1.Surgical treatment of thyroid nodules in patients with Hashimoto's disease, an analysis of 299 cases

Objective To investigate the clinical characteristics and evaluate surgical experiences in Hashimoto's disease patients suffering from coexistent thyroid nodules. Method Clinical data in 299 cases of Hashimoto's disease coexistent with thyroid nodules treated surgically from Jun 1985 to Dec 2004 were analyzed. Results The coexistent rate of Hashimoto's disease (HD) with thyroid carcinoma (TC) . thyroid adenoma.nodular goiter.hypothyroidism and hyperthyroidism was 17. 4% , 34. 4% , 5. 7% , 3. 7% and 2. 3% respectively. The coexistent rate of HD with TC was 9. 2% before 1995, and 19. 7% after 1996. There were 35 cases of papillary carcinoma, 11 cases of follicular carcinoma, 4 cases of mixed type and one case of lymphoma. There were 17 cases of occult thyroid carcinoma, accounting for 32.7% of the malignancy. Operative procedures included biopsy only and thyroidectomy according to frozen pathology. Conclusions There is a high incidence of coexistent thyroid neoplasm in patients with Hashimoto's disease, the incidence of thyroid carcinoma especially occult carcinoma is rising recently.
2.Analysis of the status and influencing factors of Helicobacter pylori infection of electrical power system workers in Shanghai

Objective To investigate Helicobacter pylori (HP) infection status and influencing factors of electrical power system employees in Shanghai. Methods A total of 29 986 employees in Shanghai electrical power system who underwent physical examination were selected as the research subjects, and received 13C-urea breath test (13C-UBT). The HP infection rate was recorded. Data including gender, age, body mass index, smoking history, drinking history, medication history, educational level, consumption of high-salt and high-fat food, presence of vomiting, and awareness of HP infection related treatment were also collected. The relationship between these factors and HP infection was analyzed. Results 13C-UBT showed that of 29,986 subjects examined, 17 580 were HP positive, and the positive rate was 58.63%. There was significant difference in HP infection rate between men and women (P < 0.05). There was a significant difference in HP infection rate among different age groups (25-40, 41 ~ 55 and 56-70 years old) (P <0.05). People who consumed high-salt and high-fat food and those who displayed no significant difference in HP infection rate (P <0.05). There were significant differences in the HP infection rate between people with and without history of HP infection (P <0.05), between smokers and non-smokers (P <0.05), and between those who knew that HP infection needed treatment and those who did not (P < 0.05). No significant difference was found in age, body mass index, educational level, drinking history and medication history (P>0.05). Logistic regression analysis showed that male, ignorance of HP infection related treatment, smoking and history of family HP infection were independent influencing factors for HP infection. Conclusion The HP infection rate was relatively high among employees in Shanghai electrical power system, and the main influencing factors of HP infection were male, ignorance of HP infection related treatment, smoking and history of HP family infection.
3.Clinical observation on the long-term effect of warming-promotion acupuncture on lumbar spinal stenosis.

OBJECTIVETo observe the difference effects between warming-promotion acupuncture and normal acupuncture on lumbar spinal stenosis (LSS).
METHODSSixty cases of LSS were randomly divided into a normal acupuncture group (30 cases) and a warming-promotion acupuncture group (30 cases). The two groups both chose Dazhui (GV 14), Mingmen (GV 4), Jiaji (EX-B 2),etc. Normal method without special manipulation was used in normal acupuncture group, while the warming-promotion manipulation was used in warming-promotion acupuncture group, all once daily, 10 treatments made one session. Compare the symptoms and spinal cord function of LSS, quality of life (QOL)and clinical effect in the two groups.
RESULTSThe comprehensive score of symptoms of LSS in warming-promotion group 3 months after treatment was 6.30 +/- 1.92, while that in normal acupuncture group was 4.67 +/- 13.70. The score of spinal cord function in warming-promotion group after treatment was 7.03 +/- 1.03, while that in normal acupuncture group was 6.33 +/- 1.12. The score of QOL in warming-promotion group after treatment was 53.67 +/- 8.91, while that in normal acupuncture group was 64.50 +/- 16.69. All the differences between these scores in two groups were statistically significant (all P < 0.05). The total effective rate was 90.0% (27/30)in warming-promotion group, and 80.0% (24/30) in normal acupuncture group. The effect of warming-promotion group was better than that in normal acupuncture group (P < 0.05).
CONCLUSIONIn the field of treating LSS, the effect of warming-promotion acupuncture is better than normal acupuncture.
