1.Effect of triple therapy plus traditional Chinese medicine in peptic ulcer caused by Helicobacter pylori

Objective To discuss the effect of triple therapy plus traditional Chinese medicine in the treatment of peptic ulcer (PU) caused by Helicobacter pylori ( Hp) .Methods 148 cases with PU were randomly divided into study group and control group of 74 cases in each group.The control group were treated with triple therapy (omeprazole +clarithromycin +amoxicillin), and the study group were given liver-discharging and heat-clearing and invigorating spleen and removing blood stasis decoction, additionally.Both groups received treatment for one course of 4 weeks.The changes of clinical symptoms, negative conversion ratio of Hp and adverse reaction were compared.Results The scores of clinical symptoms were all significantly decreased in both two groups after treatment (P<0.05), but the study group changed more than control group (P<0.05).The total effective rate was 95.95%, rather higher than 79.73% in control group (P<0.05).The negative conversion ratio of Hp was 82.43%, obviously higher than 64.86% in control group (P<0.05).The incidence of adverse reaction was 12.16% in study group, similar to 9.46% in control group.Conclusion Triple therapy plus traditional Chinese medicine can significantly improve the clinical symptoms, clinical effect and negative conversion ratio of Hp, and has a good safety and tolerability.
2.Diagnosis and treatment of primary retroperitoneal tumor:a report of 75 cases

Objective To evaluate the diagnosis and treatment of primary retroperitoneal tumor. Methods The clinical data of 75 patients with primary retroperitoneal tumor confirmed by operation and pathology in recent 20 years in our hospital were analyzed retrospectively. Results About 57.3% of the patients were diagnosed accurately before the operation. Correct diagnostic rates of ultrasound and CT were 60.0% and 84.0% ,respectively.Of them 57cases(76.0%) were malignant and18 (24.0%) benign tumor. Tumor size was not related to the pathological characteristics.Radical resection was done in 42 patients(56.0%),partial resection in 23 patients (30.7%),and biopsy in 10 patients(13.3%).Conclusions Ultrasonic and CT are the major diagnostic methods. Early operation is recommended after necessary preparation. Surgical resection is a major procedure for the management of primary retroperitoneal tumor and might provide long surrival time.
3.Value of diagnosis and treatment of Min-probe sonography for submucosal protrusion lesion in gastric

Objective To evaluate the cilincial value of Min-probe sonngraphy(MPS)for submucosal protrusion lesion in gastric.Methods 82 cases with submucosal protrusion lesion in gastric were detected by Min-probe sonography(MPS)and treated according to the diagnosis.Results 82 cases were fibroid,38 stromal tumor,among which 12 cases from muscularis mucosa,26 case from the proper muscle layer,4 malignant stromal tumor,6 lipoma,3 angioma,3 gastric cysts,11 gastric polypi,8 heterotopic pancreas,9 pressures from outside organ.29 cases were accepted EMR and 18 cases accepted surgical operation.Conclusion MPS is more valuable in the diagnosis and the differential diagnosis of submucosal protrusion lesion in gastric and can guide the further therapy.
4.Deep femoral artery third perforating flap for repair tissue defected of arrounding Pilon fracture in I stage

Objective:To investigate the clinical effect of free deep femoral artery third perforating flap repaired soft tissue loss after Pilon fracture surgery in I stage.Methods:Fifteen patients were treated from April, 2013 to January, 2020. Miller AO classification: 8 cases 43-C1, 4 cases 43-C2 and 3 cases 43-C3. All cases were accompanied with severe soft tissue contusion and skin necrosis. After fracture reduction, soft tissue defects, internal fixation exposure and tendon exposure around the wound. Free deep femoral artery third perforating flap (3.5 cm ×15.5 cm to 5.5 cm×12.5 cm) for the repair of soft tissue defects around ankle in the I stage, the blood vessels of the flap were end-to-side anastomosed with vessels of the posterior tibial or anterior tibial. Regular follow-up after surgery.Results:One case of venous crisis occurred, other 14 cases survived, were followed-up from 5 to 18 months, the ankle joint function was good, did not affect the foot shoes, with excellent color and texture, the flap restored protective sensation, and leaving only linear scar, no muscle adhesion.Conclusion:Free deep femoral artery third perforating flap repaired soft tissue loss of surgical incision after fracture operated than significantly reduce the postoperative fracture infection and protect the blood supply around the fracture. It is an effective method of repair.
