1.Comparison of efficacy between Glipizide GITS and Gliclazide in the treatment of Chinese patients with type 2 diabetes mellitus

Objective To compare the clinical efficacy of once daily regimen with Glipizide GITS (Glucotrol XL) and multiple dose regimen with Gliclazide (Diamicron) in the treatment of Chinese patients with type 2 diabetes mellitus. Methods One hundred and fifty one Chinese type 2 diabetic patients meeting the 1985 WHO diagnostic criteria were recruited from 4 clinical centers. The patients were assigned to Glipizide GITS (78 cases) or Gliclazide (73 cases) treatment group. Results The trial consisted of 1 week washout period followed by 6 week dosage regulation period and 6 week dosage maintenance period. Comparing to the baseline, HbA 1c , fasting plasma glucose (FPG) and 24 hour glucose profile levels in two groups were significantly reduced after 3 month treatment. In Glipizide GITS group, the mean reductions of blood HbA 1c and FPG level were 0.55% and 2.05 mmol/L respectively (P

Objective To analyse the lab diagnosis guideline in the patients with PM and DM,and guide the clinical diagnosis. Methods Stage the controlling condition and examine the serologic zymogram,electromyography and pathology.Results In the and Hospital of Hebei medical Vnirersity,from Jan.2003 to Jan.2005 CK,CK-MB,AST,LDH and HBDH was higher in 82.9%patients of aetive stage than the stable stage,especially changed obviously in CK.The occurance ritio(75.9%,48.3%)of the inside electricity and multiphase wave were much higher than the stable stage(46.7%,20.0%).Lots of organic fibre shrank in PM,so did the inflammation of lymohocyte soakage.In DM,the fascicle began to shrink in different degrees.Conclusion CK could become the guideline to watch the activity of illness.The number of the electromyography inside electricity and multiphase wave are related to the illness stage of patients with PM and DM.In PM pathology,muscle fibre had some infected cell soakage,with the differential recessive change.The pathology character in DM was the atrophy around the facicle and the change of tiny blood vessel.
3.The prevalence of vascular lesions in the lower extremities and their risk factors in type 2 diabetic mellitus

Objective To investigate the prevalence of vascular disease in the lower extremities and its related risk factors in type 2 diabetic patients.Methods A total of 393 type 2 diabetic patients treated in 5 hospitals in Beijing were randomly recruited for the study with their age of more than 40 years at onset,diabetes duration of more than 5 years and treatment for more than 1 year in each hospital.The case history,physical examination,biochemical tests and ultrasonic Doppler examination were conducted in these patients.Results Eight patients had gangrene (2.0%) and 28(7.1%) had intermittent claudication in 393 diabetics,while ultrasonic examination showed that 357 cases (90.8%) had vascular lesions in the lower limbs.Ankle/brachial systolic pressure ratio (API,Ankle pressure index) less than 1.0 was found in 68 cases (17.8%).The higher prevalence of diabetic nephropathy and retinopathy,higher fasting blood sugar and HbAlc,higher diastolic pressure and lower HDL ch levels ( P
4.T-lymphocyte subsets in patients with Guillain-Barré syndrome change after intravenous immunoglobulin treatment and its significance

Objective To study the changes of the T-lymphocyte subsets in patients with Guillain-Barré syndrome(GBS)before and after intravenous immunoglobulin treatment(IVIG),and to explore the possible mechanism of the IVIG curing GBS further.Methods Chose 31 cases of clinically confirmed GBS were enrolled and compared before and after the treatment.According to the effect of the therapy,31 cases of the total were sub-divided into effective and ineffective groups.Relative counting of peripheral blood T-lymphocyte subsets was preformed with flow cytometry.Results ①The percentage of CD8+ T and CD4+CD29+ T cell was significantly lower(CD8+T:28.77%±11.02% vs 31.84%±12.35%,CD4+CD29+T:56.71%±12.44% vs 62.40%±12.72%,t=2.995,3.919,P<0.05)after therapy,while the rate of CD4+/CD8+T and the percentage of CD4+CD45RA+T cell increased notably(t=2.368,3.860,P<0.05);but there was no notable difference in the percentage of CD3+T and CD4+T cell.②The percentage of CD8+T and CD4+CD29+ T cell was significantly lower(t=2.144,3.343,P<0.05)after the treatment,while the rate of CD4+/CD8+T and the percentage of CD4+CD45RA+T cell increased notably(t=2.159,3.277,P<0.05)in the good curative effect group,but there was no change in the bad curative effect group.③61.29%(19/31)of the patients significantly improved by IVIG,and there was no death case.Conclusions T-lymphocyte subsets change in a varing degree after IVIG treatment in acute GBS patients,which lays an immunological foundation for the further study of pathogenesis and mechanism of IVIG curing GBS;effective on GBS,IVIG can actively suppress pathogenetic condition and promote the recovery of nrevous function.
5.Association between serum chemerin and type 2 diabetic nephropathy

Objective To investigate the relationship between plasma Chemerin and diabetic nephropathy of type 2 diabetes mellitus (T2DM).Methods Totally 195 T2DM patients and 65 healthy controls were enrolled in this study.The T2DM patients were further divivded into three subgroups:normal albuminuria group (n =65),microalbuminuria group (n =65),and clinical albuminuria group (n =65).The plasma Chemerin level was determined by enzyme-linked immunosorbent assay (ELISA).Results The serum Chemerin level was (150.20 ±21.99) μg/L in diabetic patients,which was significantly higher than that in healthy controls (62.13 ± 18.90) μg/L) (P <0.01).Inside the T2DM group,the plasma Chemerin level was [(143.63 ± 22.33) μg/L] in the microalbuminuria group and [(173.21 ± 23.91) μg/L] in the clinical albuminuria group (P < 0.001),and both were significantly higher than that in the normal albuminuria group [(100.35 ±20.11) μg/L] (both P <0.01).Multivariate regression analysis showed that the plasma Chemerin level was independently associated with glycosylated hemoglobin (β =0.216,P =0.038),total cholesterol (β =1.867,P =0.048),high-sensitivity c-reactive protein (β =12.330,P < 0.001),and urine albumin (β =37.184,P <0.001) in T2DM patients.Conclusion Plasema Chemerin level increases in T2DM patients and therefore can be a reliable indicator for detecting early type 2 diabetic nephropathy.
6.Relation between dampness-heat syndrome of acute period of GBS and neuroendocrine-immunoregulatory network

Objective: To discuss the relation between dampness-heat syndrome of GBS and neuroendocrine-immunoregulatory network.Methods: 34 patients with GBS were divided into dampness-heat syndrome group and nondampness-heat syndrome group by syndromes differentiation of TCM.The autonomic nerve function,the level of immunoglobulin,cortisol(COR) and adrenocorticotropin(ACTH) of these two groups were detected.The datas were compared with that of normal control group.Results:① There were significant difference in function of autonomic nerve between the two groups(damp-heat syndrome group and nondamp-heat syndrome group) and normal control group.There is difference between dampness-heat syndrome group and nondampness-heat syndrome group,too.Dampness-heat syndrome group mainly showed sympathetic nerve excitement(60.0%).In nondampness-heat syndrome group,the incidence of sympathetic nerve excitement is 35.7%;parasympathetic nerve excitement is 28.6%.② In dampness-heat syndrome group the level of IgG was higher than that in nondampness-heat syndrome group and normal control group.The level of IgA was higher than that in normal control group(P
7.A comparison of efficacy and tolerance of nateglinide and acarbose monotherapy in type 2 diabetes mellitns

Objective To compare the efficacy and tolerability of nateglinide with those of acarbose in Chinese type 2 diabetes mellitus (T2DM) patients.Methods This multi-center,randomized,double-blind,parallel-arm study compared the efficacy and tolerability of nateglinide( 120 mg,3/d,n = 119) and those of acarbose( 100 mg,3/d,n = 118) during a 12-week treatment in T2DM patients uncontrolled by diet with glycosylated haemoglobin (HbA1c) 6.5% - 11.0% .Results Monotherapy with nateglinide (120 mg,3/d)or acarbose (100 mg,3/d)decreased HbA1c to a similar extent during 12-week treatment.The mean change from baseline to end-point in HbAlc was ( -0.90±0.98)% and ( -0.83±0.81 )% in patients receiving nateglinide and acarbose,respectively,with no significant difference between the two groups (P>0.05).The decrease in fasting plasma glucose (FPG)was similar between nateglinide and acarbose (P > 0.05).The mean change in 2-hour postprandial plasma glucose ( PG2h ) was ( - 1.45 ± 2.74) mmol/L and ( -2.20±2.21 ) mmol/L in patients receiving nateglinide and acarbose(P =0.0017).Body weight was significantly decreased in both groups at the end-point ( P < 0.05 ),although the decrease was more with acarbese than nateglinide [( -0.66±1.79)kg vs (-2.06±2.00) kg,P=0.0000].And the proportion of patients experiencing any presumed drug related adverse events was not significantly different between the two groups.Conclusions Nateglinide ( 120 mg,3/d) is effective and well tolerated in T2DM patients uncontrolled by diet,demonstrating similar HbA1c reductions as compared with acarbose (100 mg,3/d).

A descriptive study to investigate the situation of cross-cultural adaptation and health status among medical international students was carried out using cluster sampling method and self-designed questionnaire in Dalian Medical University.148 international students come from 24 countries.There are 5 religions belief among these students.Islam,Catholic,Hindu,Buddhism and Sikhism.The main obstacles of cross-cultural adaptation are language,accommodation and diet.Economic status and stress of interpersonal relationship are also the main causes affecting the physical and mental health of medical international students.
9.Study on physical training for the prevention of chicken′s tendon adhesion

Objective To discover the best physical training way including frequency and intensity each day ,and to guide pa‐tients to get the best cure .Methods Totally 60 chicken ,which were randomly divided into 3 groups ,20 for each ,the deep flexor tendon of the third left toes were found ,repairing after cut them off .Flexor tendon proximal were found by making knees incision then indwelling long silk line .External fixation bandage were used .Their left toes were accepted physical training ,Training stand‐ards :6 pulling backs each time ,toe flexion 1/4 arc ,but less than 2 N .For group A ,exercised once a day ,twice a day for group B , three times a day for group C .Right ones were consider as control group weren′t processed after operation .28 days later ,tendon distance was recorded by using 2 N pulling forces at tendon proximal with aspiring balance .Gross specimen was observed and histo‐logical specimens using Tang Jinen classification method to classify adhesions ,and the dates were analyzed by statistics SPSS 19 .0 . Results Group A remaining of 18 ,rate of 5 .56% ,grade Ⅰ of 1 ,Ⅱ of 2 ,Ⅲ of 4 ;Ⅳ of 10 .Group B remaining of 17 ,rate of 5 .88% , grade Ⅰ of 12 ,Ⅱ of 2 ,Ⅲ of 1 ,Ⅳ of 1 .group C remaining of 16 ,15 toes broke again ,fracture rate was 93 .75 % ,grade Ⅰ of 1 ,con‐trol group were grade Ⅳ .Between group A and B there were no significant differences in degree of adhesion(P> 0 .05) .Group A and Cs′ differences were statistically tested (P< 0 .05) .So were group B and C (P< 0 .05) .The average sliding distance for group A was (3 .01 ± 1 .58) mm ,(6 .72 ± 2 .02) mm for group B ,group C only got one sample ,8 .60mm ,and it was out of statistics .Be‐tween group A and B ,difference resulting from its tendon sliding after statistical tests(P< 0 .05) .For fracture rate ,Group A and B no significant difference was found(α′ = 0 .012 5 ,PAB > α′) .Conclusion Physical training can reduce chicken′s tendon adhesion effectively .
10.Peripheral nerve repair:theory and technology application

BACKGROUND:Recovery of motor and sensory function from peripheral nerve injury is relatively slow and incomplete. It is a difficult problem for orthopedic surgeons that mainly leads to the decline in the quality of life in patients.
OBJECTIVE: To conclude the methods and corresponding outcomes in peripheral nerve regeneration by analyzing the new treatment means for peripheral nerve injury.
METHODS:PubMed, Wanfang, CNKI databases were retrieved for relevant articles using key words of “nerve injury, regeneration”, and then retrieval data were sorted and analyzed.
RESULTS AND CONCLUSION:In recent years, in-depth studies on peripheral nerve repair have been made in the folowing aspects: surgical mode, drug, cytokine, gene transfer and biomaterials as wel as traditional Chinese medicine. If the detect size is four times longer than the diameter of nerves, the nerve regeneration chamber can achieve good outcomes. The methods of restoring nerve continuity folowing nerve injury are developed from surgical anastomosis to photochemohistological method, thermal laser welding, plastic repair and other emerging technologies. Studies have found that plasminogen activator, nerve growth factor, neurotrophic factor, recombinant erythropoietin, human tissue kalikrein, B vitamins and their derivatives, herbal preparations, immunosuppressive agents al can promote nerve regeneration.
