1.Analysis of diagnosis and treatment of acute mesenteric vascular obliteration

Objective To analyze the clinical manifestation, diagnosis and treatment of acute mesenteric vascular obliteration (AMVO). Methods 112 cases of AMVO were analyzed, including 20 cases admitted to our hospital from 1992 to 2004 and 92 cases reported by other hospitals. Results Abdominal pain, abdominal distension, nausea and vomiting, decrease or disappearance of bowel sound, elevation of white blood cell count were more frequent manifestations.Cardiovascular disease, portal vein stasis or hypercoagulation state were common findings in patients' history. The average misdiagnosis rate of AMVO in these 112 patients was 61.5%, with a misdiagnosis rate of 45.0% in 20 cases of our hospital. The average mortality rate of all these patients was 41.9%, with a mortality rate of 30.0% for 20 cases of our hospital. Conclusion The early clinical manifestation of AMVO lacks specificity, the misdiagnosis ratio and mortality rate are very high. Angiography should be done early to confirm the diagnosis and improve the therapeutic effect in the patients suspicions of having this ailment.
2.Side effects of inhalation of hyperosmotic saline on sputum induction in asthmatic patients

Conclusion Sputum induction after inhalation of short acting ? 2 agonist is safe for asthmatic patients in the remission stage.
3.Construction of eukaryotic expression vector of Der p2 gene and its expression in mouse dendritic cells

BACKGROUND: Dendritic cells, the most potent antigen presenting cells known at present, have been extensively used in the immunotherapy as adjuvant. OBJECTIVE: The present study was to construct Der p 2 eukaryotic expression vector and validate its expression in the mouse bone marrow-derived dendritic cells. DESIGN, TIME AND SETTING: A single sample observation was performed at the Institute of Respiratory Disease,Xinqiao Hospital, Third Military Medical University of Chinese PLA between May and December 2005. MATERIALS: C57BL/6 mice were included. Plambd-Der p 2 was the product of Heska Company, USA.pCI-neo plasmid was provided by the Institute of Respiratory Disease, Xinqiao Hospital, Third Military Medical University of Chinese PLA.METHODS: Mouse bone marrow-derived dendritic cells were in vitro isolated and cultured.Complete Der p 2 cDNA was spliced from prokaryotic expression vector plambd-Der p 2, and then cloned into eukaryotic expression vector pCI-neo (pCI-neo-Der p 2).The positive recombinants pCl-neo-Der p 2 transfected into dendritic cells.Non-transfected and blank vector pCI-neo-transfected dendritic cells were used as controls. MAIN OUTCOME MEASURES: ①Identification of pCI-neo-Der p 2 recombinant plasmid.②Detection of Der p 2 mRNA and protein expression by reverse transcription-polymerase chain reaction (RT-PCR) and Western Blot techniques. RESULTS: Sequencing results showed Der p 2 cDNA in pCI-neo-Der p 2 was in coincidence with the sequence registrated in Gene Bank.RT-PCR and Western Blot results showed that expression of Der p 2 mRNA and protein could be detectable in the pCI-neo-Der p 2-transfected dendritic cells. CONCLUSION: The Der p 2 cDNA was successfully constructed into the eukaryotic expression vector, and Der p 2 gene and protein could be expressed efficiently in dendritic cells.
4.Effects of neuropeptide Y gene transfection on seizure in rat and the expression of phosphorylated Tau protein in the rat hippocampus

Objective To discuss the effect of gene transfection of rAAV2/1-NPY-EGFP on KA-induced rat seizures,EEG and the expression of hippocampal phosphorylated Tau protein.Methods Altogether 72 healthy male Wistar adult rats were randomly divided into three groups:control group,KA group and NPY group(n=24).The epileptic models were established by the injection of KA 2 μl (0.4 μg/μl) five times to the right side of the hippocampus CA3 area every three days.rAAV2/1-NPY-EGFP group,in which 10 μl of rAAV2/1-NPY-EGFP (titer 5× 1011 v.g./ml) was injected to the lateral ventricle in successful rats chronic model,while KA group was injected with an equal dose of saline.The control group was injected with an equal dose of saline both in the hippocampal CA3 area and the lateral ventricle.The seizure situation,the onset latency and EEG were observed at 2 weeks and 4 weeks after vector injection.Then the expression of phosphorylated Tau protein in hippocampus were detected with Western blotting.Results (1) Scale and latency of each seizure onset in rats of rAAV2/1-NPY-EGFP group (12.13 ± 8.06) had no significant difference at 2 weeks (P> 0.05) compared with KA group (12.10± 8.07).The scale of seizure in rats of rAAV2/1-NPY-EGFP group(6.06±3.78) significantly reduced at 4 weeks(P <0.05).Latency of seizure onset (79.06±8.83min) significantly increased at 4 weeks(P<0.05),EEG epileptic discharge frequency and wave amplitude decreased (P< 0.05) at 4 weeks.The control group had no seizures.(2)Compared with the control group,the expression of phosphorylated Tau protein in KA group and NPY group significantly increased(P<0.05) at 2 weeks and 4 weeks,and the expression of phosphorylated Tau protein in the NPY group (1.15±0.16 RQ value) at 4 weeks significantly decreased (P<0.05) compared with the KA group(1.87± 0.23 RQ value).Conclusion rAAV2/1-NPY-EGFP gene transfection significantly reduces scale of seizure onset and prolongs latency of seizure onset in KA-induced rat model.rAAV2/1-NPY-EGFP gene transfection may play anti-epileptic and neuroprotective effects through inhibiting the expression of phosphorylated Tau protein in hippocampus of KA-induced epileptic rat model.
5.Study on genotypes and phenotypes of metallo-β-lactamase in multidrug-resistant Pseudomonas aeruginosa

Objective To study the generation of metallo-β-lactamase(MBLs) and its related gene carrying situation in the clini-cal isolates of Pseudomonas aeruginosa .Methods Ceftazidime and imipenem were adopted to preliminarily screen MBLs of Pseudo-monas aeruginosa .The phenotypic confirmatory of imipenem-resistant and ceftazidime-resistant Pseudomonas aeruginosa was per-formed by using 2-mercaptopropionic acid (2-MPA) or EDTA synergy test and the MBLs genotypes of the positive strains in the preliminary screen were detected by PCR .Results The positive rate of the MBLs preliminary screen test in multi-resistant strains was 10 .9% ,and the positive rate of the MBLs in multi-resistant strains detected by CAZ/EDTA ,CAZ/2-MPA ,IMP/EDTA and IMP/2-MPA was 7 .5% ,7 .9% ,8 .8% and 9 .5% respectively .The positive rates of ipm1 and vim gene by PCR were 10 .4% and 8 .3% respectively .The strains with positive spm ,sim1 and gim were not found .Conclusion The MBLs test results detected by different methods are different ;MBLs genes carying ipm1 and vim are the main reason for carbapenem-resistant multi-drug resist-ant Pseudomonas aeruginosa in the hospital .
6. The clinical value of dexmedetomidine during mechanical ventilation in icu patients of different ages

7. Microwave ablation improves the process and outcome of core needle biopsy in thyroid nodules

Objective To explore the effect of microwave ablation on the specimen quality, risk of bleeding and histopathological outcomes of core needle biopsy in thyroid nodules. Methods Core needle biopsy were performed on 251 cases of thyroid solitary nodules before and after microwave ablation, and then the specimen quality, hemorrhage of thyroid nodules after biopsy, legibility of pathological slices and the consistency of pathological diagnosis were compared. Results The specimen satisfaction rate of thyroid nodules after microwave ablation was significantly improved compared with that before microwave ablation (100% [251/251] vs 92. 03% [231/251], P=0. 000), and the incidence of bleeding after microwave ablation was significantly decreased versus that before microwave ablation (0% vs 15. 14% [38/251]; P = 0. 000). And 184 of 251 thyroid nodules were benign and 67 were papillary thyroid carcinoma. In addition to 4 thyroid nodules which could not be sliceddue to poor quality before microwave ablation, the overall consistency of pathological diagnosis in the remaining nodules before and after microwave ablation was 100%. Conclusion Core needle biopsy immediately after microwave ablation does not affect the diagnostic accuracy of thyroid nodules, and can reduce the risk of bleeding and improve the specimen quality.
8.Regulation of Fengxiaofang on Expression of Gene Gob-5 in Asthmatic Mice

Objective To observe the regulation of Fengxiaofang on the expression of gene Gob-5 in ovalbumin-challenged asthmatic mice. Methods The murine model of allergic asthma established by ovaibumin was used. The model mice were treated experimentally with Fengxiaofang and Dexamethasone as a positive control. The quantitive expression of gene Gob-5 in lung of mice was detected by real-time quantitative PCR. Results The expression of gene Gob-5 in lung of mice was down-regulated by Fengxiaofang. In the experiment, there were no significant differences between the one-fold or ten-fold dosage groups and the normal control group. Gene Gob-5 expression in five-fold dosage was significantly lower than that in the normal control group. Conclusion Fengxiaofang has the effect of lowering the expression of gene Gob-5 which maybe related to the potency on asthmatic disease.
9.A retrospective and comparative study of familial and non-familial bullous lichen planus.

In order to compare the clinical characteristics of familial and non-familial bullous lichen planus (BLP), the archival data of 36 BLP patients with positive family history and 21 BLP patients with negative family history diagnosed according to the clinical features and histopathology were collected in our department from 1956 to 2003. The clinical features were analyzed and compared. There were significant differences between familial and non-familial BLP in age of onset, duration of disease and extension of eruption (P<0.01). It was concluded that familial BLP appeared to differ from the non-familial form in its earlier age of onset, longer duration of the disease, more extensive eruption and more tendency to involve nails. Hereditary factors may play a role in the pathogenesis of familial BLP.
10. Effect of thermal ablation on HSP70 expression in benign thyroid nodules

Objective: To explore the changes of HSP70 expression after radiofrequency ablation of benign thyroid nodules and to discuss the potential relevance. Methods: Ultrasound-guided radiofrequency ablation (RFA) was conducted for treating 50 solid thyroid nodules in 50 patients. Three different regions of interest (ROI), namely the central region, marginal region and transitional region, were set on each nodule for quantitative investigation of HSP70 expression by using immunohistochemistry method. Each ROI experienced histological sampling by using core-needle biopsy before and after RFA. HSP70 expression was compared for the same ROI before and after ablation and between different ROIs after ablation. Results: HSP70 expression had no significant changes in the central and marginal regions before and after ablation in the 50 solid thyroid nodules, and the expression was significantly enhanced in the transitional region after ablation (P<0.05). After ablation the expression of HSP70 in the transitional region was significantly higher than those in the central and marginal regions (P<0.05), with no significant difference found between the latter two regions. Conclusion: Increased expression of HSP70 in the transitional region might protect normal glandular tissue against heat damage during thermal ablation treatment of benign thyroid nodules.
