1.Feminine Pelvic Lesions:Diffusion-weighted MR Imaging

Objective To investigate the value of diffusion-weighted imaging(DWI) in diagnosing the feminine pelvic lesions.Methods Conventional MRI and DWI with b value of 0 and 1000 s/mm2 respectively were performed in 135 feminine patients with pelvic lesions. Apparent diffusion coefficient (ADC) values of the lesions were measured. Contrast-enhancement MR imaging was performed in 16 patients.Results The average ADC values were (0.90?0.10)?10-3mm2/s in uterine malignant lesions (43 cases),(0.12?0.09)?10-3mm2/s in uterine benign lesions ( 24 cases ) , ( 0.98?0.11 )?10-3mm2/s in adnexal malignant diseases ( 34 cases ) and ( 2.25?0.12)?10-3mm2/s in adnexal benign ones(34 cases), respectively.There were significant differences between malignant and benign groups(P
2.Application of MR Urography in the Diagnosis of Iatrogenic Ureteral Injury

Objective To evaluate the diagnostic value of magnetic resonance urography(MRU)in patients with iatrogenic ureteral injuries.Methods MRU in 12 patients with iatrogenic ureteral injuries caused by gynecological and rectal operation were performed.Results MRU examinations were successfully done in all cases at once.The accurate rate in localized and qualitative diagnosis was all 100%.The injures of ureter in 12 cases were all located at lower segment of the ureter,of them,8 patients were purely obstruction,4 cases were obstruction with urinal leakage,all cases were accompanied by dilations of ureter and hydronephrosis.Conclusion MRU is helpful for the diagnosis of iatrogenic ureteral injury.
3.Application of serum total bilirubin mean in quality control

Objective To analyze the levels of serum total bilirubin in patients and healthy con-trols, eatablish a method of using the mean of total bilirubin in serum of patients to control internal quality, and validate the reference range. Methods Frequencies mode of SPSS13.0 statistic software package was applied to performing analysis of all data, and then the analytic interval was determined based on the frequencies of the data. The daily data mean in the analytic interval was calculated. With the daily data mean as the testing data, the quality control was carried out by the same way of quality control for quality control sample. At the end of each month, the means of various analyzers were compared. Based on the results of healthy controls, the formula mean±1.96s was used to validate the current reference range. Results With the daily data mean as the testing data for quality control, its coefficient variation was within the accepatable limit, and the 95% distribition range was the same as the current reference interval. Conclusion It is necessary to establish suitable interval in which the da-ta mean was used for internal quality control. The current reference range in our hospital is proper.
4.Persistent Improvement of Quality Control in HBV-DNA Quantity Analysis

OBJECTIVE To improve the level of HBV-DNA quantity dection.METHODS The necessity and feasibility of quality control improvement on the basis of external quality assessment results were analyzed.The source of error were songht through analyzing the whole experiment process,to improv the experiment protocol.RESULTS Four main sources of error were improved,and two of them existed in reality.Through improvements in handling protocol,coefficient of variability(CV) of internal quality control has decreased to 3.3% from 9.3% before improvement in protocol.External quality control results were also increased in large-scale.CONCLUSIONS Through the improvement in experiment process,quality assurance of HBV-DNA quantity analysis has been upgraded in essence.
5.Three-dimmentional conformal radiotherapy for mediastinal metastasis from esophageal carcinoma after surgery

Objective To evaluate the effect of 3-dimmentional conformal radiotherapy (3-DCRT) for mediastinal metastasis from esophageal carcinoma after surgery.Methods Sixty-three such patients were treated by 3-DCRT. Tumor diameter was: cm 20 patients. Positive supraclavicular lymph node was found in 9 patients. The number of tumor at the upper right, upper left and upper median mediastinum was 27, 27 and 20. All patients received 3-DCRT to the dose of 40?Gy (2?Gy/f, 5f/w), after resetting of portals, the dose was boost to 64~70?Gy by 3?Gy/f, 5f/w. Dose of the patients who had been irradiated was boosted to 50?Gy without further positioning. Results Complete response rate and partial response rate were 43% and 48% one month after the treatment was concluded,with no progression of disease. The 1-, 2- and 3-year survival rates were 75%, 43% and 15%. Conclusions Three-dimmentional conformal radiotherapy, with little side-effects, can markedly improve the local control rate and suvival rate of patients with mediastinal metastasis from esophageal carcinoma after surgery.
6.Analysis of risk factors for prognoses of 176 patients with acute paraquat intoxication

Objective To explore the risk factors influencing the prognoses by analyzing clinical data of patients with acute paraquat intoxication,and provide a basis for clinical prevention and treatment of the disease. Methods The clinical data of 176 patients with acute paraquat intoxication admitted into our Hospital were retrospectively collected during the period from January 2012 to February 2013. After admission,the conventional medical treatment was given to all the patients,and according to the prognoses of 28 days after poisoning,the patients were divided into death group and survival group. The gender,age,toxic dose,time elapsed from poisoning to admission,time elapsed from poisoning to gastric lavage,number of cases treated with drug lavage,white blood cell count(WBC),alanine amino-transferase(ALT),aspartate amino-transferase(AST),blood urea nitrogen(BUN), serum creatinine(SCr),acute physiology and chronic health evaluation system Ⅱ(APACHE Ⅱ)score,carbon dioxide combining power(CO2CP),number and time of cases revealing arterial blood oxygen saturation(SaO2)<0.90, and the survival rate within 28 days after admission were recorded. Multivariate logistic regression was used to analyze the factors related to the prognosis. Results ①Of the 176 cases,96 survived and 80 died in 28 days,the survival rate being 54.55%.②Univariate analysis showed the poisoning dose(ml),time elapsed from poisoning to admission (minute), WBC(×109/L), ALT(U/L),AST(U/L),BUN(mmol/L),SCr(μmol/L),APACHEⅡ scores, CO2CP (mmol/L),number of cases with SaO2<0.90 in the survival group were significantly lower than those in the death group〔38.14±39.44 vs. 88.50±58.79,41.11±30.29 vs. 90.00±59.31,11.78±3.61 vs. 16.13±5.02,84.46±42.53 vs. 156.35±76.62,76.21±38.69 vs. 184.00±86.48,5.34±2.89 vs. 10.51±6.80,84.17±77.38 vs. 199.74±162.13, 0.96±1.60 vs. 3.60±2.61,22.02±4.47 vs. 18.35±4.19,4(4.17%)vs. 80(100.00%),respectively,all P<0.01〕;there were no statistically significant differences in time elapsed from poisoning to admission(hour),number of cases treated with drug lavage and time of revealing SaO2<0.90(day)between survival group and death group〔7.96±3.39 vs. 8.05±4.26, 20(20.83%)vs. 24(30.00%), 62.25±18.45 vs. 65.70±45.10,respectively,all P>0.05〕.③Multivariate logistic regression revealed poisoning doses>30 ml〔odds ratio(OR)=6.455,95% confidence interval(95%CI)3.177-13.113,P<0.001〕,time elapsed from poisoning to admission>30 minutes(OR=8.639, 95%CI 4.043-18.461, P<0.001), WBC>12×109/L (OR=2.745,95%CI 1.388-5.426, P=0.004),BUN>8 mmol/L(OR=6.713,95%CI 2.650-17.006,P<0.001)and CO2CP≤22 mmol/L(OR=4.737,95%CI 2.389-9.394, P<0.001) were the risk factors correlated with the prognosis of patients with acute paraquat intoxicationl. Conclusion Poisoning doses,time elapsed from poisoning to admission,BUN,CO2CP and WBC are the independent risk factors for predicting prognosis of acute paraquat intoxication.
7.Ulinastatin attenuates paraquat-induced myocardial injury

Objective To investigate the effect of ulinastatin (UTI) on endogenous antioxidant systems in the process of myocardial oxidative stress injury induced by paraquat (PQ),so as to elucidate UTI protecting cadiocytes against PQ-induced injury.Methods Thirty Japan white rabbits were randomly (random number) divided into A,a sham control group; B,PQ intoxication model group and C,D,E,three UTI groups as per different dosages of UTI given.PQ (30 mg/kg) was administered by intraperitoneal route to the rabbits of PQ intoxication model and rabbits of three UTI groups.UTI in doses of 15 000,30 000,50 000 U/kg was administered intravenously every day to the rabbits of three UTI groups respectively for 7 days after PQ intoxication.After the rabbits were sacrificed with 10% chloral hydrate,left ventricles of rabbits were taken.Histomorphological changes of myocardium were observed by using HE staining.Hydroxyproline was measured by alkaline hydrolysis method,myocardial oxidative stress evaluated by 4-hydroxy-2-nonenal (4-HNE) immunohistochemistry staining,transcription levels of transforming growth factor-β1 (TGF-β1),adiponectin,peroxisome proliferator-activated receptor α (PPAR-α),AMP-activated protein kinase-β1 (AMPK-β1),uncoupling protein-1 (UCP-1) were assayed with reverse transcriptionpolymerase chain reaction (RT-PCR).The differences between two groups were analyzed by t test ; for multiple comparisons,data were analyzed by One-Way ANOVA followed by Dunnett' s post hoc test; for two and multiple ranked data comparisons,Mann-Whitney U test and Kruskal-Wallis test were used,and Spearman rank correlation analysis was used to investigate the correlation between UTI dose and transcription levels of endogenous antioxidant.Results Compared with group A,left ventricular cadiocytes in group B became swelled and disarranged,concentration of tissue HYP was elevated to (2.37 ± 0.49),P =0.001;scores of 4-HNE and TGF-β1 mRNA expressions were all upregulated (both P =0.001).Compared with group B,disordered myocytes in UTI treated groups tended towards normal,4-HNE scores in group D and group E descended respectively to (1.83 ± 0.53) and (1.70 ± 0.47),both P =0.001,HYP concentration reduced to (3.51 ±0.39) μg/mg and (3.29 ±0.37) μg/mg (P =0.002; P =0.001),and expressions of TGF-β1 mRNA in these two groups were decreased (both P =0.001).Transcription levels of four endogenuous antioxidant components,adiponectin,PPAR-α,AMPK-β1 and UCP-1,were all augmented in the group E with high dose UTI.The mRNA expressions of these four components positively correlated with the dose of UTI in the myocardium of PQ intoxicated rabbits.Conclusions UTI could protect myocardia against PQ-induced injury by increasing endogenous antioxidant systems.
8.Storage Problems and Countermeasures for Injection Drugs Protected from Light in Inpatient Pharmacy of Our Hospital

OBJECTIVE:To provide reference for storage management of injection drugs protected from light in inpatient phar-macy of hospital. METHODS:From Jan. to Mar. 2016,the storage items of injection drugs protected from light in inpatient phar-macy of our hospital and the current storage situation were surveyed,the quality management tool PDCA circulation was adopted to improve storage situation and evaluate the effect of PDCA. RESULTS:For improper storage of injection drugs protected from light caused by employees lacking knowledge in photophobic knowledge,unfamiliar to the list of injection drugs protected from light, no packaging in returned drugs,after developing and carrying out corresponding countermeasures,including reinforcing staff train-ing,studying characteristics of the new-coming drug timely,renewal photophobic knowledge and the list of injection drugs protect-ed from light,and ameliorating working procedure,the light storage pass rate was increased from 96.5% to 100% in 310 injection drugs protected from light in inpatient pharmacy. CONCLUSIONS:Appling PDCA can effectively improve the storage management of injection drugs protected from light and provide reliable protection for the drugs'safe and effective application.
9.Ubiquitin-proteasome system regulates adipogenic differentiation by stabi-lizing HSP90-dependent PPARγ

AIM: To investigate the role of ubiquitin-proteasome system ( UPS) in adipocyte differentiation. METHODS:Differentiation of 3T3-L1 preadipocytes into adipocytes was induced by treatment with insulin, 3-isobutyl-1-methylxanthine and dexamethasone.Western blot and immunoprecipitation were performed to detect the protein abundances and association, respectively.Oil red O staining was used to determine the intracellular lipid of 3T3-L1 adipocytes.The levels of mRNA were measured by reverse transcription polymerase chain reaction ( RT-PCR) .RESULTS:UPS inhibitor bortezomib (BZM) suppressed the differentiation of 3T3-L1 pre-adipocytes, evidenced by reduced intracellular content of triglyceride, and decreased mRNA expression of adipogenic marker proteins such as adiponectin and adipocyte protein 2.In contrast, administration of sildenafil (SDN), an activator of protein kinase G which was also found to activate UPS, promo-ted adipocyte differentiation.In addition, BZM treatment decreased the expression of heat shock protein 90 (HSP90) and peroxisome proliferator-activated receptor gamma ( PPARγ) in the soluble fraction and reduced association of HSP90 and PPARγ.Furthermore, HSP90-specific N-terminal inhibitor geldanamic mitigated SDN-induced increase in PPARγlevel and 3T3-L1 cell differentiation.CONCLUSION:UPS modulates HSP90-dependent PPARγstability, thus leading to pro-motion of adipocyte differentiation.
10.Effect of electromagnetic radiation on the expression of calmodulin kinase Ⅱ and cAMP responsive element binding protein in primary culture hippocampal neurons of rats

Objective To investigate the changes of calmodulin kinase Ⅱ (CaMK Ⅱ) and cAMP responsive element binding protein(CREB) expressing in primary cultured hippocampal neurons and its relationship with learning and memory deficit after 2000 μW/cm2 electromagnetic radiation.Methods Primary cultured hippocampal neurons in vitro were randomly divided into normal control group,sham-radiated group,and 1 h/d,2 h/d,3 h/dradiation groups.The neurons in the radiation groups were received microwave exposure of 2000 μW/cm2.The change of CaMK Ⅱ and CREB protein in hippocampal neurons of each group of rats were measured with western blot,and the expression of CaMK Ⅱ and CREB mRNA in hippocampus were determined by RT-PCR.Results Compared with control group((0.78 ± 0.07),(0.62 ± 0.12)),the expression of CaMK Ⅱ protein (1 h/d(0.59 ±0.05),2h/d(0.44 ±0.08),3h/d(0.18 ±0.04)) and its mRNA(1h/d(0.41 ±0.08),2h/d(0.34 ±0.04),3h/d(0.24 ±0.02)) was obviously decreased (P<0.05).Compared with control group((0.69 ±0.10),(0.80 ±0.12)),the expression of CREB protein(1h/d(0.49 ±0.05),2h/d(0.4 ±0.04),3h/d(0.17 ±0.03))and its mRNA (1 h/d (0.68 ± 0.11),2h/d (0.53 ± 0.08),3h/d (0.30 ± 0.03)) was obviously decreased after radiation(P<0.05).Conclusion Electromagnetic radiation of 2000 μW/cm2 exposure could weaken the learning and memory abilities of rats and the decreases in the expression of CaMK Ⅱ and CREB protein and their mRNA in hippocampus may be involved in the pathophysiological process of learning and memory deficit.
