1.Application of proximal femoral nail in the treatment of femoral intertr ochanteric fracture

Objective To evaluate the clinical effects in t he treatment of femoral peritrochan teric frac-tures by PFN(proximal femoral nail).Methods A retrospective analysis was done in the treatment of femoral peritrochanteric fractures(72cases).38cases were male and 44cases female.Their age ranged 64~96years old with the average age being 78years old.70cases were intertrochanteric f ractures(EvensⅠtype 16,Ⅱtype 31,ⅢA type 14,ⅢB type 6,Ⅳtype 3),and 2cases subtrochanteric fractu res.Results65patients were followed up for over 6months.All fractures healed and all patients could take care of themselves and were satisfied with t he clinical curative effect except one patient who had the lag screw pulled o ut and coxa vara because of serious os-teoporosis.Conclusion PFNis a better way to cure femoral peritrochanteric fractures because of few complications and simple surgical procedure.Bein g a micro-trauma technique,it is worthy of wide application in clinic.[
2.Primary Report on Hip Joint Superficial Substitution

[Objective]Primarily discuss cases selection and operation skill of hip joint superficial substitution.[Method]From Oct.2004 to Feb.2007,10 hip joints of 8 cases were made superficial substitution,paying attention on cases selection and operation skill.[Result]All cases had no such complications as femur neck fracture,infection,joint dislocation,vessel or nerve injury,deep vein embolism or ectopic ossification in early stage.The patients' score increased to 93~99 and average 95 from 35~56 and 45 before operation,the unequal limbs were corrected for DDH cases.On X-ray measurement:40?~50?for acetabula abduction angle,44?in average;134?~143?,139?for short-handle eversion angle of femur lateral;average off-set rate was 0.20 for femur neck;no loose prosthesis or narrow femur neck,no bright zone around short-handle.[Conclusion]Hip joint superficial substitution is very fit for young people with much sports,but we should pay attention to case selection and operation skill.
3.Functional analysis of + 11G > A single nucleotide polymorphisms in intron 3 of human urate transporter 1 gene

Objective We reported previously that single nucleotide polymorphisms SNP) + 11G > A in intron 3 of the human urate transporter 1 (hURAT1) gene are associated with hyperuncaemia in Han Chinese.The aim of the present study was to evaluate the effect of the variants on hURAT1 function.Methods The wild-type,mutant-type hURAT1 and exon 5-null hURAT1 were constructed,and respectively microinjected into the zebrafish embryo yolks.The subcellular localization of different genotypes of hURAT1 was detected by confocal laser scanning microscope.Results Compared with wild type,the mutant recombinant plasmid transcribed two types of mRNA spliceosome,the wild type and the exon 5-null type.The hURAT1 wild type protein was prominent localized on cell membrane,while the mutant type and exon 5-null hURAT1 proteins were distributed uniform in the cytoplasm but not on the cell membrane.Conclusion The hURAT1 variant + 11 G > A resulted in an alternative splicing of hURAT1 mRNA-exon 5-null type.Its protein product exhibited a different subcellular localization compared with that of wild type.
4.High efficient expression of hydrophilic and antigenic domain of pp65 of Human cytomegalovirus

Objective To detect the active infection of HCMV .The early infection marker pp65 was expressed and used for preparing antibody.Methods The expression vectors containing whole pp65 gene and one hydrophilic and antigenic domain of pp65(pp65 hp) were constructed and induced to express in E.coli, respectively. Western Bolt was applied to test the antigenicity of recombinant protein.ResultWhole pp65 was expressed at low level, less than 5% of total bacteria protein; while high efficient expression was observed for rp65 hp, approaching 40% of total protein in bacteria. Western Bolt result showed that rp65hp reacted with HCMV IgG antibody.Conclusion Recombinant rp65hp was antigenic and can be used to prepare antibody for detection of early HCMV infection.
5.Coexistence of aldose reductase gene and endothelial nitric oxide synthase polymorphisms associates with diabetic nephropathy

BACKGROUND: It has been demonstrated that the C-106T polymorphism at promotor region of aldose reductase gene and GLU298ASP (894G→T) polymorphism at exon 7 of endothelial nitric oxide synthase (eNOS) gene are associated with diabetic nephropathy, it should be further studied wnether the risk for diabetic nephropathy will increase when the above polymorphic sites coexist.OBJECTIVE: To investigate the association between the coexistence of the polymorphisms of both the C-106T at promotor region of aldose reductase gene and GLU298ASP (894G→T) at axon 7 of eNOS gene in chromosome 7q35 region and the genesis of diabetic nephropathy in northern Chinese Hah people.DESIGN: A case-controlled, comparative observation.SETTING: Department of Endocrinology, the Affiliated Hospital of Medical College, Qingdao University.PARTTCIPANTS; Totally 139 inpatients with type 2 diabetes mellitus were selected from the Department of Endocrinology, the Affiliated Hospital of Medical College, Qingdao University from November 2002 to April 2005,including 54 males and 85 females, (64±8) years of age. Inclusive criteria: Accorded with the standards of diabetic diagnosis and classification by WHO in 1999. According to the 24-hour urinary albumin excretion rate (UAER), they were divided into diabetic nephropathy group (n =61) and non-diabetic nephropathy group (n =78). Meanwhile, 63 healthy controls were selected as the control group, including 24 males and 39 females, 50-78 years of age. All the enrolled subjects were Han people. Informed contents were obtained from all the subjects.METHODS: The genotypes and alleles of polymorphisms of GLU298ASP(894G→T) at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene were detected by polymerase chain reaction restriction-fragment length polymorphism technique (PCR-RFLP), DNA sequencing technique and agarose gel electrophoresis separation technique.Then the frequency of genotypes and alleles were compared.MAIN OUTCOME MEASURES: ① Polymorphisms of eNOS gene and aldose reductase gene; ② Results of the multivariant stepwise regression analysis of risk factors for diabetic nephropathy; ③ Polymorphisms of aldose reductase C-106T and eNOS 894G→T and the relative risk of diabetic nephropathy.RESULTS: All the 139 patients with type 2 diabetes mellitus and 63 healthy adults were involved in the analysis of results. ① The frequencies of the T allele and TG genotype at exon 7 894G→T polymorphic site of eNOS gene in the diabetic nephropathy group were significantly higher than those in the non-diabetic nephropathy group and control group (χ2=8.261, 19.629, P < 0.05). ② The frequencies of the T allele and CT genotype at promotor region of aldose reductase gene in the diabetic nephropathy group were significantly higher than those in the non-diabetic nephropathy group and control group (χ2=6.343, 8.940, P < 0.05, 0.01). ③ After associated analysis on the above gene polymorphisms, it was found that the frequency of TG/CT genotype in the diabetic nephropathy group were significantly higher than that in the non-diabetic nephropathy group and control group (χ2=6.972, P < 0.01); whereas the frequency of GG/CC genotype was significantly lower than that in the non-diabetic nephropathy group and control group (χ2=13.304, P < 0.05). ④Glycosylated hemoglobin (GHbA1c), systolic blood pressure, total cholesterol, polymorphisms of 894G→T at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene were all the independent risk factors for diabetic nephropathy (Wald =5.627, 4.92, P < 0.05). ⑤ GG/GC genotype might be the protective genotype for diabetic nephropathy (OR=0.25, P < 0.01); The risk for diabetic nephropathy in the carrier of GG/CT or TG/CC genotype increased by 2.3 times and risk for diabetic nephropathy in the carrier of TG/CT genotype increased by 4.8 times.CONCLUSION: The coexistence of polymorphisms of 894G→T at exon 7 of eNOS gene and C-106T at promotor region of aldose reductase gene can increase the risk of diabetic nephropathy in patients with type 2 diabetes mellitus. And the TG/CT haplotype formed by the coexistence of polymorphism of these two genes is probably the predisposing genotype for diabetic nephropathy.
6.Correlation between serum uric acid and cardiovascular risk factors in Shandong coastal inhabitants

Objective To elucidate the relationship between serum uric acid (SUA) and cardiovascular risk factors in Shandong coastal inhabitants. Methods A random, stratified cluster sampling method was conducted. A population.based cross-sectional survey for hyperuricemia was performed and 4 988 dwellers of Shandong coast were investigated. Fasting SUA,lipid profiles,insulin concentrations,etc.,were determined by automatic biochemical analysis device and electrochemiluminescence analyzer.The investigation of general,healthy and nutritional conditions was carried out by questionnaire. Results In male and female subjects, the cardiovascular risk factors such as systolic blood pressure,diastolic blood pressure, selum ereatinine,total cholesterol.triglvceride,waist circumference and body mass index all increased with rising SUA,and the level of senlm high densitv lipoprotein fell accordingly.Homeostasis model assessment insulin resistance index (HOMA- IR) in female subjects increased by the quartile group,not in male subjects.Compared with SUA<280 μmol/L group.odds ratio(OR) values of hypertension in SUA 280-319,320-349,≥350 μmol/L groups were 1.54,1.88 and 2.70 respectively in female,and 2.29,4.15 and 4.96 for metabolic syndrome (MS).As for male,compared with SUA<280 μmol/L group OR value of hypertension in SUA ≥420 μmol/L group was 1.52,and OR values of MS of 1.41,1.87 and 3.27 occurred in SUA 320~379,380~419,≥420 μmol/L groups.After being adjusted for confounding factors,logistic stepwise regression revealed that elevated SUA was an independent risk factor for hypertension in both genders.Conclusion Elevated SUA level is closely related to the incidence and development of cardiovascular diseases,and should not be ignored in clinical practice.
7.A clinical study of Chuanhu anti-gout mixture in the treatment of acute gouty arthritis

In this double-blind,double-dummy,non-inferiority study,144 cases of newly diagnosed acute gouty arthritis were reassigned to Chuanhu anti-gout mixture group (CH,n =72) and colchicine group (Col,n =72).After treatment for 10 days,the total efficiency rates of CH and Col groups were 93.02% and 95.35%,respectively,showing no difference between two groups.Three months later,the overall recurrence rates in CH and Col groups were 12.50% and 13.89% respectively (P<0.05).The incidence of adverse reactions in colchicine group was significantly higher than CH group (34.72% vs 2.78%,P < 0.01).Blood uric acid,alanine aminotransferase,aspartate aminotransferase,and creatinine in CH group were decreased significantly compared with Col group (P<0.05 or P<0.01).The results suggest that the clinical efficacy of Chuanhu anti-gout mixture in the treatment of acute gouty arthritis was not inferior to colchicine,but with higher safety.

9.Plate fixation treatment for intra articular calcaneal fractures:results and complications

Objective To analyze the results and complications of the treatment of intra articular calcaneal fractures with internal plate fixation. Methods From January 1998 to October 2004, 31 patients with calcaneal fracture (36 calcanea) were treated in our hospital. According to Sanders classification, there were 2 type Ⅱ,22 type Ⅲ,and 12 type Ⅳ. They were treated with reconstruction of the joint surface, internal fixation with screw and plate through the extensile lateral approach, and bone graft. The preoperative and postoperative data were statistically analyzed with SPSS 10.0. Results Follow ups lasted from 6 to 33 months with an average of 25 months. All the fractures healed 3 months after operation. On average, Bhlers angle was improved from preoperative 13.2?to postoperative 38.5?, Gissanes angle from 75?to 115.5?, and step distance from 5.0mm to 0.3mm. Objective To determinate the risk factors of gout in patients with hyperuricemia.Methods Patients detected with hyperuricemia both in epidemiological survey of Shandong coastal areas in 2004 and in health examination of our hospital were followed up for three years to observe the incidence of gout, relationship of diet and gout, and changes of biochemical indicators.Results During 3 years, 102 patients (19%) out of 536 patients with hyperuricemia developed gout. Age(OR=1.046, P<0.05), serum uric acid(OR=1.021, P<0.05), fasting plasma glucose(OR=1.021, P<0.05), triglyceride(OR=1.008, P<0.05), tony crab intake ( OR=5.992, P<0.05),and beer intake(OR=1.012, P<0.05) were the risk factors of gout attack in patients with hyperuricemia.Conclusions Excess intake of tony crab and beer resulting in fluctuation of serum uric acid is the main risk factor of gout in patients with hyperuricemia. Correcting metabolic disorder of glucose and lipid, reducing the intake of high-purine food, and controlling the level of serum uric acid are the measures to reduce gout attack.
