed. Re?sults The levels of RDW, Scr, BNP, hypersensitive C-reactive protein (hsCRP), LVEDd, LVM, LVMI and UACR were sig?nificantly higher in chronic heart failure group than those of control, showing a trend of increase gradually with the rising of heart function classification (P<0.05). The values of Hb and LVEF showed a gradually decreasing trend with the increased heart function classification (P < 0.05). The values of UACR and RDW showed a gradually increasing trend with the in?creased heart function classification (P<0.05). There was a positive correlation between UACR and RDW, and both had a positive correlation with BNP, and a negative correlation with LVEF and Hb(P<0.05). Conclusion RDW and MAU are re?lated to the cardiac function classification, and both can be used for the evaluation of the prognosis in patients with CHF.

The prevalence of osteoporosis has increased during recent years. The activation in the function of osteo?clast is the main reason of osteoporosis. Cathepsin is expressed by osteoclast and involved in the degradation of collagen of bone,and causes osteoporosis. Cathepsin K is a kind of most important enzyme in the family of cathepsin. Odanacatib(ODN) is the inhibitor of cathepsin K, and it may be used to anti-osteoporosis thought inhibiting the degradation of bone collagen. It was found that the cortical thickness and bone minerals of cancellous increased after taking ODN in the studies, and then the density and the strength of bone increased. This study reviewed the pharmacological profile of ODN and the progresses of ani?mal study and clinical trials about ODN.

4.Performance evaluation of ABI 7300 real-time fluorescent quantitative PCR

Objective To participate in the recognition of medical laboratory and to evaluate the performance of the instruction, we assess the performance of ABI 7300 real-time fluorescent quantitative PCR instrument.Methods According to United States of America Clinical and Laboratory Standards Institute(CLSI),assess the precision,accuracy,sensitivity and linearity of HBV-DNA detection result in ABI 7300 fluorescence quantitative PCR instrument.Results The Intraassay coefficient of variation is 2.38%-4.61%,and interassay coefficient of variation was 3.87%-5.33%,both achieve the performance analysis standard of gene ampli-fication test project regulated by Medical Laboratory Quality and Competence Accreditation Criteria.Accuracy:the overall mean bias in 2011 Ministry of Health EQA results was 2.14%,which met the quality assessment requirements of the ministry of health. Functional sensitivity:the CV of 100 IU/mL detection limit concentration was close to 20%,which accords with the The limit of detection of the kit;Linear:the correlation coefficient r2 is 1.0,linear relationship met the requirements.Conclusion The perform-ance index of ABI 7300 real-time fluorescent quantitative PCR instrument is accuracy,and it can provide fast,accurate reports for clinical department.

6.Analytical performance evaluation of HBsAg ELISA kit using CLSI EP1 2-A2 document

Objective To evaluate the performance of enzyme linked immunosorbent assay(ELISA)reagent kit of hepatitis B surface antigen(HBsAg)with Clinical and Laboratory Standards Institute(CLSI)document EP12-A2,and to compare two different manufacturer kits′performance.Methods According to CLSI document EP12-A2,to evaluate the performance of two different manufacturers′HBsAg ELISA kits.Results The positive rates both were ≥95% by using the kits of two manufacturers to detect the same sample with 120%of the critical concentration.The negative rates were both ≥95%by using the kits of two manufactur-ers to detect the same sample with 80% of the critical concentration.The intra assay coefficients of variation were ≤15%,and the interassay coefficients of variation were ≤20%.The sensitivities of two manufacturers′kits were respectively 94.2%(95%CI:84.3%-98.0%)and 92.3%(95%CI:81.8%-97.0%),and the specificities were 98.0%(95%CI:89.5%-99.6%)and 100.0%(95%CI:92.8%-100.0%).The 95%interval of sensitivity of two manufacturers′s kits was 5.46%-10.19%,and which of specificity of two manufacturers′s kits was -2.00%-5.32%.Conclusion The kits of two manufacturers can provide stable re-sults of HBsAg detection with concentrations ±20% outside the critical concentrations at the cutoff point with high sensitivity and specificity.Both of the kits have no significant differences in sensitivity and specificity.
7.Comparison of different implants for internal fixation of intra-articular fractures of the proximal tibia

BACKGROUND: Intra-articular fractures of the proximal tibia are complex periarticular fractures, for which, the best implant is the focus of clinical attention.
OBJECTIVE: To explore the application methods and therapeutic effects of different implants in the internal fixation of intra-articular fractures of the proximal tibia in order to choose the appropriate implants.
METHODS:From April 2006 to May 2012, 41 patients with intra-articular fractures of the proximal tibia combined with ligament tissue injury were selected at the Second Department of Orthopedics, the 401st PLA, who had received different implant internal fixations. According to the Schatzker classification, there were 18 cases of type IV, 12 of type V, and 11 of type VI. Bone defect also be bone;simultaneously or two rows ligament tissue repair observed fracture healing and functional recovery. Bone grafting was given in patients with bone defects, and ligament tissue repair was done simultaneously or at the second stage. Fracture healing and functional recovery were observed. RESULTS AND CONCLUSION:Al the 41 patients were fol owed up for 6-36 months. Bristol scores in al the patients were 32-50, with a mean score of 46. Excellent was in 20 cases, good in 13 cases, and fair in eight cases. The excellent-good rate was 80%. Implants were removed in 29 of 41 cases within 12-28 months after treatment. Treatment methods are selected based on the type of fractures. Good reduction and stable internal fixation are necessary. Less Invasive Stabilization System can provide the maximal stability and lead to lowest probability of axial and lateral fracture deviation and the lowest incidence of complications, which is the best choice.

9.Designing a simulative model for the forecast and optimal placement of casualty evacuation assets based on the probabilistic location set covering problem

Objective:To design a simulative model for the forecast and optimal placement of mass casualty evacuation assets.Methods: An operation research module named the location selection problem was introduced in this study.The probabilistic location set covering problem was chosen as the theoretical model for optimal placement of mass casualty evacuation assets.Several indeterminate factors were analyzed in details,including the availability;capacity and velocity and effective work time of evacuation assets.The ideal statistical distributions were determined by the characteristics of these variables.Results: According to the principle of Monte Carlo simulation and based on the probabilistic location set covering problem,random numbers fitting theoretical probabilistic distributions were generated to simulate those indeterminate factors,based on which a simulative model for optimal placement of casualty evacuation assets was designed.Conclusion: This simulative model may be used as a basis for further development of a computer-assisted program and as a reference for medical administration commanders at different levels.
10.Microendoscopic discectomy in the treatment of lumbar spinal stenosis in 44 elderly patients

Objective To investigate the value of posterior approach microendoscopic discectomy (MED) in the treatment of lumbar spinal stenosis. Methods In the light of preoperative localization by anterioposterior and lateral X-ray examinations of labeled lumbar vertebrae, the MED system was established under local anesthesia or epidural blockage. Part of lamina and hyperplastic and cohesive articular processes were removed by using a self-made osteotome to fully decompress the dura mater and nerve roots. Results The operation time ranged 25~180 min, with an intraoperative blood loss of 50~300 ml. A conversion to open surgery was required in 1 case because of widespread adhesions of the dura mater and nerve roots. All the 44 patients were followed for 3~40 months (mean, 15 months). According to Nakai criteria, 30 patients had an "excellent" result, 8 patients had a "good" result and 6 were classified as "poor", the rate of "good" or "excellent" results being 86.4% (38/44). Postoperative infection of intervertebral space and symptoms on healthy limbs were observed in 1 case and 2 cases, respectively. Conclusions MED has the advantages of minimal invasion, little blood loss and rapid recovery. It can offer both full decompression to nerves and the stability of spine to the greatest possible advantage, being an effective means in the treatment of lumbar spinal stenosis in elderly patients.
