Journal of Medical Research 2006;0(06):-
Objective To study on effects of Alprostadil on 24-hour urinary albumin(U-Alb)in patients with diabetic nephropathy.Methods:38 patients with diabetic nephropathy were included in the study. The test group involving 20 patients were grouped into group A(pristine diabetic nephropathy) and group B(clinical diabetic nephropathy)randomly according to urinary albumin. The control group including 18 patients were grouped into two groups according to the same way.The subjects in both groups have the same therapy in terms of control of blood glucose, blood pressure, plasma lipids, and other therapy methods.Besides, the test group was injected Alprostadil(凯时)10ug from vein, one time a day,14 day continuously, but Alprostadilas well. Results After the treatment of two groups, the level of the urinary albumin decreased obviously compared with the index before treathent, and decreased significantly compared with normal control group. Conclusion Alprostadil can lower urinary albumin with diabetic nephropathy.

Objective To explore the change of serum sIL-2R and IL-6 levels in patients with Kawasaki disease (KD) and its role in the pathogenesis. Methods The serum slL-2R and IL-6 were detected by sandwich ELISA. The serum hs-CRP level was measured by DADE BEHRING BN ProSpec. Results The serum levels of sIL-2R and hs-CRP in KD before and after high-dose intravenous gama globulin(IVIg) were significantly higher than in healthy controls (P

Objective To investigate the clinical effects of gliclazide sustained-release tablet for recently diag-nosed type 2 diabetes mellitus. Methods 118 subjects with recently diagnosed type 2 diabetes and normal figure were divided into three groups. The three group subjects used gliclazide sustained-release tablet, repaglinide(import-ed) ,novolin 30R for 12 weeks, respectively. For all pretherapy and post-treatment cases, blood glucose and glycosy-lated hemoglobin were tested before meal. After two hours of meal, blood glucose was tested again. Meanwhile, hypo-glycaemia event was inspected. The test results were analyzed by "mean ± standard deviation" method. Results There are no significant difference(P>0.05) in fasting blood sugar and postprandial blood sugar(2h) level between the group using gliclazide sustained-release tablet and the group using novolin 30R. The clinical effects of both gli-clazide sustained-release tablet and novolin 30R to fasting blood sugar are all better than that of repaglinide(import-ed) (P < 0.01). But there are no significant difference in postprandial blood sugar(2h) level between the group of us-ing gliclazide sustained-release tablet and the following two group: repaglinide(imported) and novolin 30R. Conclu-sion The investigation results show that gliclazide sustained-release tablet is better than repaglinide(imported) in fasting blood sugar. But for the effects of postprandial blood sugar(2h), there is no significant difference between gli-clazide sustained-release tablet, repaglinide(imported) and novolin 30R.

Chinese Journal of Primary Medicine and Pharmacy 2008;15(4):562-563
Objective To investigate the effect of hyperurieemia on cardiovascular and cerebrovaseular complications in type 2 diabetes mellitus(T2DM). Methods According to the level of blood uric acid,430 patients with T2DM were divided into hyperuricemia group and non-hyperurieemia group, the differences of uric acid(UA) ,creatine(Cr) ,total cholesterol(TC) ,triglyeride(TG) ,fasting blood glucose(FBG) ,glycosylated hemoglobin(HbA1c) and body mass index(BMI) were compared, and the occurrences of coronary heart disease, hypertension and acute cerebrovascular accidence were observed. Results The FBG and HbAIc were not significantly different between the two groups(P>0.05). The BMI ,TG,TC and Cr in the hyperurieemia group were significantly higher than those in the non-hyperuricemia group(P<0.05 ). The incidence rates of coronary heart disease, hypertension and acute cerebrovascular diseases in the hyperuricemia group were also significantly higher than those in the non-hyperuricemia group (P<0.05 ). Conclusion Hyperuricemia is obviously correlated with obesity and blood lipid abnormalities. Hyperurieemia can aggravate the metabolic disturbances, and can accelerate the occurrence and development of cardiovascular and eerebrovascular diseases in the patients with T2DM.

Objective To determine the changing trend and causes of perinatal mortality in Changning District after the implementation of the universal two-child policy, and then explore effective interventions for preventing perinatal mortality. Methods Data of perinatal mortality in Changning District from 2011 to 2020 were retrospectively collected. Change of perinatal mortality, causes of death and related factors were compared in consecutive 5 years before and after the universal two-child policy. Results In total, there were 153 099 perinatal births from 2011 to 2020 in Changning District, in which 352 deaths were documented. The perinatal mortality was 2.30 per 1 000 births, showing an overall downward trend from 2011 to 2020 ( P <0.05). Residents with local household registration had lower perinatal mortality, compared to those with non-local household registration, which was observed both before and after the universal two-child policy ( P <0.05). Furthermore, the perinatal mortality showed an upward trend after the universal two-child policy ( χ 2trend=5.481, P <0.05). The major causes of perinatal death were fetus and its accessories, fetal malformation, and maternal diseases during pregnancy before the universal two-child policy; in contrast, the causes changed to maternal diseases during pregnancy, fetus and its accessories, and neonatal diseases after the policy. The proportion of pregnant women of advanced maternal age, menstrual delivery, and pregnancy complications or comorbidities were significantly higher after the policy than that before the policy ( P <0.05). The most common pregnancy complication was gestational diabetes mellitus, gestational hypertension, and hypothyroidism during pregnancy after the universal two-child policy. Of them, the proportion of gestational hypertension increased from 6.56% (4/61) to 25.88% (22/85). Conclusion Before and after the universal two-child policy, the perinatal mortality in non-local residents remains high and further shows an upward trend. Moreover, pregnant women advanced maternal age and those with complications or comorbidities may increasingly contribute to perinatal deaths after the policy. Therefore, health education should be strengthened to improve the awareness of self-health care, especially for non-local women. Hierarchical perinatal health service, primary prevention and treatment of pregnancy complications or comorbidities should be improved to further reduce perinatal mortality.

Journal of Public Health and Preventive Medicine 2024;35(3):68-72
Objective To investigate the prevalence and risk factors of cognitive impairment in the nursing home population in Changning District, Shanghai. Methods In this study, random cluster sampling method was used to select 570 elderly people from 5 nursing homes in Changning District. Multivariate logistic regression was used to analyze the risk factors influencing cognitive function. Results The total prevalence of cognitive impairment in Changning nursing home population was 22.5%. The prevalence rates of ≤80 and >80 age groups were 16.8% and 23.9%, respectively, and the prevalence rates of men and women were 19.2% and 23.9%, respectively. Multivariate analysis showed that age ≥ 80 years old (OR=1.839, 95% CI: 1.045~3.235), no habit of reading book or newspaper (OR=2.087, 95%CI: 1.282~3.398), limitation of daily activity ability (OR=1.577, 95% CI: 1.023~2.431), and having depressive symptoms (OR=2.809, 95% CI: 1.840~4.288) were all influencing factors for cognitive impairment. Conclusion More than one fifth of elderly people in nursing homes in Changning District have symptoms of cognitive impairment. It is necessary to carry out routine cognitive assessment and appropriate cognitive intervention for the nursing home population to reduce the health and economic losses caused by cognitive impairment.
