Objective: To investigate the effect of early treatment with SSRIs/Citalopram on cognitive impairment in patients with stroke.Methods: A total of forty patients with cognitive impairment following stroke,in the Department of Neurology,Southwest Hospital of the Third Military Medical University,selected by the Mini-Mental State Examination,were randomly divided into treatment group and control group.All the patients received routine treatment,besides Citalopram were given to those in the treatment group.20mg Citalopram Hydrobromide Tablets would be given after breakfast everyday for 3 mouths?Patients in both groups were investigated with the Mini-Mental State Examination,Hamilton Rating Scale for Depression,National Institute of Health Stroke Scale and Barthel Index to examine the functions before and 1 and 3months after treatment respectively.Result: The improvement of cognitive function of patients in the treatment group was obviously superior to those in the control group(19.35?2.85,21.55?3.24,P
2.The Clinical Value of Measurement of Normal Fetal Aortic Isthmus Systolic Index with Prenatal Ultrasound

Objective To obtain the fatal aortic isthmic systolic index(ISI)with prenatal ultrasound,and to establish reference ranges for normal live fatal aortic isthmic systolic index(ISI). Methods A total of 251 normal fetuses at 18?37 gestation weeks were collected and underwent Dop?pler ultrasound examination to obtain the fatal aortic isthmus waveforms. Peak systolic velocity(PSV)and systolic nadir(NS)were measured on the waveforms. ISI was derived from the ratio NS/PSV,and its relationship with gestational weeks was analyzed. Results ISI value was between 0.19 and 0.21,from 18 to 24 gestational weeks. As the gestational weeks increasing,PSV increased;however,NS decreased gradually. At about 28 weeks,a brief end?systolic deceleration wave was observed on the aortic isthmus waveforms,and this trend led to ISI towards a mean value of zero at about 30 weeks. At the end of pregnancy,ISI was about-0.33. The curve equations were established regarding the gestational age as the independent variable X,PSV,NS and ISI as the dependent variable Y respectively. Y∧=-69.116+9.542X-0.139X2,R2=0.96,P<0.001;Y∧=-99.881+10.331X-0.232X2,R2=0.99,P<0.001;Y∧=-0.671+0.086X-0.02X2,R2=0.99,P<0.001. Conclusion Prenatal ultrasound can measure the fetal aortic isthmic systolic index,which is helpful in the diagnosis of fetal cardiovascular anomalies and evaluation of fetal intrauterine status.
3.Repair of Tissue Defects of Tendon and Skin of Back of Hand by Femoral Anterolateral Free Flap with Iliotibial Tract

Objective To observe the clinical outcome of repair of the tissue defects of the tendon and skin with thigh anterolateral free flap with iliotibial tract. Methods The data was reviewed from June 2006 to June 2010, which the thigh anterolateral free flap with iliotibial tract were used to repair the defects of the tendon and skin of back of hand in 12 patients. The skin flap was 7 cm´10 cm~8 cm´12 cm in area. The iliotibial tract was 7~13 cm in length and 8~10 cm in width. Institute of Hand Surgery of Chinese Medical Association was to assess the effect. The fellow-up of the 12 patients was 6 months~4 years. Results All the flaps survived after the operation. The result showed excellent in 3 patients, good in 6 and fair in 3. Conclusion It is an effective surgical method to repair the tissue defects of the tendon and skin with thigh anterolateral free flap with iliotibial tract.
4.The repair of porta hepatic duct stricture by pedicled autograft

Objective To evaluate the repair of benign constriction of the porta hepatic duct by pedicled autograft. Methods The clinical data of repair by pedicled valve of gallbladder in 38 cases, by pedicled gastric wall in 2 cases and by round ligament of the liver in 2 cases were retrospectively analyzed. Results There were no operative mortality and no complications such as bile leaks and hemorrhage. Cholangiography through T-tube showed patency in all postoperative patients. A follow-up from 6 months to 14 years in 40 out of 42 cases found cholangitis in 2 patients with good response to conservative therapy. Conclusions Treatment of benign stricture of porta hepatic duct by pedicled autograft is physiologic, convenient and effective.
5.The prognostic value of white blood cell and hemoglobin in patients with acute aortic dissection

Objective To evaluate the prognostic value of white blood cell and hemoglobin in patients with acute aortic dissection(AAD).Methods White blood cell.hemoglobin and fibrinogen were measured in 42 AAD patients.The mortality during hospitalization was observed and the short-term prognosis in AAD patients was as-seased.Results There was 13 death during hospitalization.with the mortality of 30.95%.The value of white blood cell was higher in death group than in survival group[(13.73±6.91)×109>/L vs.(9.43±4.97)×109>/L.P< 0.05).The value of hemoglobin was lower in death group of than in survival group[(118.54±22.38)g/L vs. (131.72±18.17) g/L,P<0.05].There were no differences in the value of fibrinogen between the groups [(3.15±1.15)g/L vs.(3.48±1.24)g/L,P>0.05).The mortality in the group of elevated white blood cell(≥10.0×109>/L)was higher than that in the group of normal white blood cell(10.0×109/L)(41.18%vs.24%.P<0.05).The mortality in the group of decreased hemoglobin(≤110g/L)was higher than that in the group of nor-mal hemoglobin(>110 g/L)(50.00%vs.27.78%,P<0.05).Conclusion The value of white blood cell and he-moglobin could help to assess the short-term prognosis of patients with AAD.
6.Assessment of shape and function of right ventricle in heart transplantation patients by single beat real-time three-dimensional echocardiography

Objective To investigate the shape and function of right ventricles in patients received heart transplantation(HT). Methods Sixty healthy volunteers(control group) and 31 HT patients(HT group) were enrolled, and the HT group was further divided into the nonrejected(HTn) group and the rejected(HTp) group based on the endomyocardial biopsy(EMB) results. All the participants received routine echocardiography. Single beat real-time three-dimensional echocardiography (sRT-3DE) was performed in all to evaluate parameters concerning modality and systolic function of participants' right ventricles. Results 1) Right ventricular stroke volume (RVSV) and right ventricular ejection fraction (RVEF) were significantly different among the groups[RVSV: control group vs HTn group vs HTp group was (56.18 ± 23.72)ml vs (36.08 ± 10.94)ml vs (26.22 ± 9.84)ml, P <0.0001; RVEF:control group vs HTn group vs HTp group was (62.09± 7.18)% vs (51.04 ± 7.58) vs (35.86 ± 9.86)%, P <0.00001=.2= When taking the influence of rejection into consideration,RVEF was proved to be a stable and sensitive indicator. Conclusions sRT-3DE can quickly assess shape and systolic function of right ventricle.RVEF was the most stableand sensitive among all the RV-related indicators and is a promising indicator in the clinic follow-up of HT patients.
7.Relationship of vascular endothelial function and atherosclerosis in elderly patients with coronary artery disease

Objective To investigate the endothelial function and atherosclerosis in patients with coronary artery disease (CAD).Methods Two-dimensional echocardiography (2-DE) was used tomeasure changes in brachial artery inner diameter as parameter of endothelium-dependant and endothelium-nondependant vascular diastolic function and pathologic characters of carotid atherosclerosis in 90 elderly CAD patients and 30 controls.Results Endothelium-dependant vascular diastolic function and endothelium-non dependant diastolic function were significantly decreased in CAD group as compared with control group [(9.08±2.28)% and (6.14±2.21)% vs.(15.58±2.20)%, P<0.01].The carotid intima-media thickness and plaque score were significantly increased in CAD group as compared with control group.Conclusions There are correlations of endothelium dysfunction and carotid atherosclerosis are relatied to with coronary atherosclerosis.

9.A survey on the current status of type 2 diabetic patients who failed to achieve the glycemic control target

Objective To investigate the current status of type 2 diabetic patients who failed to achieve the glycemic control target, and provide theoretic evidences for making corresponding strategies. Methods The 2 diabetic patients who failed to reach the glycemic target were recruited from 181 hospitals in 26 cities and received a standard questionnaire, the conditions of their blood glucose level, lifestyle intervention, blood sugar monitoring, and drug therapy were recorded. Totally 3 861 questionnaires with complete information were collected. And the causes which account for glycemic control status were analyzed. Results Among these patients, the mean HbA1c was 7.9%, the mean fasting plasma glucose was 8.2 mmol/L, and the mean postprandial plasma glucose was 11.5 mmol/L. Only 25.6% of patients take their diet control strictly as prescribed and 44. 5% of patients have little exercise. 35. 8% and 47.8% of patients did not monitor their fasting and postprandial plasma glucose,respectively. Glycemic control in the patients aged > 60 years was similar to the younger patients, but the hypoglycemia incidence in the elder group reached 35.5%, which was higher than those in the other 2 groups (20.8% and 21.4%, both P<0. 05 ). The proportion of patients with mono-therapy and combination therapy was 46. 1% and 51.7%, while the proportion with combination therapy rose in the patients aged >60 years (58.7%;Compared with the other age-groups, all P<0.05 ). 75 % of patients have adjusted their drug administration regimen since initial treatment. Conclusions Inadequate or inappropriate drug therapy regimen is a major cause responsible for this poor glycemic control status. In addition, the unhealthy life styles, insufficient blood sugar monitoring, and poor compliance were also important causes. Thus, for these patients, it is necessary to further enhance patients' education, to improve life style intervention, as well as to select more effective, safer, and compliant drug therapy regimens. Finally, the glycemic control target for the elder patients should be more flexible.
10.Correlation between physical status of human papilloma virus and cervical carcinogenesis.

The prevalence of human papilloma virus (HPV)-16 in patients with cervical cancer, the physical status of HPV-16 in patients with cervical lesions, and the role of HPV-16 integration in cervical carcinogenesis were investigated. HPV genotyping was performed by using PCR approach with the primer GP5+/GP6+ and type-specific primer on biopsy specimens taken operatively from 198 women. Multiple PCR was done to detect physical status of HPV-16 in a series of cervical liquid-based cytology samples and biopsy specimens obtained from different cervical lesions with HPV-16 infection, including 112 specimens with cervical cancer, 151 specimens with CIN I, 246 specimens with CIN and 120 specimens with CINIII. The results showed that there were 112 cervical cancer samples (56.57% of total cervical cancer patients) with HPV-16 infection. The frequency of HPV-16 pure integration was 65.18% (73/112), 56.57% (47/120), 23.58% (58/246) and 7.95% (12/151) in cervical cancer, CINIII, CINII and CINI patients respectively. In situ hybridization was performed on some paraffin-embedded sections of CINII, CINIII and cervical cancer to verify the physical status of HPV-16 infection. Significant difference was observed between cervical cancer and CIN I, CINII, CINIII in the frequency of HPV-16 integration (P<0.01). It is suggested that HPV-16 is the most prevalent type and is associated with cervical cancer. In the case of HPV-16 infection there are close associations between the severity of cervical lesions and the frequency of HPV-16 integration. The application of testing HPV genotyping and physical status based on detection of HC-II HPV DNA would be in favor of predicting the prognosis of cervical precancerosis and enhancing the screening accuracy of cervical cancer.
